Abstract
Description

腸内細菌叢:
「腸内細菌叢」(以前は、腸内細菌叢(gut flora)又は微生物叢(microflora)と呼ばれていた)は、動物界に属する任意の生物(ヒト、動物、昆虫など)の腸内に生息する微生物(細菌、古細菌、真菌、ウィルス)の集団を示している。各個人は固有の微生物叢組成を有するが、60〜80の細菌類が、合計400〜500の異なる細菌種/個人に基いて、サンプリングされたヒト集団の50%以上で共有されている。
・それは、胃及び小腸が、消化できない特定の食品(主に非消化性繊維)の消化に寄与し:

患者及び方法:
患者及び研究企画:
2016年6月から2017年7月の間にフランスの7つの医療センターからのデノボAMLと診断された24〜69歳の合計62人の患者を、スクリーニングし、そして2018年6月まで追跡した(ClinicalTrials識別子NCT02928523)。急性前骨髄球性白血病の患者、及び/又は消化器障害(炎症性腸疾患、重度の大腸炎など)を含む他の重度の疾患を有する患者、又は研究を含める4日前まで抗生物質療法を受けた患者が、この試験から除外された(表1)。封入直後に採取された糞便の細菌学的安全性を評価し、そしてMDR細菌、細菌病原体、Clostridium difficile、寄生虫、ノロウィルス及び/又はロタウィルスの検出により、患者を除外した。最終的に、治療コホートは、全ての選択基準を満たす25人の患者で構成された。
糞便を、ICの開始の前、患者の入院時に収集した。糞便を、国際公開第2016/170285 (A1) 号及び国際公開第2017/103550 (A1)号に記載されているような凍結防止希釈剤を用いて、GMP条件下で、独自の装置(国際公開第2016/170290 (A1)号に記載されているものと同じ)において、72時間以内に処理し、濾過し、コンディショニングし、そして移植まで−80℃で凍結保存した。より正確には、最初の目視検査により、尿及び血液のないことを確認し、そしてブリストル便スケールに基づいて、そのテキスチャを評価した。次に、使用される凍結防止希釈剤の量を調整するために糞便を計量した。次に、希釈剤を装置に添加し、そして両方を穏やかに混合することで、懸濁液の均質化を確保した。懸濁液を、混合物が篩を通過するのと同時に、濾過した。全てのそれらの工程は、密閉された装置で実行され、空気非微生物叢と接触しないようにする。次に、懸濁液を、装置の下部ポートを介して収集し、そして密閉システム及び耐寒性プラスチックバック内の菅を介して調整し、いくつかの接続を表示し、別々の方法で生成物の出入を可能にした。サンプルを、QCとして使用されるこの工程の最後で収集した。生成物を、最終的に、−80℃で保存した。微生物学的検査(保健機関のガイドラインに従って新鮮な便に対して実施される)、及びフローサイトメトリーによる生存性評価を、生成物のリリース前、最終的に実施した。
C. difficile、Salmonella sp.、Shigella sp.、及びMDR細菌(メチシリン耐性Staphylococcus aureus、バンコマイシン及びグリコペプチド耐性Enterococci、拡張スペクトルベータラクタマーゼ(ESBL)産生細菌及びカルバペネマーゼ産生細菌)の検出を、それぞれ、PCR及び特定の分離培地上での培養を用いて、最初の3回の訪問の間に収集された糞便サンプルにおいて実施した。寄生虫、ウィルス及び病原菌、AFMT使用のための糞便の安全性を検証するために、最初の訪問の間に収集された糞便サンプルでスクリーニングした。寄生虫を、糞便濃縮の後、PCR(Microsporidia, Dientamoeba fragilis)、又は顕微鏡検査(Strongyloides stercoralis、Cyclospora sp.、Isospora sp.、 Entamoeba histolytica、 Giardia intestinalis、Cryptosporidium sp.、Blastocystis hominis)を用いて検査した。ノロウィルスを、それぞれ、PCR及び免疫クロマトグラフィーにより同定し、そして病原菌を、PCR(カンピロバクター)及び培養(Listeria sp.、Vibrio sp.、Yersinia sp.)を用いて検出した。
生化学的及び免疫学的分析を、最初の3回の訪問の間に収集された異なる血液及び糞便サンプルに対して行った。ネオプテリン及び分泌型IgA(slgA)を、それぞれネオプテリンELISA(IBL International)及びIgA分泌ヒトELISA(EUROBIO)キットを用いて、糞便上清液から測定した。総抗酸化状態(TAS)を、Hitachi 912キット (RANDOX Laboratories)を用いて、血漿から測定し、CRP及びフェリチンを、独自の内部手順に従って、種々の医療センターにおける血漿/血清サンプルから測定した。免疫学的マーカーを、血漿サンプルから測定した:IL6 及び TNFα (ヒトサイトカイン/ケモカイン磁気ビーズパネルキット (EMD Millipore));可用性CD14 (sCD14) (ヒトCD14 Quantikine ELISA キット(R&D System)。
ゲノムDNAを、NucleoSpin Soilキット (Macherey Nagel)を用いて最初の4回の訪問の間に収集された糞便サンプルから抽出した。配列決定ライブラリーを、TruSeqキット(Illumina)を用いて、その製造業者の説明書に従って、各DNAサンプルについて構築した。次に、ライブラリーを、2つのペアエンド(2 x 125 bp) HiSeq2500 (Illumina)実行で配列決定した。
Trimmomatic (Bolger, Lohse and Usadel, 2014)を用いての品質濾過の後、宿主配列の除染を、Bowtie2 (Langmead, Ben and Salzberg, 2013)を用いて実施した。従って、936060〜37212124対の読み取り(平均:34811750対の読み取り)を、異なるサンプルから取得した。公平な比較のために各サンプルの配列番号を、同じ配列決定深度、すなわちサンプル当たり1500000対末端配列に対してランダムに正規化した。次に、分類学的プロファイリングを、Kraken v.0.10.5-beta (Wood 2014) 及び RefSeqゲノムデータベース (June 2015リリース, http://www.ncbi.nlm.nih.gov/refseq/)を用いて、実施した。中央値のα−及びβ−多様性指数の測定を、vegan and phyloseqパッケージを用いて10回のサブサンプリング(R Core Team 2015, バージョン3.4.4, http://www.R-project.org)の後、R統計ソフトウェアで実行した。善玉菌の割合を、以下の有益な微生物ファミリーの相対的存在量(微生物叢の分類学的プロファイリングに基く)の合計として定義した:Lachnospiraceae、Ruminococcaceae、Bifidobacteriaceae、Streptococcaceae、Akkermansiaceae、Lactobacillaceae、Eubacteriaceae、Erysipelotrichaceae、Eggerthellaceae、Clostridiaceae、 Prevotellaceae 及び Oscillospiraceae。同様に、有害な細菌の割合は、Bacteroidaceae及び Enterococcaceaeファミリーの存在量の合計として定義された。
次のパラメーターのV3/V1及びV2/V1の比率が、両側対応t検定のおかげで比較された:

患者の特性:
合計62人のAML患者を、7つの異なるセンターにおいてスクリーニングし、25人をAFMTにより治療し、そして20人を、以下の分析が実施されたプロトコルごとの母集団として見なした。治療された、及びプロトコルごとの患者のベースライン特性を表4に示す。プロトコルごとの患者集団(比率、3:1)には、女性よりも男性の方が多く、そして年齢の中央値は50歳であった。ほとんどの患者(治療された及びプロトコルごとの患者の80%)は、中リスクのAMLであると見なされ、ところが治療された集団の3人及び2人の患者は、それぞれ、好ましいリスク及び好ましくないリスクグループであった。全ての患者は、集中的導入化学療法(古典的な「3+7」レジメン又は同等のもの)を受けた。
平均AFMT製品保持時間は、予測よりも長く(推奨される120分ではなく、第1回目及び第2回目のAFMTについては、それぞれ189.50分及び173.33分)、浣腸処理の実現可能性及び患者の不快感がないことを示している。
次に、プロトコルごとの患者の糞便微生物叢の系統発生的豊富さ及び多様性に対するIC及び続くAFMT治療の影響を試験した。本発明者らは、ICが微生物集団の劇的な変化を誘発し、種レベルでV1とV2との間のα−多様性指数の統計的に有意な減少を示すことを実証した:平均豊富さの39.3%の推定減少(960.45から589.71種; p <0.001)(図4a)及び平均Simpson指数の42.3%の推定減少(0.85〜0.50; p <0.001)(図4b及び図5)。AFMT治療の後、種の豊富さ(957.70種;p<0.001)及びSimpson指数(0.86;p<0.001)は、V1とV3との値間に統計的差異がなく、初期レベルに戻った。従って、AFMT後のV3での腸内細菌叢は、種レベルでの豊富さ及びSimpson指数の両方に基づいて、プロトコルごとの母集団で90%以上に再構築される(p<0.001)。微生物群集のこの変化はまた、β−多様性の測定でも観察される(図4c及び5)。実際、Bray−Curtis非類似性度指数(BC)は、IC後の微生物性腸内毒素症の誘発(平均BC V1−V2:0.76)、及びその組成は種レベルで初期群集の組成に近い、AFMT治療後の微生物群集の回復(平均BC V1−V3:0.40(図5)を示している。
文献から構築された34の酪産産生属のリストに基いて、発明者らは、酪酸生産体(プチコアと呼ばれる)の洗練されたリストを決定するために、ODYSSEE試験の患者における各属のレベルと、糞便ネオプテリン(炎症マーカー)との間の相関試験を実施した。Spearman相関及びSpearman相関検定を、R(統計パッケージの関数相関検定)を用いて計算した。複数の試験補正は、それらの分析には適用されなかた。
患者の糞便中のC.difficile及びMDRBの存在を、メタゲノムデータベースセットにおける耐性遺伝子の分析によりV1とV3との間で評価した。
臨床結果は、表2に要約される。死亡した患者の追跡期間の中央値は、7.13ヶ月(範囲、4.8〜8.5ヶ月)であった。6ヶ月で、全生存率(OS)は88%(範囲、4.8〜8.5ヶ月)であった(図8)。治療された患者のうち、21人(84%)が血液学的反応に基いて完全な寛解を達成し、そして1人(4%)が部分的寛解を達成した。治療された集団の1年OS率は84%(4人の死亡)であった(図8)。合計17人の患者(68%)は、12ヶ月でまだ完全に寛解しており、そして1人の患者(4%)は、部分的に寛解していた。さらに、白血病の進行が、1年で、治療された患者の3人(12%)で観察された。

Claims (15)
- 下記工程:
(i)便サンプルを収集し、そして、収集後最大5分で嫌気性条件下に置く工程;
(ii)まだ嫌気性条件下で、前記サンプルと、少なくとも凍結防止剤及び/又は増量剤を含む生理食塩水溶液とを混合する工程;及び
(iii)前記希釈されたサンプルを濾過する工程、
を含む方法により得られた、それを必要とする個人における治療誘発性炎症の予防及び/又は軽減への使用のための糞便微生物叢組成物。 - 工程(ii)において、前記生理食塩水溶液が少なくとも凍結防止剤及び増量剤を含む、請求項1に記載の糞便微生物叢組成物。
- 前記糞便微生物叢組成物が、個人からの1又はいくつかの便サンプルからの微生物叢を含む、請求項1又は2に記載の糞便微生物叢組成物。
- 前記糞便微生物叢組成物が、個人からの少なくとも1つのサンプルに存在する種の少なくとも90%を含む、請求項3に記載の糞便微生物叢組成物。
- それを必要とする個人の治療誘発性腸炎症の予防及び/又は軽減への使用のための、請求項1〜4のいずれか1項に記載の糞便微生物叢組成物。
- 抗癌治療により誘発される炎症の予防及び/又は軽減への使用のための、請求項1〜5のいずれか1項に記載の糞便微生物叢組成物。
- 少なくとも1回の糞便微生物叢移植(FMT)が、抗癌療法の終了後1〜30日で実施される、請求項1〜6のいずれか1項に記載の糞便微生物叢組成物。
- 2回のFMTが、1〜7日間隔で実施される、請求項1〜7のいずれか1項に記載の糞便微生物叢組成物。
- 前記糞便微生物叢組成物によるFMTが、腸内のネオプテリンの減少、及び/又は血清中のCRP及び/又はフェリチンの減少を導く、請求項1〜8のいずれか1項に記載の糞便微生物叢組成物。
- 前記糞便微生物叢組成物によるFMTが、胃腸管における善玉菌の割合の増加、及び有害な細菌の割合の減少を導く、請求項1〜9のいずれか1項に記載の糞便微生物叢組成物。
- 以下の15属のいくつか又は全ての割合が、FMTの前のレベルと比較して高められる:Blautia、Faecalibacterium、Alistipes、Eubacterium、Bifidobacterium、Ruminococcus、Clostridium、Coprococcus、Odoribacter、Roseburia、Holdemanella、Anaerostipes、Oscillibacter、 Subdoligranulum及びButyrivibrio、請求項1〜10のいずれか1項に記載の糞便微生物叢組成物。
- 投与される糞便微生物叢組成物は、以下の15属のいくつか又は全てを含む:Blautia、Faecalibacterium、Alistipes、Eubacterium、Bifidobacterium、Ruminococcus、Clostridium、Coprococcus、Odoribacter、Roseburia、Holdemanella、Anaerostipes、Oscillibacter、 Subdoligranulum及びButyrivibrio、請求項1〜11のいずれか1項に記載の糞便微生物叢組成物。
- 前記個人が、癌患者である、請求項1〜12のいずれか1項に記載の糞便微生物叢組成物。
- 前記個人が、造血系疾患を有する、請求項1〜13のいずれか1項に記載の糞便微生物叢組成物。
- 前記個人が、急性白血病を有する、請求項1〜14のいずれか1項に記載の糞便微生物叢組成物。
