ニューヨーク州のHospital for Special Surgeryから得られた(病理臨床検査医学教授であるLars Westblade博士により提供された)ヒト血液に由来するシュードモナス・エルギノーサ臨床分離株(CFS−1292)を用いて抗菌スクリーニングを行った。CFS−1292株を溶原性ブロス(lysogeny broth)(LB;Sigma-Aldrich)、カザミノ酸(CAA)培地(5g/Lカザミノ酸、Ameresco/VWR;5.2mM K2HPO4、Sigma-Aldrich;1mM MgSO4、Sigma-Aldrich)、又は25%ヒト血清(AB型、男性、プール;Sigma-Aldrich)を添加したCAAのいずれかにおいて培養した。本開示の目的上、特定のシュードモナス・エルギノーサの分離株は重要ではなく、市販の分離株を本実験に使用することもできた。
全ての溶解素及び修飾溶解素をgBlocks(IDT Technologies)として合成し、オーバーラップ伸長PCR又は適合性の付着末端のライゲーションによってアラビノース誘導性発現ベクターpBAD24(24)にクローニングした。全ての構築物を大腸菌株TOP10(Thermo Fisher Scientific)に形質転換した。他の市販の発現ベクター及び系を用いることもできた。
最大250個の一連の推定溶解素及び溶解素様酵素をシュードモナス・エルギノーサのゲノム配列のGenBankデータベースにおいて特定した。3つの検索方法を用いた:i)既知の溶解素の問い合わせ配列を用いた全てのシュードモナス・エルギノーサゲノムの標的化BLASTpスクリーニング、ii)溶解素(及び細胞壁ヒドロラーゼ)触媒及び結合ドメインと関連する全てのスーパーファミリー指定に焦点を合わせた全ての注釈付きシュードモナス・エルギノーサゲノムのキーワードベースの検索、並びにiii)非注釈付きゲノムのファージ配列における溶解素様遺伝子の視覚検索。特定された後、溶解素配列をgBlocks合成し、pBAD24にクローニングし、大腸菌TOP10細胞に形質転換した。次いで、大腸菌クローンを、50mM Trisバッファー(pH7.5)に懸濁した軟寒天で構成されるオーバーレイにおけるGN溶解素活性の検出が可能となるよう修正した寒天オーバーレイプレートに基づく方法(11、13)を用いる一次抗菌活性スクリーニング(生きたシュードモナス・エルギノーサに対する)において試験した。109個の溶菌クローンのセットを特定し、発現及び精製のために選択した。
広範な宿主/発現ベクターの組合せを、本開示の溶解素ポリペプチドをコードするポリヌクレオチド配列の発現に用いることができる。多数の好適なベクターが当業者に既知であり、市販されている。適切なベクターの例は、Sambrook et al, eds., Molecular Cloning: A Laboratory Manual (3rd Ed.), Vols. 1-3, Cold Spring Harbor Laboratory (2001)に提示されている。かかるベクターとしては、特に染色体ベクター、エピソームベクター及びウイルス由来ベクター、例えば細菌プラスミド、バクテリオファージ、トランスポゾン、酵母エピソーム、挿入因子、酵母染色体要素、バキュロウイルス、SV40等のパポバウイルス、ワクシニアウイルス、アデノウイルス、鶏痘ウイルス、仮性狂犬病ウイルス及びレトロウイルス等のウイルスに由来するベクター、並びにそれらの組合せに由来するベクター、例えばプラスミドとコスミド及びファージミド等のバクテリオファージ遺伝要素とに由来するベクターが挙げられる。さらに、上記ベクターは、本開示の溶解素ポリペプチドの構成的又は誘導性発現をもたらすことができる。より具体的には、好適なベクターとしては、SV40の誘導体、並びに既知の細菌プラスミド、例えば大腸菌プラスミドcolEl、pCR1、pBR322、pMB9及びそれらの誘導体、RP4、pBAD24及びpBAD−TOPO等のプラスミド;ファージDNA、例えばファージλの多数の誘導体、例えばNM989、並びに他のファージDNA、例えばM13及び繊維状一本鎖ファージDNA;2Dプラスミド等の酵母プラスミド又はその誘導体;昆虫又は哺乳動物細胞に有用なベクター等の真核細胞に有用なベクター;ファージDNA又は他の発現制御配列を用いるように改変されたプラスミド等のプラスミドとファージDNAとの組合せに由来するベクター等が挙げられるが、これらに限定されない。上述のベクターの多くがNew England Biolabs、Addgene、Clontech、Life Technologies等の供給業者(その多くが好適な宿主細胞も提供している)から市販されている。
シュードモナス・エルギノーサに対する各GN溶解素の最小阻止濃度を、米国臨床検査標準協議会(Clinical and Laboratory Standards Institute;CLSI)によって規定される標準微量液体希釈参照法の修正を用いて決定した(26)。修正は、CAA培地又は25%ヒト血清を添加したCAAのいずれかによるミューラーヒントンブロス置換えに基づくものであった。
CF−301のMBECを、修正した微量液体希釈MIC法の変法を用いて決定した(27、28)。ここで、シュードモナス・エルギノーサ株ATCC 17647の新鮮コロニーをPBSに懸濁し(0.5マクファーランド単位)、TSBg(0.2%グルコースを添加したトリプシンソイブロス)で100倍希釈し、0.15mlアリコートとしてCalgary Biofilm Device(96個のポリカーボネートペグを備える蓋を有する96ウェルプレート;Innovotech)に添加し、37℃で24時間インキュベートした。バイオフィルムを洗浄し、TSBg中の2倍希釈系列のCF−301によって37℃で24時間処理した。全サンプルを三連で試験した。処理後に、ウェルを洗浄し、37℃で風乾し、0.05%クリスタルバイオレットで10分間染色した。染色後に、バイオフィルムを33%酢酸で脱染し、抽出されたクリスタルバイオレットのOD600を決定した。各サンプルのMBECが、クリスタルバイオレット定量化によって評定される、バイオフィルムのバイオマスの95%超を除去するのに必要とされる最小薬物濃度であった。
チェッカーボードアッセイは、微量液体希釈によるMIC決定のためのCLSIの方法の修正に基づく(26、29)。チェッカーボードを、初めに横軸に沿って各ウェルが同量の2倍希釈した抗生物質を含む96ウェルポリプロピレンマイクロタイタープレートの列を作成することによって構築した。別のプレートに、縦軸に沿って各ウェルが同量の2倍希釈したGN溶解素を有する比較用の行を作成した。次いで、各列が一定量の抗生物質及びGN溶解素の2倍希釈物を含み、各行が一定量のGN溶解素及び抗生物質の2倍希釈物を含むように、GN溶解素及び抗生物質の希釈物を組み合わせた。これにより、各ウェルがGN溶解素と抗生物質との独自の組合せを含んでいた。細菌を、25%ヒト血清を含むCAA中で1×105CFU/mLの濃度で薬物の組合せに添加した。次いで、単独及び組合せでの各薬物のMICを、周囲空気中にて37℃で16時間の後に記録した。部分阻止濃度の総和(ΣFIC)を各薬物について算出し、最小のΣFIC値(ΣFICmin)を用いて相乗作用を決定した。ΣFICは、以下のように算出した:ΣFIC=FIC A+FIC B(ここで、FIC Aは、組合せでの各抗生物質のMIC/単独での各抗生物質のMICであり、FIC Bは、組合せでの各GN溶解素のMIC/単独での各GN溶解素のMICである)。組合せは、ΣFICが0.5以下である場合に相乗的、ΣFICが0.5超〜1未満である場合に強く相加的、ΣFICが1〜2未満である場合に相加的、ΣFICが2以上である場合に拮抗的とみなされる。
GN溶解素の溶血活性を、ヒト赤血球の溶解によって放出されるヘモグロビンの量として測定した(30)。簡潔に述べると、ヘパリンの入ったポリカーボネートチューブ内のプールした健常ドナー(BioreclamationIVT)から得られた3mlの新鮮ヒト血液細胞(hRBC)を1000×gにて4℃で5分間遠心分離した。得られた赤血球をリン酸緩衝生理食塩水(PBS)溶液(pH7.2)で3回洗浄し、30 PBSに再懸濁した。50μl容量の赤血球溶液を2倍希釈範囲(128μg/mL〜0.25μg/mL)の50μlの各GN溶解素(PBS中)と共に37℃で1時間インキュベートした。無傷赤血球を1000×gにて4℃で5分間の遠心分離によってペレット化し、上清を新たな96ウェルプレートに移した。ヘモグロビンの放出を、570nmでの吸光度を測定することによってモニタリングした。陰性対照として、PBS中のhRBCを0.1%Triton X−100で上記のように処理した。
シュードモナス・エルギノーサの一晩培養物を新鮮CAA培地で50倍希釈し、撹拌しながら37℃で2.5時間増殖させた。次いで、対数期細菌をペレット化し、1/5培養液量の25mM HEPES(pH7.4)に再懸濁した後、0.5のマクファーランド値に相当する光学密度に最終調整した。次いで、調整した培養物を25mM HEPES(pH7.4)又はCAA/HuSのいずれかで50倍希釈し、GN溶解素を10μg/mLの最終濃度で添加した。溶解素を添加しない対照培養物を含めた(すなわち、バッファー対照)。全ての処理物(treatments)を通気しながら37℃でインキュベートした。溶解素(又はバッファー対照)の添加前の時点、並びにその後1時間及び3時間の間隔を置いた時点で、培養サンプルをCAA寒天プレート上での定量プレーティングのために取り出した。
以下のような本ポリペプチドのin vivo活性を試験する1つ以上の実験が現在進行中である。
全身曝露を有するGN溶解素を特定するために、単回IV注射として投与されるGN溶解素で処理したCD1マウスにおいてPKスクリーニングを行う。血液PKプロファイルを、最大10個のGN溶解素について、マウス血清において適格の研究グレードの生物学的分析アッセイを用いて分析する。適切なPKプロファイルを有する複数のGN溶解素が特定されることが予想される。1を超えるAUC/MIC濃度を達成する血液曝露を示す候補を全身感染モデルにおいて試験する。このモデルについては、シュードモナス・エルギノーサ株(PAO1及び他の臨床分離株)をブタ胃ムチンに再懸濁し、対照マウスにおいて24時間〜48時間にわたる完全罹患率及び死亡率をもたらす接種材料での腹腔内投与によってCD1マウスに投与する。致死的チャレンジの2時間後に、マウスにビヒクル又はGN溶解素を外側尾静脈への静脈内(IV)注射によって投与する。罹患率及び死亡率を72時間にわたって評定する。適切な投与濃度では、GN溶解素で処理したマウスがビヒクル対照と比較して罹患率及び死亡率の低下を示すことが予想される。研究が抗生物質の同時投与を伴っていてもよい。生存データを、GraphPad prismを用いるカプランマイヤー生存分析によって分析し、50%有効濃度(EC50)を各分子について算出する。in vivo活性を有する複数のGN溶解素が特定されることが予想される。
本実験、並びに肺及び腎臓等の他のマウス感染モデルの目的は、これらのモデルにおける有効性データを生成することである。この下位目的(sub aim)で提唱された有効性モデルは概して、細菌による組織(大腿、肺又は腎臓)でのマウス感染を利用する。溶解素による処理後に、組織における細菌負荷(組織1g当たりのCFU)を実験の終了時に定量化し、処理のロバスト性を評定する。加えて、これらのモデルの各々を用いて、PK/PD指数及び有効性の大きさ、例えばAUC/MICを規定することができる。このため、これらのモデルを用いて、単独での又は抗生物質と組み合わせた、肺感染のマウスモデルにおける代表的なGN抗シュードモナス溶解素の有効性を評価し、更なるin vivo試験及び開発に最良のGN溶解素候補を決定する。アシネトバクター・バウマンニを用いて同様のモデルを確立し、本溶解素の有効性の試験に用いることができる。
大腿感染モデル
第1のモデルを確立するために、CD−1マウス(n=12)を、好中球数の99%超の減少(−4日目の150mg/kg及び−1日目の100mg/kg)をもたらす投与計画に従って腹腔内投与される20mg/mLシクロフォスファミドの投与によって好中球減少状態にする。大腿感染を、免疫抑制剤の2回目の投与の24時間後にシュードモナス・エルギノーサ(3×104cfu又は1×105cfu又は3×105cfu又は1×106cfu/大腿)の適切な接種材料の両方の外側大腿筋への筋肉内(IM)注射によって確立する。マウスは、両方の外側大腿筋への筋肉内注射による吸入麻酔下で感染させる。各大腿に、およそ1.4×105CFUのアシネトバクター・バウマンニNCTC 13301(及び/又は等量のシュードモナス・エルギノーサ株)を与えることができる。しかし、試験により適切であると示される場合に接種材料の量の調整を行ってもよい。
1処理当たり最大8匹の麻酔した(100mg/kgケタミン/6mg/kgキシラジンの混合物のIP注射)マウスの群を各鼻孔への20μlの接種材料の鼻腔内注入(鼻孔間に5分間)によって感染させ、感染後約10分間にわたって立位に維持する。適切な接種材料の強度及び量は、上記のように予め決定される。
好中球減少BALB/cマウスに、肺感染を確立するのに十分な接種材料を含有するシュードモナス・エルギノーサ細菌を麻酔下で鼻腔内注入により接種する。4匹のマウスの群(6つ、ビヒクルのみ(最終群))に感染の2時間後、6時間後及び10時間後にGN溶解素、ビヒクル又は対照溶解素を皮下(SC)注射によって投与する。感染の2時間後に、4匹の動物の対照群を、過剰量のペントバルビトンを用いて人道的に安楽死させ、処理前対照群(開始)を得る。感染の16時間後に、残り全ての群をペントバルビトンによって人道的に安楽死させる。動物の体重を測り、各動物から両肺を切除し、個別に秤量する。溶解素及び投与は、上記と同じにすることができる。
有効性と最も密接に関連するPK/PD変量(例えば、Cmax/MIC、AUC/MIC又は%時間/MIC)を描く抗感染薬を用いた動物実験は、臨床的成功にとって高度に予測的である(31)。用量分割を用いて、有効性と関連するPK/PDパラメーターを決定する。単一の総用量を1日1回(q24h)、1日2回(q12h)又は1日4回(q6h)の投与に分割することで、一定のAUCを維持した上でCmax及び遊離薬物時間>MIC(fT>MIC)の複数の値を得ることができる。複数回用量の分割は、有効性エンドポイントと比較した場合に、有効性に必要とされるPK指数及び大きさとしてのCmax/MIC、fT>MIC及びAUC/MICの識別を可能にする独自の曝露プロファイルを生じる。
耐性を発現する可能性を特定するために、マウス有効性研究からのin vivoホモジネートをMIC分析に供する。MICの2倍を超える増大が観察された場合、細菌をプレーティングし、全ゲノムシークエンシングのためにコロニーを単離する。

- GN2、GN4、GN14、GN43及びGN37からなる群から選択されるグラム陰性天然溶解素を含む溶解素ポリペプチド、又は溶菌活性を有するそのフラグメント、又は溶菌活性を有し、前記溶解素ポリペプチドと少なくとも80%の配列同一性を有するその変異体であって、該天然溶解素又は変異体又はフラグメントが非荷電アミノ酸残基による1つ〜3つの荷電アミノ酸残基の置換によって修飾されており、修飾された天然溶解素又はフラグメントが溶菌活性を保持する、溶解素ポリペプチド又はフラグメント又は変異体。
- 前記溶解素ポリペプチドがGN156、GN121、GN108及びGN123の1つ以上、又はその活性フラグメント、又は溶菌活性を有し、前記溶解素ポリペプチドと少なくとも80%の配列同一性を有するその変異体からなる群から選択される、請求項1又は2に記載の医薬組成物。
- 前記細菌がバイオフィルム中にあり、方法により該バイオフィルムの破壊がもたらされる、請求項11に記載の方法。
