房室弁(僧帽弁及び三尖弁)の弁尖は、負荷状態で弁の能力を維持するために長く薄いコード状の支持体のシステムに依存する薄い透視構造である。これらの支持体である腱索は、乳頭筋を弁尖に取り付ける。

Claims (15)
- 腱索修復のためのデバイスであって、前記デバイスは、
第一の端部及び第二の端部を有する可撓性腱と、
前記可撓性腱の第一の端部及び第二の端部の一方に取り付けそして前記可撓性腱を心臓弁の弁尖に固定するように構成されたらせん状ワイヤと、
チャンネルを有し、前記心臓弁の弁尖をクランプしそして前記チャンネルを通して前記可撓性腱をデリバリーして、前記らせん状ワイヤを前記弁尖に固定するように構成された捕捉デバイスと、
を含む、腱索修復のためのデバイス。 - 穿刺針をさらに含み、前記捕捉デバイスは、前記穿刺針及び可撓性腱を通過させるように構成されたチャンネルを含み、前記穿刺針は前記捕捉デバイスが前記弁尖をクランプしている間に、前記弁尖を穿刺するように構成されている、請求項1記載のデバイス。
- 前記穿刺針は前記可撓性腱を通過させるように構成された管腔を含む、請求項2記載のデバイス。
- 患者の心臓の組織壁において前記可撓性腱を固定するように構成されたアンカーをさらに含む、請求項1〜3のいずれか1項記載のデバイス。
- 前記捕捉デバイスは捕捉デバイスを開閉するように構成されたヒンジを含む、請求項1〜4のいずれか1項記載のデバイス。
- 前記弁尖を通る可撓性腱の配置のために前記弁尖を捕捉するように構成された吸引デバイスをさらに含む、請求項1〜5のいずれか1項記載のデバイス。
- 腱索の修復方法であって、前記方法は、
捕捉デバイスを使用して患者の心臓弁の弁尖を捕捉することと、
前記捕捉デバイスにより前記弁尖が捕捉されている間に、可撓性腱を前記弁尖を通して配置することと、
らせん状ワイヤを使用して、前記可撓性腱の第一の端部を前記弁尖内に固定することと、
前記患者の心臓の組織壁内に前記可撓性腱の第二の端部を固定することと、
を含む方法。 - 前記組織壁は前記患者の左心室壁の乳頭筋であり、前記可撓性腱の第二の端部を固定することは前記可撓性腱の第一の端部を固定する前に行われる、請求項7記載の方法。
- 前記可撓性腱の第二の端部を固定することは、前記可撓性腱の第二の端部に連結されたアンカーを挿入するために前記組織壁を穿刺針で貫通することを含む、請求項7〜8のいずれか1項記載の方法。
- 前記捕捉デバイスは、前記穿刺針及び可撓性腱を通過させるように構成されたチャンネルを含み、前記穿刺針は前記可撓性腱を通過させるように構成された管腔を含む、請求項9記載の方法。
- 前記捕捉デバイスは捕捉デバイスを開閉するように構成されたヒンジを含む、請求項7〜10のいずれか1項記載の方法。
- 第一の端部及び第二の端部を有する可撓性腱と、
前記可撓性腱の第一の端部及び第二の端部の一方に取り付けそして前記可撓性腱を心臓弁の弁尖又は心臓の組織壁に固定するように構成されたアンカーと、を含む、腱索修復デバイス。 - 前記アンカーはらせん状ワイヤであり、そしてフィルムで包まれている、請求項12記載のデバイス。
- 前記アンカーは前記アンカーが配置される心臓弁の弁尖における穿刺穴を保護又は充填するように構成されている、請求項12又は13のいずれか1項記載のデバイス。
- 前記可撓性腱の第二の端部に配置された第二のアンカーをさらに含み、前記アンカーは前記可撓性腱の第一の端部に配置されており、前記第一のアンカー及び前記第二のアンカーは前記弁尖に固定されずに前記組織壁を貫通する、請求項12記載のデバイス。
