細胞は、ヒト血清アルブミン(HSA)を含有する緩衝食塩水中で照射され、これに懸濁され得る。可能性のある汚染源を回避するために、細胞は、血清を含まない合成培地で培養され得る。細胞は、凍結保存剤として20%ジメチルスルホキシドを補足した同様の培地中に貯蔵され得る。
当技術分野で知られているように、本発明の細胞は、凍結保存、および照射を含むその後の操作が可能であるように製剤化されねばならない。細胞製剤は、抗原に対する免疫応答を刺激する組成物中で好ましいpH範囲、塩または個体に抗原を提示する他の構成要素を維持するために、緩衝液を含有し得る。細胞は、適当な生理溶液、例えば、食塩水または他の薬理学的に許容される溶媒もしくは緩衝溶液に懸濁され得る。当技術分野で公知の緩衝溶液は、pHを約4.0〜5.0にするために、水1ml当たり0.05mg〜0.15mgのエデト酸二ナトリウム、8.0mg〜9.0mgのNaCl、0.15mg〜0.25mgのポリソルベート、0.25mg〜0.30mgの無水クエン酸および0.45mg〜0.55mgのクエン酸ナトリウムを含有し得る。製剤はまた1つ以上の薬学的に許容される賦形剤を含有し得る。賦形剤は当技術分野でよく知られており、緩衝液(例えば、クエン酸緩衝液、リン酸緩衝液、酢酸緩衝液および炭酸水素緩衝液)、アミノ酸、尿素、アルコール、アスコルビン酸、リン脂質、タンパク質(例えば、血清アルブミン)、EDTA、塩化ナトリウム、リポソーム、マンニトール、ソルビトールおよびグリセロールを含む。
細胞を増殖したら、一般に単回使用バイアル内にアリコートする。細胞を、予め標識された1.2mLのクライオジェニックバイアルに手動で分注する。クライオジェニックバイアルをコールドパック上に維持し、温度を管理するために30mL単位で分注する。約1000個のクライオジェニックバイアル(製造規模)を、充填ラックに充填し、充填が完了するまで、予冷したポリカーボネートクライオジェニックボックスに配置する。いくつかの実施形態において、細胞アリコートは105〜107であり、106が好ましい。
細胞をアリコートした後、凍結する。充填完了後、照射前に、クライオジェニックボックスを制御速度フリーザーで凍結し、液体窒素フリーザーの気相中に貯蔵する。この段階で、バイアルを製品の照射前特性評価およびリリース試験のために除去する。
本明細書で考察されるように、本発明は、凍結後の細胞の照射に関する。
照射のためのクーラーの充填構造を図2に示す。クーラーの構造は、各層が2.5インチの層のドライアイスで分離された、4個のクライオジェニックボックスからなる3層(合計12個のクライオジェニックボックス、各クライオジェニックボックスは81個のバイアルを含有)を含有する発泡スチロールボックスである。クーラーは972個のクライオジェニックバイアル(バッチサイズ)を含有する。容器の仕様、ワクチン製品のストレージボックスの数および容器内でのその配向、1ストレージボックス当たりの凍結製品バイアルの数、並びに容器中のドライアイスの量および位置をすべて同定し、標準操作手順に記入した。
照射施設での輸送および操作手順が細胞生存率にもgp96−Ig発現にも影響を与えないことを確実にするために、12個の各クライオジェニックボックスは、各クライオジェニックボックスの内部または外部領域に配置された非照射HS−110ワクチンの2個の凍結バイアル(1バイアル当たり12×106細胞/0.6ml)を含有していた。各クライオジェニックボックスの残りのバイアルスロットは、凍結保存培地の凍結バイアルを含有していた。操作手順は、実際の照射プロセスの工程を模擬し、輸送LN2デュワーからクーラーに12個のクライオジェニックボックスを移す工程と;充填されたクーラーを室温で2時間貯蔵して、照射プロセスの最悪の場合の持続時間を模擬する工程と;クーラーから輸送LN2デュワーに戻す工程とを含んでいた。模擬照射をSteris照射施設で行った。照射の模擬後、クライオジェニックボックスを、LN2輸送デュワーに戻し、試験のために返送した。クライオジェニックボックスの外部からの12個のバイアルおよびクライオジェニックボックスの内部からの12個のバイアルを、生存率およびgp96−Ig発現について試験し、照射の模擬のために輸送せず、製造所に保管された凍結保存細胞で得られたデータと比較した。クライオジェニックボックスの内部領域に配置された細胞、クライオジェニックボックスの外部領域に配置された細胞、または細胞を凍結保存した製造所に保管された細胞の間で差異は観察されなかった。これらのデータは、異なる施設で細胞に照射するための輸送および操作手順が、ワクチン細胞に悪影響を与えなかったことを示している。
輸送後、照射バイアルは、長期貯蔵のために液体窒素フリーザーの気相中に貯蔵される。
線量マッピング試験を行って、約120グレイ(Gy)線量を、発泡スチロールクーラーに含有される12個のクライオジェニックボックス内の異なる位置に送達できたかを確認した。これは、氷点下の温度での線量計の較正の問題を克服するために室温で行われた。(塩ペレットはドライアイスと類似の密度を有するため)ドライアイスを模擬するために、塩ペレットを使用した。線量計はクライオジェニックボックス内の様々な位置にあり、クーラーはターンテーブル上で照射された。これを3回繰り返し、各位置の平均照射線量を計算した(%RSD約2.0%)。この構造に基づき、線源からの距離に応じて最小および最大照射線量率を、クーラーの下層の中心(最小照射)および上層の外部の角(最大照射)にあるバイアルに対して、各々1分当たり11.7および14.2グレイと計算した。約120グレイの照射線量を得るために、バイアルは8.5〜10.3分間照射されねばならず、必要に応じて照射日に線源の減衰に対して補正される。結果を考えると、このプロセスにおいて個々の製品バイアルが受ける照射の予想範囲は最小約108グレイ〜最大約132グレイであろう(図3参照)。cGMP処理を進めると、製品に送達される実際の線量は、照射日の線量率および曝露時間に基づいていた。今後の製品バッチのために、照射は、輸送前に1ワクチンバッチ当たり単一のアラニン線量計の挿入により、および(照射中の適当なクーラーの回転を確認するために)発泡スチロールクーラーの対向する角に配置された少なくとも2つの線量計により独立して確認される。これらの線量計をNISTで測定し、照射プロセスが行われたことを保証し、受けた照射線量を評価する。
多くの汎用される線量測定または線量計測方法は温度により影響を受けるので、大量の凍結物質中に線量計を配置することは非現実的である。実験的に決定した補正計数と共に基準線量計のモニタリング位置を使用することで、温度による線量計の応答における差異を補正する必要が排除される。提案された対象物質または市場に流通しない実際の物質の密度および分布を模倣する模擬物質を周囲温度で使用して、線量率(および得られた補正係数)を確立することができ、したがって、温度が線量計の結果を損なうことが回避される。代表的な物質を周囲温度で使用して線量率を決定したら、常套的な線量測定システムを使用して、基準線量を測定することができる。次いで、確立された補正係数を測定された基準線量に適用することにより、最小および最大線量を計算できる。製品に送達される必要がある線量範囲が、照射時に使用中の線量測定システムの測定能力を下回る場合、線量率を処理中の線量計の代わりに使用できる。最小と最大両方の線量率は、同じ処理条件に基づいて行われた3回の照射ランにわたって付与され、放射線源の減衰に対して補正される、曝露時間、平均最小送達線量および平均最大線量に基づき製品に対して決定され得る。計算された最小および最大線量率は、ターンテーブルおよびこれらが計算された位置に特異的である。計算後、線量率を使用して、照射処理時間および照射中に送達される線量を決定できる。
本明細書で使用する「模擬または代用物質」は、実際の被験物質と類似の特性を有する物質を指し、これは実際の製品または市場に流通しない実際の製品の代わりに用いられ得る。いくつかの実施形態において、バイアル構造では、81個のバイアルが各9個のバイアルからなる9列に配列され、各々0.6mLの凍結保存細胞を含有する。バイアルボックス一部が含まれるべきではないが、0.6mLの代用品が充填されているべきである。照射されるクーラーの数(3つのデュワーごとに1つ)をカートンの数として入力する。製品に必要な線量範囲を、kGyで線量範囲の欄に入力する。
いくつかの実施形態において、照射クーラーの1つの面にプロトコルごとに「正面」の印を付ける。2つの段ボールセパレータを使用し、クーラーを少なくとも30分間冷却する。いくつかの実施形態において、2層半(2と2分の1)のドライアイスを照射クーラーの底に配置し、1つの調製済み段ボールセパレータで覆い、蓋を交換する。照射クーラーの蓋を交換した時間を記録し、署名および日付を記載する。
いくつかの実施形態において、移動は5分以内に完了しなければならない。いくつかの実施形態において、移動はドライアイスの添加の少なくとも30分後に開始しなければならない。いくつかの実施形態において、デュワーは、各々が必要とされる番号順で開かれる。いくつかの実施形態において、バイアルボックスはすべて、標識と共に照射クーラーの「後方」向かって配向される。デュワーからのラックの除去は番号順である。照射クーラー内のバイアルボックスは、段ボールセパレータの上に配置される。照射クーラー内に第1のバイアルボックスが配置される時間(時間および分)を記録する。ラックをデュワーに戻し、デュワーを閉じる。第2の調製済み段ボールセパレータをバイアルボックスの上に配置し、ドライアイスで覆う。照射クーラーをODMS−RTシステムに収容する。要求された最小線量を0.00kGyとして入力し、0.01を要求された最大線量として入力する。
いくつかの実施形態において、凍結保存細胞の照射日は線量率チャートに入力される。いくつかの実施形態において、最小曝露時間は、要求された最小線量(Gy)÷最小線量率(Gy/分)=曝露時間(分)で計算される。いくつかの実施形態において、最大曝露時間は、要求された最大線量(Gy)÷最大線量率(Gy/分)=曝露時間(分)で計算される。いくつかの実施形態において、平均曝露時間は、(最小曝露時間+最大曝露時間)/2=平均曝露時間で計算される。曝露時間は、分および秒でプロセスタイマーに入力されねばならない。このためには、平均曝露時間からの残りの分(小数)を、以下のように秒に変換しなければならない:[15.11.6]からの任意の残りの分(小数位)×60秒/分=秒。矢印が上向きであり、照射中に矢印が再配向されないように、照射クーラーは、ボトムダウンで照射される。
いくつかの実施形態において、移動は5分以内に完了しなければならない。最後のバイアルボックスをクーラーからデュワーに移す時間は、最初のバイアルボックスをクーラーに配置した時間から2時間を超えない。「照射済み」のステッカーは、端部が互いに張り付くように、各デュワーのラックのハンドルの周りで二つ折りにする。ステッカーの両端をホッチキスで閉じる。クーラーを開き、上層の氷および上部の段ボールセパレータを除去する。第1の技術者はデュワー3を開き、ラックを除去する。第2の技術者は上層のバイアルボックスを一度に除去し、これを第1の技術者に手渡す。第1のバイアルボックスを照射クーラーから除去した時間(時間および分)を記録する。バイアルボックスをデュワーに移し、時間を記録する担当の技術者は署名および日付を記載する。第1のバイアルボックスをクーラーに配置してから最後のバイアルボックスをクーラーからデュワーに移し終わるまでの全経過時間を記録する。
いくつかの実施形態において、細胞は、修飾および分泌型熱ショックタンパク質(すなわち、gp96−Ig)を発現する。いくつかの実施形態において、細胞は分泌型熱ショックタンパク質(すなわち、gp96−Ig)、例えば、Viagenpumatucel−Lを発現する。
驚くべきことに、照射前の細胞性ワクチン細胞の凍結は、一般にその特性を変化させず、顕著な利点をもたらす。これらの特質は、以下に記載される細胞生存率、複製能および代謝機能を決定するために1つ以上のアッセイを用いて一般に確認される。
一実施形態において、細胞生存率アッセイが行われる。いくつかの実施形態において、CellTrace(商標)バイオレット細胞増殖キットが、細胞生存率を評価するために用いられた。CellTrace(商標)バイオレット染色は、原形質膜を横切り、細胞内で共有結合し、そこで蛍光色素は細胞培養環境において一貫したシグナルを数日間発する。色素は細胞表面および細胞内のすべての遊離アミンに共有結合し、わずかな細胞毒性を示し、細胞の増殖能またはバイオロジーに対する影響が最小限であることが観察される。複製および分裂する細胞については、各細胞の色素濃度は各分裂と共に希釈される。増殖しない細胞は同様の色素の希釈を示さない。したがって、膜色素が分裂親細胞と2つの得られた娘細胞との間でほぼ等しく希釈されるので、2つの集団は、蛍光の減少に基づき区別され得る。
一実施形態において、複製能アッセイが行われる。本明細書に概説されるように、ワクチンとしての使用のための細胞性組成物は、一般に複製不能であるが、しばらくの間生存可能なままある。
一実施形態において、代謝機能性アッセイが行われる。いくつかの実施形態において、代謝機能性アッセイは、代謝率を評価することにより;ATPの産生について好気性(酸化的リン酸化)対嫌気性(解糖)プロセスの相対的貢献度を評価することにより;マイクロプレート中の接着細胞を測定することにより;またはマイクロプレート中の懸濁細胞を測定することにより、培養物中の細胞が生存しているかどうかを示す。
本明細書に開示される発明をより効率的に理解できるように、以下の実施例が提供され得る。これらの実施例は例示のみを目的とし、いかなる方法によっても本発明を限定するとは解釈されるべきではないことを理解されるべきである。
Vesigenurtacel−L(HS−410)医薬品の製造プロセスは、5つの工程;製剤化、バイアル充填、凍結、照射および貯蔵(図1参照)からなる。原薬(Vesigenurtacel−L細胞のバルク採取物)を、貯蔵せずに、所望の濃度で最終凍結保存培地にすぐに再懸濁し、単一用量のクライオジェニックバイアルに分注して、所望の用量レベルを得る。次いで、照射前に、バイアルを制御された速度で凍結し、液体窒素フリーザーの気相中に貯蔵する。照射されたバイアルは、最終医薬品を構成する。細胞の培養および増殖の開放操作すべてを、ISOクラス7以下のISOクラス5安全キャビネット(BSC)において無菌条件下で行う。
原薬(40×106細胞/mL)を、貯蔵せずに、すぐに処理して、医薬品を製造する。高強度製剤(高線量)については、原薬細胞を、6%ペンタスターチ、5%DMSO、0.5%HSA、0.007%炭酸水素ナトリウムおよび0.567%塩化ナトリウムを含有する、20×106生細胞/mLの最終薬物濃度を得るために、凍結保存培地との1:1希釈により新たに製剤化する。
細胞生存率を評価するために、CellTrace(商標)バイオレット細胞増殖キットを使用した。CellTrace(商標)バイオレット染色は、原形質膜を横切り、細胞内で共有結合し、そこで蛍光色素は細胞培養環境において一貫したシグナルを数日間発する。色素は細胞表面および細胞内のすべての遊離アミンに共有結合し、わずかな細胞毒性を示し、細胞の増殖能またはバイオロジーに対する影響が最小限であることが観察される。複製および分裂する細胞については、各細胞の色素濃度は各分裂と共に希釈される。増殖しない細胞は同様の色素の希釈を示さない。したがって、膜色素が分裂親細胞と2つの得られた娘細胞との間でほぼ等しく希釈されるので、2つの集団は、蛍光の減少に基づき区別され得る。
第1のHS−410GMPバッチ(高線量および低線量)からのクライオジェニックバイアルを、2つの全く異なる試験方法:CellTraceバイオレット染色(CTVアッセイ)および組織培養処理ポリスチレン上の単層培養物を調べるクローン原性アッセイ(CFUアッセイ)で複製不能について試験した。トリチウム化チミジン評価はDNA複製活性および実際の複製を検出するが、CTVアッセイおよびCFUアッセイは各々細胞性複製を特異的に評価するため、トリチウム化チミジン取り込みではなく、CTVアッセイおよびCFUアッセイを使用した。HS−410製品の製造において、照射プロセス後、細胞は、DNA損傷を維持するが、生存可能および代謝的に活性なままであることが予想され、したがって、細胞はDNA複製を試み得るが、最終的に細胞は複製不可能であることが予想される。軟寒天試験は、これらの細胞の複製能を評価するのに可能性のある試験方法と見なされた。しかし、軟寒天でのこれらの細胞の試験では、HS−410細胞株が比較的接着依存性であり、軟寒天中で十分に増殖しないことが示された。したがって、軟寒天は、HS−410製品における複製能のある細胞の検出について感受性のあるアッセイ方法である可能性が低い。
CTVアッセイをさらに支持するために、第2のアッセイ方法を使用して、新しい照射プロセスがHS−410細胞を複製不可能にすることを確認した。このCFU試験において、培養基質は、製造プロセスで細胞の増殖のために使用した組織処理ポリスチレン上の同じタイプの単層培養物であった。このCFUアッセイにより、21日間の培養後の複製細胞のコロニーについて照射HS−410細胞(および適当な対照)を調べた。このアッセイの条件を、FDAの推奨事項に準拠するように設計した。フェーズ2プロセスで製造した最初のGMPバッチのCFU試験について、5つの高線量HS−410バイアル(12×106細胞/0.6mL)および5つの低線量(12×106細胞/0.6mL)をバッチごとに5つの異なるボックスから選択した。各バッチから4つのバイアルは最低照射バイアルを表し、バッチごとに1つのバイアルは、最高レベルの照射を受ける位置を表す。各ボックスの層(T、MおよびB)および両方のバッチ(高線量および低線量)について試験したバイアルの相対位置を図6に示す。
照射の実現可能性試験
異なる施設で細胞に照射するための輸送および操作手順が生存率またはgp96−Igの発現に影響を与えないこと、および標的照射線量を発泡スチロールクーラーの全領域に送達できたことが証明された後、試行照射試験を行った。模擬照射試験と同様に、12個の各クライオジェニックボックスは、クライオジェニックボックスの外部または内部領域に配置されたHS−110ワクチンの2つの凍結バイアル(1バイアル当たり12×106細胞/0.6mL)を含有していた。各クライオジェニックボックスの残りのバイアルスロットは、凍結保存培地の凍結バイアルを含有していた。確立された標準操作手順(SOP)後、これらの細胞を輸送し、これに照射した。次いで、照射バイアルを製造業者にLN2デュワーで返送し、細胞を生存率、HLA A1およびgp96−Ig発現、ならびに複製能について試験した。各アッセイを照射前および照射後のバイアルの3つのバイアルで行った。さらに、凍結保存培地を含有するバイアルを容器の閉鎖完全性について試験した(色素浸漬試験)。以下の表3に示すように、生存率、生細胞の回復またはHLA−A1もしくはgp96−Ig発現について、照射前試料と照射後試料との間の差異は観察されなかった。さらに、線量マッピング試験により決定されたように、最大、最小または中程度の照射線量を受けることが予想された細胞バイアル間で差異は観察されなかった(図8参照)。
照射した被験物について複製不能を証明することにより照射プロセスを検証するために、12個の各クライオジェニックボックス中の凍結HS−110ワクチンを含有するクライオジェニックバイアル(12×106細胞/0.6mL)を、確立されたSOPによる照射のために送った。40個のワクチンバイアルを10個の各クライオボックスの外部および内部から選択し、複製能アッセイで試験した。この数は、無菌性を評価するためのUSP<71>のサンプリングモデルに基づき、バッチ全体を表す統計的に適当な数の試料である。
照射前手順において、プロトコルはa)細胞採取、b)照射(12,000Rad、懸濁液中、CM1培地、氷)、c)洗浄、凍結保存、バイアル化、およびd)−70℃に凍結→LN2(図11参照)を含んでいた。具体的には、細胞を培養し、(遠心管に)採取し、9%FBSを含有するIMDM培地に再懸濁し、120グレイの線量で、湿氷上で250mLの遠心管中のバルク細胞懸濁液(約20×106細胞/ml)として照射した。照射後、無菌充填および凍結の工程の前に、バルクワクチンを洗浄培地で2回洗浄し、最終的に細胞を医薬品凍結保存培地に懸濁することにより、バルクワクチンをさらに処理した。製造した2つの膀胱ワクチン製品バッチにこの手順で照射した。これらのバッチ(HBIB05およびHBIB06)を試験すると、照射後許容レベルの細胞生存率およびgp96−Ig発現を保持することが示された。さらに、照射細胞は、FAC CellTrace(商標)バイオレットまたはトリチウム化(3H)−チミジン取り込み法によりアッセイしたように複製不能であることが示された。このプロセスでの結果は許容できるものであったが、細胞生存率を維持するために、プロセスは、細胞培養製造施設に近接する(およびこれと密に一体化した)照射施設を必要としていた。この組み合わせは業界では珍しく、したがって規模拡大および操作の非学術的な製造業者への移行後、これを保持することは実現不可能であった。

Claims (21)
- a)容器中に新たに採取した細胞を得る工程と、

を含む、細胞保存方法。 - 細胞を液体窒素中に貯蔵する工程をさらに含む、請求項1に記載の方法。
