Abstract
Description
したがって、このような結合タンパク質薬物複合体(binding protein drug conjugates conjugate)(BPDCs)、特に、抗体薬物複合体(antibody drug conjugates)(ADCs)は、腫瘍の治療のための高い医療上および商業的関心がある。
腫瘍の治療のための有効かつ安全なBPDCsまたはADCsを開発するためには、いくつかの点に対処する必要がある:
まず、結合タンパク質または抗体は、ほとんどあるいは理想的に、正常または健康な組織細胞によって発現されていない所定の腫瘍特異的抗原(TSA)に特異的であることが必要である。
第二に、薬物と結合タンパク質間の共有結合、または結合(linkage)は、循環中に十分に安定である官能基を有する必要があり、血流中において、毒性低分子薬物(payload)の望ましくない放出を阻止し、しかし、腫瘍細胞に結合および/または内部移行(internalization)の際に、薬物を、効果的に放出しなければならない。
第三に、潜在的に限られたTSAの量が、腫瘍細胞に発現され、そして、そのために、ADCの限られた量のみが内部移行(internalize)される場合でさえ、または、腫瘍細胞への結合時、または腫瘍細胞への内部移行(internalization)の際に、毒性低分子薬物(payload)のリリースが、十分に効率的に影響しない場合、毒性低分子薬物(payload)は、腫瘍細胞の破壊を達成するために、十分に高い毒性、または効力が必要である。
第一に、化学的マレイミドベースのリンカーは、ヒト血清アルブミンの存在下で望ましくない不安定性と関連しており、そして、したがって、ADCsを含むマレイミドリンカーで治療された患者の循環器において、毒素の放出につながることが見出されている(アリー(Alley)ら、2008年を参照)。
第2に、マレイミドリンカー反応基による古典的な化学的結合(conjugation)は、結合(conjugation)が起こるアミノ基またはチオール基を制御することができないので、均一でない(heterogeneous)、BPDCsまたはADCsを生じる。
したがって、3.5と4の間の範囲の、平均薬物対抗体比(DAR)を、複合化された(conjugated)ADCsが持つように、抗体当たり、共有結合した薬物の数のガウス分布(Gaussian distribution)が得られる。
しかし、個々の複合体(conjugates)は、薬物が抗体に結合していないもの(平均薬物対抗体比(DAR)=0)、または、システイン複合体(conjugates)の場合に、8つまでの薬物が抗体に結合しているもの(平均薬物対抗体比(DAR)=8)、そして、リシン複合体(conjugates)の場合に、抗体当たり、さらにより多くの薬物を有するもの(>平均薬物対抗体比(DAR)=10)がある。
古典的な化学的複合化された(conjugated)ADCsは、したがって、異なる機能的性質を示す、異なる分子の不均質な混合物を表し(Panowskiら、2014年参照)、それは、明らかに、癌患者の治療のためのADCsの開発において、規制の観点から、望ましくない。
これは、現在、結合タンパク質−薬物複合体(binding protein drug conjugates)において、使用するための毒素と、古典的マレイミドリンカー反応基を回避し、部位特異的な方法で(site−specific manner)、上記結合タンパク質にこれらの毒素を結合するための、必要に応じて、新しい技術を、提供する。
化学的に結合し、マレイミドリンカーを含有するBPDCsまたはADCsの、上記の血清における不安定性、および、不均一性の、両方は、患者において、このようなADCsの毒素の非特異的放出(すなわち、「脱薬剤化(de−drugging)」とも呼ばれる)を助長するので、癌患者に対して、これらの薬物の安全性に重要な責任(liabilities)を負う。
ヒト血清アルブミンのシステイン−34は、毒素が転送され、そして、共有結合でヒト血清アルブミン(HSA)に結合される、いわゆる逆マイケル反応の方法により、マレイミドリンカーのチオエーテル結合を切断することができる。次いで、毒素−HSA複合体(conjugate)は、腫瘍選択性を有さずに、循環中または体内に、毒素を分布させる(Alleyら、2008年参照)。
毒素関連の副作用のために、2000年にFDAで承認されていた最初のADCである、ファイザー/ワイスのマイロターグ(R)(ゲムツズマブ オゾガマイシン)は、FDA承認後10年で市場から回収する必要があった(http://www.fda.gov/NewsEvents/Newsroom/PressAnnouncements/ucm216448.htm)。
腫瘍の治療における化学療法薬として、それらの臨床的有効性が証明されているので、BPDCsまたはADCsの低分子薬物(payload)として使用するための、非常に興味深いクラスのDNAインターカレート毒素は、アントラサイクリンである(Minotti(2004年)参照)。
さらに、毒性低分子薬物として、ドキソルビシンを有する、多発性骨髄腫および他の血液腫瘍のための、フェーズI/II臨床試験中の1つの抗CD74 ADCである、ミラツズマブ−ドキソルビシンもある(clinicaltrials.gov identifier: NCT01101594参照)。
現在、臨床試験で評価されている、最初の、ドキソルビシン−ADCの例にもかかわらず、従来のアントラサイクリンをADC戦略の有毒な低分子薬物(payload)として使用することは困難を極める可能性がある。
最初の好ましい実施形態によれば、アントラサイクリンに特徴的な4環性のアグリコン構造のC14炭素と、それに結合したヒドロキシ基を欠くPNU−159682誘導体を含むアントラサイクリン(PNU)誘導体複合体(conjugates)が記載されている。
第2の好ましい実施形態として、アントラサイクリンの特徴的な4環性アグリコン構造のC13のカルボニル基およびC14のヒドロキシ基を有するC13およびC14炭素の両方を欠いた、アントラサイクリン(PNU)誘導体複合体(conjugates)が記載されている。
その好ましい実施形態は、図3A,6Aおよび6Bに示される。

BPDCsとADCsの生成のために、伝統的なマレイミドリンカーの化学(chemistry)の主な制限を克服するために、我々は、以前に、BPDCsとADCsの生成のために、配列特異的なトランスペプチダーゼ酵素を用い、ソルターゼ酵素か、または、いわゆる、分割−インテイン(split−inteins)のいずれかを用いる、酵素的アプローチを開発した(WO2014140317A1を参照)。

キメラのモノクローナル抗体のcAc10および、ヒト化モノクローナル抗体トラスツズマブは、ソルターゼA認識配列(Sortase A recognition sequence)および追加のStrep IIアフィニティー精製タグ(Strep II affinity purification tag)(HCタグ配列:LPETGGWSHPQFEK;LCタグ配列:GGGGSLPETGGWSHPQFEK)で、C末端がタグ付けされた、それらの重鎖と軽鎖を有して、当業者に公知の方法を用いて作製された(図11Aおよび11Bを参照)。
その後、N−ヒドロキシサクシンイミド(N−hydroxysuccidimide)(NHS、46mg、400μmol)およびエチル(ジメチルアミノプロピル)カルボジイミド(EDC、100mg、523μmol)のジクロロメタン(DCM)溶液を、1(51mg、81μmol)の6mlDCM溶液に添加した。30分後、混合物を、水(2x6mL)で洗浄し、Na2SO4で乾燥させ、そして、蒸発させた。
MS m/z 955.2(M+H)。
薬物負荷は、疎水性相互作用クロマトグラフィー(HIC)と、逆相クロマトグラフィーの両方によって評価した。
HICは、TOSOHの、ブチルNPR 4.6ミリメートルx3.5センチメートル、2.5μmのカラムを使用し、0.8ml/分で、A(1.5Mの(NH4)2SO4、25mMのNaPi、pH値=6.95±0.05)とB(75%の25mMのNaPi、pH値=6.95±0.05、25%のIPA)の間の12分間の直線勾配で溶出して、実施した。
逆相クロマトグラフィーは、ポリマー ラボ PLRP 2.1ミリメートルx5センチメートル、5μmのカラムを使用して、1mL/分/80℃で、0.05%のTFA/H2Oと、0.04%のTFA/CH3CNとの間の、25分間の直線勾配で溶出して行った。サンプルは、DTTと共に、15分間、37℃、pH8.0でインキュベートすることにより最初に還元した(reduced)。PNU−EDA−Gly5ベースのADCsの両方は、主に単量体であり、そして、それぞれ、最大理論量の4に近接する薬物対抗体比(drug−to−antibody−ratio)を有した。
ソルターゼA結合 ブレンツキシマブ−PNU−EDA−Gly5およびトラスツズマブ−PNU−EDA−Gly5 の ADCsを用いた、インビトロの細胞毒性アッセイ。
ブレンツキシマブ−PNU−EDA−Gly5の細胞毒性は、Karpas−299(CD30を高レベルで発現する非ホジキンリンパ腫細胞株)およびL428(CD30のレベルは低〜中程度の発現のホジキンリンパ腫細胞株)を用いて調べた(図4)。
対照として、cAc10−PNU−EDA−Gly5の効果は、市販のCD30特異的cAc10−vcPAB−MMAE複合体(conjugate)である、Adcetris(R)(陽性対照として)および、市販のHER−2特異的トラスツズマブ−DM1複合体(conjugate)である、Kadcyla(R)(陰性対照として)の効果と比較した。
したがって、トラスツズマブに対する、PNU−EDA−Gly5のソルターゼの媒介する結合は、商業的に入手可能であり、そして、FDAで承認された対照物である、ADC Kadcyla(R)のそれを超える、非常に高い効力を有するADCを齎し、さらに、HER2LO ヒト乳癌細胞にさえ効果的である。
トラスツズマブ エムタンシン(Kadcyla(R))を含むマレイミドリンカーと比較した、ソルターゼ Aで結合されたcAc10−PNU−EDA−Gly5 ADCの、インビトロの血清安定性。
簡潔に述べると、cAc10−PNU−EDA−Gly5は、マウス(シグマ社、M5905)、ラット(シグマ社、R9759)およびヒトの血清(シグマ社、H6914)で希釈され、そして、37℃でインキュベートされた。試料は、0、3、7、14日目に、液体窒素中でスナップ凍結(snap−frozen)され、そして、ELISA分析まで、−80℃で保存された。
ADCの血清濃度と総IgGsは、既知濃度の同じADCのサンプルとの比較により、試料滴定の半最大値(half maximal values of the sample titrations)から計算した。
0日目と14日目の間の時間点を、0の最終濃度を達成するように規定された(constrained)、一相指数関数的減衰関数(one−phase exponential decay function)にフィットさせることによって、各血清において、cAc10−PNU−EDA−Gly5およびKadcylaの半減期の値を決定した。Kadcylaの半減期は、それぞれ、マウス、ラットおよびヒトの血清において、3.7日、4.4日および2.9日であったのに対して、cAc10−PNU−EDA−Gly5の半減期は、マウス、ラットおよびヒト血清に対し、14日より大きかった。

著しく対照的に、トラスツズマブ−PNU−EDA−Gly5で処置した全ての動物において、腫瘍は、治療の間に、退縮し続け、そして、細胞の移植後30日目までには、本質的に検出不能となった(図10D)。腫瘍は、60日目までに、ほとんどの動物で検出可能ではなかった。そして、腫瘍の再発は、40日目位に、一匹の動物だけで観察された。

- 請求項2乃至21のいずれか1項に記載の、または、請求項22の方法で生成された、結合タンパク質−薬物複合体(binding protein drug conjugates)(BPDC)の使用であって、

・ に罹患し、
・ に発症するリスクがあり、および/または
・ として診断された、
ヒトまたは動物被験体の治療のための使用。 - 病態が、腫瘍性疾患である、請求項23に記載の使用。
- 腫瘍性疾患が、
*IHCまたはISHによって決定される、1+、2+または3+のHER−2発現スコアを有する腫瘍で、腫瘍は、好ましくは乳癌であり
*IHC、ELISAまたはフローサイトメトリーによって決定される、CD30陽性である腫瘍、好ましくは、リンパ腫、より好ましくはホジキンリンパ腫(HL)または全身性未分化大細胞型リンパ腫(systemic anaplastic large cell lymphoma)(sALCL)
である、請求項24に記載の使用。 - 請求項2乃至21のいずれか1項に記載、または、請求項22の方法で製造された、結合タンパク質−薬剤複合体(BPDC)および少なくとも1つの他の薬学的に許容される成分を含む、医薬組成物。
