Abstract
Description
−厳密に腫瘍特異的な抗原。おそらくすでに臨床的に検討されているこの群の唯一のメンバーは、神経膠芽腫で頻繁に過剰発現し、また非小細胞肺癌ならびに正常組織にはない前立腺癌、乳癌、頭頸部癌および卵巣癌でも認められる表皮成長因子受容体(EGFRvIII)の変異体IIIである。
−腫瘍上および非生命維持健常組織上に発現した表面抗原。この群の潜在的なCAR抗原は、B細胞系譜に主として制限されている分化関連分子である。これら(および多数の臨床試験における標的抗原)のうちで著名なものは、CD19であり、B細胞分化のごく初期に獲得され、B細胞受容体(BCR)によるシグナル伝達に関与する膵B細胞マーカーである。膜前立腺抗原は、このカテゴリーの別のクラスの抗原を構成する。
−非悪性腫瘍促進細胞によって通常発現する抗原。1つのこのような抗原は、線維芽細胞活性化タンパク質(FAP)であり、多様な原発癌および転移癌において腫瘍関連線維芽細胞によってほぼ常に発現する細胞表面セリンプロテアーゼである。別の抗原は血管内皮成長因子(VEGF)であり、これは腫瘍血管形成の間に多量に発現し、多くの生命維持器官における血管細胞およびリンパ内皮細胞上に通常発現する。
−生命維持健常組織と共有される腫瘍関連抗原(TAA)。
−コンビナトリアル(または「スプリット」)抗原認識。真の腫瘍特異的表面抗原はまれであるが、所与の腫瘍によって共発現する腫瘍関連抗原として必ずしも分類されない2つの異なる抗原の組み合わせは、新たな腫瘍特異的シグネチャーを定義することができる。CAR T細胞の活性をこのような抗原対に制限することは、重要な安全基準を提供し、結果として、腫瘍特異的標的の範囲を拡大し、実質的な治療的価値となり得る。第2世代および第3世代のCARは、CARエンドドメインで2つ以上のシグナル伝達部分をつなぎ合わせることによって単一の抗原と係合すると、治療用T細胞に活性化シグナルおよび共刺激シグナルを提供するように設計されてきた。しかしながら、活性化および共刺激が、それぞれ異なる抗原に特異的な2つのCARの間で同じT細胞において分けられる場合、完全な生じる応答は、2つの抗原の存在下でのみ達成され得る2つの相補的シグナルの協力を必要とするであろう。この原理はいくつかの前臨床試験で実証されている(Kloss et al.,2013、Lanitis et al.,2013、Wilkie et al.,2012、WO2016/126608)。
1.aCARの活性化部分(FcRγ/CD3−ζ)を認識単位および共刺激要素(例えば、CD28、4−1BB)から、それらを2つの異なるポリペプチド産物上に遺伝的に置くことにより脱連結させること。aCAR機能に必須であるこれらの要素の再連結は、各ポリペプチドに別個に組み込まれたそれぞれの結合部位を架橋することができるヘテロ二量体化薬の添加によってのみ起こるであろう(図2B)。ヘテロ二量体化薬によって同様に分割された認識部分および活性化部分を架橋することによる完全に機能的なCARの再構築は、近年、Wuらによって報告された(Wu et al.,2015)。この目的のために、本著者らは、ラパマイシン類似体AP21967の存在下でヘテロ二量体化するFK506結合タンパク質ドメイン(FKBP、104アミノ酸)およびFKBP−ラパマイシン結合ドメインのT2089L変異体(FRB、89アミノ酸)を使用した(スキームI)。この薬剤は、ラパマイシンと比較して1000倍低い免疫抑制活性を有し(Bayle et al.,2006、Graef et al.,1997、 Liberles et al.,1997)、市販されている(ARGENT(商標)、Regulated Heterodimerization Kit,ARIAD)。
2.2つの膜内切断部位を含有するRIP制御受容体の膜貫通ドメインの2つの表面上に、それぞれpCAR認識単位および欠損活性化ドメインを移植すること(図2A)。pCARをその抗原へ結合することは、まず外部ドメインを除去する細胞外ディスインテグリンおよびメタロプロテイナーゼ(ADAM)ファミリーのメンバーによって、次いで細胞内γ−セクレターゼによって、コードされたポリペプチドの二重切断を誘起し、それによってpCARの細胞内ドメインを遊離する。この事象は、欠損している活性化要素の機能的な膜固定構造へのアクセスを獲得するための切詰型aCARの能力を破壊し、それにより作動様式を獲得すると予測される(図2C)。この原理は、CAR T細胞活性を腫瘍細胞上の2つの異なる抗原の同時認識に制限して、RIPを介して機能する2つのよく研究されている受容体であるNotch受容体(Morsut et al.,2016、Roybal et al.,2016b )または上皮細胞接着分子(EpCAM、Pizem,Y.,本発明者の監督下での科学修士論文)のいずれかを適用するように設計された新たな遺伝子スイッチの開発において近年利用された。これらの研究では、RIP系のCARが1つの抗原に結合すると、遺伝子操作された細胞内ドメインが放出され、それが細胞核に輸送され、そこで第2のCARの発現がオンになる。これとは異なり、本発明は、防御抗原の存在下でいかなる潜在的なaCAR活性も解除するためにこの過程を利用する。
−まとめると、選択された認識部分は、全22のヒト常染色体の2本の腕の各々に存在する一連の遺伝子の細胞表面産物を標的とする。隣接する遺伝子間の距離が短ければ短いほど、網羅性は広くなり、使用の普遍性は大きくなる。
−選択された遺伝子の各々について、一組の対立遺伝子特異的認識部分が単離され、各々は、ヒト集団において優勢である異なる対立遺伝子変異体間の厳密な識別を可能にする。標的変異体の数が多ければ多いほど、患者へ提供することができる治療用遺伝子対の数は多い。
本明細書で使用する「核酸分子」という用語は、DNA分子またはRNA分子を指す。
iCAR標的として役立つことができる遺伝子を同定するために、次の必要条件を採用した。
1.遺伝子は膜貫通タンパク質をコードし、それゆえ細胞表面上に発現してiCAR結合を可能にする部分を有する。
2.遺伝子は少なくとも2つの発現する対立遺伝子を有する(チェックされた少なくとも1つの民族集団において)。
3.当該遺伝子について見出された対立遺伝子変異は、タンパク質の細胞外領域において参照配列と比較してアミノ酸変化を引き起こす。
4.遺伝子は癌においてLOHを受ける染色体領域に位置する。
5.遺伝子は、対応する領域がLOHを受けることが判明した腫瘍型の起源組織において発現する。
Exome Aggregation Consortiumデータベース(ExAC、exac.broadinstitute.org)を解析への入力として使用した。ExACデータベースは、合計60,706のエクソームを含む、さまざまな集団レベルの配列決定研究からのエクソームの編集物である(Lek et al.,2016)。ExACは、変異体対立遺伝子の計数の数と比較した参照対立遺伝子の計数の数(対立遺伝子頻度−染色体の総数からの変異体対立遺伝子の計数の数)を含む各変異体についての情報を含有する。対立遺伝子頻度情報は、表2に詳述されるようにデータベース内の下位集団にわたっており、閾値は少なくとも1つの集団において5%以上の対立遺伝子頻度であった。
1)変異体は、コードされたタンパク質のアミノ酸組成に影響しなければならない。ii)変異体は、ExACデータベースに現れる少なくとも1つの集団において0.05(5%)以上のマイナー対立遺伝子の頻度を有しなければならない。マイナー対立遺伝子が0.5(50%)よりも大きな対立遺伝子部分を有するシナリオについて、分析を修正した。ある部位に3つを超える対立遺伝子が観察された場合、最も一般的な置換を用いた。
2)変異体(この場合、一塩基多型(SNP))は、その変異が次の変異体クラスのいずれかに分類される場合、タンパク質の組成に与える影響を有するものと注釈された:「ミスセンス変異体」、「インフレーム欠失」、「終止喪失」、「終止獲得」、「インフレーム挿入」、「終止保有変異体」、「フレームシフト変異体」、「終止喪失」、「コード配列変異体」、「タンパク質変化変異体」。
遺伝子型−組織発現(GTEX)データベースv6p(dbGaP Accession phs000424.v6.p1)を、種々の組織型において発現する遺伝子の同定のために使用した(https://gtexportal.org/home,Consortium GT.Human Genomics,2015)。GTEXデータベースは、多様な健常組織型からの8,555のヒト試料のRNA配列決定からなる。
タンパク質がCAR−T媒介性治療薬のための優れた標的となるためには、その一部を細胞表面に発現させる必要がある。細胞表面タンパク質の同定に役立つように、本発明者らは、いくつかのデータベースを使用した。第1のデータベースは、Human Protein Atlasであり、32種類の組織を解析するために使用された24,028の抗体に由来する解析である(Uhlen et al.,2015)。第2のデータベースは、Cell Surface Protein Atlasであり、N−グリコシル化した細胞表面タンパク質の質量分析に基づくデータベースである(Bausch−Fluck et al.,2015)。第3のデータベースは、細胞表面発現を含むタンパク質の細胞内局在を同定するための、高処理量免疫蛍光法および自動画像解析を用いた近年公開された解析物である(Thul et al.,2017)。各SNPに、タンパク質が出現したデータベースの数を注釈づけた。3つのデータベース全てにおいて遺伝子によってコードされるタンパク質は、「高い信頼度」、2つのデータベースで「中等度の信頼度」、1つのデータベースで「低い信頼度」で細胞表面に発現すると言われた。表3に示すように、10,322個の遺伝子のうちの3,359個が上述のように膜発現の何らかの証拠を有した。
iCARが膜タンパク質のうちの1つの対立遺伝子を喪失した癌細胞だけを有効に認識するためには、タンパク質の構造はどの対立遺伝子がコードされているかに基づいて十分に異なっているべきである。SIFT(耐容性から非耐容性を選別すること)アルゴリズムは、タンパク質変異体がタンパク質構造に影響を及ぼし、それゆえ機能するであろうかどうかを予測しようと試みる(Ng and Henikoff,2003)。スコアは0(有害)から1(良性)の範囲であり得る。SIFTスコア(第sift5.2.2版)は、スコアが利用可能であるあらゆるSNPについて含めた。
iCARが対立遺伝子を認識するためには、対立遺伝子はタンパク質の細胞外部分に収まっていなければならない。各SNPについて、コンセンサス翻訳において影響されたアミノ酸の位置を抽出し、www.uniprot.org/downloadsからダウンロードした、UniProtデータベースから細胞外として注釈を付けたドメインと比較した。全てのタンパク質のドメインの特徴付けの欠如のため、多くの偽陰性が起こり得る。1146個の遺伝子における合計4577個のSNPに細胞外として注釈づけた。
優れたiCAR標的は、腫瘍の大部分でヘテロ接合性の喪失(LOH)を受けるSNPであろう。セグメントコピー数ファイルを、TCGAデータベースhttp://www.cbioportal.orgのcbioポータルからダウンロードした(Cerami et al.,2012、Gao et al.,2013)。各遺伝子についてLOHを受けているTCGAデータベース中の32の腫瘍のうちの腫瘍の比率を、HLA遺伝子についてより詳細に後述の通り決定した。
HLAクラスI遺伝子は、それらの既知の特徴、すなわち、いずれの対立遺伝子からも発現する細胞表面タンパク質、幅広い組織分布、高レベルの多型、および腫瘍の回避機序としての腫瘍における記録されたLOHにより、第1のセットの潜在的なiCAR標的として選択した。このゆえに、本発明者らは、HLAクラスIタンパク質が様々な腫瘍型において喪失された割合を決定することによって解析を開始した。本発明者らは、表4に列挙するHLA−A、BおよびCのゲノム遺伝子座におけるヘテロ接合性の喪失の存在についてこれらのコピー数特性を解析した。
異なる腫瘍において同定された数対の保存されおよび喪失した対立遺伝子変異体を選択するので、それらのポリペプチド産物は変異体特異的mAbの生成に役立つであろう。候補mAbの識別力は、次のように、二重染色およびフローサイトメトリー実験または免疫組織化学によってアッセイされる。
凍結組織をしばしばホルマリン系の溶液中で固定し、試料の凍結切片化を可能にするOCT(Optimal Cutting Temperature化合物)中で包埋する。OCT中の組織を−80℃で凍結保存する。凍結ブロックを、切片化の前に−80℃から取り出し、クリオスタットチャンバー中で平衡化し、薄い切片(しばしば5〜15μm厚)に切断する。切片を組織学的スライド上に載せる。スライドは−20℃〜−80℃で保存することができる。IHC染色の前に、スライドを室温(RT)で10〜20分間解凍する。
組織をホルムアルデヒド固定液中に包埋した。パラフィン蝋の添加前に、室温で特定の回数および持続時間で、漸増濃度のエタノール(70%、90%、100%)およびキシレン中での徐々の浸漬によって組織を脱水する。次に、組織をパラフィン蝋中に包埋する。
パラフィン包埋組織をミクロトームで5〜15μm厚の切片へと切断し、56℃の水浴中で浮遊させ、組織学的スライド上に載せる。スライドは室温で保つことができる。
IHC染色の前に、パラフィン包埋切片は再水和ステップを必要とする−
再水和−切片をキシレン中での浸漬(2×10分)、続いて低下する濃度のエタノール中での10分ずつの浸漬によって再水和させる。100%×2
95%エタノール−5分
70%エタノール−5分
50%エタノール−5分
dH2O中でのすすぎ

4.ブロッキング−ブロッキング緩衝液中で組織を室温で30分間ブロッキングする。ブロッキングバッファーは検出方法に依存し、通常はPBS×1中5%の動物血清、またはPBSX1中1%のBSAである
5.一次抗体−抗体の製造元の説明書により、一次抗体をインキュベーション緩衝液(すなわち、PBS中1%BSA、1%ロバ血清)中に希釈する(他のインキュベーション緩衝液も使用することができる)。希釈した一次抗体中で組織を4℃で一晩インキュベートする。一次抗体は、先に詳述したように、モノクローナル抗HLA−A、抗HLA−Bまたは抗HLA−C対立遺伝子特異的抗体であり得る。
結合型一次抗体を使用する場合は、遮光してステップ8へ進む
陰性対照として、一次抗体を含まないインキュベーション緩衝液のみで組織をインキュベートする
また、実験に使用したモノクローナル抗体のアイソタイプを一致させた対照を行う
6.洗浄−スライドを洗浄緩衝液中で洗浄する(3×5〜15分)。
7.二次抗体−抗体の製造元の説明書により、インキュベーション緩衝液中に二次抗体を希釈する。室温で30〜60分間、希釈した二次抗体中で組織をインキュベートする。遮光する
8.洗浄−スライドを洗浄緩衝液中で洗浄する(3×5〜15分)。
9.DAPI染色−DAPIインキュベーション緩衝液を希釈する(約300nM〜3μM)。各切片に300μlのDAPI溶液を加える。室温で5〜10分間インキュベートする。
10.洗浄−スライドをX1PBSで1回洗浄する
11.退色防止用マウントメディアでマウントする
12.スライドを遮光したままにする
13.蛍光顕微鏡を使用してスライドを可視化する

11.製造元の指針により、ABC−HRP試薬を添加する。室温で60分間インキュベートする。
12.製造元の指針により、DAB溶液(または他の色素原)を調製し、組織切片に適用する。色素生成反応は、エピトープ部位を褐色にする(通常、数秒〜10分間)。シグナルの強度が撮像に適しているとき、次のステップへ進む
13.洗浄−スライドを洗浄緩衝液中で洗浄する−3×5〜15分間。
14.スライドをdH2O中で洗浄する−2×5〜15分間。
15.核染色−ヘマトキシリン溶液を添加する。室温で5分間インキュベートする。
16.組織切片を脱水する−

本研究の目的は、CAR−T療法の標的に当たるが「腫瘍オフ」効果を阻害するであろう合成受容体を創製することとする。その程度まで、活性化CARおよび阻害性CARから構成されるCARコンストラクトのライブラリーが確立されるであろう。
標的外細胞に対するaCARの活性を阻害する上でのiCARコンストラクトの機能性を検査するための試験管内組換え系が確立されるであろう。この目的のために、aCARエピトープ、iCARエピトープまたは両方を発現する標的細胞が作出されるであろう。aCARエピトープを発現する組換え細胞は、標的上の「腫瘍オン」細胞を表すのに対し、aCARエピトープおよびiCARエピトープの両方を発現する細胞は、標的上の「腫瘍外」健常細胞を表すであろう。
iCARの阻害効果は試験管内および生体内の両方で検査されるであろう。
ルシフェラーゼ細胞毒性アッセイ−ホタルルシフェラーゼを発現するように組換え標的細胞(T)を操作するであろう。試験管内ルシフェラーゼアッセイは、Bright−Glo Luciferaseアッセイ(Promega)により行われるであろう。形質導入されたエフェクター(E)T細胞(iCARおよびaCARまたは偽CARの両方で形質導入)を、異なるエフェクター対標的比で、HLA−A2、CD19またはその両方を発現する組換え細胞と共に24〜48時間インキュベートする。細胞殺滅は、Bright−Glo Luciferaseシステムを用いて定量化するであろう。
CTLが標的細胞を殺滅する経路のうちの1つは、Fasリガンドを介してアポトーシスを誘導することによる。CASP3タンパク質は、システイン−アスパラギン酸プロテアーゼ(カスパーゼ)ファミリーの一員である。カスパーゼの連続的な活性化は、細胞アポトーシスの実行相において有意な役割を担っている。プロカスパーゼ3のカスパーゼ3への切断は、立体配座の変化および触媒活性の発現を結果的にもたらす。カスパーゼ3の切断された活性化型は、モノクローナル抗体によって特異的に認識されることができる。
標的細胞はレポーター遺伝子(例えば、mCherry)で標識されるであろう。形質導入されたT細胞は、最長5日間、「腫瘍」細胞または「腫瘍外」細胞のいずれかと共にインキュベートされるであろう。時間差顕微鏡は殺滅を可視化するために使用されるであろう。あるいは、終点時点で標的細胞数を決定するための生細胞数染色およびCountBrightビーズ(Invitrogen)を使用するフローサイトメトリー分析を実施するであろう。
形質導入されたT細胞を組換え標的細胞と共にインキュベートし、BioLegendのELISA MAX(商標)Deluxe Setキットにより細胞培養上清中のサイトカイン分泌を測定することによって、またはサイトカインを産生するT細胞の百分率のフローサイトメトリー解析によってのいずれかで、サイトカイン産生(IL2またはINFγ)を定量するであろう。フローサイトメトリー解析については、サイトカインの分泌を防止するためにゴルジ体の停止を使用するであろう。形質導入されたT細胞を標的細胞と共に6および18〜24時間インキュベートした後、T細胞を内側染色キット(Miltenyi)によって浸透および固定し、T細胞マーカー(CD3およびCD8)に対する抗体ならびにサイトカインIL2およびINFγに対する抗体で染色するであろう。
T細胞の脱顆粒は、リソソーム関連膜タンパク質(LAMP−1)であるCD107aの表面発現によって同定することができる。LAMP−1の表面発現はCD8T細胞の細胞毒性と相関することが示されてきた。この分子はリソソームの内腔側に位置する。活性化の際、CD107aは、活性化されたリンパ球の細胞膜表面へ輸送される。CD107aは、一過性に細胞表面上に発現し、エンドサイトーシス経路を介して迅速に再内在化する。それゆえ、CD107aの検出は、細胞刺激中の抗体染色によって、およびモネンシンの添加(酸性化およびそれに続くエンドサイトーシスを受けたCD107a抗体複合体の分解を防ぐため)によって最大限となる。
NOD/SCID/γc−マウスに腫瘍細胞を静脈内接種するであろう。腫瘍細胞の1つの可能性は、ホタルルシフェラーゼを発現するように操作されるであろうCD19陽性NALM6(ATCC、ヒトB−ALL細胞株)細胞であり得る。さらに、「標的オン」、「腫瘍オフ」細胞の確立のために、NALM6もiCARエピトープ(例えば、HLA−A2)を発現するように操作され、それによって健常細胞を提示するであろう。マウスを試験群に分け、一方の群にNALM6細胞を注射し、もう一方の群にiCARエピトープを発現するNALM−6を注射するであろう。数日後、マウスに、aCAR、aCAR/iCARおよび形質導入していないT細胞のまたはT細胞を有さない対照群であろう。マウスを屠殺し、腫瘍量を総フラックスにより定量するであろう。
iCARコンストラクトを発現するT細胞が同じ生物内での生体内で標的細胞と標的オフ細胞を識別できるかどうかを検査するために、「腫瘍オン」/「腫瘍オフ」のNALM−6細胞の1:1混合物をマウスに注射し、続いてaCAR単独またはaCARおよびiCARの両方のいずれかを発現する形質導入したT細胞の注射が行われるであろう。マウスの屠殺の際に、脾臓および骨髄における「腫瘍オン」および「腫瘍オフ」細胞の存在が、2つのマーカー、すなわちCD19およびiCARエピトープについてのフローサイトメトリーによって解析されるであろう。

(i)既知の変異体の少なくとも1つのデータベースからタンパク質をコードする遺伝子のヒトゲノム変異体のリストを検索することと、
(ii)(i)で検索した変異体の前記リストを
(a)対応する参照対立遺伝子と比較して、前記それぞれの遺伝子によってコードされる前記タンパク質においてアミノ酸配列変異を結果的に生じる変異体を選択すること、
(b)前記アミノ酸配列変異が前記コードされたタンパク質の細胞外ドメインにある遺伝子の変異体を選択すること、

それにより、LOHにより前記少なくとも1つの腫瘍において喪失し、前記少なくとも1つの腫瘍の起源の組織において少なくとも発現する、前記それぞれの遺伝子によってコードされるタンパク質中の細胞外ドメインにおいてアミノ酸配列変異を有する変異体のリストを得ることと、
(iii)(ii)において得られた前記リストから少なくとも1つの単一の変異体を含む配列領域を定義し、前記少なくとも1つの単一の変異体を含む前記配列領域および前記対応する参照対立遺伝子を含む配列領域をサブクローニングして発現させ、それにより、前記それぞれのエピトープペプチドを得ることと、
(iv)(iii)において得られた、前記クローニングされた配列領域によってコードされるエピトープペプチドへ、または前記対応する参照対立遺伝子によってコードされるエピトープペプチドへのいずれかへ特異的に結合するiCAR結合ドメインを選択することと、
(vii)(iv)において定義されたiCAR結合ドメインをそれぞれ含む、請求項1〜8のいずれか1項に定義されたiCARを調製することと、を含む、方法。 - 各変異体についてのマイナー対立遺伝子頻度が1、2、3、4または5%以上である、請求項19に記載の方法。
- 安全なエフェクター免疫細胞を調製するための方法であって、(i)腫瘍関連抗原に指向されるTCR操作されたエフェクター免疫細胞に、請求項1〜8のいずれか1項に記載のiCARをコードするヌクレオチド配列を含む核酸分子をトランスフェクトすること、もしくは前記細胞に請求項9に記載のベクターを形質導入すること、または(ii)請求項1〜8のいずれか1項に記載のiCARをコードするヌクレオチド配列を含む核酸分子および請求項10〜13のいずれか1項に定義されるaCARをコードするヌクレオチド配列を含む核酸分子を、未処置のエフェクター免疫細胞にトランスフェクトすること、または請求項10〜18のいずれか1項に記載のベクターをエフェクター免疫細胞に形質導入すること、を含む、方法。
- 請求項21に記載の方法によって得られる安全なエフェクター免疫細胞。
- 抗原の非多形細胞表面エピトープへ特異的に結合する細胞外ドメインを含むaCAと、前記aCARの前記細胞外ドメインが結合する異なる抗原の多形細胞表面エピトープの単一の対立遺伝子変異体に特異的に結合する細胞外ドメインを含むiCARと、をその表面上に発現する、請求項22に記載の安全なエフェクター免疫細胞。
- 前記aCARの前記細胞外ドメインが、CD19などの、表1に列挙された抗原から選択される非多形細胞表面エピトープへ特異的に結合する、請求項22または23に記載の安全なエフェクター免疫細胞。
- 前記aCARおよび前記iCARが別個のタンパク質として前記細胞表面上に存在する、請求項22に記載の安全なエフェクター免疫細胞。
- 前記iCARをコードするヌクレオチド配列の発現レベルが、前記aCARをコードするヌクレオチド配列の発現レベル以上である、請求項22に記載の安全なエフェクター免疫細胞。
- LOHを特徴とする腫瘍に罹患している対象の個別化されたバイオマーカーを選択する方法であって、
(i)前記対象から腫瘍生検を得ることと、
(ii)前記対象から正常組織の試料、例えばPBMCを得ることと、
(iii)LOHのため前記腫瘍の細胞によって発現されないが前記正常組織の細胞によって発現される多形細胞表面エピトープの単一の対立遺伝子変異体を同定することと、を含み、
それにより、前記対象について個別化されたバイオマーカーを同定する、方法。 - 請求項22に記載のエフェクター免疫細胞を患者へ投与することを含む、LOHを特徴とする腫瘍に罹患している前記患者における癌を治療するための方法であって、前記iCARが、ヘテロ接合性の喪失(LOH)のため、前記腫瘍の細胞には存在しないが、少なくとも前記患者の関連哺乳類組織の全ての細胞に存在する多形細胞表面エピトープをコードする単一の対立遺伝子変異体に指向される、方法。
- 前記iCARが、ヘテロ接合性の喪失(LOH)のため、前記腫瘍の細胞には存在しないが、少なくとも前記患者の関連哺乳類正常組織の全ての細胞に存在する多形細胞表面エピトープをコードする単一の対立遺伝子変異体に指向される、LOHを特徴とする腫瘍に罹患している患者を治療することにおける使用のための、請求項22に記載の安全なエフェクター免疫細胞。
- 前記治療が、前記癌患者へ(iii)のaCARを発現するが(iii)のiCARが欠失している免疫エフェクター細胞の少なくとも1つの集団を投与することを含む治療と比較して、標的上の腫瘍外の反応性の低減を結果的に生じる、請求項29に記載の使用のための安全なエフェクター免疫細胞。
- 腫瘍関連抗原または抗原の非多形細胞表面エピトープへ特異的に結合する細胞外ドメインを含むaCARと、前記aCARの前記細胞外ドメインが結合する抗原とは異なる抗原である、前記腫瘍の起源の組織において少なくとも発現する抗原の、またはHLA−Aなどのハウスキーピングタンパク質の、単一の対立遺伝子変異体へ特異的に結合する細胞外ドメインを含むiCARとをその表面上に発現する、請求項29に記載の使用のための安全なエフェクター免疫細胞。
- 自家またはユニバーサル(同種異系の)エフェクター細胞である、請求項28に記載の使用のための安全なエフェクター免疫細胞。
- T細胞、ナチュラルキラー細胞またはサイトカイン誘導性キラー細胞より選択される、請求項28〜32のいずれか1項に記載の使用のための安全なエフェクター免疫細胞。
- 2つ以上の核酸分子の組み合わせであって、それぞれが、制御されたエフェクター免疫細胞活性化システムの異なるメンバーをコードするヌクレオチド配列を含み、前記核酸分子が、単一の連続核酸分子を形成するか、または2つ以上の別々の核酸分子を含み、前記制御されたエフェクター免疫活性化システムが、ヘテロ接合性の喪失(LOH)により1つ以上の染色体またはその画分を喪失した腫瘍細胞を殺滅するようにエフェクター免疫細胞に指示し、関連正常組織の細胞を温存する、組み合わせであって、
(a)第1のメンバーが、抗原の非多形細胞表面エピトープへまたは異なる多形細胞表面エピトープの単一の対立遺伝子変異体へ特異的に結合する第1の細胞外ドメインを含む活性化キメラ抗原受容体(aCAR)ポリペプチドを含み、前記非多形性または多形性の細胞表面エピトープが、腫瘍関連抗原であるか、または関連する異常なおよび正常な哺乳類組織の細胞によって共有されており、
(b)第2のメンバーが、LOHにより、異常な哺乳類組織によって発現しないが関連する哺乳類正常組織の全ての細胞の上に存在する多形細胞表面エピトープの単一対立遺伝子変異体へ特異的に結合する第2の細胞外ドメインを含む調節ポリペプチドを含む、組み合わせ。 - 前記第1のメンバーが、
(a)エフェクター免疫細胞を活性化させおよび/または共刺激する少なくとも1つのシグナル伝達要素を含む細胞内ドメインをさらに含む構成aCAR、および
(b)ヘテロ二量体化小分子のための結合部位の第1のメンバーを含む細胞内ドメインと、場合により少なくとも1つの共刺激シグナル伝達要素とをさらに含むが、活性化シグナル伝達要素を欠く条件付きaCARより選択され、前記第2のメンバーが、
(c)エフェクター免疫細胞を抑制する少なくとも1つのシグナル伝達要素を含む細胞内ドメインをさらに含む抑制性キメラ抗原受容体(iCAR)、または
(d)シェダーゼのための基質を含む細胞外調節領域と、膜内切断プロテアーゼのための基質を含む膜貫通正準モチーフと、細胞内ドメインと、をさらに含む保護的キメラ抗原受容体(pCAR)であって、前記細胞内ドメインが、エフェクター免疫細胞およびヘテロ二量体化小分子のための結合部位の第2のメンバーを活性化させおよび/または共刺激する少なくとも1つのシグナル伝達要素を含む、pCARより選択される、請求項34に記載の組み合わせ。 - (i)前記iCARまたはpCARの前記細胞外ドメインが、前記aCARの前記細胞外ドメインが結合する抗原とは異なる抗原である、前記抗原の多形細胞表面エピトープの単一の対立遺伝子変異体に特異的に結合する、(ii)前記pCARまたはiCARの前記細胞外ドメインが、前記aCARの前記細胞外ドメインが結合するのと同じ抗原の異なる多形細胞表面エピトープの単一の対立遺伝子変異体に特異的に結合する、あるいは
(iii)前記pCARまたはiCARの前記細胞外ドメインが、前記aCARの前記細胞外ドメインが結合するのと同じ多形細胞表面エピトープの異なる単一の対立遺伝子変異体に特異的に結合する、請求項34または35に記載の組み合わせ。 - 前記シェダーゼのための基質が、ディスインテグリンおよびメタロプロテイナーゼ(ADAM)またはベータ−セクレターゼ1(BACE1)のための基質である、請求項34に記載の組み合わせ。
- 前記基質が前記細胞外ドメインの一部を形成し、Lin12/Notch反復とADAMプロテアーゼ切断部位とを含む、請求項37に記載の組み合わせ。
- 前記膜内切断プロテアーゼの基質が、SP2、γセクレターゼ、シグナルペプチドペプチダーゼ(spp)、spp様プロテアーゼまたはロンボイドプロテアーゼのための基質である、請求項34に記載の組み合わせ。
- 前記基質が、前記膜貫通正準モチーフの一部を形成し、ノッチ、ErbB4、E−カドヘリン、N−カドヘリン、エフリン−B2、アミロイド前駆体タンパク質またはCD44の膜貫通ドメインと相同である/に由来する、請求項39に記載の組み合わせ。
- 前記条件付きaCARの細胞外ドメインおよび細胞内ドメインを別々のタンパク質としてコードするヌクレオチド配列を含み、各ドメインが独立して膜貫通正準モチーフに融合し、ヘテロ二量体化小分子に対する結合部位の異なるメンバーを含む、請求項34に記載の組み合わせ。
- ヘテロ二量体化小分子のための前記結合部位の前記第1および第2のメンバーのそれぞれが、
(i)タクロリムス(FK506)結合タンパク質(FKBP)およびFKBP、
(ii)FKBPおよびカルシニューリン触媒サブユニットA(CnA)、
(iii)FKBPおよびシクロフィリン、
(iv)FKBPおよびFKBP−ラパマイシン関連タンパク質(FRB)、
(v)ジャイレースB(GyrB)およびGyrB、
(vi)ジヒドロ葉酸還元酵素(DHFR)およびDHFR、
(vii)DmrBホモ二量体化ドメイン(DmrB)およびDmrB、
(viii)PYLタンパク質(別名アブシジン酸受容体およびRCARとして)およびABI、
(ix)GAIシロイヌナズナ(Arabidopsis thaliana)タンパク質(別名ジベレリン酸非感受性およびDELLAタンパク質GAI( GAI))およびGID1シロイヌナズナ(Arabidopsis thaliana)タンパク質(ジベレリン受容体GID1としても既知)(GID1)より選択されるタンパク質に由来する、請求項34に記載の組み合わせ。
