本発明の実施を目的とした小胞の生産のために、非限定例として以下のプロトコルが使用される:
心細胞の小胞の産生のためには、Barileらの文献[44]に記載のプロトコル。
間葉系幹細胞の小胞の産生のためには、Laiらの文献[46]に記載のプロトコル。
神経幹細胞の小胞の産生のためには、Baulchらの文献[47]に記載のプロトコル。
本実施例では、本発明のバイオマテリアルの作製時に選択された高分子内の小胞の統合または包含を検証するために、以下で詳述される2つの小胞標識法、着色剤による標識およびカルセイン−AM(AM=アセトキシメチル)による標識、のうちの一方または他方を使用する。
得られた細胞外小胞の全サブタイプの膜を一律に標識するため、疎水性着色剤、DiD
Vybrant細胞トレーサー(商標)(参照:V−22887、Vybrant(商標)Cell−Labeling Solutions、モレキュラープローブス社)(以後「DiD」)を使用する。清澄化した条件培地(上記参照)を超遠心用チューブに移す。体積をPBSで15〜20mLにメスアップする。均質化された条件培地1mL当たり、1μLのDiD Vybrant細胞トレーサーを添加する。この条件培地を4℃で3時間、100,000gで超遠心する。ペレットをPBS中で再懸濁する。その体積を再度20mLにメスアップし、懸濁液を4℃で90分間、100,000gで再度超遠心して、遊離混入DiDを除去する。得られたペレットをPBS中で再懸濁する。このようにして標識された小胞は遠赤外の蛍光を発する。
Gray WDらの文献、「Identification of therapeutic covariant microRNA clusters in hypoxia−treated cardiac progenitor cell exosomes using systems biology」、Circ. Res. 2015 Jan16; 116(2):255−63[86]に記載されているプロトコルを用いる。カルセイン−AMの前駆物質は、細胞および細胞外小胞の脂質二重層を透過する非蛍光性の疎水性分子である。機能的なエステラーゼの存在下で、この前駆物質は切断され、蛍光性且つ親水性となり、細胞内部のシグナルを捕捉する。カルセイン−AM(カ
タログ番号C3100MP、ライフテクノロジーズ社(Life Technologie))はDMSO中に溶解し、最終濃度は1mMとする。iPS−Pgの培養から得られた条件培地を、37℃で1時間、2μMのカルセインAMと共にインキュベートする。この培地を100,000gで16時間超遠心する。標識された細胞外小胞のペレットをPBS中で再懸濁する。ペレットの洗浄のために、新たなPBSを17mL添加した後、懸濁液を再度100,000gで16時間超遠心する。内部移行実験用に、この洗浄された細胞外小胞のペレットをきれいなPBS中に再懸濁する。前記洗浄から得られた上清は、遊離カルセインでは起こり得る混入を確認するための対照として使用する。蛍光性カルセインで緑色に標識された細胞外小胞は、顕微鏡イメージングをフローサイトメトリーと組み合わせることを可能にする装置の一部である、ImageStream(ImageStream(登録商標)Mark II Image Flow Cytometer、メルクミリポア社)によって、可視化される。
3.a)フィブリンパッチの作製:
フィブリンパッチは、本実施例に開示された通りに作製された。原理的には、遠心分離後に得られた細胞外小胞(上記の実施例1を参照)をフィブリノーゲン水溶液中に再懸濁し、その後、この溶液をトロンビン溶液と混合する。フィブリン重合体の形成が行われ、その後、同時に、細胞外小胞が被覆および捕捉される。
ヒトフィブリノーゲン(50〜90mg/mlの溶液を2ml)
ヒトトロンビン(800〜1200U/mlの溶液を2ml)
フィブリノーゲン/トロンビン(F/T)の濃度を変えて、いくつかのパッチを作製した。本発明者らは、フィブリノーゲン/トロンビンにおける濃度が高いほど、パッチがより速く重合されることを観察した。この現象の主な効果は、パッチ内部のメッシュのサイズの変動にあり、重合が速いほど、細孔はより大きくなる。これは、添付の図4に示されているように、クライオスタットとヘマトキシリン/エオジン染色とにより、パッチの切断部で容易に確認できる。フィブリン線維は暗紫色に見える。

パッチの弾性も、本発明者らは、F/Tにおける濃度によるものであると考えた。AirExplorerR超音波システム(スーパーソニック・イマジン社(Supersonic Imagine))を用いて、ゲル内部の領域を選択した後、弾性を直接測定した。Shear Wave Explorerデバイスを用いて得られた結果は、添付の図6に示されるように、F/T濃度の増加に伴う、パッチの剛性の増加を示している。この図には、パッチの弾性(単位kPa)に対するF/Tにおける濃度の影響が示されている。心臓用パッチの場合、弾性の最適範囲は6〜15kPaであり、好ましい値は10kPa付近である。しかし、弾性は標的組織の性質に応じて調節されるべきである。
備品:
5%無菌アガロース
無菌PBS中の、実施例1により作製されたiPS−Pgの細胞外小胞、
無菌PBS中のフィブリノーゲン溶液(Evicel、オムリックス・バイオファーマシューティカルズ社、ベルギー)
無菌PBS中のトロンビン溶液(Evicel、オムリックス・バイオファーマシューティカルズ社、ベルギー)
α−MEM培地(参照:22571020 GIBCO、ライフテクノロジーズ社)
24ウェルプレート(「P24」)(参照:009224、TPP、ダッチャー社(Dutscher))
1:5%アガロースを調製し、オートクレーブ滅菌する(134℃での液体用プロトコル)。
2:まだ液体のこのアガロース溶液をP24ウェル内に入れ、ウェルの底を被覆し、20分間硬化させる。このアガロースは、フィブリンパッチがウェルの底に付着することを防ぐ。
3:所望のパッチの物理的特性を持たせるために、下記の表に従って、フィブリノーゲンにおける濃度およびトロンビンにおける濃度を選択する。
4:所望のフィブリノーゲン濃度を有し、20×106個の細胞外小胞と混合される、P24サイズのパッチ当たりの、150μLの最終体積の、溶液Aを、PBSまたはα−M
EM中で調製する。
5:所望のトロンビンにおける濃度を有する、150μL中の最終体積の、P24サイズのパッチ当たりの、溶液Bを、PBSまたはα−MEM中で調製する。
6:適当なホモジナイズと泡の発生回避のために、ピペットで前後運動させて、150μLの溶液Aと150μLの溶液Bとを十分に混合する。
7:37℃で20〜25分間インキュベートする。
8:α−MEM培地またはPBS培地を添加してパッチを覆い、乾燥を防ぐ。24ウェルプレートの1ウェルのサイズのパッチを覆うのに約1mlの液体が必要である。
9:休ませるためにパッチを静置する、有利には4時間である。
細胞外小胞をカルセインAMで標識し、フィブリンパッチに包含させる。このパッチを次に、OCT(商標)(参照:TFM−5、MMフランス社(MM France))でコートし、液体窒素中で凍結し、クライオスタットで切断する。切断の厚さは7〜10μmの間で変動する。切片を蛍光顕微鏡法で観察する。切片を、40倍または63倍の倍率で撮影された写真の形態で、図7に示す。
しかし、「細胞外小胞パッチ」では、使用された種々の濃度における2つのパッチで、緑色の点の存在が分かる。これらの緑色点の存在は、カルセインで標識された細胞外小胞が存在することを意味している。これらの細胞外小胞はパッチ内部に存在している。
カルセイン−AMで標識された20×106個の細胞外小胞を含有する上記(表I)の種々のF/T濃度のパッチを、純粋α−MEM培地、すなわち、細胞外小胞のサイズの粒子を含有しない培地中で48時間インキュベートした。
このパッチは、Eckhouseら[24]に記載されているパッチであってよい。これは、特に、皮膚修復を目的とした製品Hyalomatrix(登録商標)であってもよいし、あるいは、Gutowski KAの文献[25]で提示された美容外科で使用されるフィラー製品であってもよい。上記のように、小胞含侵用にα−MEM培地またはPBS培地を使用することが好ましい。
パッチは、Polizzotiらの文献[15]に記載されているようなGelfoam(商標)などの、止血性スポンジであってもよいし、あるいは、Weiらの文献[16]に記載されるような膜のパッチであってもよい。
a)細胞外小胞の生物活性の解析−瘢痕形成試験(「創傷治癒アッセイ」):
ヒト臍帯血由来の血管内皮細胞(HUVECシングルドナー、参照:C−12200、プロモセル社(PromoCell)、ハイデルベルク、ドイツ)を、42,000細胞/cm2の密度で無コート96ウェルプレート上に播種し、内皮細胞用の完全培地(Endothelial Cell Growth Medium、参照:C−22210、プロモセル社、ハイデルベルク、ドイツ+Endothelial Cell Growth Medium Supplement Pack、参照:C−39210、プロモセル社)中、37℃、5%CO2で、コンフルエントになるまで(一晩)培養した。
心筋芽細胞の不死化ラット細胞株であるH9c2を、0.2%ゼラチンで予めコーティングされた6ウェルプレート上に、25,000細胞/cm2の密度で播種し、5%CO2、37℃で、以下の完全培地中で培養する:10%FBSおよび1%ペニシリン/ストレプトマイシン/アンホテリシン(PSA)を含有するDMEM glutamax(参照:10566−016、ギブコ社、米国マサチューセッツ州ウォルサム)。24時間または48時間後、2つのウェルから得られる細胞を、Muse(商標)Cell Analyzerを用い、供給業者の説明書に従って、カウントする(T0)。
生存率試験を利用して、カルセインAMで標識された細胞外小胞が内部に取り入れられるかどうかを確認した。上記のように、細胞外小胞を添加した、または添加していない、欠乏培地中で、H9c2を培養する。ここで、細胞外小胞はカルセインで予め標識されて
いるか、または標識されていない。20時間のインキュベーション後、細胞を回収しImageStreamで解析する。H9c2細胞内の蛍光の存在は、標識された細胞外小胞の内部移行を示している。添付の図11から分かるように、標識細胞外小胞と共にインキュベートされた細胞は蛍光性であるが、対照は蛍光性ではないことが観察された。
小胞を含有する、または含有しないパッチを、上記の実施例に記載のプロトコルに従って作製する。作製後、これらのパッチを、欠損心臓上に、縫合により配置もしくは固定するか、または、接着剤で接着させる。
本実施例では、高い吸収能を有する変性したコラーゲンまたはゼラチンである、Gelfoam(登録商標、ファイザー社)について考えた。
1.細胞外小胞の作製
ヒト臍帯内皮細胞(HUVEC;C−12200;プロモセル社))を、0.2%ゼラチンでコーティングされたフラスコ内で、5000細胞/cm2の密度で、補体(キットC−39210;プロモセル社)を添加した内皮細胞の完全培地(内皮細胞用増殖培地;C−22010;プロモセル社)中で、培養する。培地はD1およびD4に新しくする。細胞がコンフルエントになったら、完全培地を欠乏培地(サプリメント無しの培地)と交換する。2日後、この条件培地を1200gで6分間遠心して壊死細胞片を除去し、その後、細胞外小胞の単離に移る。
フィブリンを生成するために使用されたフィブリノーゲンおよびトロンビンは、EVICELキット(オムリックス・バイオファーマシューティカルズ−エチコン・バイオサージェリー社、ベルギー)から得られる。パッチ作製用に、フィブリノーゲントロンビン混合物を堆積する1時間前に、1mlの無菌アガロースを各ウェルの底に堆積することにより、24ウェルプレートを用意する。この工程により、後のパッチの回収が容易となるように、形成されたパッチは、底に粘着せずに、ウェルの壁面のみに付着する。
フィブリンパッチを、作製の4時間後(すなわちD0)および3日後(すなわちD3)に、ウェルから回収する。回収したパッチは、凍結保存液(OCT(登録商標))で満たしたウェル内に配置し、液体窒素中で徐々に凍結させ、その後−80℃で保存する。これらのパッチを、クライオスタットで10μm厚の切片に切断し、以下で凍結されることとなる顕微鏡用スライド上に配置する。これらのスライドをヘマトキシリン/エオジンで着色し、その後、スライドスキャナーで解析する。そのようにして得られた画像の解析は、NDPViewソフトウェアを用いて行った。
a.)蛍光顕微鏡:DiDで標識されたFBSの細胞外小胞を含有するパッチを回収し、D0およびD3に凍結する。クライオスタットで切断後、スライドをFluoroshieldで封入し、Quicam CDDカメラ(キューイメージング社(Qimaging corp.)、サレー、ブリティッシュコロンビア州、カナダ)に接続されたLeica DM 2000蛍光顕微鏡(ライカ社、ウェッツラー、ドイツ)を用いて調べた。画像はMetamorphソフトウェアを用いて解析した。
a.)ナノ粒子トラッキング解析:細胞外小胞の作製および単離後、ナノ粒子トラッキング解析(NTA)を用いて細胞外小胞を定量化する。NTAまたはNanoSight(LM10レーザー488nm、マルバーン・インスツルメンツ社、モルバーン、英国)は、ナノ粒子を個別に追跡できる、ナノ粒子の解析法である。この技術は、動的な光の拡散およびブラウン運動の解析を介して、粒径当たりの、濃度および分布曲線を得ることを
可能にする。NTAは、密閉容器に含有される溶液中に懸濁された粒子によって反射された光を検出することを可能にする光学顕微鏡を含む。
DiDで標識されたFBSの細胞外小胞を21 109個含有するフィブリンパッチF5T1を、24ウェルプレート内に流し込んだ。1時間の重合の後、1mlのαMEM培地を添加した。このプレートを37℃の恒温器内で保管する。様々な制御用時間(control time)にパッチおよびその懸濁培地を回収し、−80℃で凍結した。
カルセインAMで標識されたHUVECの細胞外小胞を21 109個含有するフィブリンパッチF5T1を、24ウェルプレート内に流し込んだ。フィブリノーゲンにトロンビンを添加した1時間後に、1.5mlのαMEMを各ウェルに添加する。各時点において、2つの異なるスキームに従って培地を回収した:
培地の変更:培地を完全に、すなわち1.5mlを回収し、等量のαMEM培地に置き換える。
一定分量:追加の培地は添加しないで、各時点に一定分量の250μlを回収する。
供給業者のプロトコルに従いDダイマー用ELISAキット(DHDDIMER、Human D−DIMER ELISA KIT、サーモサイエンティフィック社)を用いたDダイマーの定量化により、フィブリンパッチの分解の定量化を行う。カルセイン標識アッセイのために、細胞外小胞が回収された培地の同じ試料に対して、Dダイマーをアッセイし、パッチからの細胞外小胞の放出特性をフィブリン分解と比較した。
この取扱い性試験の目的は、外科医が手術中に難なく使用できるパッチの最適組成を決定することである。最適なパッチとは、従来のピンセットで回収することが容易であり、ウェルの壁面から、裂けることなく容易に剥離し、剥離後に縮まず、且つ、手術中に組織上に配置された後に、または試験中に保存液中で、その初期形状に戻すことができる、パッチである。パッチの取扱いの比較用に、3つのパラメーターが選択された:
ピンセットでつかめること。
壁面から取り外した後に円形状の維持すること。
冷凍保存液中においてパッチが初期形状に復帰すること。
EMを用いて作製されたパッチが最良の結果となると結論付けられた。これらの特徴はこのバッチを最良な臨床使用のための候補とするものである。PBSを用いて作製されたパッチも良好な結果を示している。
フィブリンパッチは、約1ナノメートルのサイズを有する細胞外小胞を運搬することを目的とする。各作製条件で得られた種々の構造を比較するために、および、細胞外小胞の追加と協働し得る最適条件を決定するために、これらのパッチの微視的構造を研究した。様々なパッチを重合後の異なる時点で凍結した。クライオスタットで得られた切片を、ヘマトキシリン/エオジンで着色し、スライドスキャナーでデジタル化した。
本研究の目的は、液相中の高分子におけるその混合後に重合されたパッチ上の細胞外小胞の存在を明らかにすること、並びに、これらの粒子がフィブリンパッチ内部でどのように組織化されるのかを確認し、これらの結果が、必ずしも他のバイオマテリアルにそのまま外挿することはできないにしても、異なる高分子を用いた後の開発のために有用な参照ベース(reference base)を提供することを知ることであった。
局在し、パッチの細孔上にはまず観察されないことが好ましい。
細胞外小胞の放出実験ではF5T1 αMEMパッチを試験したが、これはこの濃度で得られるパッチがF20T4パッチほど厚くないためであり、この濃度選択によって、フィブリンネットワークの細胞外小胞の培地に向かってのより良好な拡散および特にインビトロ条件下でのパッチのより速い分解速度がもたらされ得る。
プロトコルに従って種々の培地を異なる時点で回収する。その培地を−80℃で保存した。解析の当日、培地吸引時にIS装置のキャピラリーを詰まらせ得る、フィブリンの分解により生じる大型デブリを除去するために、全ての培地を40μmフィルターで濾過した。
この実験用の陽性対照は以下である:
qEV DiD+:フィブリンパッチに包含されるDiDで標識された細胞外小胞に相当する量、すなわち、21.109個の、0.1μmで濾過された50μlのPBSに懸濁された、NTAで定量化された粒子(qEV NTA)、すなわち、4.2.108粒子/μlの濃度([EV]NTA)、を含有する試料。
PBS DiD+:細胞外小胞の標識に必要な量と等しい量のDiDで標識された、0.1μmで濾過されたPBSの試料。
αMEM DiD+:細胞外小胞の標識に必要な量と等しい量のDiDで標識された、αMEMの試料。
陰性対照は以下である:
qEV DiD−:21 109の非標識の細胞外小胞を含有する試料。
濾過PBS:0.1μmで濾過された50μlのPBSを含有する試料。
ISによる100%においての収率:([EV]IS/[EV]NTA)×qEV NTA
すなわち、(1.01×105/4.2×108)×21×109
ために必要と考えられたこの濾過工程は、フィルターによって除去されるフィブリンデブリに捕捉されたままとなり得る一定量の粒子の喪失の後に、細胞外小胞の定量化に偏りをもたらす可能性があった。DiDを含有するPBS培地およびαMEM培地のISによる解析は、このマーカーが、DiDで標識された細胞外小胞で観察されたものと比較可能な懸濁粒子の形態をとることを示しており、これが、ポジティブな現象の数を定量化する際に、DiDの粒子を培地中に放出された細胞外小胞としてカウントすることによって、第二の偏りをもたらす。この理由により、残りの細胞外小胞定量化実験用のマーカーの選択は、カルセインAMへと移った。
この実験では、いくつかの条件を、培地を回収する方法、培地を交換する方法、または一定分量を交換する方法に従って実験し、試料の保存温度は−80℃または37℃とした。パッチの培地交換には、培地へと向かうパッチの粒子の拡散現象を促進し、フィブリンの分解現象を促進する目的がある。37℃での試料保存は、内側に捕捉されたままとなり得る粒子の最大数が放出されるように、チューブ内に回収されたフィブリンデブリの分解現象の継続を可能にするために選ばれ、これにより、ISでの解析の前に、試料の濾過工程と関連付けられる細胞外小胞の定量化の偏りを克服することが可能となるだろう。
細胞外小胞の放出を定量化するためにISで解析された同じ培地を、Dダイマーの投与に用いた。Dダイマーはフィブリンの分解により生じる産物であるが、その投与を、パッチからの細胞外小胞の放出が実際に高分子の分解によるものであるかどうかを判定するための参照として用いる。一定分量だけ回収され37℃で保存された試料中を除く全ての条
件で、Dダイマーの投与はD3にピークを示す。なお、Dダイマーの割合はこの条件下では異なって変化し、D3においてピークは存在せず、長期間に渡りフィブリンの分解生成物中で徐々に増加する。この曲線と同じ条件下での細胞外小胞の投与との比較は、類似の変化プロファイルを示す。培地中の細胞外小胞の量はこのパッチの分解速度に準じており、試験された他の試料と比べて、この条件下において不十分な、D3における細胞外小胞の放出ピークの存在は、他のパッチよりも、この期間で不十分な分解レベルの、パッチのテクスチャに関連があると思われる。他の条件においては、Dダイマーの変化は互いに類似している。D3にこれらのパッチの分解の増加に対応する高い割合があり、その後、Dダイマーの量は減少する。これは、パッチがその重合および硬化の後に高次構造を変化させる時に該当し得る。
本実施例では、再灌流心筋梗塞のブタモデルが選択されるが、次のいくつかの理由による:これらの種の冠動脈の解剖学的形態、および、胸郭の形状によってヒツジよりも心エコー評価に適していること、および心臓サイズ/体重の比がヒトのそれに近いことである。プロトコルには、まず、冠動脈のネットワークの可視化のための血管形成術用カテーテルの大腿部への再導入(transfemorative introduction)と、その後の、左前下行枝(LAD)の中央部における血管形成術用バルーンの位置付けを可能にするガイドワイヤーの挿入とが含まれる。次に、蛍光透視で確認される、第二主対角枝(second major diagonal branch)の下流の、LADの遠位域の血流を断絶するために、バルーンを90分間かけて徐々に膨張させる。バルーンの収縮後、血管の透過性を確認するために、2回目の血管造影を行う。毎日のアミオダロンおよびβ遮断薬の経口による前投与、並びにアミオダロンの術中灌流により、不整脈が予防され得る。手術後、動物は一日量のアスピリンおよびクロピドグレルを与えられ得る。2つの主な臨床的状況を模倣するために、2通りの実験を行うべきである。

Claims (7)
- 幹細胞がヒト胚の破壊を含む方法由来ではない、心組織の治療を目的とする医薬として使用される、前記幹細胞に由来する細胞外小胞を包含する生体適合性生分解性高分子を含むバイオマテリアル。
- 前記幹細胞由来細胞外小胞が万能性幹細胞、多能性幹細胞またはその分化誘導体から選択される幹細胞に由来する、請求項1に記載のバイオマテリアル。
- 前記小胞が、心臓の血管細胞から選択される細胞由来の、万能性幹細胞、多能性幹細胞またはその分化誘導体に由来する、請求項2に記載のバイオマテリアル。
- 前記生体適合性生分解性高分子が、以下を含む群から選択される天然起源の高分子である、請求項1〜3のいずれか一項に記載のバイオマテリアル:フィブリン、キトサン、コラーゲン、アルギン酸、ヒアルロン酸およびその2種または数種の混合物;脱細胞化細胞マトリックス、または、脂肪族ポリエステル類、特に、ポリ乳酸、ポリグリコール酸、乳酸−グリコール酸共重合体、ポリリンゴ酸、ポリカプロラクトン、ポリグリセロールセバケート、ポリウレタンもしくはポリ(N−イソプロピルアクリルアミド)、これらの脂肪族ポリエステル類のうち2種または数種の混合物、脂肪族ポリエステル類の共重合体、またはこれらの共重合体の混合物から選択される合成高分子;または脱細胞化細胞外マトリックス。
- 前記生体適合性生分解性高分子がフィブリン、コラーゲンおよびヒアルロン酸を含む群から選択される天然起源の高分子である、請求項1〜4のいずれか一項に記載のバイオマテリアル。
- 前記生体適合性生分解性高分子が、幹細胞由来もしくはその分化誘導体由来の細胞外小胞を含む生体適合性液体培地中に浸漬する工程を含む方法によって得られる、または前記生体適合性生分解性高分子および/または対応する1種もしくは複数種の単量体と、幹細胞由来もしくはその分化誘導体由来の細胞外小胞、との混合によって得られる、請求項1〜5のいずれか一項に記載のバイオマテリアル。
- 前記生体適合性生分解性高分子を、幹細胞由来もしくはその分化誘導体由来の細胞外小胞を含む生体適合性液体培地中に浸漬する工程、または前記生体適合性生分解性高分子または対応する1種もしくは複数種の単量体と、幹細胞由来もしくはその分化誘導体由来の細胞外小胞との混合する工程を含む、請求項1〜6のいずれか一項に記載のバイオマテリアルのバイオマテリアル製造方法。
