抗体抗原複合体と免疫系の細胞との相互作用は、抗体依存性細胞傷害、脂肪細胞の脱顆粒及び食作用といったエフェクタ機能から、リンパ球増殖及び抗体分泌の調節といった免疫変調シグナルにまで及ぶ、幅広い応答をもたらす。全てのこれらの相互作用は、抗体又は免疫複合体のFcドメインが、造血細胞上の、特別な細胞表面受容体に結合することによって開始される。抗体及び免疫複合体によってトリガされる細胞応答の多様性は、Fc受容体の構造的異質性によるものである。Fc受容体は、構造的に関連するリガンド結合ドメインを共有し、これが恐らく、細胞内シグナリングを仲介する。
B細胞は、抗体の産生の役割を果たす免疫系細胞である。更にB細胞は、抗原を提示し、サイトカインを分泌する。抗原に対するB細胞の応答は、正常な免疫系に必須の構成要素である。B細胞は、特別な細胞表面受容体(B細胞受容体「BCR」)を有する。B細胞が、該細胞のBCRに結合できる抗原に出会うと、B細胞は刺激されて増殖し、結合した抗原に対して特異的な抗体を産生する。抗原に対する効率的な応答を生成するために、BCR関連タンパク質及びT細胞の支援も必要となる。抗原/BCR複合体は内在化され、抗原はタンパク質分解処理される。抗原のごく一部は、B細胞の表面上の主要組織適合性複合体II(MHC‐II)分子と複合体化したままとなり、ここで上記複合体をT細胞によって認識できる。このような抗原提示によって活性化されたT細胞は、CD40Lと、B細胞の成熟を誘導する様々なリンホカインとを分泌する。
炎症は、体内の白血球及び化学物質が、微生物及びウイルス等の外来物質による感染から我々の身体を保護するプロセスである。炎症は通常、患部の痛み、腫れ、発熱及び発赤を特徴とする。サイトカイン及びプロスタグランジンとして知られる化学物質はこのプロセスを制御し、規則正しくかつ自己制限的なカスケードで血液又は罹患組織中へと放出される。化学物質のこの放出は、傷害又は感染領域への血流を増加させ、発赤及び発熱をもたらす場合がある。一部の化学物質は、組織への体液の漏出を引き起こし、これは腫れをもたらす。この保護プロセスは神経を刺激し、痛みを引き起こす場合がある。これらの変化は、関連する領域において限られた期間しか発生しなければ、身体にとって有益に作用する。
A.二重特異性抗体
未修飾天然抗体(例えばIgG)の、抗原のエピトープに結合する能力は、上記抗体のエピトープ結合部位を形成するための、免疫グロブリン軽鎖及び重鎖上の可変ドメイン(即ちその軽鎖可変ドメイン(VLドメイン)及びその重鎖可変ドメイン(VHドメイン))の存在及び相互作用に左右される。単一種の軽鎖及び単一種の重鎖しか存在しないと、その結果として、天然抗体は1つのエピトープ種にしか結合できない(即ち上記天然抗体は単一特異性である)が、当該種の複数の複製に結合できる(即ち2価性又は多価性を示す)。
従来技術は、2つ以上の異なるエピトープ種に結合できる(即ち2価性又は多価性に加えて二重特異性又は多重特異性を示すことができる)点で天然抗体とは異なるダイアボディを産生できる可能性について記述している。ダイアボディの設計は単鎖Fv(scFv)構造をベースとし、これは、介在リンカーによって隔てられたVLドメイン及び対応するVHドメインを有し、上記介在リンカーは、これらのドメインを互いに相互作用させることができる。VL及びVHドメインの上記相互作用が、不十分な長さ(約12アミノ酸残基未満)のリンカーの使用によって不可能となった場合、2つのこのようなscFv構造が互いに相互作用することによって、一方の鎖のVLドメインが他方の鎖のVHドメインと結合する、2価ダイアボディ分子を形成できる(非特許文献31、32;特許文献59、60;非特許文献33〜35;特許文献61;非特許文献36〜39に概説されている)。

CD32B×CD79B二重特異性Fcダイアボディの生体内投与の評価
本発明のCD32B×CD79B結合分子の安全性及び耐容性を評価するために、実施例1のCD32B×CD79B Fcダイアボディを、年齢18〜50歳、BMI18〜30kg/m2の健康なヒト被験者に投与した。被験者は、妊婦又は出産の可能性がある女性を含まなかった。更に被験者は:重大な急性若しくは慢性の医学的疾患を有する個体;投薬の4週間以内にいずれの処方薬を使用した、若しくは投薬の1週間以内に市販薬を使用した個体;1日10本超喫煙する個体;結核、B型肝炎感染症、C型肝炎感染症若しくはHIV感染症を有する個体;自己免疫障害若しくは血管障害の既往歴を有する個体;又は薬物検査結果が陽性の個体を含まなかった。被験者は更に、450msec超の補正済み(QT)、45bpm未満若しくは120bpm超の心拍数、140mmHg超の収縮期血圧(SBP)、又は90mmHg超の拡張期血圧(DBP)を有する個体を含まなかった。本研究に関与した被験者のベースライン統計を表2に提示する。
図6は、ヒト被験者への投与時の、例示的なCD32B×CD79B結合分子の薬物動態を示す。結合分子の濃度は、検証済みのELISAアッセイで測定し、PKパラメータはノンコンパートメント分析によって算出した。図示されているように、被験者はCD32B×CD79B Fcダイアボディを0.01mg/kg体重、0.1mg/kg体重、0.3mg/kg体重、1mg/kg体重、3mg/kg体重又は10mg/kg体重の投薬量で投与され、投与後最大57日間にわたって、血清ダイアボディ濃度を測定された。ダイアボディの血清濃度、最大血清濃度(Cmax)及びAUC(曲線下面積)は全て、投薬濃度の上昇と共に上昇したことが分かった。ダイアボディの半減期は4〜8日であった。ダイアボディの迅速な排出が、最低用量(0.01mg/kg)で観察された。平均クリアランス時間(CL)は、1.426mL/h/kg(0.01mg/kg用量)〜0.350mL/h/kg(10mg/kg用量)であり、CLと用量との間の関係は非線形であり、用量の増大と共にCLは低下した。血液体積内のダイアボディの分布の指標である定常状態分布体積(VSS)は、用量とは無関係であることが分かった。血清半減期(T1/2)は、0.03mg/kg用量で投与されたダイアボディに関する92時間(〜4日)から、10mg/kg用量で投与されたダイアボディに関する191時間(〜8日)までであり、用量の増大と共に増大し、これはCLの用量依存性低下と一致していた。PKプロファイルの崩壊が記録され、これは抗薬物抗体(anti-drug-antibody:ADA)の存在を示唆していた。薬物動態データを表5にまとめる。
投与されたCD32B×CD79B結合分子は、末梢B細胞に結合できることが分かり、これは比較的高い用量レベルにおいて≧1mg/kg体重の最大占有度と、継続的な50%B細胞占有率とを示した。継続的な20%B細胞占有率が、≧0.3mg/kgにおいて観察された。図7は、研究過程全体にわたる、末梢B細胞に対する結合の生体外フローサイトメトリー分析をまとめたものである。
フローサイトメトリー分析を実施して、ダイアボディの投与がB細胞計数(図8A)、T細胞計数(図8B)、B細胞とT細胞との比率(図8C)、及びCD4+T細胞とCD8+T細胞との比率(図8D)の継続的な変化に関連するかどうかを評価した。図8A〜8Bに示すように、末梢T細胞の個数には継続的な変化が観察されず、本研究の比較的高い用量では、B細胞の個数の一時的な減少が観察された。図8C〜8Dに示すように、末梢B細胞の個数と末梢T細胞の個数との比率には、継続的な変化が観察されなかった。
生体外カルシウム動員アッセイを実施して、例示的なCD32B×CD79B結合分子のレシピエントのB細胞機能を評価した。簡潔に述べると、投薬前及び投薬後の様々な時点で回収した血液試料からPBMCを新たに単離し、BCR連結によってCa++フラックスを誘導した。イオノマイシンを導入することによって、最大Ca++フラックスを誘導した。AUC又はピークの値を、イオノマイシンによって生成された値で正規化することにより、比(AUC)=AUC(IgM)/AUC(イオノマイシン)とした。図9A〜9Bは、本研究で採用した実験手順及びデータ分析方法を示す。
例示的なCD32B×CD79B結合分子の投与の効果を更に評価するために、フローサイトメトリーを用いて、膜結合免疫グロブリンレベルを決定した。図10A〜10Cは、CD32B×CD79B結合分子の投与が、膜結合IgG(mIgG)(図10A)、膜結合IgM(mIgM)(図10B)、及び膜結合IgD(mIgD)(図10C)の発現によって決定されるように、CD27+メモリーB細胞上でのBCR発現を下方制御することを示している。
CD32B×CD79B結合分子の投与の、IgM、IgA及びIgG免疫グロブリンの血清レベルに対する影響を評価した。上記結合分子は、これらの免疫グロブリンの血清レベルを変調することが分かり、IgMレベルが最も大きな低下を示し、IgGレベルは概ね維持された(それぞれ図12A〜12C)。IgGは割愛することが望ましい。血清IgMレベルの低下は、形質芽細胞への影響を示唆している。これらの結果は、形質細胞上では発現されないCD79Bの発現と一致している。
CD32B×CD79B結合分子の投与は、末梢B細胞の表面共刺激分子のフローサイトメトリー分析によって決定されるように、B細胞上でのCD40表面発現レベル(図13A)を低下させることが分かった。
1.生体外BCR誘導型Ca++動員の低減;
2.CD27+メモリーB細胞サブセット上での表面IgG‐BCR発現の下方制御;
3.IgM+ナイーブB細胞の個数の低減
4.ナイーブB細胞上での表面IgD‐BCR発現の下方制御;及び
5.B細胞上でのCD40発現の下方制御。
1.BCR仲介型Ca2+流入の低減;
2.表面免疫グロブリン発現の下方制御;
3.血清IgMレベルの低減;及び
4.CD40発現の下方制御。
CD32B×CD79B結合分子の薬物動態及び薬力学的特性の調査
Emax(最大効果)PK/PDモデルを用いて、本発明のCD32B×CD79B結合分子の薬物動態(PK)及び薬力学(PD)的特性をより十分に調査した。簡潔に述べると、B細胞を様々な濃度の上記結合分子の存在下でインキュベートし、Emaxの50%、60%、80%及び90%の阻害をもたらす結合分子の濃度を決定した。
体液性免疫応答に対するCD32B×CD79B二重特異性Fcダイアボディの生体内投与の評価
本明細書中に記載されているように、本発明のCD32B×CD79B結合分子、特に二重特異性Fcダイアボディは、活性化‐阻害カップリングに基づく生理学的ネガティブフィードバックループをトリガすることによって、活性化されたB細胞を阻害するよう設計される。キーホールリンペットヘモシアニン(Keyhole Limpet Hemocyanin:KLH)等の免疫原によるワクチン接種に応答した体液性免疫応答に対するCD32B×CD79B結合分子の阻害効果を調査した。特に非ヒト霊長類からのPKデータの外挿に基づくと、0.3mg/kgで投与した場合、循環するCD32B×CD79B Fcダイアボディは、末梢血B細胞上のCD32B×CD79B Fcダイアボディ結合部位の20%の占有率を6日間持続させるようなレベルより高く維持される。インビトロ研究に基づくと、上述の占有率レベルは、BCR仲介型B細胞活性化の最低50%の阻害を少なくとも6日間持続させると予測される。ヒトにおけるKLHワクチン接種に対する免疫応答におよそ4〜6日かかることを考慮すると、≧0.3mg/mlの用量でCD32B×CD79B結合分子を被験者に投与した場合のKLHワクチン接種の実施は、KLH抗原によるワクチン接種に対するB細胞応答の阻害によって証明されるように、検出可能な薬力学的活性をもたらす。従ってKLHは、0.3mg/kg以上のCD32B×CD79B結合分子の用量を投与された全ての被験者に対して、2日目に1.0mgの皮下(SC)用量で投与してよい。更にワクチン接種前に各被験者に関して上記CD32B×CD79B結合分子の安全性及び耐容性を確立するために、KLHワクチン接種を、CD32B×CD79B結合分子の注入から24時間以上後に行ってよい。
体液性免疫応答に対するCD32B×CD79B二重特異性Fcダイアボディの生体内投与の評価
上述のように、体液性免疫応答に対するCD32B×CD79B結合分子の阻害活性を、ワクチン接種に応答して評価できる。不活性化A型肝炎ワクチン(HAV)は、B細胞枯渇療法(Van Der Kolk LE, et al. (2002) “Rituximab Treatment Results In Impaired Secondary Humoral Immune Responsiveness,” Blood 100:2257-9)を使用する患者間で、免疫不全状態における免疫応答を評価するために使用される、確立された新抗原である(Valdez H, et al. (2000) “Response To Immunization With Recall And Neoantigens After Prolonged Administration Of An Hiv-1 Protease Inhibitor-Containing Regimen,” ACTG 375 team. AIDS clinical trials group. AIDS 14:11-21)。この研究では、上述の例示的なCD32B×CD79B結合分子の阻害効果を、陰性血清A型肝炎力価を有する正常な健康な志願者におけるHAV投与に応答して評価する。簡潔に述べると、単一用量のCD32B×CD79B結合分子又はプラセボを、3mg/kg又は10mg/kgでIV注入によって投与し、被験者は2日目に、単一用量のHAV(VAQTA(登録商標)(A型肝炎ワクチン、不活性化、Merck、50U/1‐mL)も筋肉内投与される。単一用量のHAVに対する免疫応答に対する、CD32B×CD79B結合分子の投与の影響を、ワクチン接種された被験者における血清IgG特異的抗HAV特異性抗体の出現を監視することによって評価する。ARCHITECT HAVAb‐IgGアッセイ(Abbott Laboratories)を用いて、HAV特異性IgGの存在を検出した。ARCHITECT HAVAb‐IgGアッセイは、ヒト血清又は血漿中のHAVに対するIgG抗体の定性的検出のための、化学発光微粒子イムノアッセイ(chemiluminescent microparticle immunoassay:CMIA)である。修正されたAbbottのシステムを用いた定量的アッセイも採用した。
CD32B×CD79B結合分子は、CD40依存性B細胞応答をブロックする
上述のように、CD32B×CD79B結合分子の投与は、共刺激分子CD40のレベルを低下させる。CD40依存性応答に対する例示的なCD32B×CD79B結合分子の活性を、インビトロで評価した。これらの研究は、二次又は三次リンパ系器官の胚中心に発生する濾胞CD4ヘルパー細胞によって提供される刺激シグナルの存在下での抗体(例えばIgG)分泌細胞へのB細胞分化のプロセスを模倣するアッセイシステムを使用する。簡潔に述べると、ヒトB細胞を、陰性選択キットを用いて健康なドナーの末梢全血から精製した。精製されたヒトB細胞を、5%CO237℃インキュベーター内において、上述の例示的なCD32B×CD79B結合分子(20μg/mL)の存在下又は不在下で、希釈せずに若しくは3倍希釈を続けて(1、1/3、1/9及び1/27)使用される刺激因子(CD40リガンド(500ng/mL)、IL‐4(100ng/mL)及びIL‐21(20ng/mL))を用いて又は用いずに、完全RPMI1640培地中で5日間培養した。培養物上清を回収し、分泌されたヒトIgGをELISAで決定した。本研究の結果は、図19にプロットされている。

Claims (23)
- 治療的有効量のCD32B×CD79B結合分子を、治療を必要とする被験者に投与するステップを含む、炎症性疾患又は状態を治療する方法であって、前記CD32B×CD79B結合分子は、CD32Bのエピトープ及びCD79Bのエピトープに免疫特異的に結合でき、前記CD32B×CD79B結合分子は、約3mg/kg〜約30mg/kgの用量、及び1週間に1用量〜8週間に1用量の投薬レジメンで投与される、方法。
- 治療的有効量のCD32B×CD79B結合分子を、治療を必要とする被験者に投与するステップを含む、免疫応答を低下させる又は阻害する方法であって、前記CD32B×CD79B結合分子は、CD32Bのエピトープ及びCD79Bのエピトープに免疫特異的に結合でき、前記CD32B×CD79B結合分子は、約3mg/kg〜約30mg/kgの用量、及び1週間に1用量〜8週間に1用量の投薬レジメンで投与される、方法。
- 前記CD32B×CD79B結合分子は、約3mg/kgの用量で投与される、請求項1又は2に記載の方法。
- 前記CD32B×CD79B結合分子は、約10mg/kgの用量で投与される、請求項1又は2に記載の方法。
- 前記CD32B×CD79B結合分子は、約30mg/kgの用量で投与される、請求項1又は2に記載の方法。
- 前記投薬レジメンは、2週間に1用量(Q2W)である、請求項1〜5のいずれか1項に記載の方法。
- 前記投薬レジメンは、3週間に1用量(Q3W)である、請求項1〜5のいずれか1項に記載の方法。
- 前記投薬レジメンは、4週間に1用量(Q4W)である、請求項1〜5のいずれか1項に記載の方法。
- 前記CD32B×CD79B結合分子は、CD32Bのエピトープ及びCD79Bのエピトープに結合する二重特異性抗体、又は前記二重特異性抗体のCD32B及びCD79B結合ドメインを含む分子である、請求項1〜8のいずれか1項に記載の方法。

- 前記炎症性疾患又は状態は自己免疫疾患である、請求項1〜11のいずれか1項に記載の方法。
- 前記自己免疫疾患は:アジソン病;自己免疫性肝炎;自己免疫性内耳疾患;重症筋無力症;クローン病;皮膚筋炎;家族性腺腫性ポリポーシス;移植片対宿主病(GvHD);グレーヴス病;橋本甲状腺炎;エリテマトーデス;多発性硬化症(MS);悪性貧血;ライター症候群;関節リウマチ(RA);シェーグレン症候群;全身性エリテマトーデス(SLE);1型糖尿病;原発性血管炎(例えばリウマチ性多発筋痛症、巨細胞性動脈炎、ベーチェット病);天疱瘡;視神経脊髄炎;抗NMDA受容体脳炎;ギランバレー症候群;慢性炎症性脱髄性多発ニューロパチー(CIDP);グレーヴス眼症;IgG4関連疾患;特発性血小板減少性紫斑病(ITP);及び潰瘍性大腸炎からなる群から選択される、請求項12に記載の方法。
- 前記炎症性疾患又は状態は、GvHD、MS、RA又はSLEである、請求項13に記載の方法。
- 免疫グロブリンの血清レベルは、前記CD32B×CD79B結合分子の最初の用量の投与後36日目までに低下する、請求項1〜14のいずれか1項に記載の方法。
- 前記免疫グロブリンはIgM、IgA又はIgGである、請求項15に記載の方法。
- BCR仲介型末梢B細胞活性化は、前記CD32B×CD79B結合分子の最初の用量の投与後24時間までに低下し、前記B細胞活性化は、生体外カルシウム動員アッセイによって決定される、請求項1〜16のいずれか1項に記載の方法。
- 前記BCR仲介型B細胞活性化は少なくとも50%阻害され、前記阻害は少なくとも6日間持続する、請求項17に記載の方法。
- 末梢B細胞上の前記CD32B×CD79B結合部位の少なくとも20%が、前記CD32B×CD79B結合分子の最初の用量の投与後6時間占有される、請求項1〜18のいずれか1項に記載の方法。
- (A)B細胞上のCD40の発現が下方制御され;及び/又は
(B)CD40仲介型IgG分泌が阻害される、請求項1〜19のいずれか1項に記載の方法。 - 前記被験者はヒトである、請求項1〜20のいずれか1項に記載の方法。
- 前記CD32B×CD79B結合分子は:
(A)配列番号30のアミノ酸配列を含むVLCD32Bドメイン;
