Abstract
Description

希少細胞は、生物学的サンプルから抽出又は単離され得る。抽出された細胞は、他の細胞からの単離を伴わないで、液体サンプルから抽出された細胞である。単離された希少細胞は、液体サンプルに存在する他の種類の細胞から単離された希少細胞である。生物学的サンプルから抽出された又は単離された非希少細胞に対する希少細胞の割合は変化し、従って、抽出された又は単離された希少細胞の純度は可変であり得る。
本明細書に開示される方法は、生物学的サンプルから希少細胞を抽出するか、又は単離するための最も適切な手段として濾過を用いることによりそれらの問題や課題を解決する。濾過によるそれらの抽出又は単離の後、希少細胞は、複数の又はさらに同時分析方法のために適切な状態で存在する。この方法は、生物学的サンプルから希少細胞を効果的に単離するか又は抽出し、希少細胞を同定し、そして次に、診断目的のために、及び治療を選択し、ガイドし、モニターするために、及び特に、標的化された治療を選択し、そしてそれらに対する応答及び/又は耐性をモニターするために、希少細胞を分子的に特徴づける。
サンプル。生物学的サンプルは、静脈及び動脈の血液、リンパ液、尿、精液、腹水、脳脊髄液、胸膜液、唾液、痰、鼻液、関節液、涙液、尿道及び尿管からの液体、胆液、膵液、胃液、腸液、直腸液、膣液を非排他的に含んで成る生体液、粘膜及び器官、例えば口、喉頭、咽頭、子宮、子宮頸部、膣、食道、胃、小腸及び大腸の粘膜から非排他的に集められたサンプル、乳房、前立腺、肝臓、肺、骨髄及び他の器官からサンプルを非排他的に含んで成る、生検又は他の外科的介入により非排他的に集められたサンプルを、非排他的に含んで成る。
それは、非常に高い感度で希少細胞の単離、例えば1mlの血液において、濾過の前に、スパイクされ得、従って数百万の白血球及び数十億の赤血球と共に混合され得る1個の希少細胞の収集を可能にする。
−乳癌及び卵巣癌:ER、PR、BRCA1、BRAC2

−MTOR、MEK、STAT3、AKT、RET融合体

濾過。新鮮な腫瘍細胞を、公開された米国特許出願第2009−0226957号に記載のように、濾過により血液から抽出し、そして富化した。

肺癌。好ましい実施の形態によれば、希少細胞を、肺癌患者の血液から、濾過により単離し、腫瘍細胞を、細胞形態分析により、単離された希少細胞間で同定し、そしてALK特異的抗体及び分子プローブにより特徴づけることができる。
この作業の目的は、1)肺腺癌を有する87人の患者においてISETにより単離された、悪性細胞病理学的基準を有するCTCにおけるALK状態を評価することとして、及び2)CTC及びその対応する腫瘍組織に見出されるALK状態を比較することとして、下記に記載される。このためには、ALK−遺伝子再配列についての二重免疫化学及びFISHアプローチに基づくアッセイが使用され、そしてCTC及び対応する腫瘍組織サンプルの両者に適用された。
NSCLCのために手術を受ける患者のためのISET方法を用いてのこれまでの研究によれば、細胞形態悪性特徴を有するCTCを、208の症例のうち76(37%)で検出した。
ICC及びFISHを、6のスポットに対してMGG染色することにより検出される悪性特徴を有するCTCを含むその対応するフィルターの染色されていないスポット上でISET方法により単離されたCTCに対して行った[15]。フィルター当たり、2つのスポットをICCのために使用し、そして2つのスポットをFISHのために使用した。ICCのために、スポットを、室温で30分間、ALKタンパク質(1:50, 5A4; Abcam, Cambridge, UK)に対する一次抗体と共にインキュベートした。反応を、3,3′−ジアミノベンジジンにより可視化し、続いてヘマトキシリンにより対比染色した。細胞質染色は、ALKに対して陽性であると見なされた[30](補足データ)。複数のスポットに対して行われるFISHは、ALK遺伝子(Vysis LSI ALK Dual Color, Abbott Molecular, Abbott Park, IL)のためのブレーク−アパートプローブを、その製造業者の説明書に従って使用した。スプリットシグナル又は単独の3′シグナルを示す細胞は、ALK再配置に対して陽性であると見なされた[31]。フィルターを独立して試験し、そして臨床、IHC、ICCデータ及び組織遺伝子型を盲検した。我々は、手術の前、及び手術の7及び15日後、血液サンプリングを受けた34人の患者のCTCに対するALK検出についてのICC及びFISH結果の再現性を試験した。
陽性ALK免疫染色が、腫瘍にALK−再配置を有する患者に対応する5人の患者において単離されたCTCに見出された(図1A、A1及びB1、及び図1B)。それらの5人の患者の臨床病理学的データが、表3に詳述されている。
FISH分析を、ALK遺伝子についてブレーク−アパートプローブを用いて、腫瘍サンプルに対して実施した(Vysis LSI ALK Dual Color, Abbott Molecular, Abbott Park, IL)(補正データ)。スライドを、63倍の対物レンズを用いて落射蛍光顕微鏡(B×51、Olympus, Tokyo,Japan)上で読み取り(MI、EL、CB)、そしてイメージを、Soft Imagingシステム(Cell、Olympus)ソフトウェアを用いて分析した。結果は、臨床的及び免疫組織化学的データ及び遺伝子型について独立して盲検評価された。3人の病理学者の間での不一致が認められる場合、スライドは、コンセンサスを得るために再検討された。少なくとも50の解釈できる腫瘍細胞核が、各腫瘍について分析された。適切に解釈されるためには、腫瘍細胞核は、少なくとも1つの共局在シグナルを有すべきである。
ICCに関しては、熱誘導エピトープ回収を、ALK用標的回収溶液(pH9)(Dako, Carpinteria, CA)により実施した。スポットを、3%過酸化水素により20分間、処理し、内因性ペルオキシダーゼ活性を阻止し、続いて脱イオン水により2−4分間、洗浄した。次に、スポットを、ALKタンパク質(1:50, 5A4; Abcam, Cambridge, UK)に対する一次抗体と共に、室温で30分間、インキュベートした。反応を、3、3′−ジアミノベンジジンにより可視化し、続いてヘマトキシリンにより対比染色した。細胞質染色は、ALKに関して陽性であるとして見なされた[30]。染色の強さ及び陽性細胞の百分率を、上記のようにして、3人の病理学者(MI、VH及びPH)により、半定量的に評価した。フィルターを、独立して試験し、そして臨床、IHCデータ、及び組織及び細胞遺伝子型について盲検した。3人の病理学者間での不一致が認められる場合、スライドはコンセンサスを得るために再検討された。
「センチネル」(sentinel)循環腫瘍細胞は、慢性閉塞性肺疾患の患者における肺癌の早期診断を可能にする。
症例1
1995年、患者XB(男性、49歳)は、肺機能試験、50〜79%の間のFEV1(1秒での強制呼気量)及び胸部X線に基づいて、中程度(GOLD2)の慢性閉塞性肺疾患(COPD)と診断された。彼は45PY(パックイヤー:45年間、1日1パックのタバコ)、喫煙した。14年後(2009年10月)、彼は、ISETにより試験され、そして細胞病理学的分析により同定される10mlの血液に67CTC及び3CTM(循環腫瘍微小塞栓)を有することが見出された。同じ日に実施された低線量スパイラルCTスキャンは、COPDの診断を確認したが、しかし肺結節を示すことには失敗した。次に、CTスキャンを、その後、毎年企画し、そして1年後(2010年10月)、右下肺葉に直径1.5cmの肺結節の存在を、最初に示した。手術が1ヶ月後、行われた。病理学的分析及び癌の病期分類は、リンパ節又は遠隔転移への広がりを有さない菅状乳頭線癌ステージIAを示した(pT1aN0M0)。腫瘍の遺伝子型判定は、コドン12にK−Ras変異を示した。患者は何れの追加の治療も受けなかった。彼は、手術後9ヶ月でISETにより試験され、そしてCTCは彼の血液には見出されなかった。
患者AC(男性、54歳)は、肺機能試験、30〜49%のFEV1及び胸部X線に基づいて、1998年、重度(GOLD3)のCOPDと診断された。彼は60PY、喫煙した。11年後(2009年5月)、彼は、ISETにより試験され、そして細胞病理学的分析により同定される10mlの血液に43CTC及び1CTMを有することが見出された。同じ日に実施された低線量スパイラルCTスキャンは、COPDの診断を確認したが、しかし肺結節を示すことには失敗した。次に、CTスキャンを、毎年反復し、そして3年後(2012年9月)、左上肺葉に直径2.4cmの肺結節の存在を、最初に示した。患者は手術を1ヶ月後、受けた。病理学的分析及び癌の病期分類は、リンパ節又は遠隔転移への広がりを有さない菅状乳頭線癌ステージIAを示した(pT1aN0M0)。腫瘍DNAの分析は、コドン12にK−Ras変異を示した。患者は何れの追加の治療も受けなかった。彼は、手術後12ヶ月でISETにより試験され、そしてCTCは彼の血液には見出されなかった。

ISET方法は、8ミクロンの孔を有するポリカーボネート膜上で循環CTC及びCTMを富化するエンジン式血液濾過ベースのアプローチである[50、51]。末梢血液(10ml)を、緩衝されたEDTAに収集し、4℃で維持し、そして収集の1時間以内で処理した。膜上の7個のスポットを、免疫細胞化学のために処理し、そして3個のスポットを、細胞分析のためにMay Grunwald Giemsa (MGG)染色により処理した。免疫細胞化学を、室温で45分間、フィルターに適用される、パン−サイトケラチン抗体(マウス、クローンKL−1、Immunotech、Marseille)及び抗−ビメンチン(マウス、クローンV9、Dako、Paris)による二重免疫標識を用いて、前述のようにして実施した。
ISETを用いれば、168(1.8%)のCOPD患者のうち3人が、ISETによる単離、及び前に定義された基準に従って特徴的悪性特性を有する細胞の検出を可能にするMGG染色による単離された細胞の細胞病理学的分析に基づいて、CTCに関して陽性であることが見出された[51]。
肺癌は、高い侵襲性癌であることが知られており、そして75%以上の患者が診断での手術の対象とならない[75]。その高い割合及び侵襲性特性のために、それは世界中の癌関連死の主要原因である[53]。この分野では、診断及び非侵襲性バイオマーカーの発見が、低線量スパイラルCTスキャン検診及び早期の外科的介入の次の段階を示すことが重要である可能性がある。肺癌の高度な悪性挙動性が侵襲能力に結合されるので、CTCの高感度診断検出の使用がCTスキャン調査を補完し、そしてCTスキャン検診に関連する誤った陽性及び陰性結果の低減を助けると思われた。従って、発明者は、COPDを有する168人の患者集団を標的にした。COPDは、米国において死因の第3位であり、そして喫煙比率の上昇により、2030年までに世界の死亡原因の第4位になると予測される。COPDは、肺癌の新生物発生前の状態として見なされ、そして全体として、COPD患者の2.2%が、毎年肺癌を発症すると計算されている。しかしながら、COPDの進行は、肺発癌に対する感受性を4〜6倍まで高め、この観察は、COPD及び肺癌の両者における共有機構であると思われる。従って、DOPDの早期診断は、早期COPD疾患過程における禁煙が疾患の進行を遅くし、そして羅患率及び死亡率を低めるので、重要である[54]。
栄養膜。好ましい実施形態によれば、栄養膜細胞は、経子宮頸サンプルからの濾過により抽出され、細胞形態学的分析により同定され、そしてそれらのゲノムは、レーザー顕微解剖及び完全ゲノム増幅の後、PCRにより、それぞれ特徴づけられ得る。
現代の妊婦検診の主要目的は、胎児損失の1〜2%の危険性に結合される[55]、侵襲性出生前診断を、完全に安全な[非侵襲性]により置換することである。胎児DNAを、次の3種の源から非侵襲的に取得することができる:分娩又は流産後に血液中に保持されない、母体血液中の循環胎児細胞、特に循環赤血球及び栄養膜細胞;子宮膜から子宮頸部へのトランジットにおける経子宮頸栄養膜;及び母体血液中を循環する全細胞遊離DNAの一部である遊離胎児DNA。胎児細胞の非侵襲的回収は、純粋(母性DNAと混合されていない)胎児DNAを提供することが予測され、羊水穿刺及びCVSの非侵襲性且つ完全に信頼できる代替物の開発を可能にする。しかしながら、循環胎児細胞及び子宮頸栄養膜は、非常にまれであり、そしてそれらの単離は技術的挑戦である。細胞遊離胎児DNAを標的とする非常に強力な次世代配列決定アプローチは現在、利用でき、そして信頼でき、且つ非侵襲的出生前異数性の検出を提供することが証明されている[56,57,58,59,60]。しかしながら、それらの方法は、純粋な胎児DNAを標的としないので、羊水穿刺及びCVSを置換することはできない。さらに、それらのアプローチは、妊娠の早期で適用され得ず、そして洗練された高価な技術を必要とする。
サンプルの収集:
TCCサンプルを、絨毛サンプリングの直前、単一遺伝子疾患を有する胎児をキャリーする危険性のある妊婦(Hopital Necker-Enfants Malades)から、及び妊娠の選択的終了(TOP)を受ける女性(Antoine Beclere)から収集した。すべての女性は、7〜12週の妊娠期間であった。細胞は、従来のTCCサンプリング法とは異なり、細胞採取用ブラシを用いて得られ、我々の研究においては、ブラシは子宮頸管に挿入されないで、むしろ外部osで回転された。細胞採取用ブラシは、10mlのメタノール含有保存剤溶液に移された。
1mlの各サンプルを、1mlの1.1%アルシアンブルー8GX/3%氷酢酸溶液(Sigma-Aldrich, St. Louis, MO, USA)と共に混合し、そして30−50分間インキュベートした。次に、サンプルを、水により25倍に希釈し、全体積を50mlにした。各サンプルを十分に混合し、そして5分間インキュベートした。
ISETを、単なるわずかな変更を伴って、前述のようにして実施した[61]。手短には、希釈されたサンプル(50ml)を、較正された8μmの直径の円筒形の孔を有するポリカーボネートフィルターを通して濾過した。1mlのサンプルからの細胞を、フィルター上で10の直径0.6cmのスポット上に濃縮した。
フィルター上の濃縮された細胞中の核を染色するために、各スポットを、0.1%核ファーストレッド染色/5%硫酸アルミニウム溶液(Sigma-Aldrich, St. Louis, MO, USA)により被覆し、2分間インキュベートし、そして次に、水により十分にすすいだ。フィルターを空気下で乾燥した。
典型的には、サンプル当たり2つのフィルターを構成し、そして従って、平均2mlの各サンプルを処理した。アルシアンブルーにより染色されなかった、及び細胞栄養膜−様又は合胞体栄養細胞−様形態を示す単一細胞を、Nikon TE 2000-U (Nikon Paris, France and MMI Zurich, Switzerland)レーザー装備の顕微鏡を用いてレーザー捕獲顕微解剖によりフィルターから取得した。各単一細胞を、PCRに適した微量遠心管の蓋上に放した。
各顕微解剖された細胞を、15μlの溶解緩衝液(100mモル/lのトリス−HCl、pH8;400μg/mlのプロテイナーゼK)に、60℃で2時間、溶解し、続いて、プロテイナーゼKを94℃で15分間、不活性化した。プライマー伸長前増幅(PEP)[73]のために、溶解された細胞に、ランダムプライマー(Kit genPEP 75 OD, Genetix, Boston, USA)の400μM溶液5μl、6μlのPCR緩衝液(25 mMの MgCl2/ゼラチン(1 mg/mL)、100 mM のTris−HCl、 pH 8.3、500 mMの KCl)、4種のdNTP(それぞれ、2mMでの)の混合物3μl、及び1μl(5U)のTaqポリメラーゼ(Applied Biosystem, Foster City, CA, USA)を、60μlの最終体積で添加した。単一細胞遺伝子型判定を、父系及び母系ゲノムDNAの分析を通して有益であることが見出されたSTRプライマーを用いることにより実施し、胎児ゲノムを有する細胞を同定した。増幅を、6 μL のPEP生成物、10 mM のTris−HCl、 50 mMの KCl、 2.5 mM のMgCl2、 200 μM の各デオキシヌクレオチド、0.5 μM の各STR 「外部」プライマー及び 2 UのTaq Gold (Applied Biosystems, Foster City, CA, USA)を含む溶液60μlにおいて行った。10倍に希釈されたPCR外積2μlを、「内部」フルオレセイン化STRプライマー及び同じPCRプロトコルを用いて、20μlの最終体積で再増殖した。次に、1μlの20倍に希釈された内部PCR生成物を、13.5μlの脱イオン化されたHi−Diホルムアミド及び0.5μlのGenescan 400 HD (ROX)マーカー(Applied Biosystems)と共に混合し、そしてABI Prism 3100自動配列決定機 (Applied Biosystems)中に負荷した。プロファイルを、Genescan及びGenotypeソフトウェアプログラム(Applied Biosystems)を用いて分析した。

前記生物学的サンプルが、心血管性状態、障害又は疾患を有するか、あるいは心血管性状態、障害又は疾患を有する疑いがあるか、又は有する危険性がある対象から得られる;又は
前記生物学的サンプルが、感染状態、障害又は疾患を有するか、あるいは感染状態、障害又は疾患を有する疑いがあるか、又は有する危険性がある対象から得られる、請求項1〜3の何れか1項に記載の方法。 - (a)生物学的サンプルをポリカーボネートフィルターに通すことにより希少細胞を単離又は抽出し、そしてポリカーボネートフィルター上のその単離又は抽出された希少細胞を回収することを含んで成るか、又は
(a)生物学的サンプルをPET(ポリエチレンテレフタレート)フィルターに通すことにより希少細胞を単離又は抽出し、そしてPET(ポリエチレンテレフタレート)フィルター上のその抽出又は単離された希少細胞を回収することを含んで成る、請求項1〜5の何れか1項に記載の方法。 - 濾過により単離又は抽出された前記希少細胞が、(b)におけるように、又は培養のために、さらなる分析の前に支持体に移される、請求項1〜6の何れか1項に記載の方法。
- 前記単離又は抽出された希少細胞が、(b)の前に培養される、請求項1〜7の何れか1項に記載の方法。
- 前記単離又は抽出された希少細胞が、(b)の前に固定されるか;又は
前記単離又は抽出された希少細胞が、(b)の前にフィルター上で染色又は免疫染色される、請求項1〜8の何れか1項に記載の方法。 - (b)前記単離又は抽出された希少細胞の細胞形態を決定することを含んで成る、請求項1〜9の何れか1項に記載の方法。
- (b)前記単離された希少細胞を免疫標識することを含んで成る、請求項1〜10の何れか1項に記載の方法。
- (b)前記単離された希少細胞に対する分子分析を実施することを含んで成る、請求項1〜11の何れか1項に記載の方法。
- (b)前記単離又は抽出された希少細胞のDNA、RNA又はマイクロRNAの分子分析を含んで成る、請求項1〜12の何れか1項に記載の方法。
- (b)前記単離又は抽出された希少細胞のタンパク質、ペプチド又はポリペプチドの分子分析を含んで成る、請求項1〜13の何れか1項に記載の方法。
- 希少細胞の有無の検出方法であって、
(a)生物学的サンプルをフィルターに通し、そしてフィルター上のその単離された希少細胞を回収することにより希少細胞を単離か又は抽出し;ここで前記フィルターは、希少細胞を保持するが、しかし他の種類の細胞の通過を可能にする、孔サイズ、孔密度又は他の物理的特性を有し;
(b)任意には、前記単離又は抽出された希少細胞を培養し;
(c)任意には、前記単離又は抽出された希少細胞、又は任意には、培養された希少細胞を固定するか又は染色し;
(d)希少細胞DNA、RNA及び/又はマイクロRNAの免疫標識、及び/又はin situ分子分析、及び/又は分子分析により、及び/又は希少細胞タンパク質分子の分子分析により、(a)、(b)又は(c)からの単離又は抽出された希少細胞を分析することを含んで成る方法。 - 前記単離又は抽出された希少細胞が溶解され、そして(d)前記溶解された希少細胞におけるALK突然変異の有無を検出することを含んで成り、ここで前記工程がさらに、対象のための治療法を選択し、治療法の有効性を追跡し、又はALK突然変異の有無に基づいて、治療法に対する耐性を検出することを包含し;
前記単離又は抽出された希少細胞が溶解され、そして(d)前記溶解された希少細胞におけるK−RAS及び/又はEGFR突然変異の有無を検出することを含んで成り、ここで前記工程がさらに、対象のための治療法を選択し、治療法の有効性を追跡し、又はK−RAS及び/又はEGFR突然変異の有無に基づいて、治療法に対する耐性を検出することを包含し;
前記単離又は抽出された希少細胞が溶解され、そして(d)前記溶解された希少細胞におけるB−RAF及び/又はHER2突然変異の有無を検出することを含んで成り、ここで前記工程がさらに、対象のための治療法を選択し、治療法の有効性を追跡し、又はB−RAF及び/又はHER2突然変異の有無に基づいて、治療法に対する耐性を検出することを包含し;又は
(d)希少細胞の上皮から間葉への移行の状態を決定し;希少細胞が転移性又は侵襲性細胞に関連する遺伝子発現徴候を有するかどうかを決定するか;又は希少細胞が転移又は侵襲に関連する決定基を発現するかどうかを決定することにより、前記単離又は抽出された希少細胞を分析することを含んで成る、請求項15に記載の方法。 - 異なった時点で同じ対象から得られる生物学的サンプルを用いて、請求項15又は16に記載の方法を反復することを含んで成る個別化された医学療法であって、
前記生物学的サンプルが、希少細胞に関連する状態についての治療の間、又は希少細胞に関連する状態、障害又は疾患についての異なった治療レジメンの間、異なった点で、治療の前後、同じ患者から得られ;
前記個別化された医学治療がさらに、(e)治療レジメンの有効性を決定するためにか、又は治療レジメンに対する耐性を検出するために、異なった時点で得られたサンプル間の希少細胞の数を比較することを含んで成り、ここで検出された希少細胞の相対数の低下が、治療レジメンの相対的有効性を示し、そして検出される希少細胞の相対数の上昇が、治療レジメンに対する耐性又は治療レジメンの無効力を示し;そして任意には、
(f)(e)に基づいて対象についての効果的な個別化された標的治療法を選択することを含んで成る治療法。 - 希少細胞の分子特性に対する候補薬物又は候補治療の効果を評価し、そして薬物又は治療を付与しない対照と比較して、対象における希少細胞の数を減じる薬物又は治療を選択し、そして対照と比較して、希少細胞の相対数を減じるか、又は希少細胞の分子又は免疫学的特性を改変する薬物又は治療を選択することを含んで成る、請求項15又は16に記載の方法。
- 生物学的サンプルをフィルターに通して、そしてフィルター上の単離された希少細胞を回収することにより単離又は抽出された1又は2以上の希少細胞を含んで成る組成物であって、ここで前記フィルターは、希少細胞を保持するが、しかし他の種類の細胞の通過を可能にする、孔サイズ、孔密度又は他の物理的特性を有する、組成物。
- 体液から希少細胞を抽出又は単離するための1又は2以上のフィルター、
濾過の前に、体液を処理するための1又は2以上の緩衝液、希釈剤又は他の剤、
希少細胞が体液から抽出又は単離された後に、その希少細胞を懸濁し、洗浄し、又は他方では処理するための1又は2以上の緩衝液、

希少細胞を免疫標識するための1又は2以上の抗体又は他の試薬、又はそのための緩衝液、
フィルター又は他の支持体上の希少細胞のin situ分析のための1又は2以上の試薬、
希少細胞を溶解するための1又は2以上の溶解剤又は溶解緩衝液、
希少細胞タンパク質の分子分析のための1又は2以上の抗体又は他の試薬、又はそのための緩衝液、
PCR包含をする、希少細胞核酸の分子分析のための1又は2以上のプローブ、プライマー、ヌクレオチド、酵素又は他の試薬、
