Description

b)前記ヒト血液サンプル中のhGDF-15の決定されたレベルに基づいて、前記生存の確率を予測する工程とを含み、前記ヒト血液サンプル中のhGDF-15のレベルの低減が、生存の確率の増大を示す方法。
2.工程b)が、工程a)において決定された前記hGDF-15のレベルを、hGDF-15閾値レベルと比較する工程を含み、工程a)において決定された前記hGDF-15のレベルの、前記hGDF-15閾値レベルとの比較に基づいて前記確率が予測され、前記hGDF-15閾値レベルと比較して低減される前記ヒト血液サンプル中のhGDF-15のレベルは、生存の確率が前記hGDF-15閾値レベルでの生存の確率又はそれを上回る生存の確率と比較して増大されることを示す、項目1に記載の方法。

GDF-15に対するモノクローナルマウスGDF-15抗体のエピトープマッピングは、抗原に由来する13マーのペプチドを有する直鎖状エピトープを全く示さなかった。この知見によれば、モノクローナルマウス抗体GDF-15が、低い親和性の部分的エピトープを有するコンフォメーショナル又は不連続エピトープを認識する可能性が極めて高い。二次抗体のみのバックグラウンド染色を上回る任意のGDF-15シグナルが明らかにないことにより、PepSlide(登録商標)分析器を用いるスポット輝度の定量化及びそれに続くペプチドアノテーションを省略した。
抗体B1-23と結合する組換えヒトGDF-15のエピトープは、エピトープ切り出し法及びエピトープ抽出法によって同定された(Suckauら Proc Natl Acad Sci U S A. 1990年12月;87(24):9848〜9852頁;R.Stefanescuら、Eur.J.Mass Spectrom. 13、69〜75頁(2007年))。
組換えヒトGDF-15を、トリプシンを用いて37℃(溶液中)で2時間消化し、タンパク質中のトリプシン切断部位に従って種々のペプチドが得られた。完全に消化した後、ペプチドを、固定された抗体B1-23を含有する親和性カラム上にロードした。GDF-15の、結合していないペプチド並びに結合している可能性があるペプチドを、質量分析解析に使用した。質量分析によるペプチドの同定は可能ではなかった。これは、免疫複合体B1-23中のGDF-15の結合領域が、不連続又はコンフォメーショナルエピトープを含むという更なる指標であった。連続直鎖状エピトープの場合には、消化されたペプチドはエピトープペプチド中にトリプシン切断部位がない限り、その相互作用パートナーと結合するはずである。不連続又はコンフォメーショナルエピトープは、以下の部分に記載されるエピトープ切り出し法によって確認され得る。
次いで、親和性カラム上に固定された抗体B1-23を、組換えGDF-15とともに2時間インキュベートした。次いで、親和性カラム上に形成された免疫複合体を、トリプシンとともに37℃で2時間インキュベートした。切断は、組換えGDF-15に由来する種々のペプチドをもたらした。固定された抗体自体は、タンパク質分解的に安定である。抗体によって遮蔽され、このように、タンパク質分解による切断から保護された消化されたGDF-15タンパク質の得られたペプチドを酸性条件下(TFA、pH2)で溶出し、集め、質量分析によって同定した。

患者、材料及び方法患者 組織学的に確認された黒色腫を有する、Department of Dematology、University of Tubingen、Germanyからの患者を、ドイツ中の60を超える皮膚科学センターからの患者を将来を見越して記録する中央悪性黒色腫レジストリー(Central Malignant Melanoma Registry)(CMMR)データベースにおいて同定した。(a)2008年1月から2012年2月の間に採取された記録保存された血清サンプルを有する、(b)フォローアップデータを入手可能な、(c)採血の時点での局所領域又は遠隔転移の病歴又は存在を有する761人の患者を選択した。CMMRによるデータ収集の目的及び方法は、これまでに詳細に公開されている(Lasithiotakis,KGら、Cancer/107/1331〜9頁 2006年)。各患者について得られたデータは、年齢、性別、最後のフォローアップの日付及び該当する場合には死亡の日付及び原因を含んでいた。すべての患者は、CMMRレジストリーによって記録される臨床データを有するための所与の書面でのインフォームドコンセントを行っていた。施設内倫理委員会Tubingenは研究を承認した(倫理的表明125/2015BO2)。血清サンプリングの時点で、年齢、遠隔転移のパターン(ステージIV患者のみ)、AJCC分類(Balch, CMら、J Clin Oncol / 27 / 6199〜206頁 2009年)に従うサブステージ(IIIA対IIIB対IIIC;ステージIII患者のみ)、血清LDH及び血清S100B(Elecsys S100電気化学発光イムノアッセイ;Roche Diagnostics AG社, Rotkreuz, Switzerland)を評価した。hGDF-15血清濃度は、市販のELISAキットを製造業者の使用説明書に従って使用して2連で定量化した(R&D systems社, Wiesbaden, Germany):

1.プレート調製:
a.捕捉抗体を、PBSで2μg/mlの作業濃度に希釈した。96ウェルマイクロプレート(Nunc maxisorp(登録商標))を、外側の列(A及びH)を除いて、ウェルあたり50μlの希釈捕捉抗体を用いて直ちにコーティングした。列A及びHを、実験の間のサンプルの蒸発を防ぐためにバッファーで満たした。プレートを穏やかに軽くたたいて、各ウェルの底が完全に覆われることを確実にした。プレートを湿潤チャンバー中に入れ、室温(RT)で一晩インキュベートした。
b.各ウェルを吸引し、PBS-Tween(0.05%)を用いて3回洗浄した。
c.各ウェルに150μlのブロッキング溶液を添加し、続いて、RTで1時間インキュベートした。

a.各サンプル/GDF-15標準希釈を二連で適用した。GDF-15力価を調べるために、2連の平均を算出し、バックグラウンド(GDF-15を含まないサンプル)を差し引いた。
b.標準曲線を作製するために、線形範囲から得た値をX-Y-図(X軸:GDF-15濃度、Y軸:OD450)にプロットし、線形曲線フィットを適用した。既知濃度を有する標準希釈のOD450値から補間することによって、試験サンプルのGDF-15血清力価を算出した。
c.サンプルの最終GDF-15濃度を算出するために、それぞれの希釈係数を考慮した。標準範囲を下回る又は上回るOD値をもたらすサンプルを、適当な希釈で再解析した。
生存解析のためのフォローアップ時間は、血液サンプリングの日付から最後のフォローアップ又は死亡までと定義した。カプラン・マイヤーに従う累積生存確率を、95%の信頼区間(CI)と一緒に算出し、両側ログ・ランク検定統計学を使用して比較した。OSの解析のために、最後のフォローアップで生存していた患者を打ち切り、死亡していた患者を「イベント」と考えた。OSに対するsGDF-15の影響を解析するために、患者を、1:2の比を使用して2つのコホートに無作為に割り当てた(それぞれ同定及び検証コホート)。同定コホートでは、種々のカットオフ点を適用してsGDF-15に従って患者を、各々患者の≧25%を含む2つのバランスのとれたグループにカテゴリー化した。各カットオフ点について、高い対低いsGDF-15を有する患者間のOSの相違を解析し、以前に公開された最適化アルゴリズム(Camp, RLら、Clin Cancer Res / 10 /7252〜9頁 2004年)と同様に、最低のログランクp値をもたらすものを選択した。その後、同定コホートにおいて定義されたような最適カットオフ点を、507人の患者の検証コホートにおいて解析した。
ステージIII患者の全コホートのコックス回帰解析を実施して、その他の予後因子と比較したsGDF-15の相対的影響を決定した(Table 3(表4))。第1のモデルでは、直接比較を可能にするために3種のバイオマーカーsGDF-15、sS100B及びsLDHを含めた。単変量解析が、サブステージIIICに対してIIIA/Bのより良好なOSへの傾向を示したので、結果をサブステージについて調整した(p=0.088)。多変量解析では、上昇したsGDF-15に加えて(HR2.3;p<0.001)、上昇したsS100Bが、悪いOSと強力に関連しており(図5A)、予後に対して独立した負の影響を有していた(HR3.2;p<0.001)。両マーカーが上昇していた1年生存率の56.2%とは強く対照的に、両バイオマーカーにおいて好適な結果を有する患者については、1年生存率は最高であり、97.4%であった。上昇したsS100B又は高いsGDF-15のいずれかを有する患者の1年後の生存の確率は、それぞれ、88.4%又は95.1%であった。第2のモデルでは、年齢及び性別を更に考慮した。ここで、sGDF15及びsS100Bに加えて、ステージIIIC及び年齢>63歳が、予後に対して独立した負の影響を有していた。それら4種の因子を考慮して好適ではない値の数は、生存と強く関連していた(図5)。ステージIII黒色腫について予測されるように、sLDHは、いずれのモデルでもアウトカムとの相関を示さなかった。
実施例1に記載される患者サンプルの代替評価 本発明に従う代替実施例として、既に実施例1に記載された同一患者サンプルを、以下に記載されるような代替法で評価した。

- hGDF-15閾値レベルが、1.5ng/mlである、請求項4に記載の方法。

- 工程a)が、前記ヒト血液サンプル中のS100Bのレベルを決定する工程を更に含み、
工程b)において、前記生存の確率がまた、決定された前記ヒト血液サンプル中のS100Bのレベルに基づいても予測され、前記ヒト血液サンプル中のS100Bのレベルの低減が生存の確率の増大を示す、請求項1から8のいずれか一項に記載の方法。 - 工程b)が、工程a)において決定された前記S100BのレベルをS100B閾値レベルと比較する工程を含み、前記確率が、工程a)において決定された前記S100Bのレベルの前記S100B閾値レベルとの比較に基づいて予測され、前記S100B閾値レベルと比較して低減されるか又は前記S100B閾値レベルである前記ヒト血液サンプル中のS100Bのレベルは、生存の確率が、前記S100B閾値レベルと比較して増大されるか又は前記S100B閾値レベルを上回ることを示す、請求項9に記載の方法。
- 工程b)において、前記生存の確率がまた、前記患者の年齢に基づいても予測され、年齢の増大が生存の確率の低減を示す、請求項1から10のいずれか一項に記載の方法。
- 工程b)が、前記患者の年齢を閾値年齢と比較する工程を含み、前記確率が、前記患者の年齢の前記閾値年齢との比較に基づいて予測され、前記閾値年齢と比較して同等であるか又は増大される前記患者の年齢は、生存の確率が、前記閾値年齢での生存の確率と比較して低減されるか又は前記閾値年齢での生存の確率を下回ることを示す、請求項11に記載の方法。
- 前記閾値年齢が、60から65歳の範囲から選択される、請求項12に記載の方法。
- 前記閾値年齢が、63歳である、請求項13に記載の方法。

前記検出複合体が構造(A)-(B)-(C)-(D)を有し、ビオチン化された第1の抗体又はその抗原結合部分が第1のhGDF-15エピトープと結合し、ルテニウム錯体標識された第2の抗体又はその抗原結合部分が前記第1のhGDF-15エピトープとは異なる第2のhGDF-15エピトープと結合し、

- (i)ストレプトアビジンコーティングされたビーズ、
(ii)hGDF-15と結合可能な、ビオチン化された第1の抗体又はその抗原結合部分、
(iii)好ましくは、4〜7の範囲のpHを有する、組換えhGDF-15を含む緩衝溶液の形態の組換えhGDF-15、
(iv)hGDF-15と結合可能な、ルテニウム錯体標識された第2の抗体又はその抗原結合部分、及び任意選択で、
(v)使用のための説明書、好ましくは、請求項1から32のいずれか一項に記載の方法における使用のための説明書、及び好ましくは、
(vi)前記組換えhGDF-15を包含する容器であって、組換えhGDF-15と接触する容器の表面が非接着材料でコーティングされている容器
を含む検出キットであって、
ビオチン化された第1の抗体又はその抗原結合部分が第1のhGDF-15エピトープと結合可能であり、ルテニウム錯体標識された第2の抗体又はその抗原結合部分が前記第1のhGDF-15エピトープとは異なる第2のhGDF-15エピトープと結合可能である、検出キット。 - hGDF-15と結合可能な第1の抗体又はその抗原結合部分、及びhGDF-15と結合可能な第2の抗体又はその抗原結合部分の一方が、請求項26から28のいずれか一項に定義される抗体又はその抗原結合部分である、請求項35に記載の検出キット。
- ヒト黒色腫患者の生存の確率の予測のためのin vitro方法における、請求項35又は36に記載の検出キットの使用。
