Abstract
Description
図1は、例示的な外科用ステープル留め及び切断器具(10)を表し、この器具は、ハンドル組立体(20)と、シャフト組立体(30)と、エンドエフェクタ(40)と、を含む。エンドエフェクタ(40)及びシャフト組立体(30)の遠位部分は、外科的処置を行うために、図1に描写されるような非関節運動状態で、トロカールカニューレを通って患者内の手術部位まで挿入するように寸法決めされている。単なる例示として、患者の腹部内に、患者の2本の肋骨の間に、又はその他の部位に、かかるトロカールを挿入してもよい。一部の状況では、器具(10)は、トロカールなしで使用される。例えば、エンドエフェクタ(40)及びシャフト組立体の遠位部分(30)を、開胸術又は他の種類の切開によって直接挿入することができる。本明細書では、「近位」及び「遠位」といった用語は、器具(10)のハンドル組立体(20)を握っている臨床医を基準として使用されていることを理解されたい。したがって、エンドエフェクタ(40)は、より近位にあるハンドル組立体(20)に対して遠位にある。便宜上、また説明を明確にするため、本明細書では「垂直」及び「水平」といった空間的な用語が、図面に対して使用されている点も更に認識されるであろう。しかしながら、外科器具は、多くの配向及び位置で使用されるものであり、これらの用語は、限定的かつ絶対的なものであることを意図するものではない。

図3〜図5にも示されているように、本例のエンドエフェクタ(40)は、下側ジョー(50)及び枢動可能なアンビル(60)を含む。アンビル(60)は、下側ジョー(50)の対応する湾曲スロット(54)に位置付けられている一対の一体的な、外側に延在するピン(66)を含む。アンビル(60)は、開放位置(図2及び4に示す)と閉鎖位置(図1、3、及び7A〜7Bに示す)との間で、下側ジョー(50)に向かって、及びそれから離れるように、枢動可能である。「枢動可能」という用語(及び「枢動」を基体とした類義語)の使用は、必ずしも固定軸線を中心とした枢動運動を必要とすると理解されるべきではない。例えば、本例において、アンビル(60)は、ピン(66)により画定される軸線を中心に枢動し、このピンは、アンビル(60)が下側ジョー(50)に向かって動くと、下側ジョー(50)の湾曲スロット(54)に沿って摺動する。かかる形態では、枢動軸線がスロット(54)によって画定された経路に沿って並進する一方で、アンビル(60)はその軸線を中心として同時に枢動する。追加的にあるいは代替的に、まず枢動軸線がスロット(54)に沿って摺動し、次いで枢動軸線がスロット(54)に沿ってある一定の距離を摺動した後に、アンビル(60)が枢動軸線を中心として枢動してもよい。そのような摺動/並進枢動運動は、「枢動」、「枢動する」「枢動の」、「枢動可能な」、「枢動している」などの用語内に包含されることを理解されたい。当然のことながら、一部の形態は、固定されたままである、かつスロット又はチャネルなどの内側を並進しない、軸線を中心としたアンビル(60)の枢動運動を提供してもよい。
本例において、アンビル(60)は、閉鎖用リング(36)をエンドエフェクタ(40)に対して遠位側に前進させることによって、下側ジョー(50)に向かって駆動される。閉鎖用リング(36)は、カム動作を介してアンビル(60)と協働し、エンドエフェクタ(40)に対する閉鎖用リング(36)の遠位への並進に反応してアンビル(60)を下側ジョー(50)に向かって駆動する。同様に、閉鎖用リング(36)は、アンビル(60)と協働し、エンドエフェクタ(40)に対する閉鎖用リング(36)の近位側への並進に反応してアンビル(60)を下側ジョー(50)から離れて開放することができる。単に例として、閉鎖リング(36)とアンビル(60)とは、2014年8月28日に公開された「Jaw Closure Feature for End Effector of Surgical Instrument」と題する米国特許出願公開第2014/0239036号の教示の少なくとも一部に従って、及び/又はその開示内容が参照により本明細書に組み込まれる、2014年6月25日に出願された「Jaw Opening Feature for Surgical Stapler」と題する米国特許出願第14/314,108号の教示のうちの少なくとも一部に従って相互作用してもよい。エンドエフェクタ(40)に対して閉鎖用リング(36)を長手方向に並進するのに使用できる例示の機構は、以下により詳細に記載される。
本発明の例では、器具(10)は、発射ビーム(82)の電動制御を提供する。特に、器具(10)は、発射トリガー(26)のピストル把持部(22)に向かう枢動に応答して発射ビーム(82)を遠位に駆動するように構成された、電動構成要素を備えている。一部の形態では、モーター(図示せず)がピストル把持部(22)内に含まれ、電池パック(28)から電力を受信する。このモーターは、モーターの駆動シャフトの回転運動を、発射ビーム(82)の線形移動に変換する伝送組立体(図示せず)に連結される。一部のこのような形態では、アンビル(60)が下側ジョー(50)に対して完全閉鎖位置にあるとき、発射ビーム(82)は遠位に前進しかできない。図7A〜7Bを参照して上述されるように、発射ビーム(82)が遠位に前進して組織を切断し、ステープル(77)を駆動した後、発射ビーム(82)用の駆動組立体は、自動的に反転し、発射ビーム(82)を近位に後退位置に戻すように駆動できる(例えば、図7Bに示される位置から図7Aに示される位置まで戻す)。あるいは、オペレータは、発射ビーム(82)用の駆動組立体を反転して発射ビーム(82)を近位位置に後退できる、発射ビーム反転スイッチ(27)を作動してよい。本例のハンドル組立体(20)は、機械的脱出をもたらすように操作可能であり、オペレータが手動で発射ビーム(82)を近位に後退させることを可能にする(例えば、発射ビーム(82)が遠位位置にある間に電力損失した場合など)、脱出機構(21)を更に備えている。
上記の外科用器具(10)が患者内の組織をステープル留めし及びそれを切断するために使用され得るエンドエフェクタ(40)の一例を提供する一方、人体が、患者の体全体にわたる領域にアクセスすることが時々難しく異なって位置する様々な組織から成ることを認識されるであろう。例えば、肝臓は、肝臓のあらゆる箇所を通る血管又は導管を含む組織を含む。肝臓が腫瘍を含む設定では、腫瘍を含有する肝臓の一部を切除することが望ましい場合がある。切除は、解剖学的(例えば、肝臓の右側若しくは左側(その側の葉を含む)の切除)又は非解剖学的(例えば、肝組織の単なる単一の葉又はくさび状のものの切除)であり得る。この切除は、以下の少なくとも3種類のステップを伴う。(第1のステップ:血管又は導管の周囲の組織(例えば、肝実質)を解剖することによって、血管又は導管を分離する又は見せる;第2のステップ:その血管又は導管を結紮する;第3のステップ:結紮された血管又は導管を切断する。)
図9〜図13は、上側圧搾表面(414)、下側圧搾表面(416)、ステープルカートリッジ(418)、及びナイフ部材(419)を有するエンドエフェクタ(412)を伴う、外科用器具(410)を示す。上記に述べたように、組織を圧搾することによって、組織を切断するように構成される圧搾表面(414,416)を有するエンドエフェクタ(412)を伴う外科用器具(410)を提供する一方で、また、組織を通る1つ以上の導管を選択的に結紮する隣接するステープルカートリッジ(418)を提供することが望ましい場合がある。追加的に、ナイフ部材(419)は、周囲組織の完全除去のために1つ以上の血管を切断するように構成される。それによって、外科用器具(410)は、オペレータが組織をより速く評価することと、更なる組織切断及び/又は組織結紮を続けることとを可能にするであろう。本例の外科用器具(410)は、また、上記により詳細に説明された、ハンドル組立体(20)及びシャフト組立体(30)を含む。下記に別に説明されるものを除いて、エンドエフェクタ(412)は、ハンドル組立体(20)及びシャフト組立体(30)と連結して、エンドエフェクタ(40)と同様に構成され、操作することができる(図1参照)。ほんの一例として、エンドエフェクタ(412)は、約40mmの長さ及び約7mmの幅を有してもよい。あるいは、その他の任意の好適な寸法を用いてもよい。
図14〜図15に示されるように、下側ジョー(420)及び上側ジョー(422)はそれぞれ、アンビル(424)及びステープルカートリッジ(434)を通って延在する、細長いチャネル(439,441)を有する。より詳細に本明細書に説明されるように、細長いチャネル(439)は、患者の組織内のステープル(444)を選択的に形成するために、近位位置から遠位位置まで楔形スレッド(456)を押すように構成されている並進するナイフ部材(419)を受け入れる。追加的に、ナイフ部材(419)は、チャネル(441,439)からそれぞれ突出する上側のフランジ(470)及び下側のフランジ(472)を有する。フランジ(470,472)は、ナイフ部材(419)によって画定される垂直面から横方向に延在する。ナイフ部材(419)がエンドエフェクタ(412)を通って並進するにつれて、上側のフランジ(470)は、上側ジョー(422)の上部側で下向きに向かうように構成され、ナイフ部材(419)がエンドエフェクタ(412)を通って並進するにつれて、下側フランジ(472)は、下側ジョー(420)の底部側で上向きに向かうように構成されている。したがって、ナイフ部材(419)がエンドエフェクタ(412)を通って並進するにつれて、フランジ(470,472)は、ジョー(420,422)によって既にかけられた圧縮を高めるように協働し得る。
図16A〜図16Eは、エンドエフェクタ(412)を使用して肝実質組織(310)等の組織を切除し、組織内の血管又は導管(316)を結紮する一例を示す。上記に述べたように、血管又は導管(316)は、肝静脈又は肝動脈を備えてもよい。また、本方法は、更に、門脈及び肝外血管等の他の組織を結紮するためのエンドエフェクタ(412)の使用を含み得ることを理解されたい。

Claims (10)
- 患者の組織を処置するための外科用器具であって、
(a)シャフト組立体と、
(b)ジョー中心線に沿って前記シャフト組立体から延在するエンドエフェクタであって、
(i)第1のジョーであって、
(A)アンビルに接して押圧される複数のステープルを形成するように構成されているアンビルと、
(B)前記第1のジョーを通って延在する上側の細長いチャネルと、
(C)前記第1の細長いチャネルに接近して位置付けられた第1の複数のしるしと、を備える、第1のジョーと、
(ii)第2のジョーであって、前記第1及び前記第2のジョーが開放構成と閉鎖構成との間で移行するように構成され、前記第2のジョーは、
(A)前記第2のジョーを通って延在する第2の細長いチャネルと、
(B)前記第2の細長いチャネルに接近して位置付けられた第2の複数のしるしと、を備える、第2のジョーと、
