Abstract
Description
本出願は、2015年8月25日提出の米国仮特許出願第62/209351号及び2016年6月29日提出の第62/356337号(両者とも「人工静脈弁」と題する)に対する優先権を主張する。上記の両出願の全体が、あらゆる目的のために参照によって本明細書において援用される。
Claims (30)
- 静脈疾患の治療のために静脈に埋植するための人工静脈弁インプラントであって、
前記インプラントが、
管腔と、第1端部と、第2端部と、前記第1端部と第2端部との間の中間弁部分とを備える拡張可能な固定フレームであって、前記中間弁部分が、前記第1端部又は前記第2端部のいずれかの直径より小さい直径まで拡張する、拡張可能な固定フレームと、
前記第2端部によりも前記第1端部の近くにおいて前記固定フレームに取り付けられた弁座と、
前記第1端部によりも前記第2端部の近くにおいて前記固定フレームに取り付けられたボール保持部材と、
ボールであって、該ボールが、前記ボール保持部材に接触して前記インプラントを通過する下流方向への血液の前進流を許容する完全開放位置と、前記弁座に接触して前記インプラントを通過する上流方向への血液の逆流を防止する完全閉鎖位置との間で、前記中間弁部分内で前後に移動するように、前記固定フレームの前記管腔内において前記弁座と前記ボール保持部材との間に配置された、ボールと、
を備える、インプラント。 - 前記固定フレームが、管状のステント様格子構造を備え、前記インプラントが更に前記固定フレームの少なくとも一部分を覆って配置されたコーティングを含む、請求項1に記載のインプラント。
- 前記コーティングが、重合体、ヒアルロン酸、ヘパリン及び抗凝固剤から成るグループから選択された少なくとも1つの物質で作られる、請求項2に記載のインプラント。
- 前記固定フレームの前記第1端部又は前記第2端部の少なくとも一方が、前記固定フレームのすぐ隣の部分より広い直径まで拡張する、より広い拡張可能部分を含み、前記より広い拡張可能部分が、前記固定フレームが拡張したとき複数の移動防止先端を形成する、請求項2に記載のインプラント。
- 前記コーティングが前記移動防止先端以外の前記固定フレームの表面積全体を被覆する、請求項4に記載のインプラント。
- 更に、前記静脈内での前記インプラントの下流方向の移動を防止するために前記固定フレームに複数の移動防止逆とげを備える、請求項1に記載のインプラント。
- 前記固定フレームが、送達カテーテルを介した送達のための折畳み状態から前記送達カテーテルからの放出時の拡張状態へ自己拡張可能である、請求項1に記載のインプラント。
- 前記第1端部及び第2端部付近の前記固定フレームの部分が、前記インプラントが前記静脈の中に埋植されたとき前記静脈を拡張させるサイズを持つ、請求項1に記載のインプラント。
- 前記固定フレームの前記中間弁部分も、前記インプラントが前記静脈の中に埋植されたとき前記静脈を拡張させるサイズを持つ、請求項8に記載のインプラント。
- 前記中間弁部分が、実質的に直線的な管状形を持つ、請求項1に記載のインプラント。
- 前記中間弁部分が砂時計形を持つ、請求項1に記載のインプラント。
- 前記弁座が、前記固定フレームの外面又は前記固定フレームの内面の少なくとも一方に取り付けられた拡張可能かつ折畳み可能リングを備える、請求項1に記載のインプラント。
- 前記ボール保持部材が、前記固定部材の前記管腔を横切って延びる少なくとも1本の縫合糸を含む、請求項1に記載のインプラント。
- 前記ボール保持部材が、前記固定フレームの外面、前記固定フレームの内面又は前記弁座の少なくとも1つに取り付けられた少なくとも1つのU字形部材を含み、前記少なくとも1つのU字形部材が、前記固定フレームの前記管腔を横切って延びる、請求項1に記載のインプラント。
- 前記ボールがシェルとコアとを含む、請求項1に記載のインプラント。
- 前記シェルが第1の材料を含み、前記コアが第2の材料を含む、請求項15に記載のインプラント。
- 前記コアが磁気材料を含む、請求項15に記載のインプラント。
- 前記ボールがボール直径を有し、かつ前記弁座と前記ボール保持部材との間の距離が前記ボール直径の2倍〜4倍である、請求項1に記載のインプラント。
- 前記ボール直径が、前記ボールが前記弁座と前記ボール保持部材との間で前後に移動するとき前記ボールが前記中間弁部分の内面に接触するようなサイズを持ち、前記ボールと前記中間弁部分との間の接触が、前記中間弁部分の前記内面、前記弁座、前記ボール又は前記ボール保持部材の少なくとも1つに付着した物質を取り除くように構成される、請求項1に記載のインプラント。
- 前記ボールが、1.06グラム/立方センチメートル〜2.5グラム/立方センチメートルの密度を有する、請求項1に記載のインプラント。
- 前記ボールが、前記ボールの周りの血流を容易にするように構成された少なくとも1つの表面特徴を含み、前記表面特徴が、窪み、スリット及び溝から成るグループから選択される、請求項1に記載のインプラント。
- 前記弁座及び前記固定フレームの前記中間弁部分が、前記インプラントに付着した物質の取り除きを容易にするために、前記インプラントの外部から圧縮可能である、請求項1に記載のインプラント。
- 静脈を治療する方法であって、該方法が、
前記静脈の中へインプラント送達カテーテルを前進させるステップと、
前記送達装置の遠位端部から前記静脈の中へ人工静脈弁インプラントを前進させて、前記インプラントを拡張させ、前記静脈の内壁に前記インプラント自体を固定するステップであって、前記インプラントが、
管腔と、第1端部と、第2端部と、前記第1端部と第2端部との間の中間弁部分とを含む拡張可能な固定フレームであって、中間弁部分が前記第1端部又は前記第2端部のいずれかの直径より小さい直径まで拡張する、拡張可能な固定フレームと、
前記第2端部によりも前記第1端部の近くにおいて前記固定フレームに取り付けられた弁座と、
前記第1端部によりも前記第2端部の近くにおいて前記固定フレームに取り付けられたボール保持部材と、
ボールであって、該ボールが、前記ボール保持部材と接触して前記インプラントを通過する下流方向への血液の前進流を許容する完全開放位置と、前記弁座に接触して前記インプラントを通過する上流方向への血液の逆流を防止する完全閉鎖位置との間で、前記中間弁部分内において前後に移動するように、前記固定フレームの前記管腔内において前記弁座と前記保持部材との間に配置された、ボールと、
を含む、人工静脈弁インプラントを前進させて前記インプラントを拡張し、固定するステップと、
前記静脈を通過する血流を容易にするために前記静脈内の所定の場所に前記インプラントを残して、前記静脈から前記送達カテーテルを除去するステップと、
を含む、方法。 - 更に、少なくとも前記第1端部に隣接する前記固定フレームの第1部分及び前記第2端部に隣接する前記固定フレームの第2部分によって前記静脈を拡張させるステップを含む、請求項23に記載の方法。
- 更に、前記固定フレームの前記中間弁部分によって前記静脈を拡張させるステップを含む、請求項23に記載の方法。
- 更に、前記ボールが前記弁座と前記ボール保持部材との間で前後に移動するとき前記ボールが前記中間弁部分の前記内面に接触するように構成された直径を前記ボールが備えることによって、前記固定フレームの前記中間弁部分の内面、前記弁座、前記ボール又は前記ボール保持部材の少なくとも1つに付着した物質を取り除いて、前記固定部材を自己浄化するステップとを含む、請求項23に記載の方法。
- 更に、前記インプラントから障害物を吐き出すために前記インプラントに外部圧縮を加えるステップを含む、請求項23に記載の方法。
- 前記ボールが、磁気材料を含み、前記方法が、更に前記インプラントの外部で磁石を移動させて、前記インプラントから障害物を吐き出すために前記中間弁部分内で前記ボールを前後に移動させるステップを含む、請求項23に記載の方法。
- 更に、前記静脈の中に前記インプラントを残したまま、前記インプラントから前記ボールを除去するステップを含む、請求項23に記載の方法。
- 更に、前記静脈の中に前記インプラントを残したまま、前記インプラントから前記弁座を除去するステップを含む、請求項23に記載の方法。
