Description
Claims (43)
- 近位端部及び遠位端部を有する第1カテーテルであって、遠位部分が少なくとも1つの第1流出口を有する、第1カテーテルと、
近位端部及び遠位端部を有する第1チューブ状収容部であって、前記少なくとも1つの第1流出口より近位の前記第1カテーテルに連結された第1チューブ状収容部と、
前記第1チューブ状収容部の前記遠位端部に連結された1つ又は複数の圧力センサと、
近位端部及び遠位端部を有する第2カテーテルであって、遠位部分が少なくとも1つの第2流出口を有し、前記第2カテーテルが第1位置にあるとき、前記第2カテーテルの前記遠位端部が、
(i)前記第1カテーテル、又は
(ii)前記第1カテーテル及び前記第1チューブ状収容部のうちの1つ又は複数に連結された第2チューブ状収容部、のうちの1つの中に実質的に位置決めされるように構成された第2カテーテルと、を備える機器。 - 前記第1カテーテルの前記遠位端部が湾曲した区画を備える、請求項1に記載の機器。
- 前記湾曲した区画が約3cmから約500cmの範囲の半径を有する、請求項2に記載の機器。
- 前記第1カテーテルの前記遠位端部と前記第1チューブ状収容部の前記遠位端部との間の距離が約3cmから約40cmの範囲である、請求項1〜3のいずれか1項に記載の機器。
- 前記第1カテーテルの前記遠位端部に連結されたバルーンをさらに備える、請求項1〜4のいずれか1項に記載の機器。
- 前記第2カテーテルが前記第1位置にあるとき、前記第2カテーテルの前記遠位端部が実質的に前記第1カテーテル内に位置決めされる、請求項1〜5のいずれか1項に記載の機器。
- 前記第2カテーテルが、前記第1位置と第2位置との間で移動可能であり、前記第1カテーテルが、前記少なくとも1つの第1流出口より近位に配置された開口部を有し、前記第2位置で前記第2カテーテルの前記遠位端部が前記第1カテーテルの前記開口部を通って延在する、請求項6に記載の機器。
- 前記第1カテーテルの前記開口部がポートに連結される、請求項7に記載の機器。
- 前記第2カテーテルがカテーテルダイバータと接触し、それによって、前記第2位置で、前記第1カテーテルの前記開口部を通して方向付けられるように、前記第1カテーテルの内腔を少なくとも部分的に閉塞するように構成されたカテーテルダイバータをさらに備える、請求項7又は8に記載の機器。
- 前記第2チューブ状収容部が、前記少なくとも1つの第1流出口より近位に配置されるように、前記第2チューブ状収容部が、前記第1カテーテル及び前記第1チューブ状収容部のうちの1つ又は複数に連結され、前記第2カテーテルが第1位置にあるとき、前記第2カテーテルの前記遠位端部が実質的に前記第2チューブ状収容部内に配置される、請求項1〜5のいずれか1項に記載の機器。
- 前記第2カテーテルが、前記第1位置と第2位置との間で移動可能であり、前記第2位置で、前記第2カテーテルの前記遠位端部が、前記第2チューブ状収容部の前記遠位端部の開口部を通って延在する、請求項10に記載の機器。
- 前記1つ又は複数の圧力センサに接続され、前記第1チューブ状収容部120内に位置決めされた圧力トランスデューサラインをさらに備える、請求項1〜11のいずれか1項に記載の機器。
- 前記第1カテーテルが、約3フレンチから約15フレンチの範囲の内径を有する、請求項1〜12のいずれか1項に記載の機器。
- 前記第2カテーテルが、約1.5フレンチから約9フレンチの範囲の内径を有する、請求項1〜13のいずれか1項に記載の機器。
- 前記少なくとも1つの第1流出口が、約3cmから約40cmの範囲の長さを有する前記第1カテーテルの前記遠位部分に沿って形成され、前記少なくとも1つの第2流出口が、約3cmから約40cmの範囲の長さを有する前記第2カテーテルの前記遠位部分に沿って形成される、請求項1〜14のいずれか1項に記載の機器。
- 前記少なくとも1つの第1流出口及び前記少なくとも1つの第2流出口の中に配置された止血バルブをさらに備える、請求項1〜15のいずれか1項に記載の機器。
- 前記機器を脈管に固定する固定機構をさらに備える、請求項1〜16のいずれか1項に記載の機器。
- 前記固定機構が自己膨張構造を備える、請求項17に記載の機器。
- 前記固定機構が、脈管を穿刺するように構成されたコルクスクリュー機構を備える、請求項17に記載の機器。
- 前記第1カテーテル及び前記第2カテーテルのうち少なくとも1つに連結された第1ポンプと、
前記第1ポンプに連結された貯蔵部と、をさらに備える、請求項1〜19のいずれか1項に記載の機器。 - 前記貯蔵部内に位置決めされ、前記貯蔵部内に残る治療液の容積を判断するように構成されたセンサと、
前記判断された容積を計算装置へ送信するように構成された無線通信インタフェースと、をさらに備える、請求項20に記載の機器。 - 前記第1ポンプ及び前記貯蔵部の周囲に位置し、生体適合性の外表面を有する筐体をさらに備える、請求項20又は21に記載の機器。
- 前記第1ポンプに連結されたコントローラをさらに備え、
前記コントローラは、
1つ又は複数のセンサにより血圧が閾値より大きいことを判断し、
前記血圧が前記閾値より大きいという判断に応え、前記第1ポンプを駆動し、治療液を前記第1カテーテルの前記少なくとも1つの第1流出口及び前記第2カテーテルの前記少なくとも1つの第2流出口のうちの少なくとも1つから外へ前進させるように構成される、請求項20〜22のいずれか1項に記載の機器。 - 前記コントローラが、
前記1つ又は複数のセンサにより血圧が前記閾値未満であることを判断し、
前記血圧が前記閾値未満であるという判断に応え、前記第1ポンプを停止するようさらに構成される、請求項23に記載の機器。 - 前記コントローラが、
酸素飽和度又は心拍数のうちの1つ又は複数が閾値より大きいことを判断し、
前記酸素飽和度又は前記心拍数のどちらかが前記閾値より大きいという判断に応え、前記第1ポンプを駆動し、治療液を前記第1カテーテルの前記少なくとも1つの第1流出口及び前記第2カテーテルの前記少なくとも1つの第2流出口から外へ前進させるようさらに構成される、請求項23又は請求項24に記載の機器。 - 前記コントローラが、
1つ又は複数のセンサにより酸素飽和度又は心拍数を判断し、
前記酸素飽和度が閾値未満である、又は前記心拍数が閾値を超えることのどちらかという判断に応え、前記第1ポンプを停止するようさらに構成される、請求項25に記載の機器。 - 前記第1ポンプが前記第1カテーテルに連結され、
前記機器が前記第2カテーテルに連結された第2ポンプをさらに備え、
前記貯蔵部が前記第1ポンプ及び前記第2ポンプの両方に連結される、請求項20〜26のいずれか1項に記載の機器。 - 前記第1ポンプが前記第1カテーテルに連結され、
前記機器がさらに、前記第2カテーテルに連結された第2ポンプを備え、
第1貯蔵部が両方の前記第1ポンプに連結され、
第2貯蔵部が前記第2ポンプに連結される、請求項20〜26のいずれか1項に記載の機器。 - 前記第1貯蔵部が第1治療液を有し、前記第2貯蔵部が第2治療液を有する、請求項28に記載の機器。
- 前記第1貯蔵部及び前記第2貯蔵部が一意の識別子を持つ、請求項27又は28に記載の機器。
- 前記一意の識別子が、前記第1貯蔵部及び前記第2貯蔵部上のスタンプ又は放射線不透過マーカーのうちの1つである、請求項30に記載の機器。
- 前記一意の識別子が、前記第1貯蔵部及び前記第2貯蔵部の形状である、請求項30に記載の機器。
- 肺動脈内へ請求項1〜32のいずれか1項に記載の前記機器の遠位端部を導入することと、
前記機器の前記第2カテーテル内へガイドワイヤを導入することと、
前記第2カテーテルの前記遠位端部が前記第1チューブ状収容部の前記遠位端部を越えて延在するように、前記第2カテーテルを前記ガイドワイヤに沿って移動させることと、
前記第1カテーテルの前記少なくとも1つの第1流出口及び前記第2カテーテルの前記少なくとも1つの第2流出口から外に及び肺動脈内へ治療液を前進させることと、を含む方法。 - (a)前記機器を肺動脈内へ導入後、及び、(b)前記第1カテーテルの前記少なくとも1つの第1流出口及び前記第2カテーテルの前記少なくとも1つの第2流出口へ前記治療液を前進させた後、1つ又は複数の圧力センサにより血圧を監視することをさらに含む、請求項33に記載の方法。
- 前記第1カテーテルの前記遠位端部に連結されたバルーンを膨張させることと、
前記バルーンにより前記第1カテーテルの前記遠位端部を前進させ、下大静脈から右心室へ、肺動脈弁を通って肺動脈へ前進させることと、をさらに含む、請求項33又は34に記載の方法。 - 前記第1カテーテルの前記遠位端部を肺動脈の第1分岐内に位置決めすることと、
前記第2カテーテルの前記遠位端部を前記第1カテーテルのポートを通して前進させることと、
前記第2カテーテルの前記遠位端部を肺動脈の第2分岐内に位置決めすることと、をさらに含む、請求項33〜35のいずれか1項に記載の方法。 - 前記第1カテーテルの前記遠位端部を肺動脈の第1分岐内に位置決めすることと、
前記第2カテーテルの前記遠位端部を前記第2チューブ状収容部の前記遠位端部から外へ前進させることと、
前記第2カテーテルの前記遠位端部を肺動脈の第2分岐内に位置決めすることと、
をさらに含む、請求項33〜36のいずれか1項に記載の方法。 - 第1時間において1つ又は複数の圧力センサにより第1血圧を測定することと、
第2時間において1つ又は複数の圧力センサにより第2血圧を測定することと、
前記第1血圧を前記第2血圧と比較し血圧の変化を判断することと、をさらに含む、請求項33〜37のいずれか1項に記載の方法。 - 1つ又は複数の圧力センサにより血圧が閾値未満であることを判断することと、
血圧が前記閾値未満であるという判断に応え、前記機器を肺動脈から取り除くことと、をさらに含む、請求項33〜38のいずれか1項に記載の方法。 - ポンプを皮下に埋め込むことをさらに含み、前記ポンプは前記第1カテーテル及び前記第2カテーテルに連結されている、請求項33〜39のいずれか1項に記載の方法。
- 前記ポンプが臀部、大腿、又は腹部のうちの1つの中に埋め込まれるように構成される、請求項40に記載の方法。
- 1つ又は複数の圧力センサにより血圧が閾値より大きいことを判断することと、
前記血圧が閾値より大きいという判断に応え、ポンプを駆動し、前記治療液を前記第1カテーテルの前記第1少なくとも1つの流出口及び前記第2カテーテルの前記第2少なくとも1つの流出口から外へ前進させることと、をさらに含む、請求項33〜41のいずれか1項に記載の方法。 - 1つ又は複数の圧力センサにより血圧が閾値未満であることを判断することと、
血圧が閾値未満であるという判断に応えて、前記ポンプを停止することと、をさらに含む、請求項33〜42のいずれか1項に記載の方法。
