Abstract
Description
本出願は、2014年10月14日に出願された米国仮特許出願第62/063,793号の利益を請求し、その全体の内容が、参照によって本明細書に組み込まれる。
・血小板リッチ血漿は、良好な標準的創傷ケアと比較して、糖尿病性潰瘍の治癒を改善しなかった。
・3つの研究からのプールされた結果におけるDermagraft(登録商標)生物学的皮膚等価物は、標準的ケアと比較して有意でない改善を示すが、潰瘍治癒はDermagraftの方が良い(35%対24%)。
・Apligraf(登録商標)生物学的皮膚等価物二重層は、良好な標準的創傷ケアを超えて治癒を改善することが報告されている(55%対34%、p=.001;2研究)
・5、Regranex(登録商標)rhPDGF−BBは、プラセボまたは良好な標準的創傷ケアと比較して、治癒された潰瘍の百分率の改善を示した(58%対37%、p=0.04;7研究)。
・V.A.C.(登録商標)陰圧創傷治療は、良好な標準的創傷ケアよりも治癒を改善した(43%対29%、p<0.05;1研究)。
・全ての研究について、潰瘍治癒までの向上した時間に関連する低いまたは不十分な証拠が存在した。
さらに、4つの研究において、Regranexについての完全な創傷閉鎖の発生率は、50%またはそれ未満(48%、50%、44%および36%)と報告された。
rhPDGF−BBは、整形外科的および歯周適応症においても使用されており、ここでは、治癒環境および治癒プロセスは、真皮創傷とは非常に異なっている。2つのかかる製品には、共に活性成分としてrhPDGF−BBを含む、Augment Bone GraftおよびGEM21Sが含まれる。GEM21Sは、rhPDGF−BB溶液および粒状合成骨代用品からなり、2005年にFDA承認され、慢性歯周欠陥において骨治癒を改善することが示されている。Augment Bone Graftもまた、rhPDGF−BB溶液および粒状合成骨代用品からなる。Augmentは、米国およびカナダにおける434人の患者の中心的臨床試験に基づいて、手術の間の骨欠陥中への単回の移植後の足および足首の癒合において骨癒合を改善するとしてFDA承認されている。しかし、これらの製品のいずれも、DFUなどの真皮創傷を処置することに適応されておらず、両方とも、単回の手術内適用を介して、皮膚創傷治癒とは非常に異なる細胞性および生理学的プロセスである骨の成長および癒合を促進するために製品を使用することに焦点を当てている。Regranexと同様、GEM21SおよびAugment Bone Graft製品は、冷蔵(約2〜8℃)貯蔵されなければならず、このことは、使用者の利便性およびコンプライアンスを損なう。

この新規処置方法は、新規治療組成物を創傷表面に適用する前にそれを調製するステップをさらに含み得、この組成物は、PDGFと生物学的マトリックスとを含む。この組成物を調製する方法は、以下を含み得る:
(2a)凍結乾燥(フリーズドライ)無菌PDGF粉末を無菌の水、生理食塩水、緩衝液または生理的溶液で再構成して、特定の安全な治療濃度のPDGFを提供するステップ;および

III.創傷を処置するための新規治療組成物
配列番号1:
a)無菌rhPDGF−BB凍結乾燥粉末のバイアル、

このキットは、最大3年間にわたって室温で貯蔵され得る。一部の実施形態では、この貯蔵は、16℃と32℃との間である。一部の実施形態では、キット中に含まれる粉末は、所定の量のPDGFを含む。一部の実施形態では、PDGFの量は、本明細書で提供される値と一致する。一部の実施形態では、担体は、所定の量の担体を含むブリスターパック中に含まれる。一部の実施形態では、担体の量は、本明細書で提供される値と一致し、担体の型は、本明細書に記載される材料と一致する。

V.さらなる治療要素
0.3mg/mlの組換えヒト血小板由来増殖因子−BB(rhPDGF−BB)を含有するコラーゲン創傷包帯の効力を、レプチン受容体中の変異によって糖尿病にされたマウス(db/db)において外科的に誘導した全厚創傷の処置において評価した。
A.研究設計
群1−Regranexゲル0.01%rhPDGF−BBを、添付文書によって処方されたように、21日間毎日適用した;
群2−緩衝液と合わせたコラーゲン創傷包帯を、0日目、7日目および14日目に適用した;ならびに
群3−0.3mg/mlの組換えヒト血小板由来増殖因子−BB(rhPDGF−BB)を含有するコラーゲン創傷包帯を、0日目、7日目および14日目に適用した。
各動物は、個体番号に対応する耳パンチによって同定した。0日目に、平均出発重量を記録して、平均出発重量が群間で同等であることを確実にした。ケージカードを使用して、研究番号(LYN−01)、処置群番号および動物番号でマークした各ケージまたはラベルを同定した。

(1)Regranexの21回の適用を群1において施したが、緩衝液/コラーゲンまたはrhPDGF/コラーゲン創傷包帯の適用3回だけを、それぞれ群2および3において適用した。
(2)群1(Regranex)の3匹の動物は、死亡していることが見出されたか、または研究の生前部分の間に安楽死させなければならなかった。
(3)全ての処置群は、ノギス測定値およびImageJソフトウェア分析を使用した創傷トレースの両方によって決定される、0日目から21日目までの創傷面積における減少を示した。屠殺時(21日目)に、5つのRegranex処置創傷のうち2つ、5つのrhPDGF/コラーゲン処置創傷のうち3つ、および5つのコラーゲン包帯処置創傷のうち0個が治癒した。
(4)各処置からの生画像は、群3(rhPDGF/コラーゲン)が、緩衝液で処置したコラーゲン創傷包帯対照群(群2)と比較して、肉芽組織の形成および再上皮化において実証可能な加速を生じることを示している。さらに、Regranexで毎日処置した創傷(群1)もまた、対照コラーゲンスポンジ処置動物と比較して、より良い閉鎖速度を示した。
(5)創傷再上皮化によって評価される治癒は、Regranexの21回の適用(群1)または生理食塩水で湿らせたコラーゲン創傷包帯の3回の適用と比較して、rhPDGF/コラーゲンの3回の適用(群3)で処置した創傷において最大であった。
(6)rhPDGF/コラーゲンの3回の毎週適用(群3)は、コラーゲン創傷包帯(群2)と比較して、肉芽組織形成および再上皮化を含む創傷閉鎖を加速させ、21回の毎日用量のRegranexゲル(群1)と少なくとも同等に有効なようである。
(7)rhPDGF/コラーゲンは、安全かつ有効であり、市販のコラーゲン創傷包帯と比較して、糖尿病性創傷のより良好な治癒を促進する。5つのrhPDGF/コラーゲン処置創傷のうち3つは、5匹のコラーゲン創傷包帯処置動物のうち0匹と比較して、完全な再上皮化によって明らかなように、完全に治癒した。
(8)rhPDGF/コラーゲンは、安全かつ有効であり、市販のコラーゲン創傷包帯と比較して、組織学的に実証されたように、血管形成、肉芽組織形成および再上皮化を促進する。
(9)rhPDGF/コラーゲン無菌製品は、組織学的に実証されたように、高度に生体適合性である。
(10)rhPDGF/コラーゲンは、Regranexゲルよりも適用がかなり容易であり、患者コンプライアンスを改善するはずである。
(11)Regranexを受けた動物は高い死亡率を有したが、かかる死亡率はrhPDGF/コラーゲンまたはコラーゲン創傷包帯では観察されなかったということを考慮すると、rhPDGF/コラーゲンはRegranexよりも安全であり得る。

Claims (194)
- 無菌rhPDGFと無菌多孔性マトリックスとを含む治療組成物であって、
(a)前記rhPDGFの少なくとも80%が非切断型rhPDGFであり、
(b)前記マトリックスと合わせる前に、前記rhPDGFが、少なくとも約6カ月間にわたって16〜32℃で貯蔵されたときにその生物活性の少なくとも80%を保持することが可能であるように、前記rhPDGFが生体安定性であり、
(c)前記マトリックスが、細胞付着および孔中への内殖を可能にする孔を含む、
治療組成物。 - 前記無菌rhPDGFが、約0.05mg/ml〜約5mg/mlの間のrhPDGF−BBを有するrhPDGF−BB溶液を含む、請求項1に記載の治療組成物。

- 前記包帯が、半密封包帯または密封包帯である、請求項48または51に記載の組成物。
- (3)約0.05mg/ml〜約5mg/mlの間のrhPDGF−BBを含むrhPDGF−BB溶液であって、
a)凍結乾燥rhPDGF−BBを含有する無菌粉末であって、重量基準で前記凍結乾燥rhPDGF−BBの少なくとも約80%が非切断型凍結乾燥rhPDGF−BBであり、前記粉末が、約16℃と約32℃との間で貯蔵されることが可能であり、少なくとも約1年間にわたって前記rhPDGF−BBの少なくとも80%の生物活性をなおも維持する、無菌粉末、および
b)無菌の水、生理食塩水、緩衝液または生理的溶液
を合わせ、それにより前記非切断型凍結乾燥rhPDGF−BBを溶液へと再構成することによって形成されるrhPDGF−BB溶液;ならびに
(4)生体適合性の吸収性担体
をさらに含み、前記rhPDGF−BB溶液対前記担体の比が、1cm3の担体当たり約4μlと1cm3の担体当たり約40mlとの間である、またはrhPDGF−BB対前記担体の比が、1cm3の担体当たり約1.2μgのPDGFと1cm3の担体当たり約12mgのPDGFとの間である、またはrhPDGF−BB対前記担体の比が、1cm3の担体当たり約1.2μgのPDGFと1cm3の担体当たり約12mgのPDGFとの間である、請求項48または51に記載の組成物。 - 前記担体が、天然ポリマー、合成ポリマーまたはそれらの組合せである、請求項85に記載の組成物。
- 前記担体が、コラーゲン、ゼラチン、フィブリン、アルギネート、セルロース、キトサンおよびフィブロネクチンからなる群から選択される天然ポリマーである、請求項85に記載の組成物。
- 前記担体が、ポリ(DL−ラクチド−co−グリコリド)(PLGA)、ポリ(DL−ラクチド)(PDLA)、ポリ(L−ラクチド)(PLLA)、ポリ(e−カプロラクトン)(PCL)およびポリウレタンからなる群から選択される合成ポリマーである、請求項85に記載の組成物。
- 前記担体がコラーゲンスポンジである、請求項85に記載の組成物。
- 前記担体が、約50μm〜約1mmの間の範囲の孔径を有する多孔性細胞足場を提供する、請求項85に記載の組成物。
- 前記担体が、約50ミクロン〜約500ミクロンの間の平均孔径を有する多孔性細胞足場を提供する、請求項85に記載の組成物。
- 前記担体が、約100ミクロン〜約300ミクロンの間の平均孔径を有する多孔性細胞足場を提供する、請求項85に記載の組成物。
- 前記担体の孔の大部分が相互接続されている、請求項90、91または92に記載の組成物。
- 前記rhPDGF−BB溶液が、約0.1mg/ml〜約1mg/mlの間のrhPDGF−BBを含む、請求項85に記載の組成物。
- 前記rhPDGF−BB溶液が、約0.2mg/ml〜約0.4mg/mlの間のrhPDGF−BBを含む、請求項85に記載の組成物。
- 前記rhPDGF−BB溶液が、約0.3mg/mlのrhPDGF−BBを含む、請求項85に記載の組成物。
- 前記rhPDGF−BB溶液が、約0.5mg/mlのrhPDGF−BBを含む、請求項85に記載の組成物。
- 前記rhPDGF−BB溶液が、約1.0mg/mlのrhPDGF−BBを含む、請求項85に記載の組成物。
- 重量基準で前記凍結乾燥rhPDGF−BBの少なくとも約85%が、非切断型凍結乾燥rhPDGF−BBである、請求項85に記載の組成物。
- 重量基準で前記凍結乾燥rhPDGF−BBの少なくとも約90%が、非切断型凍結乾燥rhPDGF−BBである、請求項85に記載の組成物。
- 重量基準で前記凍結乾燥rhPDGF−BBの少なくとも約95%が、非切断型凍結乾燥rhPDGF−BBである、請求項85に記載の組成物。
- 重量基準で前記凍結乾燥rhPDGF−BBの少なくとも約97%が、非切断型凍結乾燥rhPDGF−BBである、請求項85に記載の組成物。
- 前記治療組成物中に含まれる前記rhPDGF−BBが、配列番号1に対して少なくとも約90%の相同性を有するアミノ酸配列を含む、少なくとも約90%のrhPDGF−BBを含む、請求項85に記載の組成物。
- 前記治療組成物中に含まれる前記rhPDGF−BBが、配列番号1に対して少なくとも約90%の相同性を有するアミノ酸配列から本質的になる、少なくとも約80%のrhPDGF−BBから本質的になる、請求項85に記載の組成物。

- 前記rhPDGF−BB溶液対前記担体の比が、約0.1ml/cm3〜約1ml/cm3の間である、請求項85に記載の組成物。
- 前記rhPDGF−BB溶液対前記担体の比が、約0.5ml/cm3〜約1.5ml/cm3の間である、請求項85に記載の組成物。
- 前記rhPDGF−BB対前記担体の比が、約30μgのPDGF/cm3〜約9mgのPDGF/cm3の間である、請求項85に記載の組成物。
- 前記rhPDGF−BB対前記担体の比が、約60μgのPDGF/cm3〜約6mgのPDGF/cm3の間である、請求項85に記載の組成物。

- 前記rhPDGF−BB対前記担体の比が、1cm3の担体当たり約75μgのPDGFと1cm3の担体当たり約225μgのPDGFとの間である、請求項85に記載の組成物。
