Abstract
Description
樹状細胞(DC)は、免疫応答の中心的な役割を担う。プロフェッショナルな抗原提示細胞として、DCは、組織微小環境をサンプリングし、病原体由来の生成物および腫瘍細胞を含む死滅宿主細胞の両方を食菌する(Banchereau J、Steinman RM、Dendritic cells and the control of immunity. ヒト末梢血において、自然循環するDCの二つの主な集団が区別され得る;CD1cの発現およびCD19(CD1c+(BDCA−1)CD19−)の発現の欠如を特徴とする骨髄性DC(mDC)、並びにCD304(CD304+(BDCA−4))の発現を特徴とする形質細胞様DC(pDCs)である。これらのサブタイプは、機能、局在、および表現型が異なる。mDCは主にリンパ節の周辺ゾーンに移動するか、またはそこに存在し、細菌抗原および真菌抗原を認識し応答すると考えられている。一方、pDCは、主にリンパ節のT細胞領域に存在し、ウイルス抗原認識に特化しているようである(Liu YJ. −癌の処置または治療を必要とする患者(自己)の血液から樹状細胞(CD1c(BDCA−1)+/CD19/CD14細胞)を精製する。これにより、自系のmDCの細胞集団が得らる。この精製は、すなわち、以下の抗体を使用して行われる、すなわち
−CD1c+細胞を濃縮するためのCD1c(BDCA−1)に結合することができる少なくとも第一の抗体、
−Bリンパ球に結合できる少なくとも第2の抗体、および
−抑制性DCに結合することができる少なくとも第三の抗体。
第1に、癌治療を必要とする患者由来のアフェレーシス細胞を、所望の細胞を単離するために磁気的に標識する。特に、標識は、抗CD1c−ビオチン(すなわち、抗体がビオチンで標識されているCD1cに結合する抗体)、CD14マイクロビーズ(すなわち、CD14に結合することができる抗体、すなわち、磁気ビーズ、特に磁気的に選別可能なマイクロビーズ)、およびCD19マイクロビーズ(すなわち、CD19に結合することができる抗体で、抗体はマイクロビーズ、特に磁気的に分類可能なマイクロビーズに結合する)を含む。マーカーCD19は、上に説明したように、B細胞を枯渇させるために使用される。

細胞の単離:
異なるサブセットを単離するために、最初のPBMCを、フィコール−勾配(Lucron Bioproducts)を用いて健康なドナーまたはステージIIIまたはステージIVメラノーマ患者から分離した。BDCA1+DC単離キット(Miltenyi Biotec GmbH、Bergisch Gladbach、Germany)を用いてPBMCからBDCA1を発現する全集団(BDCA +DCおよびBDCA1+CD14+集団を含む)を単離し、続いて抗CD14マイクロビーズ(Miltenyi Biotec)により単球分離を行った。2つのBDCA1発現サブセットは、抗CD14−PerCP(BD Biosciences、San Jose、CA)を用いることにより、CD14発現に基づいて遅れて識別された。
CD1c(BDCA1)およびCD14の同時発現を特徴とする特異的骨髄性サブセットは、腫瘍と関連している
進行性腫瘍が循環中のBDCA1+DC数に影響を及ぼすかどうかを決定するために、この集団のパーセンテージをステージIII/IVメラノーマ患者および健常ドナーの末梢血で評価した。メラノーマ患者におけるBDCA1+DCのより低い傾向が観察されたが、それは有意ではなかった(図1A)。興味深いことに、単球マーカーCD14を共発現するBDCA1+CD11c+HLA−DRhi集団(ゲーティング手順の場合、図8も参照)は、健康なドナー(図1A)と比較してメラノーマ患者において有意に上昇した。このBDCA1+CD14+集団の観察された増加は、CD14+HLA−DRLow(ゲーティング手順の場合、図8も参照)として特徴付けられる循環骨髄由来サプレッサー細胞(MDSC)の割合の有意な増加と同時に生じた。BDCA1+CD14+集団の存在は、卵巣癌患者由来の炎症性腫瘍腹水においてもこの集団が検出されたので、循環に限定されない。事実、腹水材料は、BDCA1+DCと比較してBDCA1+CD14+集団の有意に高い含量を示した(図1B)。
BDCA1+CD14+集団は、単球およびBDCA1+DCの両方と特異的マーカーの発現を共有するので、この集団の特異性を決定するためにさらなる解析が必要であり、単なるDCまたは単球亜集団ではない。この疑問に対処するために、3人の健康なドナーの血液から単離したBDCA1+CD14+細胞、BDCA1+DC、単球およびMDSCのトリスコピートームをRNAシークエンシングによって比較した。5000個の最高発現遺伝子を用いた異なるサンプルの階層的クラスタリングは、BDCA1+CD14+細胞を特異的集団として明確に示す。この集団は多く範囲で共有していたが、BDCA1+CD14+集団は、他の集団(図2A)よりもBDCA1+DCに密接に関連していた。本発明者らは、BDCA1+CD14+細胞の起源および発生をより詳細に知るために、マウスにおける骨髄発生に関与することが記載されている転写因子(図2B)および成長因子受容体(図2C)の発現を評価した。マウスにおけるDCの発生重要なIRF4、BATF3およびFLT3のRNA発現は、BDCA1+DC(青色のバー、左から1番目)中において最も高かったが、単球(オレンジバー、左から3番目)およびMDSC(紫色のバー、左から4番目のバー)と比較した場合、BDCA1+CD14+(緑色棒グラフ、左から2番目)の細胞は、より高い発現を示した。DC発生のためのもう1つの重要な因子であるIRF8のRNA発現は、4つの集団にわたって均一であった。同様に、マウスおよびヒトにおけるDC系統に特異的な転写因子であるZBTB46およびZNF366(DC−SCRIPT)のRNA発現は、BDCA1+DCに続いてBDCA1+CD14+細胞が最も高かった。さらに、DC24で特異的に発現することが明らかにされた長い非コードRNALOC645638(Inc−DC)のほとんどがBDCA1+DCによって独占的に発現されていた。対照的に、BDCA1+CD14+細胞、単球およびMDSCは、MAFB、EGR1およびEGR2、マウスのマクロファージ分化に関与するすべての転写因子のRNA発現に関して、BDCA1+DCよりも同等に高いRNA発現を有していた。さらに、マウスのマクロファージ分化にも関与するCSF1RのRNA発現は、最も高いBDCA1+CD14+細胞であったが、他の集団は等しい発現レベルであった。結論として、BDCA1+CD14+細胞は、独特の遺伝子発現プロファイルを有する別個の集団を形成する。
RNAおよびタンパク質分析は、BDCA1+CD14+細胞とBDCA1+DCとの間の密接な関係を喚起するが、2つのサブセット間の機能的類似性は、この密接な関連性のさらなる証拠として測定する必要がある。DCは、T細胞を活性化することによって適応免疫応答を上昇させることができる専門的な抗原提示細胞である。したがって、本発明者らは、BDCA1+CD14+細胞が周囲環境から抗原を取り込む能力を最初に評価した。その目的のために、BDCA1+CD14+細胞、BDCA1+DCまたは単球による蛍光標識ウシ血清アルブミン(BSA)の取り込みを、37℃で異なる時間インキュベートした後のフローサイトメトリーによって測定した。BSAの細胞への自発的結合に起因する蛍光を避けるために、4℃コントロール(対照)を各時点で行い、これらの4℃サンプルで測定した平均蛍光強度(MFI)値を、37℃サンプルから差し引いた(ΔMFI)。図4Aに示すように、BDCA1+CD14+細胞は、蛍光標識BSAとの15分間のインキュベーション後、より高いΔMFI値によって反映された著しく高い抗原取り込み能力を示す。興味深いことに、この取り込みアッセイでは、BDCA1+CD14+細胞と単球との間の差は有意ではなかったが、このプロセスにおいて単球が最良であることが明らかとなった(図4A、左パネル)。
DC由来の共刺激シグナルIIは、T細胞活性化に不可欠である。共刺激分子に加えて、シグナルIIは、DCによって発現され、弱毒化T細胞応答をもたらす共阻害分子を介して媒介されてもよい。これらの共阻害分子の中には、PD−L1およびPD−L2がある。PD−L2は、刺激の有無にかかわらず(データは示さず)、サブセットのいずれかにおいても発現されないが、PD−L1は、すべてのサブセットによるTLRライゲーションの後に発現する。しかし、BDCA1+CD14+細胞は、BDCA1+DCと比較して有意により高いレベルのPD−L1を示す(図6A)。BDCA1+CD14+細胞のT細胞刺激能力に対するPD−L1発現の影響を評価するために、ナイーブCD4+T細胞の活性化の間に抗体によってPD−L1分子をブロックした。実際、PD−L1分子を中和することは、BDCA1+CD14+細胞誘導T細胞増殖を有意に増強した(図6B)。したがって、BDCA1+CD14+細胞による高いPD−L1発現は、この細胞集団の機能を妨害することに関与する。
臨床試験では、BDCA1+Cワクチンを用いてメラノーマ患者を治療した。これらのワクチンに使用されたBDCA1+DCは、磁気活性化細胞選別(MACS)によって患者の末梢血から単離された。この単離方法は、Bリンパ球枯渇後のBDCA1発現細胞の陽性選択に依存する。したがって、単離されたBDCA1+集団は、BDCA1+DCおよびBDCA1+CD14+細胞の両方を含む。後者のパーセンテージ(割合)は、全ワクチン調製物の6%〜45%の間で変動した。これに基づき、BDCA1+DCワクチンを投与された患者は、BDCA1+CD14+細胞含有量が25%未満のワクチンを受けたグループと、BDCA1+CD14+細胞含有量が25%を超えるワクチンを受けたグループの2つのグループに分けられた。ワクチンに使用されたBDCA1+細胞に、メラノーマ特異的抗原gp−100およびチロシナーゼおよびキーホールリンペットヘモシアニン(KLH)を免疫原性対照抗原として負荷した。2群間のKLHに対する免疫応答を比較すると、BDCA1+CD14+細胞の含有量が低いBDCA1+ワクチンを受けた患者は、BDCA1+CD14+細胞が25%を超えるワクチンを受けた患者と比較して、有意に高いKLH特異的T細胞応答を示した(図7A)。この弱毒化免疫応答は、BDCA1+CD14+細胞の弱いT細胞刺激能力に起因し得る(図5)。しかしながら、T細胞免疫応答を減衰させる別の経路は、MDSCに類似したこれらの応答を積極的に抑制することによるものである。この仮説を検証するために、BDCA1+CD14+細胞、MDSC、単球およびBDCA1+DCの抑制特性を比較した。これらのサブセットのいずれも健常ドナーから単離された場合、自己CD4+T細胞のαCD3/αCD28誘導性増殖を抑制することができなかったが、MDSCのみがこれらの細胞をメラノーマ患者から単離したときにT細胞の湿潤を示した(図7C)。驚くべきことに、CD4+T細胞をKLH負荷単球で刺激すると、BDCA1+CD14+細胞のみがKLH特異的CD4+T細胞の増殖を抑制することができた(図7B、上のパネル)。興味深いことに、この負荷された抑制は、白金ベースの化学療法剤であるオキサリプラチンの存在下で止められた(図7B、下部パネル)。従って、BDCA1+CD14+は、抗原特異的様式でT細胞応答を抑制する能力のみ有し、これは、BDCA1+CD14+細胞の含有量が高いBDCA1+DCワクチンを受けているメラノーマ患者における弱毒化KLH応答を説明することできる。
ヒト末梢血由来の樹状細胞は、MHCクラスII分子(例えば、HLA−DR)の高発現レベル、および、例えばCD3(T細胞の場合)、CD14(単球の場合)、CD19(B細胞の場合)およびCD56(NK細胞の場合)などのいわゆる系統マーカーの発現の欠損により特徴付けられる。HLA−DR+Lin−細胞内には、CD1c+CD14−血液DCのサブセットが含まれる(図8)。Lin+細胞はCD14+細胞を含み、CD1c発現のレベルに応じてさらに分けることができる。CD1c+は、上記のような抑制機能を有するCD1c+CD14+細胞に相当する。CD1c−CD14+細胞は、2つのさらなるサブセットを含み、これらはHLA−DRの発現レベルにしたがって区別することができる。最初に発現する高レベルのHLA−DRは単球に対応し、HLA−DRの低いサブセットはMDSCに対応する(図8)。この知見と一致して、富化されたCD1c+細胞は異なるレベルでCD14を発現する(図9)。CD14+単球(図9、R4)と同じレベルでCD1c+CD14high細胞(図9、R3)がCD14を発現し、これは図8に示すようなゲーティング手順と一致する。CD14−フルオロフォア結合体の輝度に応じて、CD1c+CD14−DCとCD1c+CD14high細胞との間の中間レベルでCD14を発現するさらなるCD1c+サブセットを区別することができる。これらの細胞は、CD1c+CD14lowと称することができ、図9の領域R2に含まれる.CD1c+CD14low細胞およびCD1c+CD14high細胞は、共に、上記のようにおよび図8に示されるCD1c+CD14+サブセットを構成する.CD1c+CD14−は、R1に示されている。
免疫回避は、自然免疫応答および癌に対する免疫療法のアキレス腱である。免疫から回避する際の腫瘍の豊富さは、それらが発症する防御機構の過多によって反映される。腫瘍は、T細胞誘引の中心的なケモカイン回路を破壊することが示唆されている。さらに、腫瘍が腫瘍反応性T細胞の表面に血管障壁を形成し、これらの細胞を積極的に抑制するために、腫瘍が血管内皮を操作することが示されている。一旦T細胞が内皮関門を通過すると、これらのT細胞は腫瘍細胞に遭遇する前に敵対する免疫抑制性腫瘍間質を交渉しなければならない。実際、腫瘍は、Treg細胞、MDSCおよびM2マクロファージのような免疫抑制性白血球集団の拡大を促し、促進する。この研究では、発明者らは、免疫抑制性白血球ファミリーに新しいメンバー;BDCA1+CD14+細胞を導入した。発明者らは、主に末梢血におけるこの新規集団を特徴づけたが、卵巣癌患者の腹水においてこの集団を追跡することができた。MDSCと同様に、この集団は、黒色腫患者の末梢血において有意に上昇し、腫瘍が末梢組織における抑制要素の蓄積を促進するという概念を支持する。MDSCとBDCA1+CD14+細胞との間の共通の特徴は、CD14の発現であり、これは共同起源を意味し得る。しかし、これらの2つの抑制的集団は抑制の様式が異なる。MDSCはバイスタンダーT細胞の増殖を普遍的に抑制するが、BDCA1+CD14+細胞はT細胞のみを抗原特異的に抑制する。この差異は、CD4+T細胞への抗原提示に重要なHLA−DRの発現レベルと関連している可能性がある。MDSCは、定義上HLA−DRlowであるが、BDCA1+CD14+細胞は、BDCA1+DCによる発現と同等の高レベルでHLA−DRを発現する。これに沿って、Nagarajらは、特別な癌マウスモデル(MC38)では、CD4+T細胞耐性が観察され、実質的なレベルのMHC−IIを発現する特殊型のMDSCに起因することを発見した。これに基づいて、BDCA1+CD14+細胞は、MDSCのMHC−IIhiサブセットとして分類することができた。しかしながら、これらの2つのサブセット間のRNA発現プロファイルの大きな違いは、MDSCとしてBDCA1+CD14+細胞を分類することと相反している。また、トランスクリプトーム解析により、マウスのマクロファージ分化の中心的な遺伝子の発現によって、BDCA1+CD14+細胞とマクロファージとの間に関係がある可能性があることが明らかにされた。それにもかかわらず、CSF1Rは、マウスおよびヒトの両方において炎症性DC(infDC)によって高度に発現されることも報告されている。さらに、マクロファージは定義上は組織常在細胞であり、一方では、BDCA1+CD14+細胞は循環および組織の両方で見出されるという事実は、2つの細胞サブセット間の別の不一致を強調し、それらを同じグループ内で分類することに相反する。しかし、BDCA1+CD14+細胞とマクロファージとの間の潜在的な類似性は、これら2つの間の関係のさらなる議論を促す。

Claims (15)
- CD1c(BDCA−1)+/CD19−樹状細胞が、CD1c(BDCA−1)+/CD19−/CD14+樹状細胞について枯渇し、CD14+細胞の少なくとも75%が、患者のCD14+CD16−血液由来の単球と比較して枯渇(減少)している、CD1c(BDCA−1)+/CD19−樹状細胞を含むかまたはそれからなる患者の癌の治療に用いる細胞集団。
- 前記細胞集団が、CD304(BDCA−4)である細胞をさらに含むかまたはそれからなる、請求項1に記載の細胞集団。
- 前記細胞の少なくとも50%に腫瘍抗原または腫瘍ペプチドが負荷されている、請求項1または2に記載の細胞集団。
- 前記細胞が、特に癌治療を必要とする患者の血液由来の血液から精製される、請求項1〜3のいずれか一つに記載の細胞集団。
- 前記癌が、固形癌または慢性骨髄性白血病などの血液癌である、請求項1〜4のいずれか一つに記載の細胞集団。
- 前記固形癌がメラノーマまたは前立腺癌である、請求項5に記載の細胞集団。
- −患者の血液から以下を使用して請求項1〜3の樹状細胞を精製すること
−CD1cに結合することができる抗体(BDCA−1)、
−Bリンパ球(CD19またはCD20)に結合することができる抗体、および
−CD1c(BDCA−1)+/CD19−/CD14+細胞に結合することができる抗体;
および
−得られた樹状細胞を腫瘍抗原または腫瘍ペプチドとインキュベートするステップを含む、患者の癌を治療するための細胞集団を作製するための方法。 - −CD304(BDCA−4)に結合する抗体を用いて樹状細胞を精製する
ステップをさらに含む、請求項7に記載の方法。 - −精製された樹状細胞の成熟を誘導すること

−Bリンパ球、特にCD19またはCD20上で発現されるマーカーに結合することができる抗体、および、任意に、
−形質細胞様樹状細胞に結合することができる抗体。 - 前記単球マーカーが、CD14、CD11b、およびCCR5からなる群より選択される、請求項12に記載の使用。
- 前記B細胞マーカーが、CD19およびCD20からなる群から選択される、請求項11または12に記載の使用。
- 腫瘍抗原または腫瘍ペプチドをさらに含む、請求項11〜13のいずれか一つに記載の使用。
- 癌に罹患している患者に請求項1〜6の細胞集団を投与することを含む、
癌を治療する方法。
