Abstract
Description

ユーザの神経経路に沿って位置決めされるように構成される少なくとも1つの電極と、
一連の電気刺激を神経組織に送達するように構成される制御ユニットと
を含み、各刺激は、少なくとも神経組織への電極の所与の相対位置において、一連のうちの第1の刺激が活動電位を生成し、且つ各後続刺激が、神経組織の脱分極も生じさせず、又は活動電位も誘発せずに、神経組織の膜電位を変更するレベルで構成され、各後続刺激は、先行刺激から神経組織の膜電位が回復する前に送達され、それにより、一連の刺激は、活動電位の伝導が阻止又は回避される変更範囲内に膜電位を維持する、装置を提供する。
感覚入力及びユーザの運動のうちの少なくとも一方をモニタリングするコンピュータプログラムコード手段と、
感覚入力及びユーザの運動のうちの少なくとも一方の検出に応答して、検出された感覚入力又はユーザの運動がマスキングを生じさせる持続時間に対応する時間期間内で、増大した刺激用量を送達するコンピュータプログラムコード手段であって、増大した刺激用量は、神経リクルートメントの増大を生じさせるように構成される、送達するコンピュータプログラムコード手段と
を含む、コンピュータプログラム製品を提供する。
i.移植片システムは、活動の閾値活動測定に達する(Tnn)まで、非誘発活動(N)をモニタリングする。
ii.閾値に達すると、刺激が生成され、任意の誘発反応が終了した後、刺激後非誘発活動の大きさが再測定される(Ns)。
iii.非誘発活動(Ns)が、点刺激が停止する第2の閾値活動測定(Tns)未満になるまで、刺激はレート(Rs)で生成される。Tnsは通常、Tnnよりも小さい値をとり、適する程度のヒステリシスを提供するように選択される。
iv.次に、移植片システムは、非誘発活動をモニタリングし続け、ステップ(i)に戻る。
i.患者に、患者が静止した状態で連続刺激を用いる従来の方法がプログラムされる。刺激の位置及び振幅は、痛みのある部位の異常感覚カバレッジを得るように調整される。痛み軽減を得るためのECAP(Ea)の振幅が書き留められる。
ii.刺激はオフにされ、非誘発活動の範囲が測定される。閾値Tnnが、患者が静止した状態での非誘発活動のベースラインよりも上にあるように設計される。

Claims (26)
- 治療神経刺激を適用する方法であって、
感覚入力及びユーザの運動のうちの少なくとも一方をモニタリングすることと、
感覚入力及びユーザの運動のうちの少なくとも一方の検出に応答して、前記検出された感覚入力又はユーザの運動がマスキングを生じさせる持続時間に対応する時間期間内で、増大した刺激用量を送達することであって、前記増大した刺激用量は、神経リクルートメントの増大を生じさせるように構成される、送達することと
を含む、方法。 - 前記増大した刺激用量は、刺激の振幅、刺激のパルス幅、及び/又は刺激の周波数のうちの1つ又は複数を増大させることにより行われる、請求項1に記載の方法。
- 前記増大した刺激用量は、高周波刺激のバーストを含む、請求項2に記載の方法。
- 感覚入力も運動も検出されないとき、刺激は低減用量で送達される、請求項1〜3の何れか一項に記載の方法。
- 感覚入力も運動も検出されないとき、刺激は送達されない、請求項1〜3の何れか一項に記載の方法。
- 前記ユーザに送達された累積刺激用量をモニタリングすることと、所望の刺激総用量を送達しようとするために、運動中又は運動間の何れかでの所要刺激レジメンを定義する基礎として前記累積刺激用量を使用することとを更に含む、請求項1〜5の何れか一項に記載の方法。
- 感覚入力及び前記ユーザの運動のうちの少なくとも一方は、神経経路上の神経活動を測定することによって検出される、請求項1〜6の何れか一項に記載の方法。
- 前記測定される神経活動は、前記神経経路に適用される電気刺激から生じる誘発神経反応を含む、請求項7に記載の方法。
- 所与の刺激から誘発される前記神経反応の変化が検出される場合、運動が検出される、請求項8に記載の方法。
- 前記測定される神経活動は、非誘発神経活動を含む、請求項7〜9の何れか一項に記載の方法。
- 前記ユーザの運動は、加速度計により検出される、請求項1〜10の何れか一項に記載の方法。
- 前記増大した刺激用量が送達される時間期間は、典型的な人間の運動の持続時間の予め定義される近似である、請求項1〜11の何れか一項に記載の方法。
- 前記増大した刺激用量が送達される時間期間は、前記ユーザによる感覚入力又は運動の停止を検出し、及び次に前記増大した刺激用量の送達を停止する更なるステップを実行することにより、適応的に決定される、請求項1〜11の何れか一項に記載の方法。
- 前記増大した刺激用量は、前記時間期間内の選択時点で送達される、請求項1〜13の何れか一項に記載の方法。
- 治療神経刺激を適用する装置であって、
ユーザの神経経路に沿って位置決めされるように構成される少なくとも1つの電極と、
感覚入力及び前記ユーザの運動のうちの少なくとも一方をモニタリングするように構成され、且つ前記検出された感覚入力又はユーザの運動がマスキングを生じさせる持続時間に対応する時間期間内で、前記少なくとも1つの電極を介して増大した刺激用量を送達するように構成される制御ユニットであって、前記増大した刺激用量は、神経リクルートメントの増大を生じさせるように構成される、制御ユニットと
を備える、装置。 - 神経ブロックを行う方法であって、一連の電気刺激を神経組織に送達することを含み、各刺激は、少なくとも前記神経組織への刺激電極の所与の相対位置において、前記一連のうちの第1の刺激が活動電位を生成し、且つ各後続刺激が、前記神経組織の脱分極も生じさせず、又は活動電位も誘発せずに、前記神経組織の膜電位を変更するレベルで構成され、各後続刺激は、先行刺激から前記神経組織の前記膜電位が回復する前に送達され、それにより、前記一連の刺激は、活動電位の伝導が阻止又は回避される変更範囲内に前記膜電位を維持する、方法。

- 前記公称閾値下レベルは、前記神経組織のリクルートメント閾値を随時繰り返し決定することにより適応的に決定される、請求項16〜22の何れか一項に記載の方法。
- 神経ブロックを行う装置であって、
ユーザの神経経路に沿って位置決めされるように構成される少なくとも1つの電極と、
一連の電気刺激を神経組織に送達するように構成される制御ユニットと
