Abstract
Description
発明の分野
本発明は埋め込み型医療器具の送達及び展開のための装置及び方法に関する。
精巧な埋め込み型医療器具及びより進歩した治療装置、特に、ステント、ステントグラフト、バルーン、フィルタ、オクルーダなどの低侵襲的手順を通して送達されそして展開される医療器具及び治療装置の急速な増加に伴って、このような器具及び治療装置を効果的に拘束し、送達させ及び/又は展開するための改良された器具及び方法を見い出すことへの興味が高まっている。
本発明は単純で、正確でそして信頼できる器具展開を提供しながら、体内における送達の間に医療器具を保護する拡張可能な送達システムを提供する。この送達システムは装填力及び展開力が器具直径、長さ又は設計に直接的に関係しないように構成されており、このため、様々な送達器具形態及び製品ラインにわたって使用されうる、より有用な送達システムを提供する。
添付の図面は本発明のさらなる理解を提供するために含まれ、そして本明細書の一部に取り込まれそして該一部を構成し、本発明の実施形態を例示し、そして記載とともに本発明の原理を説明する役割を担う。
本発明は医療器具を拘束し、送達し及び/又は展開するための改良された装置を提供する。本発明は患者内で一時的に又は永久に展開されうる広範な器具と組み合わせ使用でき、該器具としては、限定するわけではないが、ステント、ステントグラフト、バルーン、フィルタ、トラップ、オクルーダ、薬剤又は他の治療物質又は処置剤などを送達させるための器具などが挙げられる。このため、本出願において用語「医療器具」及び「埋め込み型器具」は体内に一時的又は永久に配置されるあらゆる器具を包含するように広く解釈されることが意図される。
(1)本発明は様々な埋め込み型器具の形態及びサイズに対して作用するように拡張可能である、単純で、正確でかつ信頼性がある器具展開を提供しながら、体内での送達の間の医療器具を保護する送達システムを提供する。
Claims (32)
- 埋め込み型医療器具の展開システムであって、
より大きい展開直径及び送達用のより小さい圧縮直径を有する拡張可能な医療器具、
より小さい圧縮医療器具を包囲しているシースであって、それ自体において裏返されたシース、ここで、前記シースの外側部分は前記シースの内側部分を包囲している、
前記シースの内側部分と外側部分との間に配置されたフィラメント状拘束部材、
を含み、ここで、前記医療器具は前記シース及びフィラメント状拘束部材に同時作動力を課すことにより、より大きい直径へと展開される、展開システム。 - 前記拡張可能な医療器具は自己拡張性である、請求項1記載の展開システム。
- 前記シースはチューブ状であり、そして前記シースの外側部分は前記シースの内側部分を同軸的に包囲している、請求項1記載の展開システム。
- 前記同時作動力は2つの異なる速度での前記シース及び拘束部材の取り外しを生じさせる、請求項1記載の展開システム。
- 前記2つの異なる速度は可変示差速度を含む、請求項4記載の展開システム。
- 前記埋め込み型器具は生体活性コーティングを含み、そして前記シースは前記器具の取り扱いの間にコーティングを保護するように作用する、請求項1記載の展開システム。
- 前記生体活性コーティングは薬剤コーティングを含む、請求項6記載の展開システム。
- 前記シースはらせん状にプリーツが付けられているシースを含む、請求項1記載の展開システム。
- 前記埋め込み型器具は限定された軸方向強さを有し、そして前記シースにより提供された、増大した軸方向強さにより送達の間の損傷から保護される、請求項1記載の展開システム。
- 前記シースと拘束部材との相互作用は前記埋め込み型器具の長さから展開力を切り離す役割を担う、請求項1記載の展開システム。
- 前記拘束部材は引張力を課したときに解ける、請求項1記載の展開システム。
- 前記シースは引張力を課したときに解けるフィラメントを含む、請求項1記載の展開システム。
- 埋め込み型医療器具の展開システムであって、
より大きい展開直径及び送達用のより小さい圧縮直径を有する拡張可能な医療器具、
より小さい圧縮自己拡張性医療器具を包囲している第一のシースであって、それ自体において裏返されたシース、ここで、前記第一のシースの外側部分は前記第一のシースの内側部分を包囲している、
前記第一のシースの内側部分と外側部分との間に配置された第二のシース、ここで、第一のシース及び第二のシースは、第一のシース及び第二のシースに2つの別個の引張力を課すことにより取り外される、展開システム。 - 前記拡張可能な医療器具は自己拡張性である、請求項13記載の展開システム。
- 前記第一のシースの外側部分は第一のシースの内側部分を軸方向に包囲している、請求項13記載の展開システム。
- 前記2つの別個の引張力は2つの異なる速度での第一のシース及び第二のシースの取り外しを生じさせる、請求項13記載の展開システム。
- 前記2つの異なる速度は可変示差速度を含む、請求項16記載の展開システム。
- 前記埋め込み型器具は生体活性コーティングを含み、そして前記シースは前記器具の送達の間にコーティングを保護するように作用する、請求項13記載の展開システム。
- 前記生体活性コーティングは薬剤コーティングを含む、請求項18記載の展開システム。
- 前記第一のシースはらせん状にプリーツが付けられているシースを含む、請求項13記載の展開システム。

- 展開システムに対して埋め込み型器具を装填するための方法であって、
埋め込み型器具を提供すること、
前記埋め込み型器具を超えて延在しているセグメントを含むシース要素内に埋め込み型器具を配置すること、
ファンネル及び拘束要素を提供すること、
前記シース要素のセグメントに張力を加え、前記シース要素及び埋め込み型器具を、ファンネルを通して引張り、埋め込み型器具をシース要素内及び拘束要素内で圧縮することを含み、それにより、埋め込み型器具は圧縮状態で拘束され、ここで、前記シース要素及び拘束要素は使用時に埋め込み型器具を展開するように除去される形態となっている、方法。 - 前記ファンネルを通した前記埋め込み型器具の圧縮の後に、前記埋め込み型器具を使用の前にさらに圧縮することをさらに含む、請求項25記載の方法。
- 前記シース要素のための第一の展開ラインを提供すること、及び、前記拘束要素のための第二の展開ラインを提供すること、及び、
展開を実施するために本質的に同時に作動させるように前記第一の展開ライン及び前記第二の展開ラインを配置すること、
をさらに含む、請求項25記載の方法。 - 医療器具展開システムであって、
第一のシース及び第二のシースを含み、ここで、前記第一のシース及び第二のシースの各々は前記第一のシース及び第二のシースの各々に連結された引張部材に張力を加えることにより非比例的に作動され、単一のインプット力を加えることによる展開時に、前記インプット力は前記引張部材の間で可変的に分配される、展開システム。 - インプット力はプーリーの使用により引張部材の間で可変的に分配される、請求項28記載の展開システム。
- インプット力は示差要素の使用を通して引張部材の間で可変的に分配される、請求項28記載の展開システム。
