Abstract
Description
本発明は、全般的には前立腺癌診断の分野に関する。具体的には、本発明は、前立腺癌の検出に使用することができる生体試料中のバイオマーカーを特定することに関する。特定されたマーカーはほかにも、前立腺癌患者の予後の判定および治療に対する応答のモニタリングに使用し得る。
本願は、2014年1月17日に出願された米国特許仮出願第61/928,776号の優先権を主張するものであり、上記出願の内容全体が相互参照により本明細書に組み込まれる。
前立腺癌は、米国の男性に最も高頻度に診断が下される内臓癌であり、その死因の第2位を占めている。米国癌協会(American Cancer Society)の推定では、2013年に新たに診断される前立腺癌の症例数は約238,590例にのぼり、29,720人の男性が前立腺癌で死亡するとみられている。全体でみると、男性の6人に1人が生涯の間に前立腺癌の診断が下されることになる。
本発明は、前立腺癌患者の体液または組織にグリピカン−1ヘパラン硫酸プロテオグリカン(GPC−1)レベルの上昇がみられるという発見に一部基づくものである。本発明者らは、グリピカン−1が新たな前立腺癌のマーカーになることを発見した。したがって、本発明は、グリピカン−1を検出して患者に前立腺癌が存在することを確定する方法を提供する。
実施形態1:患者の前立腺癌を検出する方法であって、患者の体液試料中のグリピカン−1のレベルを測定することと、体液試料中のグリピカン−1のレベルに基づき、前記患者に前立腺癌があるか、前立腺癌が発生する可能性が高いことを確定することとを含む、方法。
(a)患者から体液試料を採取する段階と、
(b)前記体液試料と抗グリピカン−1抗体とを接触させる段階と、

患者の体液試料中の前立腺特異抗原(PSA)のレベルを測定することと、
(i)体液試料で測定したPSAのレベルおよび(ii)前記抗グリピカン−1抗体と前記体液試料との結合に基づき、前記患者に前立腺癌があるか、前立腺癌が発生する可能性が高いことを確定することと
をさらに含む、実施形態1〜16のいずれか1つに記載の方法。
本発明は、前立腺癌患者の体液または細胞にグリピカン−1ヘパラン硫酸プロテオグリカン(GPC−1)レベルの上昇がみられるという発見に一部基づくものである。本発明者らは、グリピカン−1が新たな前立腺癌のマーカーになることを発見した。したがって、本発明は、患者に前立腺癌の存在を検出する方法を提供する。
実施例1.細胞と結合したMIL−38抗原の特徴付け。
MIL−38抗原は最初、30kDタンパク質として報告された(Russellら,2004)が、本発明者らの研究では、MIL−38抗体が、様々な細胞抽出物中の主として60kDa抗原を検出することが示された。ウエスタンブロットでは、ゲル電気泳動の前に試料を還元剤とインキュベートすると、MIL−38の抗原との反応性が失われる。
DU−145前立腺癌細胞を膜タンパク質抽出キット(MPEK)で処理した。磁気ビーズと架橋したMIL−38により膜抽出物を免疫沈降させた。免疫沈降物をゲルで泳動して切り取り、質量分析に供するか、ビーズから直接溶離させてから質量分析に供した。次いで、質量/電荷のデータに基づいてペプチドを同定することができる質量分析により、MIL−38抗体と結合した抗原を分析した。
ウサギ抗GPC−1抗体を購入し、MIL−38により検出される分子量と同じ約60kDaの分子量のグリピカン−1コアタンパク質と反応することが明らかになった。MIL−38がグリピカン−1を認識したことを確認するため、前立腺癌DU−145のMPEK抽出物を二次元電気泳動およびウエスタンブロット法に供した。
MIL−38抗体またはウサギ抗GPC−1抗体を用いて、その反応性抗原をDU−145またはC3(MIL−38陰性)のMPEK抽出物から免疫沈降させた。免疫沈降物(IP)にMIL−38抗体または抗GPC−1抗体のいずれかを用いてウエスタンブロットを実施した(図5)。
精製組換えグリピカン−1の2つの入手源について、MIL−38抗体および抗GPC−1抗体との反応性を試験した。第一の入手源はコムギ胚芽抽出物から作製された短縮型のものである(これにはしかるべき哺乳動物の翻訳後修飾が含まれていなかったことに留意されたい)。第二の入手源はマウスNS0細胞により産生された完全長グリピカン−1である。コムギ胚芽発現グリピカン−1はMIL−38との反応性が全く認められなかったが、ウサギ抗GPC−1抗体により検出することができた(データ不掲載)。これに対し、NS0細胞で産生されたグリピカン−1にはMIL−38抗体および抗GPC−1抗体との極めて強い反応性が認められた(図6)。
これまで、MIL−38抗原の分泌を示す実験的証拠は得られていない。このことを検討するため、DU−145細胞を無血清培地で洗浄した後、無血清培地で36時間インキュベートした。得られた馴化培地をMIL−38で免疫沈降させ、得られた試料をDU−145のMPEK抽出物の標準IPと比較した。訓化培地IPには、DU−145抽出物から単離された60kDaバンドに対して、約40kDaバンドおよび55kDaバンドが認められた(図7)。
これまで、正常患者でも前立腺癌患者でも血漿試料中にMIL−38抗原の分泌を示す実験的証拠は得られていない。このことを検討するため、正常患者1例(042)および前立腺癌患者1例(046)の血漿試料をMIL−38抗体で免疫沈降させ、IP試料にMIL−38抗体および抗GPC−1抗体を用いてウエスタンブロットを実施した(図9a)。
MIL−38は前立腺癌患者の尿中の細胞を検出することができる。この検出方法の感度および特異性を試験するため、年齢が一致した尿試料125例を入手した。尿から細胞を遠心沈殿させ、MIL−38間接免疫蛍光アッセイにより分析した。合計で健常対照47例、良性前立腺肥大(BPH)37例および生検で確認された前立腺癌41例を分析した。陽性前立腺癌細胞、DU−145陽性対照およびC3陰性細胞の例を示す(図10)。
MIL−38免疫蛍光アッセイ(IFA)検査とPSA検査とを併用すると、感度および特異性が増大する。このような増大は、PSA検査に適用するカットオフ値に応じて異なる。陽性診断のカットオフが2ng/mLより大きい場合、特異性はIFA単独検査時の73%から2つの検査併用時の83%まで増大する。陽性診断のカットオフが4ng/mLより大きい場合、特異性はIFA単独検査時の73%から2つの検査併用時の89%まで増大する。
図11に示されるように、3種類のELISAアッセイフォーマットを実施した。捕捉抗体としてMIL−38、検出抗体としてウサギ抗GPC−1を用いて、組換えグリピカン−1を0ng/ml、0.1ng/ml、1ng/mlおよび10ng/mlの濃度で試験した。同様の実験を、捕捉抗体としてウサギ抗GPC−1、検出抗体としてMIL−38を用いて実施した。ほかにも、捕捉抗体としてMIL−38抗体、検出抗体としてビオチン化MIL−38抗体を用いて、単一抗体ELISAを試験した。結果から、抗GPC抗体を様々なELISAフォーマットで用いることが可能であり、また、NS0 GPC−1が適切な陽性対照になり得ることがわかる。
本発明者らが実施した実験から、当時「BLCA−38ハイブリドーマ」と称された原寄託のMIL−38抗体のハイブリドーマ(ATCCアクセション番号HB11785:マウスハイブリドーマBLCA−38)は、ここで「AM3」および「AM4」と称される2つの異なる抗体集団を産生するハイブリドーマ細胞の混合集団であることが明らかになった。異なる抗体集団をそれぞれ産生するハイブリドーマ細胞を分離し、2014年8月22日、「AM4」ハイブリドーマ細胞をCellBank Australia(CBA)にアクセッション番号CBA20140026の下で寄託した。
材料および方法
−重鎖および軽鎖のシーケンシング(DNA)
別個のシーケンシングのランを3回実行した。1回目のラン(コード224945)には、1−Oと称する混合(AM4およびAM3)調製物の二クローン性ハイブリドーマ細胞を用いた。2回目のラン(コード449295−1)には、AM4の生成に用いたハイブリドーマストックであるAlfioの細胞を用いた。3回目のラン(コード449295−5)には、AM3の生成に用いたハイブリドーマストックであるAlfio IIの細胞を用いた。
−配列のまとめの表
下の表5に、検討した抗体の重鎖および軽鎖の核酸配列およびタンパク質配列の概要を記載するとともに、様々な内部領域を示す。
試料から単離した全RNAを1.5%アガロース/GelRed(商標)ゲル上でDNAマーカー(Marker III−TIANGEN社、カタログ番号MD103)と並べて泳動した。
AM3様Alfio II細胞からは一貫した重鎖配列は得られなかった。シーケンシングラン449295−5(Alfio II)から得られた軽鎖配列は一貫して得られたものであり、他の2回のシーケンシングランの配列と比較して、フレームワーク領域および相補性決定領域の両方に明らかな差がみられた。
材料および方法
本研究に使用したAM4抗グリピカン−1抗体に関する情報を下の表6に記載する。
本研究の土台となったヒトグリピカン−1(GPC−1)配列は配列番号16で定められる。以下の残基配列を用いた:

実施例1で言及した方法を用いてペプチド合成を実施した。以下に示す設計に従って、化学合成線状ペプチドおよびCLIPSペプチドを合成した:
用いたCLIPS技術の一般的原理の説明について以下で説明する。
標的分子の不連続エピトープを再構築するため、構造化されたペプチドのライブラリーを合成した。この合成はChemically Linked Peptides on Scaffolds(CLIPS)技術を用いて実施した。CLIPS技術により、単一ループ、二重ループ、三重ループ、シート様の折畳み、へリックス様の折畳みおよびその組合せに構造化されたペプチドを作製することができた。CLIPS鋳型とシステイン残基とを結合させた。ペプチド内の多数のシステインの側鎖を1つまたは2つのCLIPS鋳型と結合させた。例えば、T2 CLIPS鋳型である1,3−ビス(ブロモメチル)ベンゼンの0.5mM溶液を炭酸水素アンモニウム(20mM、pH7.9)/アセトニトリル(1:1(v/v))に溶かした。この溶液をペプチドアレイ上に加えた。固相中に存在する2つのシステインの側鎖と結合したCLIPS鋳型がペプチドアレイ(3μlウェルを備えた455ウェルプレート)のペプチドと結合した。ペプチドアレイを溶液中、溶液で完全に覆われた状態で30〜60分間、穏やかに振盪した。最後に、ペプチドアレイを過剰のH2Oで十分に洗浄し、PBS(pH7.2)中に1%SDS/0.1%ベータ−メルカプトエタノールを含有する破壊緩衝液中、70℃で30分間、超音波処理した後、H2O中でさらに45分間、超音波処理した。システインが3つであること以外は同じ方法で、T3 CLIPSを有するペプチドを作製した。

説明 置換分析、標識名に示されるベース配列から出発して、システインを除く個々のアミノ酸がすべて、天然に存在するアミノ酸に置き換わっている。
抗体と各合成ペプチドとの結合をPEPSCANベースのELISAで試験した。ペプチドアレイを一次抗体溶液とインキュベートした(4℃で一晩)。洗浄後、ペプチドアレイを1/1000希釈の抗体ペルオキシダーゼコンジュゲート(SBA社、カタログ番号2010−05)と25℃で1時間インキュベートした。洗浄後、ペルオキシダーゼ基質の2,2’−アジノ−ジ−3−エチルベンゾチアゾリンスルホナート(ABTS)および3%H2O2 2μl/mlを加えた。1時間後、発色を測定した。電荷結合素子(CCD)カメラおよび画像処理システムで発色を定量化した。
CCDカメラで得られた値は0〜3000mAUの範囲内にあり、標準的な96ウェルプレートELISAリーダーとほぼ同じである。結果を定量化し、Peplabデータベースに保存した。ウェルに気泡が含まれているために偽陽性の値が出ることがあり、カードを手作業で検査し、気泡によって生じた値はいずれもスコアを0とする。
合成ペプチドの品質を確認するため、別個のセットの陽性対照ペプチドおよび陰性対照ペプチドを同時に合成した。これらを抗体57.9でスクリーニングした(Posthumusら,J.Virology,1990,64:3304−3309を参照されたい)。
表7に抗体結合条件をまとめる。Pepscan緩衝液および前準備(SQ)について、数値は競合タンパク質(ウマ血清とオボアルブミンの組合せ)の相対量を示している。
−主要な実験結果およびシグナル/ノイズ比の決定
スクリーニングによって得られた未処理のELISAの結果の全データセットの概要を図13にグラフで示す。図中、箱ひげ図は各データセットを表し、各データセット内でのELISAシグナルの平均値、分布および外れ値を示している。実験条件(抗体量、ブロッキング強度など)に応じて様々なELISAデータの分布が得られる。
本発明者らが以前実施した分析では、MIL38−AM4が伸長348VNPQGPGPEEK358(配列番号22)上のグリピカンと結合するほか、伸長135TQNARA140(配列番号8)/135TQNARAFRD143(配列番号9)とも結合することが確認され、このことは不連続エピトープであることを示すと考えられた。
抗体MIL38−AM4の立体構造エピトープの特徴を明らかにした。MIL38−AM4に対する不連続エピトープを示唆する以前の分析で得られたリードを用いて、ループの長さが異なるマトリックスアレイを作製した。さらに、個々のリード配列の完全置換分析を実施した。いずれのアレイもMIL38−AM4抗体で探索した。

- 患者の前立腺癌を検出する方法であって、患者の体液試料中のグリピカン−1のレベルを測定することと、前記体液試料中のグリピカン−1のレベルに基づき、前記患者に前立腺癌があるか、前立腺癌が発生する可能性が高いことを確定することとを含む、方法。
- (a)患者から体液試料を採取する段階と、
(b)前記体液試料と抗グリピカン−1抗体とを接触させる段階と、
(c)前記抗グリピカン−1抗体と前記体液試料との結合に基づき、前記患者に前立腺癌があるか、前立腺癌が発生する可能性が高いことを判定する段階と
を含む、請求項1に記載の患者の前立腺癌を検出する方法。 - 前記抗グリピカン−1抗体がMIL−38である、請求項2に記載の方法。

- 抗グリピカン−1抗体と前記体液試料との結合を、対照試料の抗グリピカン−1抗体結合と比較し、前記体液試料の抗グリピカン−1抗体結合が前記対照試料よりも増大していれば、前立腺癌が存在するものとする、請求項2〜10のいずれか1項に記載の方法。
- 前記抗グリピカン−1抗体と前記体液試料との結合が前記対照試料の抗グリピカン−1抗体結合のレベルよりも50%以上増大していれば、前立腺癌があることを示す、請求項13に記載の方法。
- 抗グリピカン−1抗体と前記体液試料との結合を、抗グリピカン−1抗体と1つまたは複数のグリピカン−1標準品との結合と比較し、前記標準品の抗グリピカン−1抗体結合を用いて前記体液試料中のグリピカン−1の量を定量化する、請求項2〜10、13または14のいずれか1項に記載の方法。
- 前記体液試料中のグリピカン−1含有量が約10ng/mlを上回れば、前立腺癌があることを示す、請求項1〜15のいずれか1項に記載の方法。
- 前記患者の体液試料中の前立腺特異抗原(PSA)のレベルを測定することと、
(i)前記体液試料で測定したPSAのレベルおよび(ii)前記抗グリピカン−1抗体と前記体液試料との結合に基づき、前記患者に前立腺癌があるか、前立腺癌が発生する可能性が高いことを確定することと
をさらに含む、請求項1〜16のいずれか1項に記載の方法。 - 前記前立腺特異抗原(PSA)のレベルを前記患者の血液試料で測定する、請求項17に記載の方法。
- 測定した前記体液試料中の前立腺特異抗原(PSA)のレベルと、対照試料で測定したPSAのレベルとを比較し、前記体液試料中のPSAレベルが前記対照試料よりも増大していれば、前立腺癌が存在するものとする、請求項17または請求項18に記載の方法。
- 前記体液が、血液、血清、血漿および尿からなる群より選択される、請求項1〜19のいずれか1項に記載の方法。
- 前立腺癌を検出するキットであって、第一の抗グリピカン−1抗体と、薬学的に許容される担体と、グリピカン−1標準品とを含み、患者の体液中のグリピカン−1を検出することが可能である、キット。
- 前記抗グリピカン−1抗体がMIL−38ではない、請求項21に記載のキット。
- 前記抗グリピカン−1抗体がMIL−38である、請求項21に記載のキット。
- 前記抗グリピカン−1抗体が、請求項4、5または7のいずれか1項で言及されている抗体である、請求項21に記載のキット。
- 二次リガンドをさらに含む、請求項21〜24のいずれか1項に記載のキット。
- 前記二次リガンドが、第二の抗グリピカン−1抗体またはグリピカン−1と結合することが可能なアプタマーであり、前記第二の抗グリピカン−1抗体が、前記第一の抗グリピカン−1抗体と同じものであり得る、請求項25に記載のキット。
- 前記二次リガンドを迅速に検出するため、前記リガンドが標識とコンジュゲートされている、請求項25または請求項26に記載のキット。
- 前記標識が、免疫蛍光法、放射標識、イムノブロッティング、ウエスタンブロット法、酵素結合免疫測定法(ELISA)、フローサイトメトリー、免疫沈降、免疫組織化学法、生物膜試験、affinity ring test、抗体アレイ吸光度試験および化学発光からなる群より選択される検出方法に使用するためのものである、請求項27に記載のキット。
- ELISAを実施するための構成要素を含む、請求項22〜28のいずれか1項に記載のキット。
