Abstract
【解決手段】少なくとも70mg/mLの濃度の抗スクレロスチン免疫グロブリンと約1〜約20mMの範囲の濃度の酢酸カルシウムとを含む、約10cP以下の絶対粘度を有する、滅菌液体製剤。約5〜約15mMの濃度の酢酸緩衝液、例えば、酢酸ナトリウムを更に含むことが好ましい、滅菌液体製剤。更に安定化賦形剤及び/又は等張性剤としてスクロース等のポリオール配合することが、好ましい滅菌液体製剤。

比較的低濃度の酢酸カルシウムを、選択された抗体の製剤に添加することによって、製剤の粘度を低減することがわかっている。ここで使用される「粘度」という用語は、「絶対粘度」を指す。時に動的または単純粘度と呼ばれる絶対粘度は、運動学的粘度および流体密度の積である:絶対粘度=運動学的粘度×密度。運動学的粘度の次元は、L2/Tであり、Lは長さであり、Tは時間である。一般的に、運動学的粘度は、センチストーク(cSt)で表される。運動学的粘度のSI単位は、mm2/sであり、それは、1cStである。絶対粘度は、センチポアズ(cP)の単位で表される。絶対粘度のSI単位は、ミリパスカル−秒(mPa−s)であり、1cP=1mPa-sである。
タンパク質製剤は、概して、非経口的に投与される。非経口的に投与される場合、それらは、滅菌されていなければならない。滅菌希釈剤は、薬学的に許容され(ヒトへの投与に対して安全かつ非毒性)、凍結乾燥後の製剤再構成等、液体製剤の調製に有用である、液体を含む。例示的な希釈剤としては、滅菌水、注射用静菌水(BWFI)、pH緩衝液(例えば、リン酸緩衝生理食塩水)、滅菌生理食塩水、リンガー溶液、またはデキストロース溶液が挙げられる。希釈剤は、塩の水溶液および/または緩衝液を含むことができる。
最適な安定性のpH範囲は、予備処方研究中の早期に特定される必要がある。加速安定性試験および熱量測定スクリーニング等のいくつかの方法が、この試みに有用であることが証明されている(Remmele R.L.Jr.,et al.,Biochemistry,38(16):5241−7(1999))。いったん製剤が仕上げられると、薬物製品は、その保存可能期間全体を通して、所定の仕様内で製造および維持されなければならない。したがって、ほとんどの場合、緩衝剤が製剤中のpHを調整するために採用される。

安定剤は、抗凍結剤、リオプロテクタント、およびガラス形成剤としての機能を果たすことができる化合物群を含む。抗凍結剤は、低温での凍結中または凍結状態において、タンパク質を安定化させるように作用する。リオプロテクタントは、凍結乾燥の脱水段階中にタンパク質の天然様配座特性を保つことによって、タンパク質を凍結乾燥固形投薬形態で安定化させる。ガラス状態特性は、温度の関数としてのそれらの緩和特性に応じて、「頑丈」または「脆弱」に分類されてきた。抗凍結剤、リオプロテクタント、およびガラス形成剤が、安定性を与えるために、タンパク質と同じ相のままであることが重要である。糖、ポリマー、およびポリオールは、このカテゴリーに入り、時に全ての3つの役割を果たすことができる。
タンパク質分子は、空気−液体、バイアル−液体、および液体−液体(シリコーン油)の界面において、それらに吸着および変性を起こしやすくさせる表面と相互作用する傾向が強い。この分解経路は、タンパク質濃度に反比例して決まり、表面への吸着を介して、溶性および不溶性タンパク質凝集体の形成または溶液からのタンパク質の損失のいずれかをもたらすことが認められている。容器表面の吸着に加えて、製品の配送および取扱中に起こり得るように、物理的撹拌によって表面誘起分解が悪化する。
アミノ酸は、緩衝液、充填剤、安定剤、および抗酸化剤として、タンパク質製剤において多目的に利用されてきた。ヒスチジンおよびグルタミン酸は、タンパク質製剤を、それぞれ5.5〜6.5および4.0〜5.5のpH範囲で緩衝するために採用される。ヒスチジンのイミダゾール基は、pKa=6.0を有し、グルタミン酸側鎖のカルボキシル基は、4.3のpKaを有し、それらを、それらの個々のpH範囲における緩衝に好適なものにする。グルタミン酸は、いくつかの製剤中に見られる(例えば、Stemgen(登録商標))。ヒスチジンは、市販されているタンパク質製剤中に一般的にみられる(例えば、Xolair(登録商標)、Herceptin(登録商標)、Recombinate(登録商標))。それは、注射時に刺痛を起こすことが既知の緩衝液である、クエン酸塩の良い代替品を提供する。興味深いことに、ヒスチジンはまた、液体および凍結乾燥の提示の両方において、高濃度で使用される場合、安定効果を有することも報告されている(Chen B,et al.,Pharm Res.,20(12):1952−60(2003))。ヒスチジン(最大で60mM)はまた、この抗体の高濃度製剤の粘度を低減することが認められた。しかしながら、同一試験において、著者らは、ステンレス鋼容器中の抗体の凍結融解試験中、ヒスチジン含有製剤における凝集および変色の増加を認めた。著者らは、これが、鋼製容器の腐食から溶出された鉄イオンの効果によるものであると考えた。ヒスチジンに関する別の注意点は、それが、金属イオンの存在下で光酸化を受けるということである(Tomita M,et al.,Biochemistry,8(12):5149−60(1969))。製剤中の抗酸化剤としてのメチオニンの使用は、有望と思われる。それは、多数の酸化的ストレスに対して有効であることが認められている(Lam XM,et al.,J Pharm Sci.,86(11):1250−5(1997))。
タンパク質残基の酸化は、多数の異なる原因から生じる。特定の抗酸化剤の添加以外に、酸化的なタンパク質の損傷の防止は、大気酸素、温度、光への曝露、および化学汚染等、製造工程および製品の保存全体にわたった多数の要因の慎重な管理を伴う。最も一般的に使用される医薬用抗酸化剤は、還元剤、酸素/フリーラジカルスカベンジャー、またはキレート剤である。治療用タンパク質製剤中の抗酸化剤は、水溶性でなければならず、製品の保存可能期間を通して活性な状態を保たなければならない。還元剤および酸素/フリーラジカルスカベンジャーは、溶液中の活性酸素種を除去することによって機能する。EDTA等のキレート剤は、フリーラジカル形成を促進する微量金属汚染物を結合することによって効果的になり得る。例えば、金属イオンによって触媒されるシステイン残基の酸化を阻害するために、酸性繊維芽細胞増殖因子の液体製剤中でEDTAが使用された。EDTAは、Kineret(登録商標)およびOntak(登録商標)等の市販の製品中に使用されてきた。
一般に、遷移金属イオンは、タンパク質中の物理的および化学的分解反応を触媒する可能性があるため、タンパク質製剤中において望ましくない。しかしながら、特定の金属イオンが、タンパク質に対する補因子である場合には製剤中に、配位錯体を形成する場合にはタンパク質の懸濁液製剤(例えば、インスリンの亜鉛懸濁液)中に含まれる。近年、アスパラギン酸のイソアスパラギン酸への異性化を阻害するために、マグネシウムイオン(10〜120mM)の使用が提案されている(国際公開第WO2004/039337号)。
同一容器から2回以上の抽出を伴う複数回使用の非経口製剤を開発する場合、防腐剤が必要である。それらの主な機能は、微生物増殖を阻害し、薬物製品の保存可能期間または使用期間を通じて、製品の無菌性を確実にすることである。一般に使用されている防腐剤としては、フェノール、ベンジルアルコール、メタクレゾール、メチルパラベンまたはプロピルパラベン等のアルキルパラベン、塩化ベンザルコニウム、および塩化ベンゼトニウムが挙げられる。抗菌防腐活性を有する化合物の他の例としては、オクタデシルジメチルベンジル塩化アンモニウム、塩化ヘキサメトニウムが挙げられる。他の種類の防腐剤としては、ブチルアルコール、フェノール、ベンジルアルコール等の芳香族アルコール、アテコール、レソルシノール、シクロヘキサノール、3−ペンタノールが挙げられる。防腐剤は、長い使用歴を有するが、防腐剤を含むタンパク質製剤の開発は、困難なであり得る。防腐剤は、ほとんどの場合、タンパク質に対して不安定化効果(凝集)を有し、これは、複数回投与タンパク質製剤における防腐剤の使用を限定する主要な原因となっている(Roy S,et al.,J Pharm Sci.,94(2):382−96(2005))。
さらなる態様として、対象への投与のためのそれらの使用を容易にする方法でパッケージ化される、本明細書に記載する1つ以上の製剤を含む、キットを本明細書に記載する。一実施形態では、そのようなキットは、密閉ボトル、管、使い捨てもしくは複数回使用バイアル、充填済みシリンジ、または充填済み注入デバイス等の容器にパッケージ化される、本明細書に記載する製剤(例えば、そこに記載される抗体のいずれかを含む組成物)を含み、任意で、その方法の実施における化合物または組成物の使用を説明するラベルが、容器に貼付されるか、またはパッケージに含まれる。一態様では、化合物または組成物は、単位投薬形態でパッケージ化される。キットはさらに、特定の投与経路に従って、組成物を投与するのに好適なデバイスを含んでもよい。好ましくは、キットは、本明細書に記載する抗体、または本明細書に記載する製剤の使用を説明するラベルを含む。
本明細書に記載する状態を治療するための方法に関与する投薬計画は、薬物の作用を修正する種々の要因、例えば、患者の年齢、状態、体重、性別、および食習慣、任意の感染症の重症度、投与時間、ならびに他の臨床的因子を考慮して、主治医によって決定される。種々の態様では、毎日の処方は、体重1キログラム当たり0.1〜50mgの抗体の調製物の範囲である(化学修飾なしで、タンパク質のみの質量を計算する)。いくつかの実施形態では、投与量は、約0.5mg/kg〜20mg/kg、または約0.5〜10mg/kgである。

同一の病原体または生化学的経路を標的とする2つ以上の薬剤を組み合わせることによる病状の治療は、時に、治療上関連のある用量の各薬剤の単独の使用と比較して、より大きい有効性および副作用の減少をもたらす。場合によっては、薬物併用の有効性は、相加的であるが(併用の有効性は、各薬物の単独の効果の合計にほぼ等しい)、他の場合では、効果は、相乗的である(併用の有効性は、単独で与えられる各薬物の効果の合計よりも大きい)。ここで使用される「併用治療」という用語は、2つの化合物を同時に、例えば、並行して、または化合物のうちの1つが最初に投与され、続いて第2の薬剤が投与される方法で、例えば連続して、送達することができることを意味する。所望の結果は、1つ以上の症状の主観的な緩和、または投与量の受容者における客観的に識別できる改善のいずれかであり得る。
(実施例)
実施例1−酢酸カルシウムによる、スクレロスチン抗体製剤の有効粘度の低減
10mLの選択された抗スクレロスチン抗体(75.7mg/mL)を、2リットルの10mM Na(OAc)および9%スクロースに対して、4℃で2時間透析した。選択された抗スクレロスチン抗体(75.7mg/mL)を約160mg/mLまで濃縮し、水で、約140mg/mLおよび120mg/mLまで希釈した。希釈試料の吸光度を、それぞれ、120、142、および157mg/mLに決定した。
実施例2−製剤調節
10mLの選択された抗スクレロスチン抗体(75.7mg/mL)を、2リットルの10mM Na(OAc)、6%スクロースまたは4%スクロースに対して、4℃で2時間透析した。次いで、Amiconを用いて、各スクロース製剤を約140mg/mLまで濃縮し、次いで、標的濃度まで、水で希釈し戻した(すなわち、120mg/mL、140mg/mL、および160mg/mL)。希釈試料の吸光度値を、それぞれ、124mg/mL(4%スクロース)、119.5mg/mL(6%スクロース)、137.5mg/mL(4%スクロース)、および142mg/mL(6%スクロース)に決定した。
実施例3−他の高タンパク質濃度製剤中の酢酸カルシウムの効果
以下の実施例は、酢酸カルシウムが、スクレロスチン抗体以外に、高濃度のタンパク質を含有する製剤の粘度を低減するかどうかを判定した。
実施例4−高濃度抗スクレロスチン抗体製剤の粘度に対する非カルシウム塩の効果
非カルシウム塩が抗スクレロスチン抗体製剤の粘度を低減することが可能であるかどうかを判断するために、以下の実験を実施した。
