Abstract
Description
しかしながら、検出及び処置の進歩にもかかわらず、多くの治療プロトコールが生存率に対してわずかしか貢献しておらず、斯かる処置の費用対効果やクオリティオブライフに与える影響が問われている。最近、従来の化学療法ベース及び放射線療法ベースの癌処置の抗腫瘍効果に対する先天性及び順応性免疫系の重要な貢献が説明された(Kroemer et al., 2013; Zitvogel et al., 2008)。
そのため、(相乗効果でなくても)化学療法及び/又は放射線や免疫などの処置の間の建設的な相互作用に有利に働く、改善された癌処置の開発に関して切迫した需要が存在する。
本発明の別の態様は、癌を患っている患者を処置するための、個人に投与すると前記個人の腸微生物叢のフィルミクテス(firmicutes)/バクテロイデテス(bacteroidetes)比を低下させる、SFB及び/又はポルフィロモナス科(Porphyromonadaceae)を特に増大させる、及び/又はクロストリジウム属第IV群を減少させる化学療法薬と抗生物質組成物の組み合わせの使用である。
エンテロコッカス・ヒラエ、ラクトバチルス・ジョンソニー、エンテロコッカス・フェカリス(Enterococcus faecalis)、セグメント細菌(SFB)、ポルフィロモナス、バーンシエラ、ホールデマニア、及びその混合物から成る群から選択される細菌の断片を含む免疫原性組成物、並びに癌患者に投与される抗腫瘍薬処置へのアジュバントとしてその使用もまた本発明の一部である。
本発明は更に、細胞組成物、及び化学療法薬と組み合わせた養子細胞移植におけるその利用に関係する。
本発明はまた、化学療法に対して優れた応答者である可能性が高い患者を同定するインビトロにおける方法であって、前記患者でTLR4、NOD1、及びNOD2の機能性を測定することを含み、ここで、前記患者が機能的なTLR4及び/又はNOD1及び/又はNOD2を欠いている場合、該患者が化学療法に対する優れた応答者として同定される方法を提供する。

(ii)腸微生物叢失調の存在又は不存在を決定すること、
を含み、ここで、「好ましくない」細菌の過剰発現を伴った腸微生物叢失調が、該患者が抗腫瘍薬処置に対する優れた応答者でないことを示す、方法を提供する。
(i)前記患者からの生物学的サンプル、例えば、前記抗腫瘍薬処置の開始から3〜9週間、好ましくは6〜9週間後に入手した血液サンプルなどから、少なくとも1つの片利共生種の細菌に対する、例えばL.ジョンソニー、E.ヒラエ、及び/又はE.フェカリスに対する免疫記憶CD4+T細胞応答を分析し;
(ii)CD4+T細胞応答を分析するそれぞれの片利共生種について、応答を以下のカテゴリ:
−免疫記憶CD4+T細胞応答なし;
−Th10表現型の免疫記憶応答;
−Th1表現型の免疫記憶応答、
のうちの1つに分類することを含み、ここで、Th1表現型の免疫記憶応答が少なくとも1つの片利共生種に関して観察される場合、該抗腫瘍薬処置が続けられ、そして、斯かる応答の不存在では、該抗腫瘍薬処置が中止される、方法を提供する。
本発明はまた、インビトロにおける、患者に投与されたネオアジュバント抗腫瘍薬処置の生物学的効果を決定する方法であって、以下のステップ:
(i)例えば、患者由来の十二指腸又は回腸粘膜の生検検体から得られた、前記患者由来の適切な生物学的サンプルから、前記微生物叢中の、ラクトバチルス及びビフィドバクテリウム属を含めた第一群由来の細菌の相対量を測定し、
(ii)同じ生物学的サンプルから、前記腸微生物叢中の、パラバクテロイデス・ディスタノニス、フェカリバクテリウム・プラウスニッツイー、ゲミガー、アリスチペス及びクロストリジウム属クラスタIVを含めた第二群由来の細菌の相対量を測定し;
(iii)第一群由来の細菌の存在量と第二群由来の細菌の存在量の間の比を計算すること、
を含み、ここで、前記比が予定された閾値を超えている場合、結果は、ネオアジュバント抗腫瘍薬処置がT−bet/Th1局所的及び全身的免疫応答を引き起こしたことを示す方法に関係する。

腸微生物叢
(以前、腸内フローラ又は微生物フローラと呼ばれた)「腸微生物叢」は、動物界(ヒト、動物、昆虫など)に属するいずれかの生物体の腸内に生きる微生物集団を示す。各個人は独特な微生物叢組成を有するが(合計400〜500の別個の細菌種/個人に対して、60〜80の細菌種がサンプル採取集団の50%超を占める)、いつも同様の主たる生理機能を満たし、且つ、個人の健康に直接的な影響を有する:
・いくつかのビタミン(B及びK)の産生に貢献する。
・他の微生物からの攻撃性から保護し、そして、腸粘膜の完全性を維持する。
・適切な免疫系の発生において重要な役割を果たす。
・健康で、様々な、そして、平衡を保った腸微生物叢は本来の腸の機能を確保するのに重要である。
腸微生物叢の発生は出産時に始まる。子宮内は無菌であり、新生児の消化管は母親(腟、皮膚、乳房など)、送達が行われる環境、空気などからの微生物によって急速にコロニー形成される。三日目から、腸微生物叢の組成は、乳児がどのように給養されているかに直接依存している:乳児用調製粉乳で給養された新生児と比較して、母乳で育てられた新生児の腸微生物叢は、例えば、主にビフィドバクテリアによって占められている。
優位な腸微生物叢の全般的な組成は、健康な人々のほとんどで同様であり(4種類の主たる門、すなわち、ファーミキューテス、バクテロイデテス、アクチノバクテリア(Actinobacteria)、及びプロテオバクテリア(Proteobacteria))、種レベルでの組成は、非常に個人化されていて、個人の遺伝的、環境、及び食事に大きく影響される。腸微生物叢の組成は、一時的又は恒久的に食物成分に順応するようになり得る。例えば、日本人の人々は、彼らの微生物叢が海洋細菌から取得した特異的酵素のおかげで海草(彼らの日常食の一部)を消化する。
それは変化に順応し、且つ、高いレジリエンス能力を有するが、腸微生物叢組成の不均衡はいくつかの特別な状況で生じ得る。これは「微生物叢失調」と呼ばれ、腸内における潜在的「有害」と既知の「有益な」細菌の間の不均衡、又は主たる細菌群の組成と多様性の観点から「健康な」微生物叢と見なされるものからのいずれかの偏りである。微生物叢失調は、機能性腸疾患、炎症性腸疾患、アレルギー、肥満及び糖尿病などの健康問題につながり得る。それはまた、細胞傷害性の処置又は抗生物質処置などの処置との因果関係でもあり得る。
「抗腫瘍薬処置」は、本明細書中では、外科手術以外のあらゆる癌の処置を示す。それらには、化学療法、ホルモン療法、生物療法、及び放射線療法が含まれる。
化学療法
「化学療法」は、1若しくは複数の「化学療法薬」を用いた癌処置と本明細書中に規定される。化学療法薬は、ほとんどの癌細胞の主な特性の1つである急速に分裂する細胞を殺滅するように作用する化学分子である。化学剤にはいくつかのカテゴリが存在する:
−アルキル化剤(以下でさらに規定する);
−メベンダゾール、コルヒチンなどの紡錘体阻害剤;
−細胞傷害性/抗腫瘍抗生物質:アントラサイクリン(例えばドキソルビシン、ダウノルビシン、アドリアマイシン、イダルビシン、エピルビシン及びミトキサントロン、バルルビシン)、ストレプトマイセス(例えばアクチノマイシン、ブレオマイシン、マイトマイシン、プリカマイシン)など;
−代謝拮抗物質(ピリミジン類似体(例えばフルオロピリミジン類似体、5−フルオロウラシル(5−FU)、フロクシウリジン(FUDR)、シトシンアラビノシド(シタラビン)、ゲムシタビン(ジェムザール(登録商標))、カペシタビン;プリン類似体(例えば、アザチオプリン、メルカプトプリン、チオグアニン、フルダラビン、ペントスタチン、クラドリビン、カペシタビン、クロファラビン);葉酸類似体(例えばメトトレキサート、葉酸、ペメトレキセド、アミノプテリン、ラルチトレキセド、トリメトプリム、ピリメタミン)など;
−DNAメチルトランスフェラーゼ阻害剤:2’−デオキシ−5−アザシチジン(DAC)、5−アザシチジン、5−アザ−2’−デオキシシチジン、1−[β]−D−アラビノフラノシル−5−アザシトシン、ジヒドロ−5−アザシチジン;
−血管破壊剤、フラボン酢酸誘導体、5,6−ジメチルキサンテノン−4−酢酸(DMXAA)及びフラボン酢酸(FAA)など;
「アルキル化剤」は、タンパク質、RNA、及びDNAを含めた多くの分子をアルキル化するそれらの能力のためそのように命名されている。それらのアルキル基を介してDNAに共有するこの能力は、細胞アポトーシスを引き起こすので、それらの抗癌効果の主原因である。アルキル化剤は、細胞のサイクルから独立した薬物なので、それらの効果は通常用量依存性である。
アルキル化剤のサブタイプは、ナイトロジェンマスタード、ニトロソ尿素、テトラジン、アジリジン、及び非古典的アルキル化剤である。ナイトロジェンマスタードとしては、メクロールエサミン、シクロホスファミド、メルファラン、クロラムブシル、イホスファミド、及びブスルファンが挙げられる。ニトロソ尿素としては、N−ニトロソ−N−メチルウレア(MNU)、カルムスチン(BCNU)、ロムスチン(CCNU)及びセムスチン(MeCCNU)、フォテムスチン及びストレプトゾトシンが挙げられる。テトラジンとしては、ダカルバジン、ミトゾロミド及びテモゾロミドが挙げられる。アジリジンとしては、チオテパ、マイトマイシン、及びジアジクオン(AZQ)が挙げられる。非古典的アルキル化剤としては、プロカルバジン及びヘキサメチルメラミンが挙げられる。
抗癌「生物療法」は、直接癌細胞を標的とするか、又は癌細胞に対して作用する体の免疫系を刺激(「免疫療法」)することによって癌を処置するために、生きた生物、生物から得られた物質、又は斯かる物質の実験室製造バージョンの使用を伴う。生物療法としては、モノクローナル抗体(癌細胞表面を標的とするもの、例えば、リツキシマブ及びアレムツズマブ;抗CTLA4 Mabs、イピリムバブなど;標的増殖因子:ベバシズマブ、セツキシマブ、パニツムマブ及びトラスツズマブ;抗PD−1 Mabs;抗Tin3 Mabs、抗ICOS Mabsを含む)、免疫抱合体(例えば、90Y−イブリツモマブ・チウキセタン、131I−トシツモマブ、及びアド−トラスツズマブ・エムタンシン)、サイトカイン(IFNαなどのインターフェロン;IL−2、IL−11、G−CSM、GM−CSFなどのインターロイキンを含む)、処置用ワクチン(例えば、Sipuleucel-T(プロベンジ(登録商標))、細菌バチルスカルメット−ゲラン、癌殺滅ウイルス、遺伝子療法、並びに養子細胞T細胞移植が挙げられる。
「プレバイオティクス」は、健康に有益であると主張された方法で消化器系における細菌の増殖及び/又は活性を刺激する非消化性食品構成要素である。通常、それらは、腸微生物叢の組成及び/又は活性の両方における特有の変化を可能にする選択的発酵構成要素である。
本明細書中に使用される場合、「癌」とはすべてのタイプの癌を意味する。特に、癌は固形癌又は非固形癌である。癌の限定されることのない例は、乳癌、前立腺癌、卵巣癌、肺癌、膵臓癌及び結腸癌、肉腫、リンパ腫、メラノーマ、白血病、生殖細胞癌並びに芽細胞腫などの癌腫又は腺癌である。
本明細書中に使用される場合、「処置する」、「処置」、及び「処置すること」という用語は、癌、特に固形腫瘍の進行、重症度、及び/又は継続のいずれかの低減又は改善を指す;例えば、乳癌において、1若しくは複数の治療法の投与から生じるその1若しくは複数の症状の軽減を指す。
第一の態様によると、本発明は、癌患者に投与される抗腫瘍薬処置にアジュバントとして使用するための、エンテロコッカス・ヒラエ、ラクトバチルス・ジョンソニー、セグメント細菌(SFB)、ポルフィロモナス、バーンシエラ、ホールデマニア、及びその混合物から成る群から選択される細菌を含むプロバイオティクス組成に関係する。
いくつかの化学療法薬(特にCTX)は、抗癌ワクチンに対して効果的なアジュバントとして記述されている。本発明によるプロバイオティクス組成物の特に有用な適用は、さらに癌ワクチンの有効性を増強するための、斯かる化学療法薬と組み合わせたその利用である。
上記組成物は、抗腫瘍薬処置、例えば(単独又は抗腫瘍ワクチンと組み合わせた)化学療法などで処置された任意の患者に適切に投与され得るが、それらは前記プロバイオティクス組成物中に存在する種の過少出現を伴う微生物叢失調を有する患者において特に有用である。
本発明の別の目的は、癌患者に投与される抗腫瘍薬処置に対するアジュバントとしての使用のための、エンテロコッカス・ヒラエ、ラクトバチルス・ジョンソニー、エンテロコッカス・フェカリス、セグメント細菌(SFB)、ポルフィロモナス、バーンシエラ、ホールデマニア、及びその混合物から成る群から選択される細菌の断片を含む免疫原性組成物である。
本発明による免疫原性組成物は、皮下又は筋肉内投与のために好ましくは処方される。それらは、有利なことには、処置に対するアジュバント効果を有する免疫応答を誘発するために、CTXなどの化学療法薬の投与の前に、同時に又は後で投与され得る。
本発明は更に、化学療法薬と組み合わせた養子細胞移植の際の細胞組成物及びその利用に関係する。患者から様々な免疫細胞型を得るための技術、及び斯かる細胞を生体外でパルス又は教育するための技術は、当業者に周知である。斯かる技術は、とりわけ、Cauxら(1996)、Sallust (1994)、Palucka (2013)、Vanlint (2014)、Arrntzen (2008)、及びLesterhuis (2008)による説明されている。本発明による第一の細胞組成物は、先に記載のとおりプロバイオティクス組成物又は免疫原性組成物を用いて生体外でパルスした、樹状細胞(DC)などの抗原提示細胞(APC)を含む細胞組成物である。好ましい実施形態によると、細胞組成物中に存在する抗原提示細胞はまた、腫瘍抗原を用いて生体外でパルスされた。
本発明の別の態様は、化学療法に対する優れた応答者である可能性が高い患者を同定するインビトロにおける方法であって、前記患者においてTLR4、NOD1、及びNOD2の機能性を測定することを含み、ここで、前記患者が、機能的なTLR4及び/又はNOD1/CARD4(rs2006847、rs2066844、rs2066845、rs2066842、ND(1)+32656、rs2075820、...)及び/又はNOD2/CARD15(p.R702W、p.G908R、p.Leul007fsX1008など)を欠いている場合、該患者が化学療法(アントラサイクリン系及びオキサプラチン、及び放射線療法以外のすべて)に対する優れた応答者と同定される。2つの同時分離一塩基変異多型(SNP)−Asp299Gly及びThr399Ile−は、TLR4をコードする遺伝子内で同定された。これらのSNPは、白人の個人の約10%に存在し、いくつかの感染症で明確に関連するのがわかった。この方法の特定の実施形態において、これらのSNPのうちの一方又は両方の存在又は不存在は、例えば、PCRによって又は当業者によって知られているその他の方法によって決定される。

(ii)腸微生物叢失調の存在又は不存在を決定すること、
を含み、
先の内容では、「相対量」は生物学的サンプル中の細菌の総数のパーセンテージとして特定の分類学レベル(門から種まで)の細菌数と規定される。この相対量は、例えば、これらの細菌に割り当てられている、サンプル中に存在する16SrRNA遺伝子配列のパーセンテージを計測することによって、評価できる。それは、当業者によって知られている任意の適切な技術、例えば以下の実験の項に記載の454パイロシークエンシング及びこれらの特定の細菌16S rRNA遺伝子マーカーの定量的PCR、又は細菌群に特異的な任意の遺伝子の定量的PCR、によって計測できる。
任意のタイプの癌に関する別の実施形態によると、生物学的サンプルは、患者から入手された検便用便である。このサンプルは、診断の時点で又は例えば、処置の開始を決める前の任意の時点で採取されてもよかった。
(i)抗腫瘍薬処置の開始から少なくとも3週間後、好ましくは抗腫瘍薬処置の開始から6〜9週間後(3サイクルの化学療法に相当)に得られた前記患者からの生物学的サンプルから、少なくとも1つの片利共生種の細菌(好ましくは少なくとも2、より好ましくは少なくとも3、4つ以上の片利共生)に対して向けられた免疫記憶CD4+T細胞応答を分析し;
−免疫記憶CD4+T細胞応答なし;
−Th10:Tr1/Treg表現型の免疫記憶応答;
−Th1表現型の免疫記憶応答、
のうちの1つに分類することを含み、
方法に関係する。
この薬力学的方法の1つの特に有利な態様は、それが血液サンプルを使用して行われるということである。もちろん、どんな種類の癌を患っている患者のためにおこなわれ得る。
(i)前記患者からの適切な生物学的サンプルから、前記微生物叢中の「好ましい」細菌の相対量を測定し;
(ii)同じ生物学的サンプルから、前記腸微生物叢中の「好ましくない」細菌の相対量を測定し;
(iii)好ましい細菌の存在量と好ましくない細菌の存在量との間の比を計算すること、
を含み、
ここで、前記比が予定された閾値を超えている場合、結果は、ネオアジュバント抗腫瘍薬処置がT−bet/Th1局所的及び全身的免疫応答を引き起こしたことを示す方法に関係する。
重要なことには、先に記載した方法は、化学療法、生物療法、放射線療法、ホルモン療法などを含めた、先に規定される任意の抗腫瘍薬処置に対する癌患者の応答性を予測又は診断するために実施される。特に、これらの方法は、癌患者に関して、化学療法の、より特に、先に引用したものなどのアルキル化剤又は白金塩、及び/又は抗腫瘍ワクチンを用いたものの、(潜在的)利益を評価するのに特に有利に使用され得る。以下の実験データは、シクロホスファミド(実施例1、3及び4)、ドキソルビシン(少なくとも図14を参照のこと)及びオキサリプラチン(実施例2)によって誘発された免疫応答に対する微生物叢の役割を明確に記載している。興味深いことに、実施例3〜5は、マウスにおいて得られた結果がヒトに対して推定される可能性があることを示す。実験データは、「有益な」微生物叢もまたアントラサイクリン(図8)による処置の効率に対してプラスの影響を有し、且つ、明らかに、フェカリバクテリウム・プラウスニッツイーなどの免疫調整の役割を有する細菌種が腸微生物叢中に多すぎる場合、これらの細菌は薬剤効率にマイナスの影響を与えることを示す。
本発明によるプロバイオティクスの例は、番号I−4823で2013年11月15日にCollection Nationale de Cultures de Microorganismes(CNCM)に寄託されたラクトバチルス・ジョンソニー菌株LJFS001B、及び数号I−4815で2013年11月7日にCollection Nationale de Cultures de Microorganismes(CNCM)に寄託されたエンテロココッカス・ヒラエ菌株EHFS001である。
本発明はさらに、癌患者を処置するための、抗腫瘍薬処置と組み合わせた斯かるプロバイオティクスの使用に関連している。
少なくとも、番号I−4823で2013年11月15日にCollection Nationale de Cultures de Microorganismes(CNCM)に寄託されたラクトバチルス・ジョンソニー菌株LJFS001B(図26A)、及び/又は番号I−4815で2013年11月7日にCollection Nationale de Cultures de Microorganismes(CNCM)に寄託されたエンテロココッカス・ヒラエ菌株EHFS001(図26B)を含んでいる組成物もまた本発明の一部である。本発明による組成物は、栄養補助食品(例えば、丸薬、錠剤、シロップなど)の形態であっても、又は飲料、発酵ヨーグルトなどの機能性食品の形態であってもよい。プロバイオティクスはこれらの組成物中で優先的に生存している。
材料と方法
注意:相容れない適用がない限り、当該実施例に記載のある材料と方法は、他の実施例で使用されたものでもある。
マウス脾臓及びリンパ節からの細胞懸濁液を、コラゲナーゼ及びDNアーゼで60分間消化し、それに続いて70μmのメッシュを通して裏漉しすることによって調製した。結腸及び小腸のリンパ球を、先に記載したとおり(Schlitzer et al., 2013)単離した。要するに、結腸及び小腸を、37℃にて振盪しながら5mMのEDTA及び2mMのDTTを含むPBSで消化した。最初の消化後、結腸及び小腸組織の小片を、RPMI培地を含んでいるコラゲナーゼ/Dnase中で30分間消化した。組織小片を70μmのメッシュを通してさらに裏漉しした。フローサイトメトリー分析のために、細胞懸濁液を、以下の表面マーカー:CD11c(N418)、CD11b(ml/70)、Ly6c(HK1.4)、MHCクラスII(M5/114.15.2)、CD24(M1/69)、CD64(X54−5/7.1)、CD317(ebio927)、CD45(30F11)、F4/80(C1:A3−1)、CD8α(53−6.7)、に対する抗体で染色した。DAPIを死細胞を除外するのに使用した。抗体を、eBioscience、BD Biosciences又はBioLegendからそれぞれ購入した。細胞集団に次のようにゲートをかけた:小腸(移動画分):CD103+ DC(CD45+ CD11c+ MHC−II+ CD103+ CD24+)、CD11b+ CD103+(CD45+ CD11c+ 、MHC−II+ CD103+ CD11b+ CD24+)、CD11b+(CD45+ CD11c+ MHC−II+ CD11b+ CD24+)、炎症性DC(CD45+ CD11c+ MHC−II+ CD11b+ CD64+ Ly6c+)、大腸:CD103+ DC(CD45+ CD11c+ MHC−II+ CD103+ CD24+)、CD11b+(CD45+ CD11c+ MHC−II+ CD11b+ CD24+)、炎症性DC(CD45+ CD11c+ MHC−II+ CD11b+ CD64+ Ly6c+)。
移植後5〜7日目に脾臓を採取した。フローサイトメトリー分析を、モネンジンの存在下でのPMA/イオノマイシン再刺激の5h後の細胞内IL−17+又はIFNγ+細胞のパーセンテージを正確に評価するためにCD45.1+細胞に対してゲートをかけた。他の脾細胞を、100万個/mlにて24ウェル平底プレート内、三連でインキュベートし、1μg/mlのCBirlペプチド455−475(DMATEMVKYSNANILSQAGQ)あり又はなしで培養し、そして、上清を抗IFNγ特異的な市販のELISAを使用して分析した。
当該実施例において、小腸微生物叢に対するCTXの影響、及び抗腫瘍免疫応答に対するその後の効果を説明した。
腸上皮バリアの炎症状況は、未処置マウスにおいて、骨髄非破壊的用量のCTX又はアントラサイクリン系ドキソルビシンを用いた処置後48時間で特徴づけられた。両剤とも、粘膜固有層(LP)における小腸絨毛の短縮、上皮バリアの不連続、組織間腔の水腫、及び単核細胞の巣状滞留を引き起こした(図1A〜B)。化学療法後、杯細胞及びパネート細胞の数は、それぞれ絨毛(図1C)及び陰窩(図1D)で増加した。抗菌酵素リゾチーム(殺微生物性ペプチドRegIIIγでなく)は、CTXで処置したマウスの十二指腸で上方制御された(図1E)。経口的投与されたフルオレセインイソチオシアネート(FITC)デキストランは、CTX後18hで血液中(Yang et al., 2013)で検出可能になり、腸透過性の増大を確認した(図1F)。腸バリアの破損は、CTX後48hで良好に検出可能であった腸間膜リンパ節及び脾臓への>50%のマウスにおける共生細菌の有意な移行を伴い、ドキソルビシン処置後にそれは少なくなった(図2A)。ラクトバチルス・ジョンソニー(>40%の場合で増殖)、ラクトバチルス・ムリヌス及びエンテロコッカス・ヒラエを含めた数種類のグラム+細菌種が、これらのリンパ器官から培養される場合がある(図2B)。
最初に、発明者らは、K−Ras及びP53の発癌活性化から生じた自発性非小細胞肺癌の微小環境に対するバンコマイシンの影響を分析し、そして、CTXベースの化学療法で処置した。彼らは、化学療法後の抗腫瘍CTLsの動員に重要であることが知られているγδΤ17細胞による化学療法で処置した腫瘍床の浸潤に対するバンコマイシンの影響を分析した(Ma et al., 2011)。バンコマイシン又は広域スペクトルATBで処置したマウスでは、水で処置した化学治療法レシピエントとは対照的に、腫瘍床は治療法後のγδΤ17を欠いていた(図17)。
(T. Jacks, Cell 2012によって最初に記載されたように)制御性P53欠失に結合した発癌性K−Rasによって駆動された自所NSCLCのトランスジェニック腫瘍モデルを、オキサリプラチンとCTXの組み合わせの抗腫瘍効果に対するバンコマイシンベースの抗生物質療法の阻害的役割を試験するのに使用した。ヒト腫瘍形成を模倣するこの前臨床モデルでは、バンコマイシンによるグラム陽性細菌の根絶がCTXベースの化学療法の有効性を低下さ(図19A及び図4D)、低下した腫瘍内CD8+Tエフェクター/Foxp3+制御性T細胞比(図19B)と相関するという概念が正当であると確認された。
これにより、グラム陽性細菌が、CTX誘発抗癌免疫応答及び腫瘍体積低減に関する最適な有効性に必要であると思われる。
マウスと同様にヒトにおいてもCTXが二次リンパ組織への細菌トランスロケーションを引き起こすことをさらに実証するために、発明者らは、規則的なシクロホスファミド(CTX)での処置前後の進行性癌患者の一群の細菌に特異的な、末梢血中での、免疫記憶CD4+Th1細胞応答を評価した。観察した応答には、エンテロコッカス(E.ヒラエ及びE.フェカリス、CTXを受けたマウスにおいて両方とも免疫原性)、ラクトバチルス属(L.ジョンソニーとそれほどでもないがL.プランタルム)、及びE.コリに対するものが含まれた。結果を、CTX+アバスチンで処置した移行性卵巣癌を患っている6人の患者(Viaud et al., 2011)、DCベースのエキソソームフェーズIIワクチントライアル前にCTXで処置した3人のNSCLC(非小細胞肺癌)患者(Chaput et al., 2006)、及びCTXによって先行した標的化免疫療法のフェーズIトライアルに組み入れられた2人の黒色腫患者(Chaput et al., 2013)から得た。これらの11人の患者のうち、6人(54%)がエンテロコッカスに対して、2人がL.ジョンソニーに対して(18%)、2人(18%)がE.コリに対して免疫記憶Th1応答を発現し他のに対して、彼らのうちのひとり(9%)がL.プランタルムに対して細胞免疫反応を高めた(図20)。興味深いことに、一部の個人が、E.フェカリス(高いIL−10及び低いIFNγ産生、患者5及び6の場合)に対してTh10免疫応答を発揮した(すなわち、癌進行に関連していることが多いIL−10放出)。
(i)サイトカイン放出なし、すなわち、片利共生への免疫記憶応答がない;(ii)Th10表現型の免疫記憶応答;及び(iii)Th1表現型の免疫記憶応答、がこれらの実験条件で観察された。
原発結腸癌、膵臓癌又は胃癌のデバルキング外科手術中、十二指腸(胃及び膵臓腫瘍のため)又は回腸(右結腸癌のため)に接触することは想像できる。斯かる場合、粘膜サンプルは、掻き取り、そして、採取されてもよく(16S rRNA遺伝子パイロシークエンシング分析及び先に記載した異なった分類学レベルにおける粘膜微生物叢組成の説明のために)、並びに(qRT−PCRのためのRNAzol中で)凍ったまま維持され得る又は(免疫組織化学分析のための)パラフィン包埋組織の状態であり得る粘液であってもよい。
当該実施例では、右結腸癌(ネオアジュバントオキサリプラチンベースの化学療法中の6人の患者及び治療法前の7人の患者)を手術した患者からの回腸粘膜を分析し、回腸微生物叢の組成、並びに結腸癌を担持する患者において、化学療法を既に受けた(≪Chemo≫)又は受けなかった(≪対照≫)を意味する、アジュバント対新アジュバント化学療法の場合に、異なった属及び種の代表株(単離株)の相対的喪失又は獲得を比較した。
マウスのように、化学治療法は、ほとんどすべての患者においてクロストリジウム属クラスタIVに属する種、より詳しく述べると、属ドレア(Dorea)、コプロコッカス(Coprococcus)、ラクノスピラ、ゲミガー、アリスチペス、及び細菌種フェカリバクテリウム・プラウスニッツイー(図22と23、表3)から細菌の減少を誘発した。対照的に、ビフィドバクテリウム及びラクトバチルス属の細菌は、化学療法後に増加する傾向があった(表3、図24)。
細菌特異的な病原性TH17免疫応答の感作に対する異なった細菌種(具体的には、CTX後脾臓に移行できる細菌種)の影響を分析するために、発明者らは15日間広範囲ATBでC57BL/6マウスを処置し(糞便を滅菌した)、CTX(100mg/kg)の注射と、それに続く109個のE.ヒラエ±109個のL.ジョンソニーの経口強制飼養を実施した。細菌の単独定着又は二重定着後6日間、CD3+CD4+RORγt+又はCCR6+T細胞(以降「TH17」と呼ぶ)(図25B)から生じることがある、並びに、真正CD3+CD4+IFNγ+又はCXCR3+T細胞(以降TH1細胞と呼ぶ)(図25C)をもたらす場合がある、TH17細胞の中のIFNγ+又はCXCR3+T細胞(以降「pTH17」と呼ぶ)(図25A)に焦点を合わせたフローサイトメトリー解析のために脾細胞を採取した。得られたデータは、L.ジョンソニーではなく、E.ヒラエがCTX後の脾臓においてTH1及びpTH17応答を媒介する優位な細菌であることを明らかにし、そしてそれはL.ジョンソニーの存在下でさらに拡大される場合がある(図25A、25C)。加えて、発明者らは現在、小腸中のグラム陽性細菌のカクテル(L.ジョンソニー+E.ヒラエ)の存在と、全身的pTH17細胞の単なる惹起だけではなく(図25)、ATBで処置したマウスにおけるCTX誘発抗腫瘍の部分的回復(図26)との間の因果関係について証拠を挙げている。
E.ヒラエに対する同族TH応答が、抗癌T細胞応答を促進する場合があるかどうか調査するために、2つの異なった前臨床モデルがセットアップした。最初に、オブアルブミン抗原(OVA)を発現するように遺伝子を組み換えられた腫瘍細胞株(線維肉腫MCA205 OVA)を(又は対照として生理的食塩水)、14日間の広範囲ATB治療後にsc.移植した。動物には、OVA323−339特異的MHCクラスII−制御性OTII TCR遺伝子組み換えT細胞を養子移植し、そして、CTX(又は生理的食塩水)で処置した。発明者らは、脾臓及び腫瘍床における同種同系CD45.1+T細胞及び類遺伝子性CD45.2+OTII細胞の伸長及び活性化に対するE.ヒラエの経口強制飼養の「臨床的」影響を観察した(図27Aに提示した実験設定)。脾臓では、彼らは、pTH17細胞の蓄積で伴った(図27B、右のパネル)CD4+T細胞(図27B、中央パネル)の少なくとも一部の増殖による、宿主(CD45.1+)脾細胞(図27B、左のパネル)のE.ヒラエ誘発伸長を確認した。実際、彼らは、E.ヒラエが媒介した細胞分裂、蓄積、及び脾臓(図27C)におけるCD45.2 OTII細胞を養子移植した免疫記憶T細胞への分化を実証した。そのうえ、CD45.2+T細胞を腫瘍床から取り戻し、CD44分子を入手し、そして、E.ヒラエの経口摂取と関連して宿主TILsとほとんど同じくらい効率的に増殖した(図27D〜F)。
最大で13個の他のE.ヒラエ単離株/クローンを試験して、生体内におけるそれらの特異な免疫原性及び斯かる「抗癌プロバイオティクス」特性を媒介する能力を分析した。E.ヒラエの様々なクローンの細菌ゲノムパターンの整列を、パルスフィールドゲル電気泳動(PFGE)(図29)で分析し、発明者らによって最初に単離されたクローン(クローン「Villejuif」、番号I−4815で2013年11月7日にCollection Nationale de Cultures de Microorganismes de I'Institut Pasteur, Paris(CNCM)に寄託)がゲノム配列の点で多くの他のものと異なっていることを明らかにした。1つのヒト単離株(クローン708)が、クローンCNCM I−4815(図30)よりさらにすばらしいTH1及びTc1可能性を呈した。しかしながら、クローン708は、CTXで処置した樹立MCA205肉腫の抗癌性プロバイオティクス効果に関してCNCM I−4815クローンよりよく働かなかった(未掲載)。種々のE.ヒラエ単離株間の配列及び機能の差は、E.ヒラエ又はE.コリのCTX+/−種々クローンに晒したCaco−2上皮性腸細胞株からLDH放出(細胞死の特徴)及びヒトβ2デフェンシン分泌を観察することを目的とした試験管内におけるアッセイで確証した。クローンCNCM I−4815及びクローン708は、おそらく抗微生物ペプチドβ2デフェンシンの産生を促進することによってCTX誘発Caco2細胞死を予防し得るが、E.コリはそれができなかった(未掲載)。
どの腸免疫チェックポイントが、アルキル化剤による治療法中の細菌トランスロケーションをチェックし続け得るのか分析するために、発明者らは、脾臓pTH17細胞の惹起とMCA205担持C57BL/6マウスにおけるCTXによって促進された腫瘍制御における腸の恒常性を調整する主要なパターン認識受容体の役割を調査した。
WT対NOD1−/−xNOD2−/−未処置マウスから採取した小腸及び便の両方のバイオフィルム由来の16S rRNA遺伝子単位複製物のパイロシークエンシング分析を、CTX又はPBS投与の7日後に実施した。原理座標解析は、細菌群集構造がWT対遺伝子欠損マウスからのCTX群間の有意な違いを明らかにした(門及び属に関する図35〜36、OTUに関する表4)。主にセグメント細菌に起因する(表4)小腸(SI)におけるクロストリジウム科の(図36A〜B)、及びSIにおけるエリシペロトリクスの相対喪失を伴う(図36A〜B)、便中のポルフィロモナス科の(主にバーンシエラ)(図35A〜B)、及びPBS(図35A−B)と比較したCTX受容NOD1−/−xNOD2−/−未処置マウスの糞便中のラクノスピラの過剰発現があった。
より優れた抗癌性応答を呈したNOD1xNOD2二重ノックアウトマウスにおいて単離されたグラム陰性OTUの過剰発現を考慮に入れて、発明者らは長期でCTXに関連して使用されるOVAベースの癌ワクチンを使用した作り出される長期保護状態にあるグラム陰性菌の役割に取り組んだ。広範囲ATBは、OVA操作腫瘍細胞と共に致死的攻撃に対して癌ワクチンの長期保護を妨げた。興味深いことに、バンコマイシンは、ワクチンが動物を免疫することを妨げることなく、一方でグラム陰性菌を殺滅するコリスチンはそうした(図38)。CTXは腫瘍ワクチンを賦活するためにグラム陰性腸アジュバントを動員し得ると結論づけられる。
NOD遺伝的欠損によって媒介又は強化される微生物叢失調はCTXの治療学的な成功を改善し得るので、発明者らは、Zhang Y et al. (2014) に記載の異なったATB投薬計画が腫瘍増殖に明確に影響し得るかどうかに取り組んだ。実際、プロトコールは、ファーミキューテスが減少することを報告し、最も特に、ファーミキューテス/バクテロイド比(ネオマイシン+セファロチン又はバンコマイシン+イミペナムの組み合わせなど)(図39C〜D)を最終的に、減少させるクロストリジウム科は、CTXが誘発する抗癌効果を改善し得るが、それに対し、シフロキサシン(対照的に、バクテロイデテスの顕著な抑制を引き起こした)は効果的でなかった(図39B)。注意すべきは、ネオマイシン+セファロチンの組み合わせはSFB出現を増大させる可能性があり、それに対して、vanco+イミペナムはポルフィロモナスのそれを増強した(Zhang Y et al., 2014)。
