本願で用いる用語「抗体」は、特定の抗原と反応する任意の免疫グロブリン分子に関して用いられる。当該用語は、任意の発生源(例:ヒト、げっ歯類、ヒト以外の霊長類、ヤギ、ウシ、ウマ、ヒツジなど)から入手した任意の免疫グロブリン(例:IgG、IgM、IgA、IgE、IgDなど)を包含することが意図されている。抗体の具体的な種類/例はポリクローナル、モノクローナル、ヒト化、キメラ、ヒト、またはそれ以外の様式でヒトに適した抗体を含む。「抗体」は、本願に記載する任意の抗体の任意のフラグメントまたは誘導体も含む。
(A.イムノアッセイによる検出)
他の実施形態においては、本発明のバイオマーカーは、イムノアッセイによって検出および/または測定することができる。イムノアッセイは、バイオマーカーを捕捉するために、抗体などの生体特異的捕捉試薬/結合物質を必要とする。多くの抗体が市販されている。抗体は、たとえば動物をバイオマーカーによって免疫することなどの、技術上周知の方法によっても生成することができる。バイオマーカーは、その結合特性に基づいて試料より分離することができる。代替的に、ポリペプチドバイオマーカーのアミノ酸配列が既知である場合は、ポリペプチドを合成し、さらに技術上周知の方法によって抗体を生成するために用いることができる。
一定の実施形態においては、本発明のバイオマーカーはポリメラーゼ連鎖反応(PCR)によって検出/測定/定量することができる。一定の実施形態においては、本発明は、GFAP、S100B、MT3、ICAM5、BDNFおよび/またはNSEを含む、本願に記載の1つまたはそれ以上のバイオマーカーの定量を意図する。1つまたはそれ以上のバイオマーカーは定量することが可能であり、また発現を参照レベルと比較することができる。バイオマーカーに応じて、参照と比較した過剰発現または発現低下が損傷を示す。PCRは、定量型リアルタイムPCR(シングルプレックスとマルチプレックスの両者)などの定量型PCRを含むことができる。具体的な実施形態において、定量段階は、定量型リアルタイムPCRを用いて進められる。当業者は、本願に記載の1つまたはそれ以上のバイオマーカーと特異的に結合し、これを増幅するプライマーを、その公表された配列を用いて設計することができる。
1つの実施形態においては、本発明のバイオマーカーは、気相イオンを検出する質量分析器を使用する方法である質量分析法によって検出しうる。質量分析法の例は、飛行時間型、磁気セクター、四極子フィルター、イオントラップ、イオンサイクロトロン共鳴、オービトラップ、前述の混成または複合等である。

本発明のバイオマーカーは、他の適当な方法で検出することができる。この目的のために用いることのできる検出パラダイムは、光学的方法、電気化学的方法(ボルタメトリー法および電流滴定法)、原子間力顕微鏡、およびたとえば多極共鳴分光法などの高周波法を含む。光学的方法の例示は、共焦点および非共焦点顕微鏡に加えて、蛍光、発光、化学発光、吸光度、反射率、透過率、および複屈折または屈折率(例:表面プラスモン共鳴法、偏光解析法、共振ミラー法、回折格子結合器導波管法または干渉法)である。
本発明は、脳損傷を診断し、患者が頭部CTを必要としているか判定し、TBI患者がICHを有する可能性があると鑑別し、かつ/または短期的および長期的な脳振盪後症状およびTBI後の障害を予測するためのバイオマーカーの使用に関する。簡潔さを期すために、用語「脳損傷」は、本願を通して用いられることが理解されるが、本願に記載の方法およびバイオマーカーは、神経変性を診断する状況において適用されることが理解される。より具体的には、本発明のバイオマーカーは、個人、対象または患者において、たとえば脳損傷などを診断するためなどに、脳損傷または状態を判定、認定、および/または評価するための診断検査において用いることができる。具体的な実施形態においては、脳損傷状態は患者の脳損傷状態を判定すること、またはたとえば個人、対象または患者の脳損傷を診断するためなどに、脳損傷状態を判定することを含むことができる。より具体的には、脳損傷(例:無症候性または外傷性脳損傷)を診断する際に検出しようとするバイオマーカーはGFAP、S100B、MT3、ICAM5、BDNF、および/またはNSEを含むが、これに限定されない。関連技術分野において既知である他のバイオマーカーを、本願に記載のバイオマーカーと組み合わせて用いうる。本発明は、非修飾および修飾(例:シトルリン化または他の翻訳後修飾)タンパク質/ポリペプチド/ペプチドの両者、さらには前述のいずれかに対する自己抗体の検出、測定、定量、判定などをさらに意図し、患者の脳損傷を判定する。本願に記載の方法および組成物は、脳損傷を診断するためだけでなく、患者が頭部CTを必要としているか判定し、TBI患者がICHを有する可能性があると鑑別し、かつ/または短期的および長期的な脳振盪後症状およびTBI後の障害を予測することを含めて、損傷状態/病状にある他の脳を診断するために用いることができることが理解される。
本発明のバイオマーカーは、患者の脳損傷状態を評価、判定、および/または認定(本願では互換的に使用される)するための診断検査において用いることができる。語句「脳損傷状態」は、脳損傷を有していないことを含めた、状態の任意の判別可能な発現を含む。たとえば、脳損傷状態は、患者の脳損傷の有無、脳損傷を発症するリスク、脳損傷の病期または重症度、脳損傷の進行(例:経時的な脳損傷の進行)、脳損傷の治療の有効性またはこれに対する反応(例:治療後の脳損傷の臨床的経過観察および監視など)、患者が頭部CTを必要とするか、TBI患者がICHを有する可能性があるか、および/または短期的および長期的な脳振盪後症状およびTBI後障害を予測することを制限なく含む。この状態に基づき、追加的な診断検査または治療手技またはレジメンを含むさらなる手順を指示しうる。
具体的な実施形態においては、本発明は患者が脳損傷を発症するリスクを判定するための方法を提供する。バイオマーカーの百分率、比率、量またはパターンは、たとえば高、中、または低などの多様なリスク状態の特徴である。脳損傷を発症するリスクは、関連バイオマーカーを測定し、その後それらを分類アルゴリズムに投入するか、または参照値、すなわち特定のリスクレベルと関連する事前に定義するバイオマーカーのレベルまたはパターンと比較することによって判定する。
他の実施形態においては、本発明は患者の脳損傷の重症度を判定するための方法を提供する。脳損傷の各等級または病期は、特徴的なバイオマーカーのレベルまたはバイオマーカー群の相対的レベル/比率(パターンまたは比率)を有する可能性がある。脳損傷の重症度は、関連バイオマーカーを測定し、その後それらを分類アルゴリズムに投入するか、または参照値、すなわち特定の病期と関連する事前に定義するバイオマーカーのレベルまたはパターンと比較することによって判定する。
1つの実施形態においては、本発明は患者の脳損傷の経過を判定するための方法を提供する。脳損傷の経過は、脳損傷の進行(悪化)および脳損傷の後退(改善)を含む、脳損傷状態の経時的変化を指す。時間と共に、バイオマーカーの量または相対的な量(例:パターンまたは比率)は変化する。たとえば、バイオマーカー「X」が脳損傷に伴って増加することがある一方で、バイオマーカー「Y」が脳損傷に伴って減少することもある。したがって、脳損傷または非脳損傷に向かうこれらのバイオマーカーの経時的な増加または減少の傾向は、状態の経過を示す。このため、本方法は、患者の1つまたはそれ以上のバイオマーカーのレベルを、たとえば1回目および2回目などの少なくとも2つの測定時に測定し、かつ変化があれば比較することを包含する。脳損傷の経過はこれらの比較に基づいて判定する。
脳損傷状態を認定する方法の一定の実施形態においては、方法は状態に基づいて患者の治療を管理することをさらに含む。そのような管理は、脳損傷状態を判定することに続く医師または臨床医の措置を含む。たとえば、医師が脳損傷の診断を下す場合、その後に一定のモニタリング法を実施するであろう。本発明の方法を用いた脳損傷の経過の評価は、その後一定の脳損傷治療レジメンを必要とすることもある。あるいは、非脳損傷の診断の後で、患者が罹患しているおそれのある具体的な疾患を判定するために、さらなる検査を行うこともある。また、診断検査の結果が脳損傷状態に関する結論に至らない場合は、さらなる検査を要請することもある。
他の実施形態においては、本発明は医薬品の治療効果を判定するための方法を提供する。これらの方法は、薬剤の臨床試験を実施する際、および薬剤を使用している患者の進行をモニタリングする際に有用である。治療法または臨床試験は、薬剤を特定のレジメンで投与することを包含する。レジメンは、薬剤の1回投与または薬剤の経時的複数回投与を包含しうる。医師または臨床研究者は、患者または被験者に対する薬剤の効果を、投与の過程に渡ってモニタリングする。薬剤が病状に対して薬理学的影響を有する場合、本発明の1つまたはそれ以上のバイオマーカーの量または相対量(例:パターン、プロフィールまたは比率)は、非脳損傷プロフィールに向かって変化しうる。したがって、投与クール中、患者における1つまたはそれ以上のバイオマーカーの経過を追跡することができる。これにより、本方法は薬物療法を受ける患者の1つまたはそれ以上のバイオマーカーを測定すること、およびバイオマーカーレベル/比率を患者の脳損傷状態と(例:種々の脳損傷状態に対応する事前に定義するバイオマーカーのレベル/比率との比較により)相関させることを包含する。本方法の1つの実施形態は、たとえば1回目および2回目など、薬物療法クール中の少なくとも2つの異なる測定時について、1つまたはそれ以上のバイオマーカーのレベル/比率を判定し、かつバイオマーカーのレベル/比率に変化があれば、これを比較することを包含する。たとえば、1つまたはそれ以上のバイオマーカーのレベル/比率は薬剤の投与の前後、または薬剤投与中の異なる2つの測定時に測定することができる。治療法の効果はこれらの比較に基づいて判定する。治療が有効である場合には、1つまたはそれ以上のバイオマーカーのレベル/比率は正常に向かう傾向を示す一方、治療が無効である場合には、1つまたはそれ以上のバイオマーカーレベル/比率は脳損傷に向かう傾向を示すであろう。
一部の実施形態においては、「既知の試料」などの試料を用いて生成したデータを、その後で分類モデルを「トレーニング」するために用いることができる。「既知の試料」とは事前に分類された試料である。分類モデルを形成するために用いるデータは「トレーニングデータセット」と呼ぶことができる。分類モデルを形成するために用いるトレーニングデータセットは、生データを含むこともあれば、または事前に処理したデータを含むこともある。分類モデルは、一旦トレーニングすれば未知の試料を用いて生成したデータにおけるパターンを認識することができる。分類モデルは、その後未知の試料を階級に分類するために用いることができる。これは、たとえば特定の生体試料が一定の生物学的状態(例:疾患対非疾患)と関連するか否か予測する際などに有用となることがある。
他の態様においては、本発明は脳損傷状態を認定するためのキットであって、該キットが本願に記載するバイオマーカーを検出するために用いられるキットを提供する。具体的な実施形態においては、キットはGFAP、S100B、MT3、ICAM5、BDNFおよび/またはNSEを含むがこれに限定されない、本発明のバイオマーカーに対する抗体を含む、ELISAキットとして提供される。

TBI症例:症例は、来院より24時間以内に発生した頭部の鈍的外傷を呈し、ジョンズホプキンス病院救急科(ED)を受診した連続成人患者(18歳またはそれ以上)で、頭部CTスキャンを撮影したところ急性外傷性頭蓋内出血が証明され、さらに臨床生化学検査室で余剰の血清検体が入手できた患者であった。過去に脳手術または脳腫瘍のあった患者は除外した。
血清試料は救急科来院時に採取した。
Everett博士の研究室で開発したGFAP(グリア線維性酸性タンパク質)、ICAM5(細胞内接着分子5)、S100B、MT3(メタロチオネイン3)、神経特異性エノラーゼ(NSE)およびBDNF(脳由来神経栄養因子)に対する高感度ELISA測定法を用いた。GFAP、ICAM5、S100B、NSEおよびBDNFに対するタンパク質標品は市販のものを用い、MT3についてはジョンズホプキンス大学のEverett博士の研究室で開発されたものを用いた。
TBI症例13例、TBI参照31例、および非TBI対照196例について、入院時に採取した初回血液をELISAにより分析した。結果より、TBI群(ICHおよび非ICH)において、GFAP(図1)およびICAM5(図3)のいずれも、S100B(図4)、MT3(図6)およびBDNF(図2)と比較して、有意に上昇することが証明された。GFAP単独は非ICHからICHを予測する能力が最も高かった(図1)。我々は、以下のバイオマーカーの組み合わせによって、頭蓋内出血(ICH)を有するTBIおよびTBI対照で撮影したCTスキャンの結果に基づき、すべてのICHを有するTBI症例を鑑別できること、すなわちICHに対する100%の感度を確認した(表1):

TBI症例:来院より24時間前以内に発生した頭部の鈍的外傷により、ジョンズホプキンス病院救急科(ED)を受診した成人患者(18歳またはそれ以上)であって、ED医師によって、頭部コンピューター断層撮影(CT)スキャンを撮影して頭蓋内出血を評価するのに十分なほど重大な外傷をするとみなされた患者。そしてさらに臨床的目的で採血を受け、かつ臨床生化学検査室で余剰の血清検体が入手できた症例を含めた。
血清試料はED来院時に採取した。
Everett博士の研究室でBDNF(脳由来神経栄養因子)のために開発された高感度ELISA分析法を用いた。BDNF用タンパク質標品は市販品を入手した。
症例は計44例(CTスキャンに基づく頭蓋内出血13例および非頭蓋内出血31例)、対照は196例であった。症例におけるBDNF中央値は24.1(IQR:15.6〜29.6)ng/mLであり、対照におけるBDNF中央値は55.9(IQR:39.3〜76.8)であった。BDNFが1ng/mL変化する毎に、TBIを有するオッズ比は13%低下する(OR:0.87[95%CI:0.83〜0.91])。BDNFはAUC 0.93でTBI症例と非外傷対照を識別することができた。BDNFはICHと非ICHを識別しなかった。
BDNFレベルはTBIを有する者とTBIのない者を識別することができる。TBIを有しかつ頭蓋内出血のない患者は脳振盪を有するので、BDNFレベルの低下を用いて重大な脳振盪を有する者を鑑別することも可能である。これは、スポーツにおいて、選手が重大な脳振盪を有するか(すぐに競技に復帰してはならない)否か(すぐに競技に復帰してよい)判定するために、BDNFを用いうるような診断においてだけでなく、職務復帰についても幅広い適用可能性を有する。さらに、BDNF、その変異体または受容体はTBIの治療において治療的に用いることも可能である。

予測的コホート研究。

現行の頭部CTスキャンのみによる診断パラダイムでは、脳振盪後症状が予測できる患者は47.6%のみであるのに対し(下記の表を参照)、GFAPおよび/またはBDNF陽性の患者のみに頭部CTという方針では、脳振盪後症状を有する患者の88.2%でこの症状を予測し、8/39例の被験者でCTスキャンが回避される。障害予測についての診断精度は表4に示し、これは頭部CTと同等であった。
GFAPおよびBDNF単独またはその併用およびその変異体(タンパク質スプライシング変異体[アイソフォーム]、多形、および分解およびシトルリン化、リン酸化、アセチル化、メチル化などを含むが、これに限定されないその他の翻訳後修飾、さらにはBDNF受容体、BDNFプロホルモンおよび活性型)は、短期的および長期的脳振盪後症状およびTBI後障害を予測する。
外傷性脳損傷(TBI)の客観的診断は依然として困難である。現在、TBIを有する者とそうでない者を高い信頼性で識別する診断的検査はない。この実施例4は上記の実施例2の追跡であり、脳由来神経栄養因子(BDFN)値が、TBI症例と非外傷対象を識別できるか判定することを目的としている。以下に記載するように、我々は鈍的頭部外傷受傷から24時間以内に救急科(ED)を受診し、米国救急医学会の頭部損傷における頭部CT撮影基準を満たす患者(TBI症例)、および急性冠動脈症候群の疑いについて評価され、最終的に心疾患がないとして退院した、最近外傷を受傷していないED患者(非TBI対照)から来院時に採取した血清試料中のBDNFを測定した。BDNFはELISA測定法を用いて測定した。
軽度外傷性脳損傷(mTBI)の検出は、大きな公衆衛生的意味を伴う特有の臨床的困難さを示す。外傷性脳損傷(TBI)は、毎年170万例が罹患する「沈黙の流行病」であり、また医学的注目を求めない多数の未知の症例である。TBI症例の約75%は軽度外傷性脳損傷(mTBI)に分類されるものの、それらの症例は重大な神経学的不全および障害を患う可能性がある。TBIおよび特にmTBIの客観的診断は依然として困難である。現在、TBI患者とTBIのない患者を識別する信頼性のある診断的検査はない。持続的mTBIが疑われるスポーツ選手、軍人、および他の者に対する現在の診断パラダイムは、主観的な患者の症状の報告および理学的検査所見に基づいている。結果として、識別されない鈍的頭部損傷患者において、TBIを客観的に識別することのできる新規診断的検査に対する、満たされない臨床的ニーズがある。TBIの客観的検出は、どのTBI疑い患者がさらなる診断的検査および画像検査を必要とするか判定し;スポーツ関連TBIの正確な急性期トリアージおよび「競技復帰」ガイドラインの策定;およびTBI後遺症のリスクのある軍人を鑑別する上で有用となる可能性がある。
施設内倫理委員会の承認後に、TBI症例と非外傷対照の症例対象研究を実施した。試験は、年間65,000例の患者を診察し、1000床の三次診療大学病院に属する都市部のEDにおいて実施した。
鈍的外傷を受傷した後に都市部の救急科(ED)を受診した患者が以下の条件を満たす場合、症例としての組入れの対象となるとみなした:受傷より24時間以内に受診し;米国救急医学会(ACEP)のTBIにおける頭部CTスキャン撮影基準を満たし;その臨床評価の一環として非造影頭部CTスキャンを受け;かつ臨床生化学検査室で余剰の血清試料が入手できる。組入れ対象症例が以下の過去の医学的状態のうち1つを有する場合は除外した:脱髄性疾患;神経変性性疾患;認知症;脳卒中;脳腫瘍;頭蓋内手術;または活動性癌。対照被験者は、急性冠動脈症候群の疑いについて評価された、現在進行中の予測的コホート研究のED患者から選択した。患者が以下の基準をすべて満たす場合に対照として含めた:最近鈍的頭部外傷を受傷していない;心臓疾患がないとみなされ、EDを退院して帰宅;GCSが15である。条件を満たす対照被験者が症例の除外基準のいずれかを満たす場合は、除外した(上記を参照)。我々は、外傷性脳損傷における転換研究および臨床所見(TRACK−TBI)試験から入手した血液試料中のBDFN値も測定した。
TBI症例の人口統計学および臨床データは、BDNF値について盲検化された臨床医によってその電子カルテより要約された。ED受診時にTBI症例から採取し、4°の冷凍庫で保存した余剰の血清サンプルを、受診から1週間以内に臨床生化学検査室より入手し、−80°Fの冷凍庫で保存した。対照被験者の人口統計学および臨床データは、訓練を受けた研究助手が被験者と面接して収集した。非TBI対照被験者の血清試料は、ED来院中に採取し、処理し、−80°Fの冷凍庫で保存した。頭部CTスキャン画像は、委員会が認定した神経科医が読影し、マーシャル頭部CTスキャン分類スキームにしたがって分類した。
BDNFはバッチで測定した。BDNFを測定する技師は、試料の症例または対照の分類に対して盲検化された。
記述統計を用いてデータを要約した。カテゴリー変数(臨床および人口統計学データ)は割合として要約した。割合の差はχ2検定で評価した。連続変数(年齢およびBDNF)は中央値および四分位数間範囲(IQR)を用いて要約した。我々は、症例と対照の間のBDNF値分布が同じであるという帰無仮説を、クラスカル・ワリス検定を用いて検証した。我々は、BDNFが症例と対照を識別するための識別能力を、受信者動作特定曲線下面積(AUC)を用いて測定し、定量した。対照集団におけるBDNFの決定因子を理解するために、我々は単変量および多変量線形回帰モデルを構築した。モデルに含まれる変数(年齢、性別、人種、血圧、高血圧の既往、うつ病または統合失調症の既往)は、事前の文献検討に基づいて選択した。Fidalgo他30(1) J. AGING 115-23 (2005)を参照されたい。両側p値<0.05を統計的に有意とみなした。統計分析はSTATA/MP統計ソフトウェアバージョン11.2(Stata社、テキサス州カレッジステーション)、およびRStudio統計ソフトウェアバージョン0.97.312を用いて実施した。
我々の結果が我々の症例の定義によって影響されているか判定するために、我々は数多くの分析を実施した。我々の当初の症例の定義は、鈍的頭部外傷を有し、頭部CTを受けるためのACEP基準を満たす被験者を含んだ。これらの感度分析においては、症例を、外傷性脳損傷および心理学的健康の研究のための共通データ要素についての国際的および組織間イニシアティブの人口統計学および臨床評価作業部会によるTBIの定義を満たす被験者と再定義した。この定義を用いたところ、頭部CTを受けるためのACEP基準を満たすがTBI基準を満たさない多くの患者がいた。
(TBI症例と非TBI対照の識別)
我々は、2012年11月から2013年9月の間に受診した、頭部CTを受けるためのACEP基準を満たすTBI症例76例を検討した。さらに、我々は2012年1月から2013年2月に受診した非TBI対照被験者150例も検討した(追加図1)。非TBI対照被験者の方が症例よりも年齢が高く、女性およびアフリカ系アメリカ人が多かった(表5)。TBI症例の大半は転倒(34.2%)、運転・乗車中の衝突(27.6%)、または襲撃(25.0%)の後のいずれかに来院した(表5)。TBI症例のED受診から血清試料採取までの時間の中央値は1.8時間(IQR:0.9〜2.9時間)であった。BDNFレベルは非TBI対照(中央値60.3[IQR:41.1〜78.2]ng/mL)の方がTBI症例(17.5[IQR:11.3〜29.6]ng/mL)よりも有意に高かった(p<0.01)。BDNF値はTBIの重症度によって有意に変化しなかった(図11)。BDNFはAUC 0.94でTBI症例と非TBI対照を識別する(95%CI:0.89〜0.96)(図12)。BDNFの最小の四分位の被験者はほぼ全例(91.3%)がTBI症例であった。同様に、BDNFの最大の四分位の被験者はすべて非TBI対照であった(図13)。第2の四分位数(35.9ng/mL)のカットオフ上限値を用いると、TBIを検出するための感度、特異性、陽性予測値および陰性予測値はそれぞれ:85.5%、82.7%、71.4%、91.9%であった。
非TBI対照においては、BDNFレベルの中央値は女性の方が男性よりも高かった(それぞれ中央値69.1[IQR:41.4〜82.4]対52.7[IQR:38.7〜71.8]ng/mL)。同様に、非TBI対象において、BDNF値は平均動脈圧の上昇と共に上昇した(平均動脈圧が10mmHg上昇する毎に2.8ng/mL上昇)。年齢、性別、人種、高血圧、糖尿病、精神病の既往および平均動脈圧で調節すると、性別および平均動脈圧のみが、非TBI対照におけるBDNFの独立予測因子として残った(表6)。
TBI症例の試料と非TBI対照の試料は採取条件が異なっているので、我々は室温(RT)および4℃におけるBDNFの安定性を48時間判定した。保存条件はBDNFの安定性にほとんど影響しなかった。したがって、BDNFを凍結する前に12〜48時間保存したとき、検出の損失は最小限であり、血漿中で非常に安定であった。
我々が知る限り、これはヒトTBI症例と非TBI対照を識別するための、循環BDNFの診断精度の初めての報告である。数多くの理由により、BDNFはTBIを検出するためのバイオマーカーとして好ましい候補である。第1に、我々の研究結果よりBDNFとTBIの間で非常に強い関連が立証され、0.94と優れた識別能力が得られた(C統計量として測定)。第2に、BDNFは最も豊富に発現されるニュートロフィンであり、血清中で容易かつ高い信頼度で測定することができる。さらに、我々のデータより、BDNFは安定でありかつ低い分析変動性を有し、臨床的かつ厳しい条件において信頼度の高い結果が得られる可能性が高まる。第3に、BDNF発現は主として脳および脊髄組織にあり、多身体系外傷症例における擬陽性の可能性が限定される。
血清BDNFは、TBI症例と非外傷対照を優れた診断精度で識別し、またTBIの客観的診断のためのバイオマーカーの候補となりうる。
Claims (20)
- どの外傷性脳損傷(TBI)患者が急性頭蓋内病態を診断するための頭部コンピューター断層撮影(CT)スキャンを必要とするか鑑別するための方法であって:
a.前記患者から入手した生体試料を複数のバイオマーカーと選択的に結合する複数の結合物質と結合物質−バイオマーカー複合体を形成するのに十分な時間接触させる段階であって、前記複数のバイオマーカーがグリア線維性酸性タンパク質(GFAP)、S100B、メタロチオネイン3(MT3)、ニューロン特異性エノラーゼ(NSE)および細胞内接着分子5(ICAM5)のうち1つまたはそれ以上を含む前記段階;
b.前記結合物質−バイオマーカー複合体を検出することにより、前記生体試料中の前記バイオマーカーの発現レベルを判定する段階;および
c.前記生体試料中の前記複数のバイオマーカーの前記発現レベルを事前に定める閾値と比較する段階であって、前記複数のバイオマーカーのうち少なくとも1つの前記の事前に定める閾値を上回るかまたは下回る発現レベルが頭部CTスキャンの前記必要性を示す前記段階を含む前記方法。 - どの外傷性脳損傷(TBI)患者が急性頭蓋内病態を診断するための頭部CTスキャンを必要とするか鑑別するための方法であって:
a.前記患者から入手した生体試料を固形基材および前記基材上に固定された複数の結合物質を含む組成物と接触させる段階であって、前記結合物質のそれぞれが前記基材上の相異なる、指標となることのできる位置に固定されかつ前記結合物質が複数のバイオマーカーと特異的に結合する前記段階であって、前記複数のポリペプチドバイオマーカーがGFAP、S100B、MT3、NSE、およびICAM5のうち1つまたはそれ以上を含む前記段階;
b.前記複数の結合物質の前記複数のバイオマーカーとの結合を検出することにより、前記生体試料中の前記複数のバイオマーカーの発現レベルを判定する段階;および
c.前記生体試料中の前記複数のバイオマーカーの前記発現レベルを事前に定める閾値と比較する段階であって、前記複数のバイオマーカーのうち少なくとも1つの前記の事前に定める閾値を上回るかまたは下回る発現レベルが頭部CTスキャンの前記必要性を示す前記段階を含む前記方法。 - TBI患者が頭蓋内出血(ICH)を有する可能性が高いと診断するための方法であって:
a.前記患者から入手した生体試料を複数のバイオマーカーと選択的に結合する複数の結合物質と結合物質−バイオマーカー複合体を形成するのに十分な時間接触させる段階であって、前記複数のバイオマーカーがGFAP、S100B、MT3、NSEおよびICAM5のうち1つまたはそれ以上を含む前記段階;
b.前記結合物質−バイオマーカー複合体を検出することにより、前記生体試料中の前記バイオマーカーの発現レベルを判定する段階;および
c.前記生体試料中の前記複数のバイオマーカーの前記発現レベルを事前に定める閾値と比較する段階であって、前記複数のバイオマーカーのうち少なくとも1つの前記の事前に定める閾値を上回るかまたは下回る発現レベルが前記患者がICHを有する可能性が高いことを示す前記段階を含む前記方法。 - TBI患者がICHを有する可能性が高いと診断するための方法であって:
a.前記患者から入手した生体試料を固形基材および前記基材上に固定された複数の結合物質を含む組成物と接触させる段階であって、前記結合物質のそれぞれが前記基材上の相異なる、指標となることのできる位置に固定されかつ前記結合物質が複数のバイオマーカーと特異的に結合する前記段階であって、前記複数のポリペプチドバイオマーカーがGFAP、S100B、MT3、NSE、およびICAM5のうち1つまたはそれ以上を含む前記段階;
b.前記複数の結合物質の前記複数のバイオマーカーとの結合を検出することにより、前記生体試料中の前記複数のバイオマーカーの発現レベルを判定する段階;および
c.前記生体試料中の前記複数のバイオマーカーの前記発現レベルを事前に定める閾値と比較する段階であって、前記複数のバイオマーカーのうち少なくとも1つの前記の事前に定める閾値を上回るかまたは下回る発現レベルが患者がICHを有する可能性が高いことを示す前記段階を含む前記方法。 - 請求項1〜4のうちいずれかに記載の方法であって、前記複数のバイオマーカーがGFAPおよびS100B;GFAPおよびMT3;GFAPおよびNSE;GFAP、ICAM、およびS100B;GFAP、ICAM5およびMT3;GFAP、MT3およびS100B;GFAP、NSEおよびS100B;またはGFAP、ICAM5、MT3およびS100Bを含む前記方法。
- 請求項1〜5のいずれかに記載の方法であって、前記バイオマーカーの前記の事前に定める閾値がGFAP>0.04ng/mL、ICAM>17ng/mL、S100B>2ng/mL、MT3>0.95ng/mL、およびNSE<71ng/mLを含む前記方法。
- 請求項1〜6のいずれかに記載の方法であって、前記生体試料が血液、血漿、血清または脳脊髄液(CSF)からなる群から選択される前記方法。
- 請求項1〜7のいずれかに記載の方法であって、前記結合物質が抗体またはその抗原結合フラグメントを含む前記方法。
- 固形基材、および前記基材上に固定された複数の結合物質を含む組成物であって、前記結合物質のそれぞれが前記基材上の相異なる、指標となることのできる位置に固定されかつ前記結合物質が複数のバイオマーカーと特異的に結合する前記組成物であって、前記複数のバイオマーカーがGFAP、S100B、MT3、NSE、およびICAM5を含む前記組成物。
- 請求項9に記載の組成物であって、前記複数のバイオマーカーがさらに脳由来神経栄養因子(BDNF)を含む前記組成物。
- 請求項9に記載の組成物であって、前記結合物質が検出可能な構成部分で標識される前記組成物。
- 請求項9に記載の組成物であって、前記検出可能な構成部分が発光物質、化学発光物質、放射性同位元素、比色物質;および酵素−基質物質からなる群から選択される前記組成物。
- 請求項8に記載の組成物であって、前記結合物質が抗体またはその抗原結合フラグメントを含む前記方法。
- 患者がTBIを有すると鑑別するための方法であって:
a.前記患者から入手した生体試料をBDNFと選択的に結合する結合物質と結合物質−BDNF複合体を形成するのに十分な時間接触させる段階;
b.前記結合物質−BDNF複合体を検出することにより、前記生体試料中のBDNFの発現レベルを測定する段階;および
c.対象と比較した前記の測定されたBDNFレベルに基づいて前記患者がTBIを有すると鑑別する段階を含む前記方法。 - 請求項14に記載の方法であって、受信者動作特性曲線下面積分析を用いて前記患者がTBIを有すると鑑別する前記方法。
- 請求項14に記載の方法であって、ロジスティック回帰モデルを用いて前記患者がTBIを有すると鑑別する前記方法。
- 患者がTBIを有すると診断するための方法であって:
a.前記患者から入手した生体試料を固形基材および前記基材上に固定された複数の結合物質を含む組成物と接触させる段階であって、前記結合物質のそれぞれが相異なる、指標となりうる前記基材上の位置に固定されかつ前記結合物質が複数のバイオマーカーと特異的に結合する前記段階であって、前記複数のポリペプチドバイオマーカーがBDNFとニューログラニン(NRGN)、ミエリン塩基性タンパク質(MBP)、PAD−2、チューブリンβ−4B鎖、チューブリンα−1B鎖、CNPアーゼ、PPIA、セプチン−7、伸長因子1−α2、TPPP、TPPP3、エルミンアイソフォーム2、NDRG2アイソフォーム2、アストロタクチン1(ASTN1)、脳血管新生阻害物質3(BAI3);カルノシンジペプチダーゼ1(CNDP1);ERMIN;代謝型グルタミン酸受容体3(GRM3);ケルヒ様タンパク質32(KLH32);メラノーマ抗原ファミリーE,2(MAGE2);ニューレグリン3(NRG3);オリゴデンドロサイトミエリン糖タンパク質(OMG);溶質輸送体ファミリー39(亜鉛輸送体);レティキュロン1(RTN1);およびペプチジルアルギニンデイミナーゼ(1〜4及び6型)(PAD)のうち1つまたはそれ以上を含む前記段階;
b.前記複数の結合物質の前記複数のバイオマーカーとの結合を検出することにより、前記生体試料中の前記複数のバイオマーカーの前記発現レベルを判定する段階;および
c.前記生体試料中の前記複数のバイオマーカーの前記発現レベルを事前に定める閾値と比較する段階であって、前記複数のバイオマーカーのうち少なくとも1つの前記の事前に定める閾値を上回るかまたは下回る発現レベルが患者がTBIを有することを示す前記段階を含む前記方法。 - 患者のTBIを診断するための方法であって:
a.前記患者から試料を採取する段階;
b.質量分析法またはELISAを用いて前記患者から採取した前記試料中のパネルバイオマーカーのレベルを測定する段階であって、前記バイオマーカーのパネルがBDNF、そのアイソフォーム、その翻訳後修飾型、または前述のいずれかの組み合わせを含む前記段階;および
c.前記のバイオマーカーのパネルの前記レベルをTBIを有する患者と相関する同じバイオマーカーのパネルの事前に定義するレベルおよびTBIのない患者と相関する同じバイオマーカーのパネルの事前に定義するレベルと比較する段階であって、前記の事前に定義するレベルの1つとの相関が前記診断を提供する前記段階を含む前記方法。 - 患者の短期的及び長期的脳振盪後症状及びTBI後の障害を予測するための方法であって:
a.前記患者から試料を採取する段階;
b.質量分析法またはELISAを用いて前記患者から採取した前記試料中のパネルバイオマーカーのレベルを測定する段階であって、前記バイオマーカーのパネルがまたBDNF、そのアイソフォーム、その翻訳後修飾型、または前述のいずれかの組み合わせを含む前記段階;および
c.前記のバイオマーカーのパネルの前記レベルを持続性の脳振盪後症状を有する患者と相関する同じバイオマーカーのパネルの事前に定義するレベル、障害を有する患者と相関する同じバイオマーカーのパネルの事前に定義するレベル、脳振盪後症状のない患者と相関する同じバイオマーカーのパネルの事前に定義するレベル、および障害のない患者と相関する同じバイオマーカーのパネルの事前に定義するレベルと比較する段階であって、前記事前に定義するレベルの1つとの相関が前記診断を提供する前記段階を含む前記方法。 - a.外傷性脳損傷を有することが疑われるヒト患者に由来する生体試料内の複数のバイオマーカーと特異的に結合することのできる複数のバイオマーカー結合物質であって、前記バイオマーカーがGFAP、S100B、MT3、NSE、ICAM5およびBDNFを含む前記複数のバイオマーカー結合物質;および
b.前記結合物質の1つまたはそれ以上のバイオマーカーとの結合を示すことのできる検出試薬または検出装置を含むキット。
