Abstract
Description
この出願は、2013年1月29日に出願された米国仮出願第61/758,138号および2013年3月14日に出願された米国本出願第13/803,804号(これらの各々の開示は、参考として援用される)の利益を主張する。
本発明は、一般に、血清ホモシステイン(Hcy)濃度を有意に低下させ得、かつシスタチオニンおよびシステインのような下流代謝産物の生成を増大させ得るヒトシスタチオニンβ−シンターゼ(CBS)の形態を含む、酵素補充療法に適した組成物に関する。このような組成物は、ホモシスチン尿症およびホモシステイン再メチル化障害のような状態もしくは疾患の処置のために使用され得る。
CBSは、含硫置換基移動経路における中心的な酵素であり、真核生物におけるホモシステイン(Hcy)代謝に必須の役割を果たす(Muddら、2001, in The Metabolic and Molecular Bases of Inherited Disease, 8 Ed., pp. 2007−2056, McGraw−Hill, New York)。上記CBS酵素は、セリンおよびホモシステインのピリドキサール−5’−リン酸(PLP;ビタミンB6)依存性縮合を触媒して、シスタチオニンを形成し、次いで、上記シスタチオニンは、別のPLP依存性酵素、シスタチオニンγ−リアーゼによってシステインを生成するために使用される。ある種の遺伝的変異の結果として、CBS活性が劇的に低下しているかもしくは存在しない場合、Hcyは、組織および血中に蓄積する。含硫置換基移動経路を有する哺乳動物細胞では、CBSは、Hcyからメチオニンへの再メチル化、またはシステインの生合成におけるもう1つの使用の間で重要な調節的位置を占める。
1)ビタミンB6応答性個体においてはビタミンB6の薬理学的用量を使用するCBS活性のうちの残っている活性の増大;
2)Metの摂取を厳しく制限した食事によって血清Hcyを下げること;および
3)HcyからMetへのベタイン媒介性変換による解毒、従って、血清Hcy濃度の低下。
・未処置のホモシスチン尿症は、血管構造、結合組織、および中枢神経系に高い割合で合併症を有する。
・血清Hcyを低下させる処置(例えば、厳しくMetを制限する食事およびベタイン)は、十分に実行されれば、関連する臨床的問題を減少させる。認知能力の改善は、乳児期における処置の開始を必要とする。
・食事に伴うコンプライアンスは、一様に不十分である。新生児期に治療を開始している個体では、コンプライアンスの喪失は青年期に起こる。新生児期の後に治療を開始している個体では、コンプライアンスは全年齢で実にひどい。現在の処置アプローチが極端に困難であることは、食事によって防止可能な生命を脅かす症状を経験し、それでもなおこの処置を厳守することはできない個体をみれば、最も容易に明白である。
・食事のコンプライアンスの失敗は、増大した血清Hcy、致死的かつ身体を無能力にする(incapacitating)事象を含む、血管組織および結合組織の合併症の再燃を引き起こし、重篤な副作用(例えば、脳浮腫(過剰な血清Met濃度による)もしくは重篤な栄養失調(必須アミノ酸の欠如による)の危険がある。
・最も有効な治療ストラテジーは、ビタミンB6応答性ホモシスチン尿症にピリドキシンを与える場合に明らかであるように、酵素活性を増大させることである。このストラテジーは、変異状態に起因して、ビタミンB6非応答性個体では可能ではなく、これら個体における増大した酵素活性は、外因性酵素の送達、すなわち、酵素補充療法(ERT)(ホモシスチン尿症を処置するために一度も試みられてこなかったストラテジー)に依存する。
・実証された効力のある上記3つの現在の処置ストラテジー:(1)ビタミンB6応答性個体においてピリドキシンで酵素活性を増大させること;(2)Met制限食によって蓄積している代謝産物を減少させること;および(3)ベタイン処置においてベタイン−ホモシステインメチルトランスフェラーゼの酵素活性による解毒、のうち、全ては、血漿中の総Hcyの低下が共通している(Walter,ら、1998, Eur J Pediatr 157(Suppl 2):S71−6)。
本明細書で示されるように、本発明は、ホモシスチン尿症を有する個体において血清Hcyを特に低下させるための組成物、具体的には、組成物および方法を提供する。メチオニンが挙げられるが、これに限定されない代謝産物(例えば、システインおよびシスタチオニン)の実質的に正常なレベルを回復させる組成物、具体的には、薬学的組成物もまた提供される。本明細書でより具体的に記載されるように、ホモシスチン尿症の酵素補充療法(ERT)のための試薬および方法が提供され、ここで天然の酵素の改変形態が、インビボでの改善された安定性、活性、および医療的有用性を特に提供することによってその薬学的許容性を改善するために、組換え操作されている。
本明細書で提供されるのは、ヒトシスタチオニンβ−シンターゼ(CBS)の、具体的には、薬学的組成物に適した形態、およびホモシスチン尿症を有する個体を、例えば、酵素補充療法(ERT)によって処置するための方法である。
(a.細菌における短縮型CBSタンパク質の組換え発現)
非保存領域の特定の部分を欠いている短縮型ヒトCBS改変体(rhCBSΔC;配列番号3)を構築し、先に記載されたE.coliベースの発現系(Kozich and Kraus, 1992, 前出)を使用して過剰発現させた。上記短縮型ヒトCBSタンパク質改変体rhCBSΔCをコードする構築物(rhCBSΔCをコードするDNAは、配列番号4に示される)を、pKK388.1にクローニングされたCBS全長コード配列(配列番号1)を含む先に記載されたpHCS3 CBS発現構築物(Kozich and Kraus, 1992, Hum. Mutat. 1:113−123)の改変によって生成した。C末端欠失構築物を生成するために、所望のヌクレオチド残基にわたるCBS cDNAフラグメントを、PCR生成物の5’末端および3’末端それぞれにSph IおよびKpn I部位を組みこんだプライマーを使用して増幅した。次いで、全てのPCR生成物をSph IおよびKpn Iで切断し、Sph IおよびKpn Iで消化したpHCS3ベクターへのライゲーションによってクローニングした。Sph I部位は、上記CBS cDNAに、アンチセンスプライマーハイブリダイゼーション部位の直ぐ上流(配列番号1と番号付けされているCBS cDNAによれば、塩基対位置1012)に天然に存在する。次いで、そのようにして生成されたPCR生成物をNco IおよびSph Iで消化し、同じ酵素で切断したpHCS3プラスミドに連結した。
上記短縮型ヒトCBSをコードする配列を有するpET−28a(+)ベクターを、DE3細菌、すなわち、HMS174(DE3)もしくはBL−21(DE3)に形質転換し、カナマイシン耐性クローンを選択し、さらに使用するために、−80℃においてストックグリセロールとして維持した。
・0.001%のチアミン−HCl(pH8.0)
・0.0025%のピリドキシン−HCl(pH8.0)
・0.3mM δ−(アミノレブリン酸)(δ−ALA) pH8.0
・150μM 塩化第二鉄
・30μg/mlのカナマイシン 。
CBS活性を、標識基質として[14C]セリンを使用する先に記載された放射性同位体アッセイによって決定した(Kraus, 1987, Methods Enzymol. 143,388−394)。タンパク質濃度を、ウシ血清アルブミン(BSA)を標準物質として使用して、Bradford法(Bradford, 1976, Anal. Biochem. 72, 248−254)によって決定した。活性の1単位は、1時間37℃で、1μmolのシスタチオニンの形成を触媒するCBSの量として定義される。
変性条件および未変性条件両方の下でのCBSサンプルのウェスタンブロット分析を、以前に記載されたように行った(Majtanら、2010 J Biol Chem. 2010; 285(21):15866−73)。
Stable−Isotope−Dilution Liquid Chromatography-Mass Spectrometry法を使用して、Allenら、1993, Serum betaine N, N−dimethylglycine and N−methylglycine levels in individuals with cobalamin and folate deficiency and related inborn errors of metabolism, Metabolism 42: 1448-1460に実質的に開示されるように、マウス血漿中での含硫アミノ酸代謝化合物(sulfur amino acid metabolism compound)のレベルを測定した。
血液循環中のrhCBSΔCの保持時間および活性を評価するために実験を行い、この実験でC57BL/6Jマウスに5mg/kg 体重のrhCBSΔCを、腹腔内(IP)、血管内(IV)もしくは皮下(SQ)経路によって注射した。過剰な採血を回避するために、各々5匹のマウスの2つの実験群(1および2と称する)を、各注射経路に使用した(合計n=30)。各注射経路に関して、群1から、注射して0時間後、1時間後、8時間後および24時間後に、ならびに群2から、注射して1時間後、4時間後、10時間後、および48時間後に血液を集めた。動物を、顎下採血のために、単回使用ランセットで出血させ、ゲル(Terumo Medical Corporation, NJ, USA)入りのCapiject T−MLHGヘパリンリチウム(12.5 IU)チューブの中に血液を集めた。次いで、チューブを1200Gで10分間、遠心分離し、続いて、1.5mlチューブに血漿を集め、−80℃で貯蔵した。
循環からのタンパク質の迅速なクリアランス速度は、より高い血漿濃度を維持するために、注射の回数を増やすことによって回避され得る。よって、反復注射レジメンを、非PEG化rhCBSΔCおよびPEG化rhCBSΔCの両方で試験した。C57BL/6Jマウスに、5mg/kg 体重の非PEG化rhCBSΔC(n=5)もしくはGL4−400MA rhCBSΔC(n=5)を0時間、24時間、48時間で注射し(図2の矢印)、示された時点で採血した。血漿を、実施例1に示される放射活性アッセイを使用して、CBS活性について分析した。図2に示されるように、非PEG化酵素の反復注射は、循環において酵素活性の蓄積をもたらさず、各注射の24時間後には、ほぼ活性を生じなかった。対照的に、PEG化酵素の活性は、2回の注射の後にピークに達し、プラトーに達した。
先の実施例を、野生型マウスにおいで行い、注射したrhCBSΔCのクリアランスおよび活性の特徴付け、ならびにPEG化酵素と非PEG化酵素との間の比較に焦点を当てた。PEG化が循環時間を長期化することをいったん確立した後、PEG化CBS酵素分子の種々のレパートリーを、野生型マウスにおいてまず試験し、次いで、ホモシスチン尿症のマウスモデルにおいて評価した。
非PEG化およびPEG化rhCBSΔCを、ホモシステインを下げ、かつ低レベルのホモシステインを維持し、ならびにシスタチオニンおよびシステインレベルを増大させる能力について比較する、休薬期間を有する反復注射レジメンを、以下のように行った。6匹のHOマウスに、0日目、1日目、2日目、3日目および4日目に、GL4−400MA PEGでPEG化したrhCBSΔCを注射し(矢印,図4)、続いて、10日間の休薬期間を設け、次いで、14日目、15日目、16日目、17日目および18日目に、再び注射した。血漿サンプルを、図4に示された時点で(常に注射の前に)採取した。比較のために、同じ注射レジメンを、非PEG化酵素を注射した5匹のHOマウスにも行った。血漿代謝産物のレベルを、実施例1eに記載されるように、Stable−Isotope−Dilution Liquid Chromatography-Mass Spectrometryによって決定した。各マウスに関するホモシステイン(図4aに示される結果)およびシスタチオニン(図4b)の血漿濃度が示される。各実験群に関するホモシステインおよびシスタチオニンの平均値もまた、示される(図4c中)。図4a〜4cに示されるように、平均ホモシステイン濃度は、48時間で182μMから38μMへと低下し、週全体にわたって低いままであった。シスタチオニンは、時間0での最初の4.7μMから48時間で42μMの濃度に達し、第1週の間に高いままであった。休薬期間の間、代謝産物は、それらの初期値に戻った。その後のCBS注射は、ホモシステインレベルの鋭い低下およびシスタチオニンレベルの上昇を再びもたらした。その後、処置の中止は、再度、これらパラメーターの未処置レベルへの復帰を生じた。
未変性PAGEゲル(4−15% すなわち、ドデシル硫酸ナトリウムのような変性剤の添加なし)を、rhCBSΔCの種々のバッチ(112、13、7、27および28)で行った。図5Aに示した結果は、種々の調製物がテトラマー(T)およびダイマー(D)の種々の比、およびより高次の形態の凝集したCBSを有することを図示する。ゲル中のCBS活性(図5B)は、4℃でのタンパク質サンプルに対する電気泳動を行うことによって、未変性ポリアクリルアミドゲルで(活性染色によって)決定した。次いで、上記ゲルを、活性染色溶液(表Aを参照のこと)中に沈め、37℃で約15分間インキュベートした。約15分後に、ロードした量に依存して、濃灰色のバンド(1−413 CBS種の約45kDaダイマー)が出現した。さらに長時間の後に;最大で1時間もしくは室温で一晩で、テトラマーが視認可能になった。このゲル中活性法で未変性ゲルを分析すると、図5Bに例示されるように、図5Aにおいてバッチ112および28で出現しているテトラマー(T)、ダイマー(D)、および他のより高次の凝集した形態のrhCBSΔCのバンドがCBS関連だったことが確認される。
システイン(rhCBSΔCの15位にある)はカップリングしないので、それは、凝集形成をもたらす考えられる因子と考えられた。従って、新たな組換えプラスミドを、rhCBSΔC変異体タンパク質(本明細書でC15Sといわれる)を生成するために調製した。rhCBSΔCを作製するために実施例1bで使用した同じストラテジーを、以下のプライマーを使用することによって、C15S変異体CBS酵素(T→Aヌクレオチド変異)を生成するために使用した:
ALZET(登録商標)浸透圧ポンプは、長期間にわたってC15S酵素の一定の制御された送達を可能にした。上記C15S CBS酵素を、ALZET(登録商標)ポンプ(Micro−osmotic ALZET(登録商標) Pump, Model 1002. Lot 10268−12)を使用して投与して、反復皮下注射の代わりに、C15S CBS酵素をマウスに連続送達した。これら実験において、平均ポンプ送達速度は、0.21μl/時であった;平均充填容積は、106.6μlであった;上記ポンプは、26.2μg/μlの濃度で約106μlのPEG化C15S CBS酵素が装填された。2匹のHOマウスでの20日間にわたるC15S CBSの連続投与は、血漿ホモシステインにおいて初期の有意なおよびその後持続した低下を生じた(図8を参照のこと)。
図9は、HOマウスに、単一用量(7.5mg/kg)で、時間0で、ME−200MA0BでPEG化したrhCBSΔC、C15S変異体(C15S)および二重短縮型構築物(二重短縮型)を注射した実験の結果を示す棒グラフである。時間0(注射前に)、注射して24時間後、48時間後および72時間後でマウスから採血した。上記群(n=5〜6)の各々の血漿ホモシステインのレベルを示す。結果は、3種の酵素形態全てが、Hcy血漿濃度の匹敵する低下をもたらすことを示す。

- 上昇したホモシステインと関連する前記疾患、障害、もしくは状態は、精神遅滞、骨粗鬆症、水晶体偏位、後側弯症、脳卒中、心筋梗塞、もしくは肺塞栓症である、請求項38に記載の方法。
- 前記単離されたCBSポリペプチドは、一定量の該単離されたCBSポリペプチドを含み、ここで最大20mg/kgまでが、必要とする個体に投与される、請求項38に記載の方法。
- アネトールジチオールチオンもしくはベタインの投与をさらに含む、請求項38に記載の方法。
- ベタイン投与は、1日2回行われる、請求項38に記載の方法。
- 上昇したホモシステインと関連する疾患、障害、もしくは状態を処置もしくは改善するための医薬の製造のための、請求項1〜20のいずれかに記載の単離されたCBSポリペプチドの使用。
- 上昇したホモシステインと関連する前記疾患、障害、もしくは状態は、精神遅滞、骨粗鬆症、水晶体偏位、後側弯症、脳卒中、心筋梗塞、もしくは肺塞栓症である、請求項43に記載の使用。
- 前記医薬は、アネトールジチオールチオンもしくはベタインをさらに含む、請求項43に記載の使用。
- 緩やかなタンパク質制限食への制限をさらに含む、請求項43に記載の使用。
