Abstract
Description
X線、ガンマ線、UV線、レーザー光、マイクロ波、電子ビーム、ならびに中性子、炭素イオンおよび陽子といったような粒子ビームなどの様々な形態の放射線が、悪性疾患を治療するのに用いられている。該放射線のいくつかは、放射線増感剤との併用で、このような適応に用いられている。実際、電磁放射線および電離放射線は、細胞のDNA分子を破壊する能力があり、そのことによって細胞の増殖および分割を妨げる。この効果は、規定体積内を伝播し、この体積内にエネルギー蓄積を生成する電子および遊離ラジカルを放出する電離を創生する粒子または波の作用によって説明されうる。
(1)標的組織または標的細胞集団および高Zエレメントに対する親和性をもつ標的化分子を含む、ある量の高Z粒子を投与する;次いで、
(2)電離放射線により標的組織または標的細胞集団を照射する;ここで、高Z粒子は、的組織または標的細胞集団において、0.05金属重量%未満の濃度を達成するのに有効な量で動物に投与される。
本発明者らは、癌を治療するための使用のための無機粒子を含む有利な組成物、特に、患者の腫瘍が、電離放射線に当てられる場合に、患者の腫瘍体積中の約30%以上、好ましくは約44%以上または約47%以上、さらに好ましくは約70%以上の癌細胞の破壊(組織学的応答評価)を可能にするか、または患者において少なくとも20%以上の腫瘍サイズの減少(解剖学的応答評価)を誘導するか、または患者において少なくとも20%以上の腫瘍18F-FDG SUV(正規化取り込み値)減退(代謝的応答評価)を誘導する組成物を提供する。
癌に罹患している患者に、対応する水の体積1(Vw1)の電子密度の少なくとも5倍の電子密度の体積(Vin)を有する無機ナノ粒子を投与すること;および
患者の腫瘍(標的組織または標的細胞集団)を電離放射線に当てること;
それによって、組成物の体積(Vc)が腫瘍体積(Vt)の2〜50%を占める場合の、(i)腫瘍体積中の約30%以上の癌細胞の破壊、または(ii)少なくとも20%以上の腫瘍サイズの減少、を誘導すること。

用語「晶子の凝集体」は、互いに強く、典型的には共有的に結合している晶子を意味する。本発明のナノ粒子は、典型的に、晶子および/または晶子の凝集体である。本願明細書で用いる用語「ナノ粒子のサイズ」および「ナノ粒子の最大サイズ」は、「ナノ粒子の最大寸法」または「ナノ粒子の直径」を意味する。透過電子顕微鏡(TEM)を用いて、ナノ粒子のサイズを測定することができる。同様に、動的光散乱(DLS)を用いて、溶液中のナノ粒子の流体力学直径を測定することができる。これらの2つの方法は、一方を行った後に他方を行って、サイズ測定値を比較し、該サイズを確認するために用いてもよい。好ましい方法は、DLSである(参考文献:国際標準 ISO22412 粒子サイズ分析-動的光散乱、国際標準化機構(ISO) 2008)。
本明細書で定義されるナノ粒子の最大寸法は、典型的に、約5 nm〜約250 nm、好ましくは約10 nm〜約100 nmまたは約200 nm、さらにより好ましくは約20 nm〜約150 nmである。
粒子の形状は、その「生体適合性」に影響を及ぼしうるので、まったく均一な形状を有する粒子が好ましい。薬物動態学的理由から、形状において本質的に球形、円形または卵形であるナノ粒子が好ましい。このような形状は、また、細胞とのナノ粒子相互作用または細胞による取り込みを好む。球形または円形の形状が、特に好ましい。
典型的に、最大寸法は、円形もしくは球形の形状のナノ粒子の直径であるか、または卵形もしくは楕円形の形状のナノ粒子の最大長さである。

実効原子番号は、原子番号と類似した用語であるが、原子よりもむしろ、化合物(たとえば、水)および異なる材料の混合物について用いられる。実効原子番号は、化合物または材料の混合物についての平均原子番号を計算する。実効原子番号は、Zeffと略記される。

ナノ粒子を構成する無機材料が酸化物である場合、この酸化物が、セリウム(IV)酸化物(CeO2)、ネオジウム(III)酸化物(Nd2O3)、サマリウム(III)酸化物(Sm2O3)、ユーロピウム(III)酸化物(Eu2O3)、ガドリニウム(III)酸化物(Gd2O3)、テルビウム(III)酸化物(Tb2O3)、ジスプロシウム(III)酸化物(Dy2O3)、ホルミウム酸化物(Ho2O3)、エルビウム酸化物(Er2O3)、ツリウム(III)酸化物(Tm2O3)、イッテルビウム酸化物(Yb2O3)、ルテニウム酸化物(lu2O3)、ハフニウム(IV)酸化物(HfO2)、タンタル(V)酸化物(Ta2O5)、レニウム(IV)酸化物(ReO2)、ビスマス(III)(Bi2O3)から選ばれるのが有利である。本発明との関連において、無機酸化物は、本発明のナノ粒子を製造するのに用いられうる。
ナノ粒子を構成する無機材料が硫化物である場合、この硫化物が、硫化銀(Ag2S)であるのが好ましい。
ナノ粒子を構成する材料(晶子または晶子の凝集体)の電子密度は、電子/cm3 (e-/cm3)で表される材料の体積当りの電子の数である。
電子密度は、以下の方程式を用いて計算される:
i. ρe-materialは、ナノ粒子を構成する材料の電子密度に対応し、cm3当りの電子の数(e-/cm3)で表される;
ii. dmaterialは、ナノ粒子を構成する材料の理論的(かさ)密度(Handbook of Chemistry and Physics (David R. Lide Editor-In-Chief、88th Edition 2007-2008)の4−43頁に記載の無機化合物の物理定数の表を参照)に対応し、g/cm3で表される;
iii. e- materialは、ナノ粒子を構成する材料のグラムあたりの電子の数(The Physics of Radiation Therapy Fourth Edition、Faiz M. Khan 2010の63頁の表5.1などを参照)に対応し、電子/g (e-/g)で表される。

好ましい実施態様において、対象の組成物を製造するために本発明に関連して用いられるナノ粒子は、ステルス特性を示す物質から選ばれる生体適合性材料でコーティングされうる。実際、本発明のナノ粒子を、静脈内(IV)経路で患者に投与する場合、ステルス特性を示す物質から選ばれる材料による生体適合性コーティングは、ナノ粒子の生体内分布を最適化するのに特に有利である。該コーティングは、ナノ粒子のいわゆる「ステルス特性」を担当する。
本発明の特定のナノ粒子は、さらに、標的細胞に存在する認識要素との、その相互作用を可能にする標的化剤を含むことができる。このような標的化剤は、いったんナノ粒子が標的部位に蓄積されると、典型的に、作用する。標的化剤は、ヒトまたは動物の身体分子に対する親和性を示す任意の生物学的または化学的構造でありうる。たとえば、それは、ペプチド、オリゴペプチドまたはポリペプチド、タンパク質、核酸(DNA、RNA、SiRNA、tRNA、miRNAなど)、ホルモン、ビタミン、酵素、病原細胞によって発現される分子のリガンド、特に腫瘍抗原のリガンド、ホルモン受容体、サイトカイン受容体または増殖因子受容体でありうる。該標的化剤は、たとえば、LHRH、EGF、葉酸塩、抗-B-FN抗体、E-セレクチン/P-セレクチン、抗-IL-2Rα antibody、GHRHなどから選ばれうる。
本明細書に記載のもう1つの目的は、好ましくは医薬的に許容しうる担体またはビヒクルとともに、上述したナノ粒子を含む医薬組成物に関する。
組成物は、固体、液体(懸濁液中の粒子)、エアロゾル、ゲル、ペーストなどの形態でありうる。好ましい組成物は、液体またはゲルの形態である。特に好ましい組成物は液体形態である。
組成物は、また、安定剤、甘味料、界面活性剤、ポリマーなどを含むこともできる。
一般に、液体またはゲル形態の組成物は、約0.05 g/L〜約450 g/Lのナノ粒子、約0.05 g/L〜約150 g/Lのナノ粒子、好ましくは少なくとも約10 g/L、20 g/L、40 g/L、45 g/L、50 g/L、55 g/L、60 g/L、80 g/L、100 g/L、150 g/L、200 g/L、250 g/L、300 g/L、350 g/Lまたは400 g/Lのナノ粒子を含む。
組成物中のナノ粒子の濃度は、乾燥抽出物によって測定されうる。乾燥抽出物が、乾燥オーブンにおいてナノ粒子を含む懸濁液の乾燥ステップに続いて測定されるのが理想的である。
本発明のナノ粒子は、
局所(腫瘍内(IT)、動脈内(IA))、皮下、静脈内(IV)、皮内、気道(吸入)、腹腔内、筋肉内、関節内、髄腔内、眼内または経口経路(口から)などの様々な可能な経路、好ましくはIT、IV またはIAを用いて患者に投与されうる。
適切である場合、ナノ粒子の反復注入または投与を行うことができる。
本発明を用いて、任意のタイプの悪性固形腫瘍、特に上皮、神経外胚葉または間葉起源の腫瘍ならびにリンパ腺癌を治療することができる。ナノ粒子は、外科手術では摘出できない進行したステージの腫瘍に用いることもできる。
腫瘍体積イメージングとして、当業者に周知のX線撮影、コンピュータ断層撮影(CT)、磁気共鳴イメージング(MRI)、超音波(US)、単光子放射断層撮影(SPECT)およびポジトロン放射断層撮影(PET)が挙げられる。すべてのこのような技術は、腫瘍体積を評価するために用いられる。CTおよびMRIは、治療計画のために最も一般的に使用される手順である。
本発明のナノ粒子は、高電子密度材料でできている。該ナノ粒子は、本質的に放射線不透過性(すなわち、それらはX線を吸収する)であり、典型的にはX線撮影およびコンピュータ断層撮影を用いて容易に視覚化することができる。
したがって、組成物の体積(Vc)−ナノ粒子によって占められた体積(典型的には120または150より上のHU)−を計算することができる。
組成物の体積(Vc)の電子密度は、CTスキャナーを用いて確立された較正曲線を用いて計算される。
第一のステップで、組成物の体積(Vc)−ナノ粒子によって占められた体積(典型的には120または150または200より上のHU)を計算する。
較正曲線から、ナノ粒子の平均(mean)濃度が計算される(Xmean g/L)。
ρe-C=組成物の体積の電子密度(電子の数/cm3)
ρe-eau=水(eau)の電子密度
ρe-material=ナノ粒子を構成する材料の電子密度。
本発明のナノ粒子は、多くの分野で、特に人間医学または獣医学で用いることができる。本明細書に記載の、本発明に対応するナノ粒子および組成物は、
動物、好ましくは哺乳動物において(たとえば、獣医学に関連して)、さらに好ましくはヒトにおいて、治療薬として、特に腫瘍学において、好ましくはナノ粒子が電離放射線に当てられる場合に用いるのが好ましい。電離放射線として、典型的に、X線、ガンマ線、UV線、電子ビームならびに中性子、炭素イオンおよび陽子などの粒子ビームが挙げられる。
腫瘍体積(Vt)の2〜50%を占める体積(Vc)を有する組成物であって、対応する水の体積1(Vw1)の電子密度の少なくとも5倍の電子密度の体積(Vin)を有する無機ナノ粒子を含む組成物を患者に投与すること;および
患者の腫瘍を電離放射線に当てること;
を含む方法に関する。
量および予定(分割線量、分割デリバリー方式、全線量単独または他の抗癌剤との併用で、などのいずれであっても)は、任意の疾患/解剖学的部位/病期患者の設定/患者の年齢(児童、成人、高齢の患者)について決定され、任意の特定の状況のためのケア標準を構成する。
2〜50 keVの表面X線:表面に近いナノ粒子を励起するため(数ミリメートルの貫通);
50〜150 keVのX線:診断および治療用;
組織厚6 cmを貫通することができる200〜500 keV(オルソボルテージ)のX線;
1000 keV〜25,000 keV(メガボルテージ)のX線。
腫瘍サイズ評価(解剖学的応答基準)
腫瘍量における変化の評価は、癌両療法の臨床的評価の重要な特徴である:腫瘍縮小(客観的応答(objective response))および疾患進行の両方が、臨床試験における有用な終点である。
抗腫瘍効果の証拠に対して新たな治療薬をスクリーニングするための有意義な終点としての腫瘍退縮(腫瘍サイズの縮小)の使用は、多くの固形腫瘍について、一部の患者において腫瘍縮小を生み出す治療薬が、全体的な生存または生活の質における改善を続いて実証する合理的な機会を有することを示唆する長年の証拠によって支持される。
1981年に、世界保健機関(WHO)が、最初に、腫瘍応答基準(tumor response criteria)を発表した。RECIST(固形腫瘍における応答評価基準(Response Evaluation Criteria in Solid Tumors))として知られる新たな基準は、2000年および2009年に発表された。上述の腫瘍応答基準において、CT、MRIまたはその他の化学技術などのイメージング技術が腫瘍サイズの評価のために用いられる。
典型的には術前療法後に、組織学的検査を用いて残留癌細胞を検出する。現在、網羅的ではないが、日本の病理学的応答基準、Abersen分類、GEPARDO分類、NSABP B18分類などの臨床試験で採用される定義を用いて、原発病変の病理学的応答の評価を行うことができる。
ダイナミック造影MRI、拡散強調MRI、MR分光法、光学的イメージングおよび造影超音波法などの治療モニタリングのためのいくつかの追及された分子イメージングアプローチのうち、グルコース類縁体である18F-FGDを用いるPET((18)フルオロデオキシグルコースポジトロン放射断層撮影)が、臨床的に最も使用される。
腫瘍による18F-FGD取り込みにおける低下が少ないかまたは無いことが、腫瘍応答の不在の兆候として見られた。経時18F-FGD取り込みにおける持続的低下を示す腫瘍は、治療の終了時に組織学による完全な病理学的応答を有する可能性が最も高い。腫瘍18F-FGD取り込みは、また、効果的治療による腫瘍サイズの低下と比べて、より急速に低下した。
2つの基本的なアプローチは、治療の代謝の変化を評価するために考慮されうる。定量分析について、SUV(正規化取り込み値(Standardized Uptake Value))は、トレーサーの組織蓄積を評価するために広範に用いられる測定基準である。SUVは、体重、除脂肪体重(SUL)または体表面積に対して正規化することができる。典型的に、腫瘍SUVにおける低下パーセンテージは、療法に対する客観的腫瘍応答を評価するための定量化の方法として用いられてもよい。しかしながら、絶対SUV決定を用いてもよい。
癌細胞によって血液中に特異的に分泌されるマーカーの測定を用いて、療法に対する腫瘍応答をモニターしてもよい。前立腺癌、卵巣癌、および甲状腺癌などのいくつかの悪性腫瘍において、腫瘍マーカー(前立腺特異的抗原、CA125およびサイログロブリン)を用いて治療に対する腫瘍応答をモニターする。
特に放射線路療法に関して、本明細書に記載される放射線に供されるナノ粒子は、すべての異なる癌療法プロトコルと共同で用いられうる。このようなプロトコルは、外科手術、放射線手術、化学療法、細胞増殖抑制剤(複数可)、細胞毒性剤(複数可)の投与、標的療法、免疫療法、放射性核種、特に免疫放射性核種、および癌を治療することを意図する任意の他の生物学的生成物または無機生成物から選択されうる。
ベースラインにおける腫瘍体積(cm3);
腫瘍内に注入され、ベースラインにおける腫瘍体積の2.5%に対応するナノ粒子(ハフニウム酸化物材料から構成される)懸濁液の体積である組成物体積(cm3);
53g/Lに等しいナノ粒子濃度;
水分子から構成される同じナノ粒子(体積Vw1)の電子密度に対する各ナノ粒子(体積Vin)の電子密度;
Claims (11)
- 腫瘍を有するヒト患者において、組成物の体積(Vc)が腫瘍体積(Vt)の2〜50%を占める場合、および腫瘍が電離放射線に当てられる場合に、(i)腫瘍体積中の30%以上の癌細胞の破壊、または(ii)少なくとも20%以上の腫瘍サイズの減少を誘導する組成物であって、対応する水の体積1(Vw1)の電子密度の少なくとも5倍の電子密度の体積(Vin)を有する無機ナノ粒子を含む組成物。
- 該組成物の体積(Vc)が、対応する水の体積2(Vw2)の電子密度の少なくとも3%の電子密度を有する、請求項1に記載の組成物。
- 組成物が、無機ナノ粒子が7x1022個以上の電子を腫瘍塊に提供する場合に腫瘍体積中の70%以上の癌細胞の死滅を誘導するための組成物である、請求項1または2に記載の組成物。
- 組成物が、無機ナノ粒子が3x1022個以上の電子を腫瘍塊に提供する場合に腫瘍体積中の44%、好ましくは47%以上の癌細胞の死滅を誘導するための組成物である、請求項1〜3に記載の組成物。
- ナノ粒子の無機材料が、少なくとも7の理論的(かさ)密度および少なくとも25の実効原子番号 (Zeff)を有する、請求項1〜4のいずれか1つに記載の組成物。
- 無機材料が、酸化物、金属、硫化物またはその任意の混合物から選ばれる、請求項5に記載の組成物。
- ナノ粒子の最大直径が、約5 nm〜約250 nmである、請求項1〜6のいずれか1つに記載の組成物。
- 組成物が、液体またはゲル形態の医薬組成物である、請求項1〜7のいずれか1つに記載の組成物。
- 腫瘍が、悪性固形腫瘍またはリンパ腺がんである、請求項1〜8のいずれか1つに記載の組成物。
- 無機ナノ粒子が、主要な医薬組成物、動脈内または静脈内経路から選ばれる経路で患者に投与される、請求項1〜9のいずれか1つに記載の組成物。
- 電離放射線源が、X線、典型的には50 KeV〜6 MeVのX線、イオンビーム、電子ビーム、ガンマ線、放射性同位体から選ばれる、請求項1〜10のいずれか1つに記載の組成物。
