Abstract
Description
本出願は、2012年10月31日に出願された米国仮特許出願第61/720,912号および2013年3月15日に出願された米国仮特許出願第61/786,500号の優先権を主張する。上記出願の内容は、全体を参考で本明細書中に引用される。
本明細書に開示される発明は少なくとも一部が国立衛生研究所の認可番号1 DP2 OD006506−01に基づく政府の支援によりなされた。したがって、米国政府が、本発明について特定の権利を有しうる。
本発明は、結腸癌の診断および処置に関する。
結腸癌は、一般的に結腸直腸癌(colorectal cancer)または大腸癌(bowel cancer)として知られているが、結腸もしくは直腸での、または虫垂での制御されない細胞増殖による癌である。例えば、Cancer Genome Atlas Network, "Comprehensive molecular characterization of human colon and rectal cancer," Nature 487: 330-337 (19 July 2012)を参照。米国では2番目に最も頻繁に診断される悪性腫瘍であり、2番目に最も一般的な癌による死亡原因である。例えば、初期の限局的な段階で検出された結腸直腸癌の患者の5年生存率は92%であるが、癌が付近の器官やリンパ節に転移している場合には生存率が64%まで低下し、さらに遠隔転移のある患者では7%にまで低下する。
本発明は、結腸癌の診断および処置のための薬剤および方法を提供することによって上記必要性に対応する。
本発明は、少なくとも一部は、共同的なmiRNA−タンパク質ネットワーク(cooperative miRNA-protein network)が転移性結腸癌による肝転移増殖(liver colonization)で無秩序になる(deregulated)というおよびmiRNA、例えば、miR−483−5p及びmiR−551aが脳型クレアチンキナーゼ依存性エネルギー(brain creatine kinase dependent energetics)を共同して標的にすることによって結腸癌転移性の生存(metastatic survival)を抑制するという予想できない知見に基づく。したがって、本発明は、結腸癌、特に転移性結腸癌の診断および処置のための新規な薬剤および方法を提供する。
本明細書に開示されるように、miR−483−5pおよびmiR−551aは、結腸癌や他のタイプの癌で癌細胞の生存、低酸素での生存(hypoxic survival)、転移性の生存(metastatic survival)、または転移増殖(metastatic colonization)の内因性の転移サプレッサー(metastasis suppressors)として同定された一方で、CKBやクレアチントランスポーターチャンネルSLC6a8が同じプロセスの転移プロモーター(metastasis promoter)として機能する。これらのmiRNAまたはタンパク質は、ヒトの転移による結果(metastatic outcome)を強力に予想するだけでなく、結腸癌や他のタイプの癌等の癌を処理するためのターゲットを提供する。
miR−483−5pおよびmiR−551aの双方ならびにこれらをコードする核酸を転移サプレッサーとして用いて、問題となる細胞または必要とする患者でのこれらの過剰発現により発明を実施してもよい。
上述したように、結腸癌の処置にCKBまたはSLC6a8の発現または活性レベルを減少する阻害物質を使用できる。阻害物質(即ち、インヒビター)は、核酸、ポリペプチド、抗体、または小分子化合物であってもよい。一例では、インヒビターは、転写、mRNA安定性、翻訳、タンパク質安定性/分解、タンパク質修飾、およびタンパク質結合のレベルで機能する。
適当な担体および上記治療剤の1つ以上を含む組成物は、本発明の範囲に含まれる。本組成物は、製薬上許容できる担体を含む薬剤組成物、飲食に許容できる適当な担体を含む飲食用組成物、または化粧品として許容できる担体を含む化粧品組成物であり得る。
上記遺伝子は、被検者が転移性結腸癌を有するまたは有する危険性があるか否かを決定するのに使用できる。または、上記遺伝子は、被検者のこのような疾患の予後を決定するのに使用できる。
一態様では、本発明は、被検者が、転移性結腸癌を有するもしくは転移性結腸癌にかかりやすい素因を有する、転移性結腸癌が再発しやすい素因を有する、化学療法に耐性のある結腸癌、もしくは癌細胞の生存、低酸素での生存(hypoxic survival)、転移性の生存(metastatic survival)、もしくは転移増殖(metastatic colonization)の特徴を有する他の病気にかかりやすい素因を有するか否かを決定するための定性的及び定量的な情報を提供する。このような疾患を有するまたはこのような疾患にかかりやすい被検者を、被検者の試験サンプルにおける上記遺伝子またはこれらの産物(mRNA、microRNA、またはポリペプチド)の発現レベル、パターン、またはプロフィールに基づいて決定できる。換言すると、産物をマーカーとして用いて、疾患が存在するか存在しないかを示すことができる。本発明の診断および予後診断アッセイは、産物の発現レベルを評価する方法を包含する。本方法によって、疾患を検出できる。例えば、一以上のプロモーター(即ち、CKBまたはSLC6a8)の発現レベルが相対的に増加することは疾患が存在することを示す。これに対して、発現レベルが低くなるまたは発現レベルがないことは疾患が存在しないことを示す。同様にして、1以上のサプレッサー(miR−483−5pまたはmiR−551a)の発現レベルが低くなるまたは発現レベルがないことは疾患が存在することを示し、その一方、発現レベルが相対的に増加することは疾患が存在しないことを示す。
必要であれば、全RNAを、RNEASY精製プラットフォーム(RNEASY purification platform)(QIAGEN, Inc., Valencia, CA)等の、自動適合(automation-compatible)96ウェルフォーマットでシリカを用いた単離を用いて細胞溶解物(または他のタイプのサンプル)から精製してもよい。シリカ被覆ビーズまたはグアニジンを用いた抽出等の、他のRNA単離方法が考えられる。RNA単離及び調製のためのさらなる方法は、当業者によって考案されうる。
上記診断方法は、結腸癌または転移性結腸癌の再発を有するまたはこれらを発症する危険性のある被検者を同定できる。加えて、生体サンプル、例えば、末梢血サンプルにおける上記遺伝子の発現レベルおよび/または傾向の変化は、回復または回復していないことの初期の指標を提供できる。例えば、プロモーター遺伝子(もしくはインヒビター遺伝子)のさらなる増加(もしくは減少)または永続的に変化した遺伝子発現レベルは、予後診断が乏しい、即ち、改善しないないまたは健康が減退している(もしくは予後が悪い)ことを示す。したがって、これらの遺伝子によって、結腸癌の処置後の回復を評価できる。この所定の遺伝子群またはそのサブセットの分析は状態の経過を示す。
バイオチップまたはアレイもまた本発明で提供される。バイオチップ/アレイは、本明細書に記載される結合プローブまたは複数のプローブを有する固体または半固体基材を含んでもよい。プローブは、ストリンジェントなハイブリダイゼーション条件下で標的配列にハイブリダイズできてもよい。プローブは、基材上の空間的に規定された場所で結合してもよい。1つの標的配列あたり1超のプローブを使用してもよく、この際、重複プローブであってもまたは特定の標的配列とは異なる部分に対するプローブであってもよい。プローブは、当業者に認識される単一の疾患に関連する標的配列とハイブリダイズできるものであってもよい。プローブは、まず合成された後バイオチップに結合されても、またはバイオチップ上に直接合成されてもよい。
他の態様では、本発明は、本明細書に記載されるポリペプチド及びmicroRNAの発現の分析のための方法、組成物、およびシステムを具現化するキットを提供する。このようなキットは、本明細書に記載される核酸および下記のいずれかまたは全てを含んでもよい:アッセイ試薬、バッファー、プローブおよび/またはプライマー、ならびに滅菌生理食塩水もしくは他の製薬上許容できるエマルジョンおよび懸濁液ベース。加えて、キットは、本明細書に記載される方法を実施することを目的として指示書(例えば、プロトコル)を含む指示材料を有していてもよい。例えば、キットは、標的mRNAまたはmicroRNA配列の増幅、検出、同定または定量化用のキットであってもよい。これを目的として、キットは、適当なプライマー(例えば、ヘアピンプライマー)、フォワードプライマー、リバースプライマー、及びプローブを有していてもよい。
本発明は、結腸癌を処置するのに、または癌細胞の生存、低酸素での生存(hypoxic survival)、転移性の生存(metastatic survival)、もしくは転移増殖(metastatic colonization)を阻害するのに有用である化合物の同定方法を提供する。
本実施例では、下記実施例2〜15に使用される材料および方法を記載する。
ルシフェラーゼレポーターを発現する1×106個のLS174T細胞を、20μl容の1:1のPBS/Matrigel混合物中に懸濁し、NOD−SCIDマウスの肝臓に肝臓内注射した。転移小塊(Metastatic nodules)を3〜4週間の期間発達させて、生物発光イメージングによってモニターした。形成した小塊を切除して、コラゲナーゼ及びヒアロニダーゼ消化によって解離させ、単細胞懸濁液を得た。細胞をインビトロで拡張した後、マウスに再注射した。インビボ選択を3回繰り返した後、転移性の高い(highly metastatic)LvM3a及びLvM3b誘導細胞系を確立した。
各細胞が単一のmiRNAを過剰に発現するように、低い感染の多重度(MOI)で611個のmiRNAのレンチウィルスLenti−miRライブラリー(lentivirus Lenti-miR library of 611 miRNAs)(System Biosciences)を細胞に形質導入した。次に、過剰発現すると、肝転移増殖(metastatic liver colonization)を促進または抑制したmiRNAをインビボで選択するために、形質導入した集団をNOD−SCIDマウスに肝臓内注射した。レンチウィルスのmiRNA挿入物のゲノムDNA PCR増幅および回収を、製造社のプロトコルに従って注射前に肝臓小塊の細胞で行った。miRNAアレイプロファイリングによって、インビボ選択の前後にmiRNA挿入物の定量を行った。
注射する細胞を細胞トラッカー赤または緑(cell-tracker red or green)(Invitrogen)で標識し、脾臓内注射によりNOD−SCIDマウスの肝臓に接種した。次に、肝臓を取り出し、McIlwain組織チョッパー(McIlwain tissue chopper)(Ted Pella)を用いて150μmスライスに切り、器官型組織培養インサート(organotypic tissue culture inserts)(Millipore)に播種し、肝細胞維持サプリメントパック(Hepatocyte Maintenance Supplement Pack)(Invitrogen)を添加したウィリアムE培地(William’s E Medium)中で培養した。所定の期間経過後、肝臓スライスをパラホルムアルデヒドで固定し、多光子顕微鏡を用いて画像を撮影した。
インビボでカスパーゼ活性を測定するために、VivoGlo Caspase 3/7基質(VivoGlo Caspase 3/7 Substrate)(Z-DEVD-Aminoluciferin Sodium Salt, Promega)を使用した。ルシフェリンは、DEVDペプチドがアポトーシスを起こした細胞で活性化したカスパーゼ−3から切断されるまで不活性である。DEVD−ルシフェリンを、ルシフェラーゼを発現する結腸直腸癌細胞を有するマウスに注射した。アポトーシスを起こした細胞によって活性化したら、生物発光イメージング(bioluminescence imaging)を行うことによってインビボでのカスパーゼ活性を測定できる。インビボでのカスパーゼ活性を測定してから5時間後に、マウスに標準化を目的として標準ルシフェリンを注射した。
miR−483−5p及びmiR−551aを、scAAV.GFP(Plasmid 21893, Addgene)のBglII及びNotI部位にタンデムにゲノム配列が隣接した双方のmiRNAプレカーサーから構成されるポリシストロン(polycistron)としてクローニングした。miR−483−5p及びmiR−551aをコードするゲノム配列(配列番号:5及び6)、相当するプレカーサー配列(下線、配列番号:3及び4)、ならびに相当する成熟microRNA配列(下線および太字、配列番号:1及び2)を以下に示す。アデノ随伴ウィルスを、Cell Biolabs製のAAV−DJヘルペスフリー発現系(AAV-DJ Helper Free expression system)を用いてパッケージ、精製し、さらに力価を測定した(titered)。
CKB及びSLC6a8を標的とするshRNAヘアピン(shRNA hairpins targeting CKB and SLC6a8)を発現するpLKOベクターを、Sigma-Aldrichで注文した。転写物レベルを最良にノックダウンした2個の独立したヘアピンをすべての実験に使用した。これらのヘアピンDNA及びRNA配列を以下に挙げる。
マウスを、10mgのシクロクレアチンまたは生理食塩水ベヒクルで処置し、腹腔内注射により投与した。上記処置レジメを腫瘍細胞を接種してから1日後に開始し、マウスを安楽死させるまで続けた。β−グアニジノプロピオン酸を腹腔内注射により0.5M溶液 200μlの投与量で投与した。シクロクレアチン処置についても同様の処置レジメを行った。
結腸癌の肝転移増殖(liver colonization)の分子調節剤を同定する最初の段階として、免疫不全マウスに癌細胞を繰り返し肝臓内注射した後肝臓コロニーを外科的に切除し細胞を解離させることにより、肝転移増殖の促進用のLS−174Tヒト結腸癌系で、インビボ選択を行った。より詳細には、5×105個のLS−Parental、LvM3a及びLvM3b細胞による肝転移増殖を、生物発光によって直接肝臓内注射後に試験した。注射してから21日目にマウスの画像を撮影し、肝臓をエクスビボ撮影(ex vivo imaging)及び肉眼による形態学的試験(gross morphological examination)用に肝臓を摘出した。グループの光束子率(Photon flux ratios)を得て、比較した。3代目の肝臓コロナイザー(third-generation liver colonizers) LS−LvM3a及びLS−LvM3bはこれらの親系に対して肝臓内注射時に肝転移増殖能が劇的に(>50倍)促進したことが分かった。重要なことに、これらの誘導体はまた、転移増殖アッセイ(metastatic colonization assays)において門脈循環注射時に肝転移能が劇的に(>150倍)促進したことから、肝転移増殖能が結腸癌の転移進行のカギとなる段階であることが示された。これらの生物発光アッセイでは、グループのそれぞれの光子束率のすべてのP値は、片側スチューデントt−検定(on one-sided Student’s t-tests)に基づき、0.05、0.001、または0.0001未満であることが分かった。
本実施例では、これらのmiRNAの内因性レベルが同系の転移性の低い細胞に比べて転移性の高い細胞でサイレンシングを示す(exhibit silencing)か否かを試験するアッセイを行った。確かに、miR−483−5p及びmiR−551aは、親系に対して転移性の高いLS−LVM3a及びLS−LVM3b肝臓コロナイザー(liver colonizers)でならびに同系の親系に対して転移性SW602誘導体でサイレンシングされることが分かった。肝転移増殖(liver colonization)でのこれらのmiRNAの抑制の役割と一致して、miR−483−5pまたはmiR−551aの過剰発現はLS−LvM3b細胞による転移増殖を確実に抑制した一方、転移性の低い親系LS−174T及びSW480での内因性のmiR−483−5pまたはmiR−551aの阻害は肝転移増殖を有意に促進した。
本実施例では、これらのmiRNAが抗転移効果を発揮するメカニズムを調べるアッセイを行った。miR−551a阻害はインビトロでは増殖に効果を奏さない一方で、miR−483−5p阻害は増殖を向上したため、転移進行へのこれらのmiRNAの効果は増殖能の調節に二次的に起こるものではなかった。加えて、これらのmiRNAの過剰発現は癌細胞の浸潤能またはアポトーシス速度を変化させなかった。これらのmiRNAが転移に影響を及ぼすメカニズムを決定するために、これらのmiRNAを過剰発現する細胞が進行しなくなる(display a defect in progression)転移プロセス中の時点を同定するアッセイを行った。驚くべきことに、肝転移増殖アッセイのために門脈循環に細胞を注射してから24時間という初期の段階で、これらのmiRNAを過剰発現する細胞がコントロールヘアピンを発現する細胞に対してこれらの提示(representation)において競合して負ける(out-competed)ことが分かった。
これらのmiRNAが肝転移増殖を抑制するメカニズムを解明するために、インビトロの器官型スライス培養系を開発した。この系により、肝臓微小環境における結腸癌細胞の単細胞播種後の肝転移中の所期事象を研究することが可能になった。転移増殖中の細胞生存への有意な選択に関する従来の研究と一致するが、時間の関数として肝臓微小環境内で細胞数が大きく減少した。転移性の高いLvM3bコロナイザー細胞(colonizer cells)は、転移性の低い親系に比して肝臓微小環境での維持が有意に良好であった−この結果は転移進行での肝臓内持続に関する積極的な役割と一致する。
本実施例では、これらのmiRNAの下流エフェクターを同定するアッセイを行った。転写プロファイリングにより、各microRNAの過剰発現によってダウンレギュレーションされ、miRNAに相補的である3’−UTRまたはコーディング配列(CDS)要素を含む転写物を同定した。興味深いことに、脳型クレアチンキナーゼ(CKB)が双方のmiRNAの標的として可能性のあるものとして同定されたことから、これらのmiRNAは、共通のインビボ及び器官型表現型を示すが、共通の標的遺伝子を介して効果を調節することが示唆された。確かに、定量的PCR検証により、microRNAの過剰発現時のCKB転写物レベルの抑制が示された。転写性の高いLvM3b細胞におけるCKBの発現レベルはmiR−483−5p及びmiR−551aの過剰発現によって抑制されることが分かった。さらに、内因性miR−483及びmiR−551aは、内因性CKBタンパク質レベルを抑制することが分かった。例えば、CKBの発現は、内因性のmiR−483−5p及びmiR−551aaがLNAで阻害される転移性の低いSW480細胞ではアップレギュレートされることが分かった。突然変異誘発およびルシフェラーゼを用いたレポーターアッセイから、miR−483−5p及びmiR−551aが3’UTRまたはCKBのCDSを直接標的とすることが示された。このため、CKBコーディング配列及び3’−UTRのルシフェラーゼレポーターアッセイを行った。miR−483−5p及びmiR−551aが、それぞれ、3’−UTR及びCKBコーディング配列の相補領域を標的とすることが分かった。これらのアッセイを、変異させた相補領域で同様にして行い、少なくとも3回行った。
本実施例では、CKBが結腸癌による肝転移増殖に十分かつ必要であるかどうかを調べるアッセイを行った。
CKBは、ホスホクレアチンからADPへの高エネルギーリン酸基の移動を触媒してATP及びクレアチンを生成することによって脳や腎臓等の組織の迅速に移動する高エネルギーリン酸塩のレザーバーを調節することが知られている。ホスホクレアチンからのATPのCKB生成は肝転移増殖中にエネルギー面での利点を結腸癌細胞に提供する可能性があるとの仮説があった。CKB触媒反応の最終産物である、ATPがCKBノックダウン時にみられる転移抑制を救出できるか否かを決定するために、実験転移アッセイで細胞を注射する前にCKBノックダウン細胞にATPを充填した。簡単にいうと、CKBをノックダウンしたまたはしない5×105個のLvM3bを注射し、100μM ATPまたはベヒクルで予め処置したマウスで、肝転移を試験した。生物発光イメージングによって転移負荷(Metastatic burden)をモニターし、注射してから21日目に安楽死させた。細胞のATP充填がCKBが枯渇した細胞で抑制された転移表現型を10倍以上有意に促進するのに十分であることが分かった。ATP充填がショートヘアピンコントロールを発現する細胞の転移活性を促進しなかったため、ATPによる救済は特異的であった。
結腸癌転移進行を制御するこの協力的miRNA調節ネットワームがヒト疾患と関連があるか否かを決定するために、1セットの67個の原発性結腸癌およびMSKCCで患者から得られた肝転移で、miR−483−5p及びmiR−551aの発現レベルを分析した。より詳細には、37個の原発性腫瘍サンプル及び30個の肝転移サンプルにおけるmiR−483−5p及びmiR−551aレベルを定量的リアルタイムPCRによって定量した。転移進行中のこれらのmiRNAに関する転移抑制役割と一致するが、miR−483−5p及びmiR−551aは双方とも、原発性結腸癌に対してヒト肝転移での発現レベルが有意に減少した(図1a;miR−483−5pでp<0.05及びmiR−551aでp<0.05;N=67)。
本実施例では、このmiRNA調節ネットワームを標的とすることの治療の可能性を調べるためのアッセイを行った。このために、多数(500k)の転移性の高いLvM3a細胞をマウスに注射し、24時間後に、1本の転写物からmiR−483−5p及びmiR−551aを発現するアデノ随伴ウィルス(AAV)を単回静脈内投与量マウスに注射した。単回治療量の双方のmiRNAをデリバリーするアデノ随伴ウィルス(AAV)が5倍以上転移増殖を劇的にかつ有意に抑制することが分かった(図1c)。
本実施例では、SLC6a8のインヒビターである、小分子B−GPAを投与することによるクレアチントランスポーターチャンネル SLC6a8を標的とする治療の可能性を確認するアッセイを行った。上記したように、LvM3b結腸癌細胞を注射したマウスにB−GPAを投与すると、処置してから2週間後に肝臓への結腸癌転移が阻害された(図1e)。この治療効果を確認するために、LvM3b結腸癌細胞を注射したマウスを、3週間、毎日腹腔内注射によりB−GPAまたはコントロールベヒクル(PBS)で処置した(図2)。3週間目にマウスを安楽死させ、生物発光イメージング及び肉眼による組織観察のために肝臓を摘出した。
本実施例では、SLC6a8を標的とするshRNAノックダウン(shRNA knockdown targeting SLC6a8)を用いたクレアチントランスポーターチャンネル SLC6a8を標的とする治療上の有用性を評価するアッセイを行った。
本実施例では、ヒト結腸癌腫瘍のクレアチントランスポーターであるSLC6a8が転移進行と相関するか否かを調べた。
上記したように、shRNAが介在するノックダウンでクレアチントランスポーターであるSLC6a8を阻害することによって、結腸癌および膵臓癌双方の転移を抑制できることが示された。また、小分子インヒビターであるB−GPAでSLC6a8を阻害することによって、インビボでの結腸癌の転移について治療上の利点があることが示された。B−GPA処置が膵臓癌で治療上の利点があるか否かを評価するために、B−GPA処置のヒト膵臓癌細胞の生存の阻害能をマウスでインビボで評価した。
上記実施例から、B−GPA処置単独で、結腸癌及び脾臓癌について治療上の利点があることが示された。本実施例では、B−GPA処置が化学療法剤である5’−フルオロウラシル及びゲムシタビンの治療活性を促進できるかを調べた。上記を達成するために、細胞生存率アッセイを行い、5’−フルオロウラシルまたはゲムシタビン単独の細胞毒性活性を、B−GPAと組み合わせた治療と比較した。
Claims (24)
- 患者において脳型クレアチンキナーゼ(CKB)またはクレアチントランスポーターチャンネルSLC6a8(SLC6a8)の発現レベルまたは活性を低下させることを有する、結腸癌の処置を必要とする患者の結腸癌の処置方法。
- 前記低下段階が、患者にシクロクレアチンを投与することによって行われる、請求項1に記載の方法。
- 患者に別の治療剤を投与することをさらに有する、請求項2に記載の方法。
- 前記別の治療剤が、β−グアニジノプロピオン酸、5−フルオロウラシル、オキサリプラチン、イリノテカン、カペシタビン、ゲムシタビン、セツキシマブ、タキソール、およびアバスチンからなる群より選択される、請求項3に記載の方法。
- 前記癌が転移性である、請求項1〜4のいずれか1項に記載の方法。
- 患者にβ−グアニジノプロピオン酸を投与することを有する癌を処置する必要のある患者の癌の処置方法であって、前記癌が、結腸癌、膵臓癌、胃癌、肝臓癌、乳癌、前立腺癌、肺癌、およびメラノーマからなる群より選択される一である、方法。
- 患者においてmiR−483−5pおよびmiR−551aからなる群より選択されるmicroRNAの発現レベルを増加させることを有する、結腸癌の処置を必要とする患者の結腸癌の処置方法。
- 前記癌が転移性である、請求項1に記載の方法。
- 前記増加段階が、miR−483−5pまたはmiR−551aをコードする核酸を患者に投与することによって行われる、請求項7または8に記載の方法。
- 前記核酸がオリゴヌクレオチドである、請求項9に記載の方法。
- 前記オリゴヌクレオチドが合成核酸である、請求項10に記載の方法。
- 前記核酸がベクター内にある、請求項9に記載の方法。

- miR−483−5p、miR−551a、またはCKBもしくはSLC6a8をコードする遺伝子の発現産物に特異的に結合する試薬を含む、被検者の結腸癌の転移の可能性を診断するためのキット。
