代謝症候群または代謝疾患とは、一個人において同時期に生じ糖尿病および心血管疾患を引き起こす一群の代謝系危険因子を定義する用語である。代謝症候群の主な特徴には、インスリン抵抗性、高血圧(血圧上昇)、コレステロール異常および凝血に関するリスクの上昇が含まれる。インスリン抵抗性とは、血液から筋肉およびその他の組織への糖グルコースの輸送を促進する際のインスリンの作用に対する細胞の応答能力が低下したことを指す。この危険因子群を有する患者はたいてい過体重または肥満体である。

aP2は、脂肪細胞型脂肪酸結合タンパク質(AFABP)、脂肪酸結合タンパク質-4(FABP-4)および脂肪細胞型脂質結合タンパク質(ALBP)としても公知である。本発明以前まで、aP2は細胞質タンパク質であると考えられていた。本発明により、分泌された脂肪性脂質シャペロンであるaP2が肝糖代謝を調節することが見出された。aP2は、マウスおよびその他の哺乳動物、例えばヒトの細胞において脂肪に分泌されるタンパク質であることが見出された。
aP2が細胞上清に放出されることを確認するため、WTまたはFABP欠損の脂肪細胞由来の馴化培地および細胞溶解物を、aP2の存在についてウェスタンブロットで分析した(図1a)。aP2は馴化培地中に豊富に存在することが見出されたが、脂肪細胞中の2つの細胞質タンパク質であるカベオリンおよびAKTは同じ条件下で検出できなかった(図1a)。脂肪細胞におけるFABPの少ないアイソフォームでありaP2に対して高い相同性を共有するmal1もまた馴化培地中に放出されていた(図1a)。まとめると、両方の脂肪性FABPとも分化した脂肪細胞から分泌される。
脂肪細胞から分泌されるホルモン(すなわちアディポカイン)は、糖代謝および脂質代謝に関与する(Rosen et al., 2006, Nature 444:847-853)。血清aP2は肥満および糖尿病状態で増加するので、増加したaP2が肥満で見られるような糖代謝の変化に関与しているならば肥満における血清aP2の減少が血糖管理を改善するかどうか判定する実験を行った。血清aP2を効果的に枯渇させるため、aP2を特異的に認識する抗体を作製した。この抗体を用いた調査により、この抗体がaP2を極めて高感度で特異的に検出することを確認した(図2a)。この抗体を肥満マウスに注射した。該マウスは16週間にわたる高脂肪食給餌により肥満に誘導した。aP2抗体の投与は、血清aP2濃度を効果的かつ迅速に抑制した(図2B)。このaP2抗体処理は、これらのマウスの体重を変化させなかったが、投与2週間で血糖レベルを有意に低下させた(図2c)。aP2抗体を注射されたマウスではまた、糖処理率も劇的に向上しかつインスリン応答も向上し、これは、耐糖試験およびインスリン耐性試験により確認した(図2dおよび2E)。実際、aP2抗体処理はこれらのマウスにおける食餌性肥満に関係する耐糖能障害を本質的に消滅させ、これにより血清aP2の減少がII型糖尿病を患う人々に臨床的恩恵をもたらすことが示された。
最近15年間における証拠の蓄積により、脂肪組織は代謝調節を担う最大の内分泌器官の一つであることが確認された。全身のエネルギー恒常性に対する脂肪組織の作用は様々なホルモンに仲介される。肥満被験体内の脂肪組織はまたより多くの炎症性サイトカインを産生することも示され、慢性的な脂肪の炎症が肥満と関連する代謝障害とを結びつける重要な特徴であることが明らかになった。したがって、生理学的および病理学的な文脈の両方で、脂肪組織は、栄養シグナル伝達分子と内因性シグナル伝達分子が相互作用して統合され、最終的にはエネルギー恒常性の全身的な調節が行われている重要な場所であるといえる。脂肪組織はまた、体内の主要な脂質保管部位として、空腹時の重要なエネルギー提供元でもある。脂肪性脂肪分解により脂肪酸の大部分が血清に提供され、これらは筋肉に取り込まれて酸化され、かつ肝臓における糖産生も活性化する。脂肪分解に伴う肝糖産生の亢進は重要な恒常性維持現象であり、これは肥満状態では調節不全に陥ることが知られている。本発明より以前、このプロセスが脂肪組織と肝臓の間で合図される機序は完全に理解されていなかった。
aP2およびmal1においてホモ接合型のnull変異を有するマウスに対して12世代超の戻し交配を行い、C57BL/6Jの遺伝的背景とした。マウスは通常固形飼料食(RD)で維持するか、または4週齢から高脂肪食(Research Diets, Inc)を20週間与えて食餌性肥満を誘導した。レプチン欠損(ob/ob)マウスはJackson laboratoryから購入した。全てのマウスを12時間照明・12時間消灯のサイクル下で維持した。耐糖試験およびインスリン耐性試験には標準的な方法を使用した。
食物停止の6時間後に尾採血によりマウスから血液を採取し、微小遠心分離機を用いて13,000rpmで30分間回転させた。血清aP2はELISAシステム(Biovendor Inc.)を用いて測定した。血清aP2の栄養調節をモニタリングするため、消灯サイクルの開始直前にマウスから血液サンプルを採取し、その後動物を食物のないケージに入れた。絶食24時間後、二セット目の血液サンプルを採取して、食物を与えた。最後の採血は給餌再開の4時間後に行った。脂肪分解時のaP2レベルを測定するため、12匹のマウスからベースラインレベルの血液を採取した。この後、6匹のマウスにイソプロテレノール(10mg/kg体重)を注射し、残りの6匹にビヒクル対照を与えた。aP2測定用に、注射から15分後に血液サンプルを採取した。
6週齢のレシピエントマウスに対して、セシウム源からの線量5Gyの照射を、放射線の毒性を最小限に抑えるために4時間の間隔をあけて2回行った(計10Gy)。骨髄は、同性のドナーマウス(6〜8週齢)由来の大腿骨および脛骨をGibco RPMI 1640培地(Invitrogen, Carlsbad, CA)でフラッシングすることによって回収した。2回目の照射から4時間後に、0.2mlの培地中の5×106個の骨髄細胞を眼窩静脈叢に注射した。骨髄移植の1週間前および4週間後、100mg/lのネオマイシンおよび10mg/lの硫酸ポリミキシンBを酸性水に加えた。
6x Hisタグを有する組換えaP2または対照Gusタンパク質を大腸菌(E. coli)中で作製し、B-PER 6x His Spin Purification Kit(Pierce Biotechnology, Inc)を用いて精製した。市販のシステム(Lonza Inc.)を用いて内毒素を除去することによりタンパク質をさらに精製した。100μgの対照またはaP2タンパク質を、通常食で維持したWTマウスに1日2回、2週間にわたり注射した。マウスaP2に対するウサギポリクローナル抗体は、組換え全長aP2タンパク質を用いて作製し、この抗体を、NAb(商標) Spinシステム(Pierce Biotechnology, Inc)を用いて最後の採血の血清から精製した。免疫前血清を同様に精製し対照として用いた。精製抗体を生理食塩水で1μg/μlに希釈し、高脂肪食(Research diet, Inc)で維持したマウスに対して50mg/kg量で注射した。
Flagタグ付きのGFP(プラスミドID 10825)およびAKT(プラスミドID 9021)をAddgeneから入手した。Flagタグ付きの全長aP2および入口(portal)を欠くaP2をpEGFP-C1にクローニングした。aP2 K22, 59I変種体を、Quickchange変異誘発システム(Stratagene)を用いて作製した。Lipofectamine 2000(Invitrogen Corporation)を用い、示された構築物をHEK293細胞にトランスフェクトした。
HEK293細胞は10%ウシ胎仔血清を含むDMEM中で維持した。FABP欠損細胞株は以前に記載された通りに樹立した。Cao, H. et al. Identification of a lipokine, a lipid hormone linking adipose tissue to systemic metabolism. Nat Med 7, 699-705 (2001)。前脂肪細胞は10%仔ウシ血清を含むDMEM中で維持し、標準的な分化プロトコルを用いて10%コスミック仔ウシ血清(cosmic calf serum;CCS)を含むDMEM中で脂肪細胞に分化させた。脂肪分解を誘導するため、分化した脂肪細胞をホルスコリン20nMまたはIBMX 1mM/ジブチリルcAMP 1mMで1時間処理した。脂質処理のために、0.25mMのパルミチン酸またはステアリン酸を10%CCS含有DMEMに溶解し、12ウェルプレート内で培養した脂肪細胞に添加した。1時間後に、培地を、同一の脂質含有培地と交換し、さらに1時間後に馴化培地を回収した。脂肪外植片を採取するため、精巣上体の脂肪組織貯留部をマウスから切り出し、PBS中で2回濯いだ。次いで、脂肪組織サンプルを10%CCS含有DMEM中に移し、平均サイズ1〜2mmになるよう刻んだ。この組織外植片をDMEMで十分に洗浄し、10%CCS含有DMEM中で培養した。脂肪分解は、脂肪細胞と同じ方法で誘導した。
細胞を6ウェル組織培養プレートのカバースリップ上で培養し、4%パラホルムアルデヒドで固定した。DAPIで核を染色し、カバースリップを、ProLong Gold褪色防止剤(Invitrogen Corporation)を含んだスライド上にマウントした。イメージングはZeiss Observor Z1蛍光顕微鏡で行った。
脂肪細胞からエキソソームを単離するため、馴化培地を採取し、1,200gで10分間遠心分離した。次いでこの培地を、0.45μmフィルターを通してろ過し、10,000gで30分間の遠心分離に2回供し、20%スクロース勾配を負荷した。エキソソーム画分を100,000gで150分間の遠心分離によりペレット化した。血清からエキソソームを単離するため、血液をマウスから採取し、微小遠心分離機において最高速度で30分間遠心分離して血漿を得た。血漿を等量のPBSで希釈し、20%スクロース勾配を負荷し、エキソソームを200,000gで90分間の遠心分離によりペレット化した。
総RNAはTrizol試薬(Invitrogen)を用いて肝臓組織から単離した。逆転写は、1μgのRNAを用いてsuperscript一本鎖cDNA合成システム(Applied Biosystems Inc.)により行った。定量的リアルタイムRT-PCRは、PCRサーマルサイクラー(Applied Biosystems Inc.)を用いて行った。PCRプログラムは、以下であった:酵素の活性化のための95℃2分30秒、伸長のための、95℃15秒、58℃30秒、および72℃1分を40サイクル。リアルタイムPCR産物を確認するために融解曲線分析を行った。全ての量を18S rRNAに対して標準化した。使用したプライマー配列は以下の通りであった。
脂肪細胞由来の組織タンパク質溶解物および馴化培地をSDS PAGEゲル上で分離し、Coomassie(Biorad Laboratory)で染色するか、または以下の抗体を用いた免疫ブロットにより検出した: Santa Cruz Biotechnology製のアディポネクチン、Calbiochem製のMFG-E8、BD Bioscience製のカベオリン-1、Cell Signaling Technology製のAKT。Flagタグ付きaP2は、30μlのFlagアガロースビーズ(Sigma)と共に4℃で一晩インキュベートした後に、トランスフェクトHEK293細胞由来の馴化培地4mlを用いて免疫沈降させた。アガロースビーズに結合したタンパク質はSDSローディングバッファーで溶出させ、SDS-PAGEで分離した。
実験4日前に、マウスをケタミン(90mg/kg体重)およびキシラジン(10mg/kg体重)の腹腔内注射により麻酔した。該マウスの右頸静脈に、ヘパリン溶液(100USP U/ml)で満たしたPE-10ポリエチレンチューブ(内径および外径はそれぞれ0.28mmおよび0.61mm;Becton Dickinson, Franklin Lakes, NJ)のカテーテルを挿入した。このカテーテルの遠位端を皮下に潜らせ、肩甲骨間部で露出させた後、結んで固定した。
上述のようにしてマウスにカテーテルを挿入した。高インスリン-正常血糖クランプは既報の手法15の改変により行った。一晩の絶食後、HPLC精製した[3-3H]-グルコース(0.05μCi/分;PerkinElmer Life and Analytical Sciences, Boston, MA)を2時間の基準期間の間に注入し、その最後に血液サンプルを採取して基礎肝糖産生速度を算出した。基準期間の後、12.5mU/kg/分の速度のヒトインスリン(Humulin R; Eli Lilly, Indianapolis, NJ)のプライミング後持続注入(primed-continuous infusion)により120分間の高インスリン-正常血糖クランプを行った。血漿グルコース濃度の直接測定のために血液サンプルを20分間隔で採取し、血漿グルコースを基準濃度で維持するために25%グルコースを様々な速度で注入した。クランプを通じて[3-3H]-グルコースの持続注入(0.1μCi/分)を行いつつ、インスリン刺激後の全身グルコース代謝回転を算出した。全ての注入は、流量制御式微小透析ポンプ(CMA/Microdialysis, North Chelmsford, MA)を用いて行った。血漿中の[3H]-グルコースおよび3H2Oの濃度を測定するために、クランプ開始から80、85、90、100、110および120分後に血液サンプルを採取した。クランプの最後に、動物を屠殺した。5分以内に肝臓組織を取り出し、さらなる分析のために-80℃で保存した。
aP2は、最初に発見されて以来、細胞質タンパク質と考えられていたが、最近、プロテオミクススクリーニングによって分化した3T3-L1脂肪細胞の上清中で、およびその後にヒト血清中で同定された。aP2が脂肪細胞により特異的に分泌されるのか又は細胞の代謝回転の間に放出されるのかを判定するための調査を行った。野生型(WT)またはFABP欠損の脂肪細胞から採取した馴化培地および細胞溶解物におけるaP2レベルの試験は、このタンパク質がWT細胞の馴化培地中に豊富に存在することを明らかにした(図4a)。対照的に、脂肪細胞における2つの豊富な細胞質タンパク質であるカベオリンおよびAktは同じ条件下では検出不可能であり、これにより、能動的分泌の可能性が示唆された(図4a)。しかし、aP2は脂肪細胞における最も豊富な細胞質タンパク質の一つであるため、馴化培地におけるその存在は、未だ、細胞の死および/または溶解に起因する非特異的な放出による可能性がある。細胞からのaP2放出の性質を確認するため、Flagタグ付きのaP2を、同じようにタグ付けした対照としてのGFPおよびAktと共にHEK293細胞にトランスフェクトし、各プラスミドの量を慎重に用量設定して、全てのタンパク質が細胞内において同様のレベルで発現されることを確実にした(図4b)。馴化培地および細胞溶解物の両方を抗Flag抗体で調査して、抗体ごとの感度の違いにより生じ得るあらゆるばらつきを排除した。これらの実験において、aP2は馴化培地において容易に検出できたが、GFPおよびAKTは同じ条件下で検出できなかった(図4b)。これらのデータは、aP2が能動的に分泌されること、およびaP2が馴化培地に存在するのは非特異的な細胞溶解または死によるものではなかったことを示した。全ての脂肪細胞が分泌するタンパク質中でのaP2の相対量を調査するため、脂肪細胞由来の馴化培地を一次元電気泳動により分離した。これらのデータは、aP2が、脂肪細胞から分泌される最も豊富なタンパク質の一つであること(図4c)、アディポネクチンに匹敵するレベルで存在することを明らかにし、aP2が細胞外で重要な生物学的機能を有していることを示した。
aP2分泌の調節を試験するため、マウスにおけるaP2の血清レベルをELISAシステムを用いて試験した。WTおよびmal1-/-マウスにおいて、aP2は血清中にかなりのレベル(100〜300ng/ml)で存在したが、aP2-/-およびaP2-mal1-/-対照においては検出できなかった(図4d)。血清aP2はレプチン(約12.5ng/ml)より10〜30倍豊富であったが、アディポネクチンレベル(5〜10μg/ml)よりは有意に少なかった。代謝疾患との関連において血清aP2の調節を調査するため、非肥満マウスと遺伝性および食餌誘導性の肥満マウスモデル両方との血清プロフィールを比較した。血清aP2は両方の肥満モデルにおいて顕著に(約4倍)増加しており(図4e)、これによりaP2がこれらの病理学的条件下での代謝調節の変化に関連している可能性が示唆された。一貫して、循環aP2レベルの上昇がヒトにおける肥満に関連していることが報告されている。
脂肪細胞から分泌され代謝的な活性を有するタンパク質が代謝状態および栄養の変動により調節されるかどうかを評価する調査を行った。このような調節は、病理学的状態下での機能不全の機序を明らかにする可能性がある。したがって、血清aP2レベルが絶食および再給餌に反応して変化するかどうかを判定する実験を行った。24時間絶食したマウスにおける循環aP2のレベルは不断給餌時のレベルと比較して有意に上昇していたが、再給餌の4時間後に急速に抑制された(図5a)。したがって、栄養および代謝状態は血清aP2レベルを調節することができ、よってこのことはaP2がエネルギー恒常性を調節する全身プログラムの一部である可能性を示唆している。
FABP欠損マウスは、代謝症候群の複数の要素、特に2型糖尿病から保護される。ヒトにおける血清aP2の増大は、糖尿病、心血管疾患およびその他の代謝障害に関連することも報告されている。しかし本発明以前は、循環aP2と脂質および/または糖質の代謝との間の因果関係は確立されていなかった。aP2分泌と脂肪分解の緊密な結びつきは、血清aP2が、脂肪酸の変動に起因する代謝調節に対して相乗効果を発揮する可能性を示唆している。
血清aP2の増加が糖代謝に対して負の影響を有するかどうかについて直接取り組むため、血清aP2が増加した以外は代謝的に正常な状態のマウスを産生した。通常固形飼料食を与えたマウスに組換えaP2を注射した。一回量の組換えaP2のマウスへの投与は血清aP2レベルの上昇をもたらし、これは数時間持続した(図11)。組換えaP2を一日二回注射して、マウスに、各24時間の期間の大部分で循環中の増大したaP2を確実に維持させた。組換えaP2の投与は、この期間の間、マウスの体重も血清中の遊離脂肪酸レベルも変化させなかった(図12)。しかし、耐糖試験により判定されたように、非肥満の健常な動物は組換えaP2を2週間与えられた後に穏やかな耐糖能障害を示した(図6i)。この知見は、血清aP2が糖代謝を調節すること、および、食餌による寄与および体重変化がない場合でさえも、血清aP2の増加のみで糖代謝を損ない得ることを示した。組換えaP2を意識下のFABP欠損マウスに注入し、糖代謝に対する血清aP2の急性効果を高インスリン-正常血糖クランプにより試験した。この設定下で、aP2の注入はマウスにおける血清aP2レベルの急激な上昇をもたらし、1時間後には高い定常状態の血清レベルを確立し、これは実験終了時点まで持続した(図13)。このクランプ試験の際、aP2を与えられたマウスは有意に高い基礎肝糖産生(bHGP)を示した(図6j)。肝糖産生を試験した時点でこれらのマウスは4時間にわたりaP2注入しか受けていなかったことを考慮すると、これは重大な効果である。これらの知見および糖代謝速度の変化がみられなかったことは、肝臓が血清aP2の主たる標的であるという仮説をさらに支持するものである。肝糖産生を調節する遺伝子の発現を、aP2注入実験およびaP2枯渇実験の両方において試験した。糖新生経路における2つの重要な遺伝子であるホスホエノールピルビン酸カルボキシキナーゼ(PEPCK)およびグルコース-6-ホスファターゼ(G6P)はどちらも、対照と比較してaP2を注入したマウスにおいて有意に上方制御された(図6k、上パネル)。反対に、aP2抗体を与えたマウスは、これらの糖新生遺伝子の劇的な減少を示した(図6k、下パネル)。これらのデータは、血清aP2が肝臓における糖新生を調節し、肥満状態における血清aP2レベルの上昇が、糖尿病状態下でしばしば観察される肝糖産生の亢進に寄与することを示している。
aP2の分泌が脂肪分解とどのように関連し、それによってどのように調節されるかをより理解するため、aP2分泌の分子機序を試験した。最初に、分化した脂肪細胞を古典的分泌経路の阻害剤で処理し、馴化培地中に分泌されたタンパク質を試験した。ブレフェルジンAおよびモネンシン処理は両方ともアディポネクチンの分泌を効果的に妨害したが、いずれもaP2放出に対する阻害効果は全く有さず(図7a)、これによりaP2が非古典的経路を通じて分泌されることが示された。この知見は、aP2がシグナルペプチド配列を欠くという事実と符合する。
膜標的化は、エキソソームを介した分泌における重要な工程である。aP2の大多数は細胞質内に局在する一方、aP2が一過的にリン脂質膜と相互作用することが見出された。aP2の3D構造は、脂肪酸侵入に対する入口となる2つの上部αヘリックスを有する10本のストランドからなるβバレルから構成されている25(図7c、左パネル)。aP2タンパク質の以下の2つの要素は、aP2の膜会合に寄与することが提唱されている:(1)負に荷電したリン脂質との静電的相互作用を仲介する、リジン残基が提供する強い正の表面電位をもつ隆起(ridge)および(2)膜と直接相互作用する入口領域。aP2のエキソソームへの移行の根底にある機序を調査するため、2つの重要な表面残基であるリジン22および59をイソロイシンに変更するか(図7c、中央パネル)、または入口領域を完全に欠損させた(図7c、右パネル)、aP2変異体を作製した。これらの変異体ではaP2の全体的なフォールディングは変化しなかったが、どちらも細胞からのaP2分泌を妨害し(図7d)、これによりaP2とリン脂質膜との会合がaP2の分泌の絶対条件であることが示された。分泌能を喪失させた上記のaP2変異体はまた、エキソソームへの局在を劇的に減少させ(図7d)、これによりエキソソーム標的化がaP2分泌のための必須工程であることが示された。
Claims (17)
- aP2を介する障害を治療するかまたはその重篤度を軽減する方法であって、分泌されたaP2の阻害剤またはaP2分泌の阻害剤を被験体に投与する工程を含む、方法。
- 前記被験体が、上昇したレベルの循環aP2を含むとして特定される、請求項1記載の方法。
- 前記障害が、代謝症候群、肥満、糖尿病、脂肪肝疾患、耐糖能障害、アテローム性動脈硬化症、ぜん息、高血圧、脳卒中、神経変性疾患、慢性血液透析(CD)、閉塞性睡眠時無呼吸、HIV感染患者における脂肪異栄養症、および脂肪分解からなる群より選択される、請求項1記載の方法。
- 前記分泌されたaP2の阻害剤が可溶性aP2ブロッキング剤を含む、請求項1記載の方法。
- 前記ブロッキング剤がaP2特異的抗体またはそのaP2結合フラグメントである、請求項4記載の方法。
- 代謝疾患を予防するかまたはその重篤度を軽減する方法であって、血清aP2濃度を低下させる組成物を被験体に投与する工程を含む、方法。
- 前記血清aP2濃度が少なくとも10%低下する、請求項6記載の方法。
- 前記組成物がaP2特異的抗体である、請求項6記載の方法。
- 前記組成物がaP2の不連続エピトープに結合する抗体である、請求項6記載の方法。
- 前記エピトープが、溶液中のaP2の3次元構造において表面に露出している、請求項9記載の方法。
- 以下の配列のうちの少なくとも1つを含む、請求項9記載の方法:
SEQ ID NO: 3の残基1〜5、残基22、残基36〜37、残基46〜47、残基57、残基59〜60、残基78、残基80、残基89、残基97〜101、残基110〜112または残基122。 - 代謝疾患の症候を軽減する方法であって、細胞によるaP2分泌の阻害剤を被験体に投与する工程を含む、方法。
- 前記症候が耐糖能障害である、請求項12記載の方法。
- 前記細胞が脂肪細胞である、請求項12記載の方法。
- 糖尿病を治療する方法であって、II型糖尿病を有するかまたはその発症リスクがあると診断された被験体に、aP2分泌の阻害剤を投与する工程を含む、方法。
- aP2分泌細胞を候補化合物と接触させる工程および細胞外aP2レベルを検出する工程を含む、aP2分泌の阻害剤を同定する方法であって、該化合物の存在下での細胞外aP2が該化合物の非存在下と比較して減少していることにより、該化合物がaP2分泌を阻害することが示される、方法。
- 前記細胞が脂肪細胞またはマクロファージである、請求項16記載の方法。
