本開示の実施形態は、腎臓、膀胱、腹腔、胃および胆管系のような内臓を排膿するために使用される排膿用カテーテルを導入する。これらのカテーテルは、新規な縫合糸ロックハブを備えたピグテールループのような保持要素を含む。ロックハブは、不注意に開放されにくく、且つ付加的なロック解除器具を要しない保持システムを設けることにより、干渉する危険性を最小限にする。ロックハブは、カテーテルの近位部に取り付けられた基部を含む。縫合糸のような可撓性部材は長尺状部材の遠位部に接続され、長尺状部材を通してハブに向かって延びる。ハブにおいて、縫合糸は、基部の小径の開口部から退出する。近位側に縫合糸を張引することにより、ピグテールループを形成する保持要素が駆動される。カラーは、基部を覆い、カラーの内側表面と基部の外側表面との間に縫合糸を保持する。基部の外側表面のねじを含むねじ係合ロックは、カラーの内側表面の対応するねじと係合し、基部とカラーとをロックする。ねじがさらに保持するように係合すると、基部の外側表面およびカラーの内側表面はさらに正方向に係合する。基部は、所定の角度にて遠位側に向かって先端ほど細くなり、カラーは、異なる角度にて先端ほど細くなる形状である。さらに、カラーは、基部より鋭い角度にて先端ほど細くなる形状であり、これにより、先端ほど細くなるカラーの径は、基部の対応する径よりわずかに小さい。

図1は、長尺状本体部材102、長尺状可撓性部材104(あるいは可撓性部材104)およびハブ106を含む例示的な排膿用カテーテルを示す。
排膿用カテーテル100の構造上の特徴は、図1Aおよび図1Bを参照して開示される。次の図(図2Aおよび図2B)は、ハブ106に可撓性部材104をロックする方法を含むカテーテルの機能的な特徴を示す。図2Aは、ハブ106がロック解除状態にある第1の位置の排膿用カテーテル100を示し、図2Bは、ハブ106がロック状態にある第2の位置の排膿用カテーテル100を示す。両図は、患者の体に排膿用カテーテル100を挿入し、ピグテールループを形成し、ピグテールループを保持するように可撓性部材104を固定する処理を連続して示す。
Claims (20)
- 排膿用カテーテルであって、
中央のルーメンを形成するとともに、遠位部および近位部を含む長尺状本体部材であって、同長尺状本体部材の少なくとも一部は、患者の体内に配置されるように構成される、前記長尺状本体部材と、
前記長尺状本体部材に作動可能に接続されるとともに前記ルーメンを通して前記長尺状本体部材の少なくとも近位部に延びる長尺状可撓性部材と、
ハブとを備え、同ハブは、
近位端、遠位端、および内部通路を有する基部であって、該近位端と該遠位端との間の外側表面と、該内部通路から該外側表面の開口部に延びる側面のルーメンとを有し、該外側表面は、係合要素および整合面部を含み、該基部の遠位端は、該長尺状本体部材の近位部に固定される、前記基部と、
近位端、遠位端、該近位端から該遠位端に延びる内側表面、および該近位端から該遠位端に延びる外側表面を有するカラーであって、同カラーは、該内側表面から該外側表面に延びる側面のルーメンをさらに有し、該カラーの該内側表面は、整合面部および対向要素を含み、該長尺状可撓性部材は、該基部の該側面のルーメンおよび該カラーの該側面のルーメンを通して延びる、前記カラーとを有し、
前記ハブは、該係合要素および該対向要素が係合しないロック解除位置、および該係合要素および該対向要素が係合するロック位置を有し、該カラーは該基部に締め付けられ、該長尺状可撓性要素は、該基部および該カラーの両整合面部間に留められることを特徴とする排膿用カテーテル。 - 前記ハブは、前記基部および前記カラーを覆う外側ジャケットをさらに含み、前記外側ジャケットは、前記基部および前記カラーを受承する腔を有することを特徴とする請求項1に記載の排膿用カテーテル。
- 前記外側ジャケットは遠位部をさらに含み、同遠位部は、遠位側に向かって先端ほど細くなるとともに前記基部および前記カラーの両遠位端を越えて遠位側に延びることを特徴とする請求項2に記載の排膿用カテーテル。
- 前記基部の整合面は、第1の角度で先端ほど細くなるテーパ面を含み、前記カラーの整合面は、第2の角度で先端ほど細くなるテーパ面を含み、該第2の角度は該第1の角度より大きいことを特徴とする請求項1に記載の排膿用カテーテル。
- 前記基部の整合面は前記基部の外側表面に配置される前記係合要素よりも遠位側にあり、前記カラーの整合面は前記カラーの内側表面に配置される対向要素よりも遠位側にあることを特徴とする請求項1に記載の排膿用カテーテル。
- 前記基部の整合面および前記カラーの整合面は前記係合要素よりも近位側にあり、前記対向要素は前記基部の外側表面に配置される前記係合要素よりも遠位側にあり、前記カラーの整合面は前記カラーの内側表面に配置される前記対向要素よりも遠位側にあることを特徴とする請求項1に記載の排膿用カテーテル。
- 前記長尺状可撓性部材は、前記ハブに固定される第1の端、前記基部の側面のルーメンおよび前記カラーの側面のルーメンを延びる第2の端部、並びに該第1の端および該第2の端部よりも先端側の中間部を有し、該中間部は、前記長尺状本体部材の前記先端部に作動的に接続されることを特徴とする請求項1に記載の排膿用カテーテル。
- 前記長尺状可撓性部材および前記ハブの基端側への相対運動により、前記長尺状本体部材の前記先端部は変形することを特徴とする請求項1に記載の排膿用カテーテル。
- 前記長尺状可撓性部材および前記ハブの基端側への相対運動により、前記長尺状本体部材の前記先端部はループを形成することを特徴とする請求項1に記載の排膿用カテーテル。
- 前記カラーが基部に締めつけられる場合に、前記長尺状可撓性部材とハブとの間の相対運動は可能ではなく、前記長尺状可撓性部材の一部は、前記カラーと前記基部との間に固定されることを特徴とする請求項1に記載の排膿用カテーテル。
- 前記基部の側面のルーメンに配置されるとともに流体が該側面のルーメンを通過して流れることを実質的に防止する弾性を備えたシールをさらに備えることを特徴とする請求項1に記載の排膿用カテーテル。
- 前記基部と前記カラーとの間に配置されるロック要素をさらに含むことを特徴とする請求項1に記載の排膿用カテーテル。
- 前記ロック要素はばねであることを特徴とする請求項12に記載の排膿用カテーテル。
- 前記ロック要素は、前記基部の前記外側表面の腔に部分的に配置されることを特徴とする請求項12に記載の排膿用カテーテル。
- 前記腔は、通常長手方向に延びるスロットであることを特徴とする請求項14に記載の排膿用カテーテル。
- 前記ロック要素は、前記カラーと前記基部との間で部分的に圧縮されることを特徴とする請求項12に記載の排膿用カテーテル。
- 前記係合要素および前記対向要素が係合しない場合に、前記ロック要素は前記カラーと前記基部との間で部分的に圧縮されることを特徴とする請求項12に記載の排膿用カテーテル。
- 前記カラーは第1のロック腔を含み、同第1のロック腔は前記カラーが前記基部にロックされると、前記ロック要素を部分的に受承するように構成されることを特徴とする請求項12に記載の排膿用カテーテル。
- 前記カラーは第2のロック腔を含み、同第2のロック腔は、前記係合要素および前記対向要素が係合しない場合に前記ロック要素を部分的に受承するように構成されることを特徴とする請求項18に記載の排膿用カテーテル。
- 排膿用カテーテルであって、
遠位部および近位部を含む長尺状本体部材であって、同長尺状本体部材の少なくとも一部は、患者の体内に配置される、前記長尺状本体部材と、
前記長尺状本体部材の前記遠位部に作動的に連結されるとともにルーメンを通して前記長尺状本体部材の少なくとも近位部に延びる長尺状可撓性部材と、
ハブとを備え、同ハブは、
近位端、遠位端、および該近位端と該遠位端との間を延びる外側表面を有する基部であって、同基部は、該外側表面に配置されるねじ山をさらに含み、該外側表面は整合面部を有し、該基部の遠位端は前記長尺状本体部材の近位部に固定され、前記長尺状本体部材および前記基部は一体的に中央の器具ルーメンを形成する、前記基部と、
近位端、遠位端、および中央のルーメン、該近位端から該遠位端に延びる内側表面、該近位端から該遠位端に延びる外側表面、該内側表面に配置されるとともに該基部の該ねじ山と係合するように構成されるねじ山を有するカラーであって、該内側表面は整合面部を含み、該基部および該カラーの両整合面は不整合な外形を有するとともに、該カラーが対応する両ねじ山を使用して該ハブに締め付けられると両整合面が強制的に一体的となり該カラーを該基部にロックするように構成される、前記カラーとを有し、
前記長尺状可撓性部材は、該基部の該近位端よりも近位側の該中央の器具ルーメンを退出する近位部を有し、且つ前記カラーが前記基部にロックされると該長尺状可撓性部材の近位部が該基部と該カラーとの間に固定されるように該基部の該外側表面の少なくとも一部と該カラーの該内側表面との間に配置されることを特徴とする排膿用カテーテル。
