Abstract
Description
本出願は、2011年9月1日に出願された米国特許仮出願第61/530031号、2011年7月1日に出願された米国特許仮出願第61/504120号、2011年4月1日に出願された米国特許仮出願第61/471030号、2011年3月10日に出願された米国特許仮出願第61/451379号、2011年9月26日に出願された米国特許出願第13/245130号の優先権を主張する。これらの先行出願の開示全体が、参照によって本明細書に組み込まれ、それらの開示を本明細書の一部とみなすべきである。
図2〜図7Bは、経皮整形手術を実施するために使用することができる拡張導入器100の実施形態を示している。以下に詳しく説明するように、図示した実施形態の拡張導入器は、アクセスカニューレ30と、第1、第2および第3の拡張器チューブ40、45および60と、を有してよい。図示の実施形態は、第1、第2および第3の拡張器チューブ40を含んでいるが、変形された実施形態では、それより多くまたは少ない拡張器チューブを含んでいてよく、および/または修正された特徴を有する拡張器チューブを含んでいてもよい。いくつかの実施形態では、アクセスカニューレ30を、導入器から無くすか、あるいは修正してもよいことが予想される。
図8A〜図13は、拡張導入器を使用して経皮的整形手術を実施する方法の一実施形態を示している。まず図8Aを参照すると分かるように、遠位部41が椎間板12に当接するかあるいは場合によっては椎間板12に貫入するまで、第1の拡張器チューブ40をカムビンの三角形20を通して配置してもよい。一構成では、次いで、第2の拡張器チューブの遠位部46が椎間板12に当接するが進入しない位置に来るまで、第2の拡張器チューブ45を第1の拡張器チューブ40を介して前進させてもよい。
図20A〜図20Dは、第1の拡張器チューブが神経監視針1108と置き換えられた拡張導入器の別の態様を示している。神経監視針1108は、遠位先端1111の所で露出し、針カニューレ1110によって密閉することができるワイヤ1109を含んでいる。針カニューレ1110は、絶縁されるように針カニューレ1110の長さに沿って誘電体コーティング1112によって囲まれていてよい。たとえば、ワイヤ1109は、ステンレススチールを含んでいてよく、誘電体コーティング1112はパリレン(Parylene)を含んでいてもよい。上記に指摘したように、ノブ1115が神経監視針1108の近位部1116上に配置されていてもよい。第1の神経監視リード線1113が第2の拡張器チューブ145の近位部177に取り付けられてもよい。第2の神経監視リード線1114が第3の拡張器チューブ160の近位部183に取り付けられてもよい。
上述のインプラント80に関して、インプラント80は、椎体同士の間に配置されるように構成された様々な種類の椎体間装置のうちのいずれかの椎体間装置を有してもよい。インプラント80は、金属(たとえば、チタン)、またはプラスチック、PEEKTM、ポリマーおよびゴムなどの非金属材料から形成されてもよい。さらに、各インプラント構成部材は、非金属材料(たとえば、PEEKTM、ポリマー)と金属の組合せで作られてもよい。インプラント80は、たとえば図13の実施形態に示されているように、一定または実質的に一定の高さ、長さおよび幅を有するように構成されていてよい。他の実施形態では、インプラントは、1つまたは2つ以上の方向に沿って拡張可能であるように構成されてもよい。たとえば、ある実施形態では、装置がアクセスカニューレを通して前進し椎体同士の間に(たとえば、線維輪内の椎間板腔内に)位置した後、インプラントの高さを拡張してもよい。
アクセスカニューレを通して使用できる手術器具の別の例には骨やすりがある。そのような骨やすりの一実施形態が図32に示されている。この図に示されているように、やすり器具800は、アクセスカニューレ30を通して椎間板腔内に挿入されるように構成されていてよい。次いで、やすり器具800を使用して、上椎骨の内面と下椎骨の上面を研磨またはやすりがけしてもよい。やすり器具800は細長い本体810と掻き取り構成部材812とを有していてよい。ハンドル816を細長い本体810の近位に取り付けてもよい。図示のように、やすり器具800は、内部に細長い本体810がスライド可能に受け入れられる開放スリーブ808を含んでいる。この構成は、細長い本体810および掻き取り構成部材812が開放スリーブ808に対してスライドすること可能にすることができる。
次に図33A〜図34Dを参照して、椎間板腔内で使用できるようにアクセスカニューレを通して挿入することのできる骨移植片移送装置を説明する。たとえば、隣接する椎骨同士の急速な固定を推進するため、骨移植片材料を椎間板腔内に挿入することができる。骨移植片材料は、椎間板インプラントを挿入する前に挿入されてもよい。あるいは、椎間板インプラントを挿入した後で骨移植片材料を挿入してもよい。いくつかの実現形態において、骨移植片材料は、椎間板インプラントが挿入される前と挿入された後の両方のときに送られる。骨移植片材料は、自家移植片、同種移植片、異種移植片または合成移植片であってもよい。骨移植片材料だけでなく、必要に応じて他の材料を治療部位に導入してもよい。たとえば、開示される移送装置を使用して、コラーゲンなどの分散媒によって骨形成タンパク質を導入してもよい。

前記第2の拡張器チューブによって下椎弓根から骨を除去することと、を含む方法。 - 第3の拡張器チューブを前記第2の拡張器チューブを介して導入することと、
前記下椎弓根から骨をさらに除去することと、をさらに含む、請求項16に記載の方法。 - アクセスカニューレを前記第3の拡張器チューブを介して導入することと、
前記アクセスカニューレを通して脊椎を手術することと、をさらに含む、請求項17に記載の方法。 - 整形手術において治療すべき患者の椎間板に到達する方法であって、
椎間孔拡大術を実施するステップと、
前記椎間孔拡大術によって形成された大きい開口部を利用して前記椎間板に装置または器具を導入するステップと、を含む方法。 - 前記椎間板にインプラントを導入することをさらに含む、請求項18に記載の方法。
- 前記椎間板内で前記インプラントを拡張することをさらに含む、請求項20に記載の方法。
- 前記椎間孔拡大術は、少なくともその一部が1本または2本以上の拡張器チューブ上の切削面を使用して実施される、請求項19に記載の方法。
