Abstract

Description
幾つかの形態では、縫合糸管理装置は、一つ又は複数のカテーテルセクション(部)を備えている。カテーテルセクションは、利用者から離れた場所に前進され得る任意の構造を有する。例えば、カテーテルセクションは、一つ又は複数のルーメン又はチャンネルを形成する、チューブ、シース、又はカテーテル本体を備えている。カテーテルセクションは、任意の適当な材料で作られ、任意の適当な大きさ又は形状を有し、任意の適当な特性を有するように作られる。カテーテルセクションのすべて又は一部はフレキシブルである。反対に、カテーテルセクションのすべて又は一部は硬いものであってもよい。当然、カテーテルセクションは、その長さ方向の一部に沿ってフレキシブルであってもよいし、その長さの一部に沿って硬くてもよい。
ここで説明する切断アセンブリは、縫合糸管理装置の作動時に縫合しを切断するために使用される。切断アセンブリは、必要なことではないが、カテーテルセクション内又は縫合糸係合部内に完全に含まれる。切断アセンブリが完全に縫合糸管理装置に含まれる形態では、切断アセンブリは縫合糸だけに接触するように構成してもよい。これにより、切断アセンブリが身体上の組織を切断又は損傷するのを防止する。また、切断アセンブリを操作する力は縫合糸管理装置内に吸収され、そのために、切断アセンブリが縫合糸管理装置に力を加えるのを防止する。これにより、組織の外傷が防止される。
バルーン(510)は、バルーン内腔(512)を介してバルーン(510)に流体を通過させることにより、膨らまされる。この流体は任意の適当なガス又は液体である。幾つかの形態では、バルーンが収縮すると、片持ち切断ブレード(506)は自然にその待機位置に戻る。その他の形態では、バルーン(510)は、片持ち切断ブレード(506)と縫合糸係合部(502)の外側壁(514)の両方に取り付けられる。その結果、バルーン(510)が収縮すると、片持ち切断ブレード(506)が待機形状に戻る。さらに他の形態では、縫合糸管理装置(500)は、バルーン(510)が収縮すると、片持ち切断ブレード(506)をその待機位置に戻すための幾つかの構造を含む。これらの形態の幾つかでは、縫合糸管理装置(500)は、片持ち切断ブレード(506)を切断位置から遠ざける一つ又は複数のスプリングを有する。これらの形態のその他では、縫合糸管理装置(500)は、片持ち切断ブレード(506)を切断位置から遠ざけるように付勢する一つ又は複数の磁石を備えている。
バルーン(716)が収縮しているとき、図7Aの断面図に示すように、ブレードハウジング(710)は待機位置に留まる。縫合糸管理装置(700)を作動するために、バルーン(716)は、流体(液体又は気体)がバルーンルーメン(718)を介して送られることにより、膨らまされる。膨張したバルーン(716)は、カテーテルセクション(720)に対して末端側にブレードハウジング(710)を押す。これにより、ブレード(704)がブレードチャンネル(722)を介して末端に向かって縫合糸係合部(706)に移動する。ブレード(704)がブレードチャンネル(722)を移動するとき、該ブレード(704)は縫合糸チャンネル(708)を通って移動し、それにより、縫合糸(図示せず)を切断する。バルーン(716)が収縮すると、スプリング(714)が作用してブレードハウジング(710)をその待機位置に戻す。同様に、縫合糸管理装置(700)は、代替的に又は追加的に、ブレードハウジング(710)をその待機位置に戻すために、磁石又はその他の構造を備えていてもよい。
ここで説明するハンドピースは、任意の適当な構造形状を有する。通常、ハンドピースは、利用者が、体内又は囲まれた空間内で縫合糸管理装置を案内する又は操作できるようにするものである。ハンドピースはまた、利用者から離れた場所で、該利用者が縫合糸を案内又は操作できるようにするものである。又、ハンドピースは、縫合糸又は縫合糸の結び目を切断するために使用される、一つ又は複数の切断アセンブリを制御することができる。
本願に記載の縫合糸管理装置は、一つ又は複数の縫合糸が通過する、一つ又は複数の縫合糸チャンネルを有する。縫合糸チャンネルは、縫合糸管理装置上の任意の2つの適当な場所に配置された端部を有する。縫合糸チャンネルの配置は、装置の切断アセンブリ又は他の構成の形状によって決まる。幾つかの形態では、図1Aと図1Bに示すように、縫合糸は、縫合糸管理装置の基端と末端の間にわたる縫合糸チャンネルを通過する。他の形態では、縫合糸チャンネルは、縫合糸管理部又はその他の開口を介して、縫合糸管理装置に入って存在する。
そこに示されているのは、チャンネル(1202)、ブレードエッジ(1206)を有する片持ち切断ブレード(1204)、及びバルーンルーメン(1210)を有するバルーン(1208)である。この形態において、縫合糸チャンネルは、縫合糸係合部(1200)の末端に縫合糸入口(1212)を有し、縫合糸係合部(1200)の壁(1216)に縫合糸出口(1214)を有する。縫合糸(図示せず)は、縫合糸入口(1212)から縫合糸出口(1214)に又はその反対に挿通され、これにより、縫合糸係合部(1200)が縫合糸の一部に係合できる。
幾つかの形態では、本願で説明する装置は、引込式ガイドを有する。図15A〜図15Cは、縫合糸管理装置(1500)の一形態を示す。図15Aは、縫合糸管理装置(1500)全体の斜視図である。そこに示されているのは、スライド式アクチュエータ(1504)とプッシュアクチュエータ(1506)を備えたハンドピース(1502)と、縫合糸チャンネル(1510)、ガイドループ(1514)を有する引込式ガイド(1512)、マーカー(1516)、及びブレード(1520)を有するプッシュロッド(1518)を備えたカテーテルセクション(1508)である。引込式ガイド(1512)は、縫合糸チャンネル(1510)内に配置され、スライドアクチュエータ(1504)の作動に基づいて縫合糸チャンネル(1510)に基端側から引込まれる。同様に、ブレード(1520)は、プッシュアクチュエータ(1506)の作動に基づいてカテーテルセクション(1508)に対して末端側に前進する。
上述の縫合糸管理装置はいずれも、利用者から離れた位置で縫合糸を切断するために利用できる。幾つかの方法では、縫合糸は目的の場所での手技で使用される。この手技は、限定的ではないが、創縫合、2つの組織部を引き寄せること、左心耳のような組織の領域を結合することなどの任意の適当な手技である。これらの方法では、結び目が目的部位で結ばれる。この結び目は、限定的ではないが、引き結びを含む任意の適当な結び目でよい。これらの方法の幾つかでは、縫合糸管理装置は、縫合糸の少なくとも一部に係合する。
Claims (25)
- 基端、末端、及び上記基端と上記末端の間にある内腔を有し、上記内腔は該内腔の少なくとも一部を縫合糸が通過するように構成されている細長い管状部材と、
上記細長い管状部材の上記末端近傍に配置された片持ちブレードと、
上記片持ち部材を作動するために、上記片持ちブレードに隣接して配置された膨張可能な部材を備えた縫合糸の一部を切断する装置。 - 上記膨張可能な部材は膨らませることができる、請求項1の装置。
- 上記膨張可能な部材がバルーンである、請求項2の装置。
- 上記膨張可能な部材が一対のジョーを備えている、請求項1の装置。
- 引込式ガイドを備えている、請求項1の装置。
- 基端、末端、及び上記基端と上記末端の間にある内腔を有し、上記内腔は該内腔の少なくとも一部を縫合糸が通過するように構成されている細長い管状部材と、
上記末端近傍に設けられた対向する切断ブレードと、
上記切断ブレードの外面周りの少なくとも一部に配置され、上記切断ブレードを作動するアクチュエータを備えた縫合糸の一部を切断する装置。 - 上記アクチュエータは、膨張可能な部材を備えている、請求項6の装置。
- 上記アクチュエータは膨らませることができる、請求項7の装置。
- 上記アクチュエータがバルーンである、請求項8の装置。
- 上記アクチュエータは、基端側に引かれるか又は末端側に押されたとき、上記対向するブレードを作動するように構成されている作動ジョーを備えている、請求項6の装置。

ブレードハウジングに接続され且つ上記内腔内に配置されたブレードを備えており、
上記ブレードは上記内腔の長軸に平行に向けられており、
上記ブレードと上記ブレードハウジングは上記内腔内でスライド可能である、縫合糸の一部を切断する装置。 - 上記スライド可能なブレードを作動するように構成されたアクチュエータを備えている、請求項12の装置。
- 上記アクチュエータが膨張可能な部材である、請求項13の装置。
- 上記アクチュエータが引きワイヤである、請求項13の装置。
- 上記アクチュエータが一つ又は複数の磁石である、請求項13の装置。
- 上記アクチュエータがプランジャである、請求項13の装置。
- 上記細長い管状部材の内側にスライド自在に配置された内側の細長い部材を備えている、請求項12の装置。
- 上記ハンドルが、一つ又は複数の安全構造を備えている、請求項18の装置。

- 基端、末端、及び上記基端と上記末端の間の少なくとも一部にある内腔を備えた第1の細長い管状部材であって、上記細長い管状部材は開口を有するが実質的に閉じられた末端を有し、上記開口は少なくとも一つの切断エッジを有する第1の細長い管状部材と、
基端、末端、及び上記基端と上記末端の間の少なくとも一部にある内腔を備えた第2の細長い管状部材であって、上記細長い管状部材は開口を有するが実質的に閉じられた末端を有し、上記開口は少なくとも一つの切断エッジを有する第2の細長い管状部材を備えており、
上記第1と第2の細長い管状部材は、互いに回転可能であり、回転すると上記第1の細長い管状部材の上記切断エッジと上記第2の細長い管状部材の上記切断エッジが互いに相手に向かって移動するようにしてある、縫合糸を切断する装置。 - 縫合糸を切断する方法であって、
縫合糸上に切断アセンブリを前進させる工程を有し、
上記切断アセンブリは、
基端、末端、及び上記基端と上記末端の間の少なくとも一部にある内腔を備えた第1の細長い管状部材であって、上記細長い管状部材は開口を有するが実質的に閉じられた末端を有し、上記開口は少なくとも一つの切断エッジを有する第1の細長い管状部材と、
基端、末端、及び上記基端と上記末端の間の少なくとも一部にある内腔を備えた第2の細長い管状部材であって、上記細長い管状部材は開口を有するが実質的に閉じられた末端を有し、上記開口は少なくとも一つの切断エッジを有し、上記第1と第2の細長い管状部材が一列に配置された状態で上記切断アセンブリの上記末端を上記縫合糸が通過する、第2の細長い管状部材を備えており、
上記方法はまた、上記第2の細長い管状部材に対して上記第1の細長い管状部材を回転する工程を有し、
上記第2の細長い管状部材に対して上記第1の細長い管状部材を回転すると、上記第1と第2の細長い管状部材の上記切断エッジが互いに接近する、縫合糸を切断する方法。 - 縫合糸を切断する方法であって、
細長い管状部材内にブレードアセンブリを引き込んで縫合糸に接触して切断する工程を有し、

上記ブレードアセンブリはブレードとブレードハウジングを備えており、
上記ブレードアセンブリは、上記細長い管状部材の上記内腔内にスライド自在に配置されており、
上記ブレードは上記内腔の長軸に平行に向けられている、縫合糸を切断する方法。 - 上記ブレードアセンブリを引き込む工程は、一つ又は複数の磁石を用いて上記ブレードアセンブリを引き込む、請求項23の方法。
- 上記ブレードアセンブリを引き込む工程は、上記管状部材内にスライド自在に配置されたシャフトを用いて上記ブレードアセンブリを引き込む、請求項23の方法。
