Abstract
Description
本発明は、一般に、単純ヘルペスウイルス(HSV−1およびHSV−2)、特にHSV−2の獲得および伝播を防ぐことにおいて使用するための抗ウイルス活性を有する化合物の処方物に関する。
ヒト免疫不全ウイルス(HIV)感染および関連する疾患は、世界的に主要な公衆衛生問題である。抗ウイルス剤での処置によって、AIDS患者の生命を延長することにおいて大きな前進をしたが、完全な治療は存在しない。
本発明は、局所用(例えば、膣、直腸など)適用に適した抗ウイルス活性を有する化合物、特に[2−(6−アミノ−プリン−9−イル)−1−メチル−エトキシメチル]−ホスホン酸(テノホビル、PMPA)の処方物、ならびにHSV−1感染およびHSV−2感染の獲得および伝播を妨げることおよび/または防ぐことにおけるそれらの使用に関する。

用語「治療上有効な量」は、疾患を処置するために被験体に投与される場合に、上記疾患に対する処置を達成するために十分である化合物の量を指す。「治療上有効な量」は、化合物、処置されるべき被験体の疾患、およびその重症度、年齢、体重などによって変動し得る。
以下の実施例は、本発明の範囲内の特定の実施形態をさらに記載し、実証する。実施例は、単に例示のために与えられ、本発明の趣旨および範囲から外れることなく多くのバリエーションが可能であるので、限定するものとして構築されない。以下の実施例は、例示することだけが意図され、あらゆる様式において本発明の範囲を限定することは意図されない。「活性成分」は、上に記載されるように、1つ以上のNRTI、好ましくはテノホビルまたは生理学的に機能的なその誘導体を意味する。
細胞。ヒト胚肺(HEL)線維芽細胞(HEL−299;ATCC CCL−137)を、10%のウシ胎仔血清(FCS)、1%のL−グルタミン、1%の可欠アミノ酸、および1%のピルビン酸ナトリウムを補ったMEMアール培地(Gibco,Invitrogen Corporation,UK)において培養した。初代ヒトケラチノサイト(PHK)を新生児の包皮から単離した。組織片をトリプシン−EDTAとともに37℃にて1時間インキュベートした。上皮細胞を分離し、以下の補充物:1ml当たり0.5μgのヒドロコルチゾン、1ml当たり10ngの上皮増殖因子、2mMのL−グルタミン、10mMのHEPES、1mMのピルビン酸ナトリウム、1×10−10Mのコレラ毒素、1ml当たり5μgのインスリン、1ml当たり5μgのヒトトランスフェリン、および1ml当たり15×10−4mgの3,3’,5−トリヨード−2−サイロニンを含むケラチノサイト無血清培地(ケラチノサイト−SFM)(Gibco,Invitrogen Corporation,UK)において培養した。上記PHKを、単層における抗ウイルスアッセイのために、および器官型ラフト培養物(organotypic raft culture)の調製のために使用した。TZM−bl細胞は、Dr.G.Van Ham(ITG,Antwerp,Belgium)によって、親切にも提供された。
複数の許容細胞型におけるHSV−1およびHSV−2の複製のテノホビルによる阻害
実験室HSV株に対するテノホビルの活性をまず、HEL細胞単層および初代ヒトケラチノサイト(PHK)において評価し、ヌクレオシド(すなわち、アシクロビル、ペンシクロビル、ガンシクロビル、およびブリブジン)類似体および非環式ヌクレオチドホスホネート(ANP)(すなわち、シドホビルおよびアデホビル)類似体の抗HSV活性と比較した。テノホビルは、PHKにおいて、HSV−1およびHSV−2に対して約100μg/ml〜約200μg/mlのEC50値でウイルス誘導細胞変性性を阻害した。アデホビルについてのEC50値は、3.6μg/ml〜13μg/mlであり、シドホビルについては、0.63μg/ml〜4.4μg/mlであった。HSV−1に対してよりもHSV−2に対して著しく低い活性を有することが知られているブリブジンを除いて、ヌクレオシド類似体は、試験した非環式ヌクレオチドホスホネート類似体のうちのどれよりも活性があることが証明された。
上皮TZM−Bl細胞は、HIV(コ)レセプターCD4、CXCR4、およびCCR5でトランスフェクションされたHeLa細胞に由来する。それらは、HIVおよびHSVの両方によって感染し得る。発明者らは、これらの細胞をHIV−1(NL4.3)およびHSV−2(G)に共感染させ、HIVの複製をLTR駆動ルシフェラーゼ発現(発光)によってモニターし、HSV−2の複製をウイルス誘導細胞変性性の顕微鏡読み取りによってモニターした。テノホビルは、共感染培養物において、単独でHSV−2に感染したTZM−bl細胞培養物を阻害するために必要されるテノホビル濃度と同様である約60μg/mlの薬物濃度でHSV−2の感染を防いだ。また、テノホビルによって、進行中のHSV−2感染の存在下、または非存在下でHIV−1を等しく抑制した(データは示されない)。従って、HIV−1およびHSV−2の二重感染の実験条件下で、各ウイルスは、他のウイルスに対するテノホビルの抗ウイルス活性に影響を与えなかった。
分化したケラチノサイトは、インビボでのHSVの産生性感染に対する主要な標的細胞であるので、ケラチノサイトの器官型ラフト培養物におけるテノホビルの抗ウイルス活性を評価した。この3次元培養モデルにおいて、ケラチノサイトは完全に分化し、従って、インビボでの組織状態を忠実に真似ていた。発明者らは、この系において、テノホビル、アデホビル、およびシドホビルのHSVの複製を低減するための能力を比較した。上記器官型上皮ラフト培養物を分化の10日後に感染させ、試験化合物の連続希釈物を用いて処理した。持ち上げ後15日後(すなわち、処理の5日後)に、組織学的実験のために、およびウイルスの定量化のためにラフトを加工した。培養切片の組織学的実験は、対照ラフトにおいて、特徴的な層を有する完全に分化した上皮を示したが、HSV感染ラフトは、明白なウイルス感染および上皮一帯にウイルスの広がりを示した(図2)。ラフトのHSV感染は、細胞変性効果を生じ、核内好酸性封入体の出現、代表的な上皮内小胞の形成および多核化と一緒にケラチノサイトの気球状化および網状変性をもたらした。
HSV−1株F(HSV−1F)、HSV−2株G(HSV−1G)、およびHSV−2株MS(HSV−2MS)の複製に対するテノホビルの効果を、感染したヒト扁桃組織においてエクスビボで調査した(図4)。この系は、外因性刺激または活性化なしで、種々のウイルスの複製を支持する(17)。このインビボ様組織系における接種の際に、組織ブロックを浸している培養培地におけるウイルスDNAの存在によって、示されるように、HSV−1F、HSV−2G、またはHSV−2MSを効率的に複製した。全ての試験したドナーからの組織は、培養の9日間にわたって培養培地において、HSV−1F、HSV−2G、およびHSV−2MSそれぞれについて、7.8log10 DNAコピー/ml[四分位数間範囲(IQR)7.1〜8.1、n=5]、7.25log10コピー/ml(IQR7.2〜7.9、n=6)、および6.4log10コピー/ml(IQR6.1〜7.5、n=3)に達するウイルスDNAゲノムのメジアン蓄積を有する、明白な産生性ウイルス感染を支持する。
頸膣組織においてテノホビルの抗HSV活性を調査するために、この組織のブロックを前に記載されるように培養し、HSV−2Gを接種し、薬物で処理した。166μg/mlの濃度におけるテノホビルをHSV−2G感染時に加え、各培地の取替えでそれを補給することによって実験の持続期間にわたって維持した。HSV−2Gの複製を、頸膣組織ブロック(1条件当たり16ブロック)を浸している培養培地へのウイルスのDNAの放出によって評価した。テノホビルで処理されていない対照組織において、5つの全ての試験したドナーからの組織において、12日間の培養にわたって6.6log10コピー/ml(IQR5.3〜8.2)のメジアン累積生成を有するウイルスの複製を検出した。テノホビルで処理した頸膣組織において、ウイルス生成を5.5log10コピー/mL(IQR4.8〜5.7、n=5)(図4Cおよび図4D)まで低減し、各実験におけるウイルス複製の低減を平均する場合に78±9%の低減を反映した(p<0.01)。
2つの異なるビヒクル−DMSO(図5)およびCAPRISA 004研究において使用されるゲルを用いて、テノホビル、アデホビル、およびシドホビルを、HSV−1感染マウスおよびHSV−2感染マウスにおける抗ヘルペス薬(antiherpetics)として評価した(データは示されない)。全ての化合物を1%の濃度で、感染の日から始めて5日間、HSV−1感染マウスおよびHSV−2感染マウスに投与した。HSV−1に感染し、1%のテノホビルで処置したマウスの罹患および生存曲線の両方は、プラセボ処置群と比較して有意に遅延した。予期されたように、動物がDMSOまたはゲル中1%のアデホビルを受けた場合、それぞれ80%および100%の生存を観測したので、アデホビルは、テノホビルと比較して、DMSO(図5A)およびゲル処方物(データは示されない)の両方においてより効果的であることが証明された。DMSO処方物中1%のアデホビルで処置されたマウスが皮膚病変を発症せずに死亡したことに言及すべきである(図5A)。DMSOまたはゲル処方物のいずれかにおけるシドホビルでの処置は、ウイルス誘導罹患および死亡に対してマウスを完全に保護した。
テノホビルのその抗ウイルス性活性(ジホスホリル化(diphosphorylated))代謝産物への代謝変換を、以前に確立された手順(19)に従って、ヒトリンパ球CEM(HIV感染実験に使用した)ならびにヒト上皮TZM−blおよび線維芽細胞HEL(HSV感染実験に使用した)細胞培養物において研究した。培養物の開始後72時間における2μMの[2,8−3H]テノホビルを用いた細胞の24時間の間の処理は、テノホビル−ジホスフェートの形成をもたらし、Tリンパ細胞、上皮細胞、および線維芽細胞において、それぞれ13pモル/109細胞、6pモル/109細胞、および15pモル/109細胞のレベルに達した。従って、インビボでのHIVまたはHSV感染のいずれかに対して関連がある標的細胞を代表する複数の異なる細胞型において、テノホビルをその抗ウイルス性活性代謝産物に転換した。
ウイルスをコードするヘルペスウイルスDNAのポリメラーゼまたはHIV−1逆転写酵素のインヒビターとしての活性を有するために、テノホビルは、細胞酵素によって、そのジホスホリル化誘導体(テノホビル−DP)に転換される必要がある。活性代謝産物は、プライマー/鋳型として活性化仔ウシ胸腺DNA、および競合基質として[2,8−3H]dATPを用いて、標的酵素に対するその阻害活性について評価されてきた。テノホビル−DPは、HIV−1 RT(IC50:4.3μM)およびHSV−1 DNAポリメラーゼ(IC50:1.3μM)の両方を効率的に阻害する。従って、細胞培養物、器官型上皮ラフト培養物、ヒトリンパ球および頸部エクスビボ組織、ならびにウイルス感染マウスにおけるテノホビルの抗ヘルペス活性を、その活性代謝産物テノホビル−DPによるウイルスDNAポリメラーゼの阻害によって説明し得る。

1%のBIDを用いたテノホビル膣ゲルは、限定された全身吸収性(systemic absorption)を有し、膣のミクロフロラに対して起こり得る有益な効果を有して、禁欲的および性的に活発なHIV(−)およびHIV(+)女性において忍容性が良好であった。
本研究の目的は、膣に適用される場合に、単純ヘルペスウイルス、特にHSV−2の伝播を防ぐことにおけるテノホビルゲルの有効性を評価することであった。
・単に割り当てられた製品を膣に適用すること。
・ゲル挿入後2時間は膣洗浄もしくは他の方法で膣を洗浄しないこと、または他の物体もしくは膣製品を挿入しないこと。女性が性交後に膣洗浄する計画である場合、膣洗浄後にゲルを挿入することを彼女に勧めた。
・彼らの研究製品を適切に保存すること(直射日光の当たらない涼しい乾燥した場所で)。
・コンドームを使用してもしなくても、研究製品を使用すること。
HSV−2の存在を、Kalon HSV−2型特異的EIA(Kolon Biological Ltd.,United Kingdom)を用いて、採った血液サンプルにおいて決定した。登録時、888人の女性のうち454人(1人の女性は、試験のために保管された血液を有さなかった)は、既存のHSV−2感染を有し、51.1%の有病率をもたらした。残りの434人の女性を、試験への参加時にHSV−2に感染しやすいとみなし、終了時のHSV−2状態をこれらの女性のうちの426人において決定した。これらの女性のうちの4人は、保管された研究終了時の標本を有さず、4人は、研究終了時において不確定なHSV−2の結果を有したことに留意すべきである。
