Abstract
Description

−proBNP(1−108)またはRAPRSP配列(SEQ ID NO:1)を含むproBNP(1−108)のフラグメントを検出することに対して好適な少なくとも1つの抗体;および
−proBNP(1−108)またはRAPRSP配列(SEQ ID NO:1)を含むproBNP(1−108)のフラグメントを、好ましくは1〜1000pg/mlの濃度で含む少なくとも1つのキャリブレータを含む、卒中を診断するためのキットにも関する。
ここで意図されるとおりでは、「診断する」または「診断」を確立するということは、卒中が個体に生じたかどうかを判断することに関する。
好ましくは、この発明に従うと、卒中は、虚血性卒中および出血性卒中からなる群から選択される。より好ましくは、ここに意図される卒中は急性の虚血性卒中である。
「proBNP(1−108)」は、前駆体BNP(32)およびNT−proBNP(1―76)に関する。ここに意図されるとおりでは、「proBNP(1−108)」はすべてのその天然の変異体を含む。例示的に、proBNP(1−108)はSEQ ID NO:4によって表わされる。
a.サンプル
proBNP(1−108)およびH−FABPのレベルを、発症後3時間以内に卒中が生じた個体から得られた70の血清サンプル(15の出血性卒中サンプル(HM)および55の虚血性卒中サンプル(IM))ならびに明らかに健康な個体からの148の対照血清サンプルにおいて測定した。
proBNP(1−108)レベルの測定を、BioPlex(商標)2200 proBNP(1−108)アッセイ(Bio−Rad)を用いて行なった。
患者の状態に従ったバイオマーカの分布をボックスプロット表現によって表現した、というのも、それは、数値データの群を、5つの数による概要(最小観測値、下位四分位数(Q1)、中央値(Q2)、上位四分位数(Q3)、および最大観測値)を介して図形的に表現する便利な方法であるからである。
a.ボックスプロット表現によるバイオマーカの分布
図1および図2は、それぞれ、卒中母集団および対照母集団におけるH−FABP濃度およびproBNP(1−108)濃度の分布を表わす。
卒中サンプルと対照サンプルとの間におけるH−FABPレベルおよびproBNP(1−108)レベルにおける差異の統計学的有意性を判断した:

臨床状態に対する受信動作特性(ROC)分析に基づき、および表3にまとめられるように、proBNP(1−108)に対する曲線下領域は、90%の感度、85%の特異度で、卒中について診断された患者を健康な被験者と区別することに成功している。さらに、この識別能力は、虚血性卒中患者および出血性卒中患者の双方に対して、それぞれ、0.936および0.935のAUCで、非常に高い。
a.サンプル
proBNP(1−108)のレベルを、発症後3時間以内に卒中が生じた個体から得られた23の血清サンプル(5つの出血性卒中サンプル(HM)および18の虚血性卒中サンプル(IM))、TIAが臨床診断された個体から得られた7つの血清サンプル、ならびに明らかに健康な個体からの94の対照血清サンプルにおいて測定した。
proBNP(1−108)レベルを実施例1に示されるように測定した。
proBNP(1−108)レベル分布のボックスプロット表現およびROC分析を実施例1に示されるように行なった。
a)ボックスプロット表現によるバイオマーカの分布
図3は、卒中母集団、TIA母集団および対照母集団におけるproBNP(1−108)濃度の分布を表現する。
臨床状態に対する受信動作特性(ROC)分析に基づき、および表6にまとめられるように、proBNP(1−108)に対する曲線下領域は、91%の感度、84%の特異度で、卒中について診断された患者を健康な被験者と区別することに成功している。
a.サンプル
マーカのレベルを、発症から3時間以内に卒中が生じた個体から得られた41の血漿サンプル、および明らかに健康な個体からの対照血漿サンプルにおいて測定した。
proBNP(1−108)レベルを実施例1に示されるように測定した。
統計学的分析を実施例1に示されるように行なった。
a)ボックスプロット表現によるバイオマーカの分布
図4、図5および図6は、それぞれ卒中母集団および対照母集団におけるproBNP(1−108)濃度、S−100b濃度およびNSE濃度の分布を表現する。
卒中サンプルおよび対照サンプルにおけるproBNP(1−108)レベル、NSEレベルおよびS−100レベルにおける差の統計学的有意性を判断した:

臨床状態に対する受信動作特性(ROC)分析に基づき、および表9にまとめられるように、proBNP(1−108)に対する曲線下領域は、95%の感度、95%の特異度および0.974のAUCで、卒中について診断された患者を健康な被験者と区別することに成功している。
−卒中発症の3時間以内に救急部に受入れられた虚血性卒中患者からの32のクエン酸塩添加血漿サンプルを検査した。卒中重症度は米国立衛生研究所の脳卒中スケール(NIHSS)で評価した。
対照母集団および虚血性卒中母集団に対するproBNP(1−108)値の分布を表10に示す。proBNP(1−108)のレベルは、対照群と比較して虚血性卒中群において有意に高かった(Mann-Whitney、p<0.0001)。これらの結果により、proBNP(1−108)はしたがって虚血性卒中の早期診断に対して有用な血漿バイオマーカであることが確認される。
Claims (17)
- 個体における卒中および一過性の虚血性発作(TIA)のインビトロ診断のための方法であって:
(a)前記個体の生体サンプルにおいてproBNP(1−108)またはRAPRSP配列(SEQ ID NO:1)を含むproBNP(1−108)のフラグメントのレベルを測定するステップと;
(b)前記測定されたレベルをカットオフ値と比較するステップと;
(c)それから、卒中またはTIAが前記個体に生じたかどうかを判断するステップとを含む、方法。 - 前記proBNP(1−108)のフラグメントはFGRKMDR配列(SEQ ID NO:2)をさらに含む、請求項1に記載の方法。
- 前記proBNP(1−108)のフラグメントはBNP(32)の配列(SEQ ID NO:3)を含む、請求項1または2に記載の方法。
- 前記卒中は、虚血性卒中、出血性卒中、および一過性の虚血性発作(TIA)からなる群から選択される、請求項1から3のいずれかに記載の方法。
- ステップ(a)は前記個体において卒中を示す少なくとも1つの症状の発症後6時間以内に実施される、請求項1から4のいずれかに記載の方法。
- proBNP(1−108)またはproBNP(1−108)のフラグメントのレベルはイムノアッセイを用いて測定される、請求項1〜5のいずれかに記載の方法。
- 前記イムノアッセイは、RAPRSP配列(SEQ ID NO:1)を含むエピトープを標的とする抗体を含む、請求項6に記載の方法。
- 前記抗体は、ブダペスト条約の下2005年4月29日に登録番号I−3073でCNCM(フランス、パリ)に寄託されたハイブリドーマによって分泌される、請求項7に記載の方法。
- 前記イムノアッセイは、FGRKMDR配列(SEQ ID NO:2)を含むエピトープを標的とする抗体を含む、請求項6〜8のいずれかに記載の方法。
- 前記抗体は、ブダペスト条約の下2007年4月13日に登録番号I−3746でCNCM(フランス、パリ)に寄託されたハイブリドーマによって分泌される、請求項9に記載の方法。
- 前記カットオフ値は少なくとも1pg/mlである、請求項1から10のいずれかに記載の方法。
- (a’)個体の生体サンプルにおいてproBNP(1−108)またはRAPRSP配列(SEQ ID NO:1)を含むproBNP(1−108)のフラグメントのレベルを測定するステップと、
(b’)前記個体の生体サンプルにおいて少なくとも1つのさらなる卒中マーカのレベルを測定するステップと、
(c’)proBNP(1−108)またはproBNP(1−108)のフラグメントのレベルおよび前記少なくとも1つのさらなる卒中マーカのレベルを、1つまたはいくつかのカットオフ値と比較するステップと、
(d’)それから、卒中またはTIAが前記個体に生じたか否かを判断するステップとを含む、請求項1〜11のいずれかに記載の方法。 - 前記少なくともさらなる卒中マーカのレベルは、proBNP(1−108)またはproBNP(1−108)のフラグメントのレベルが測定される生体サンプルと同じサンプルにおいて測定される、請求項12に記載の方法。
- 前記生体サンプルは、血液サンプル、血清サンプル、血漿サンプル、脳脊髄液サンプル、尿サンプルおよび唾液サンプルからなる群から選択される、請求項1〜13のいずれかに記載の方法。
- 少なくとも1つの心血管疾患のマーカのレベルを測定することをさらに含む、請求項1〜14のいずれかに記載の方法。
- 前記少なくとも1つの心血管疾患のマーカはCRPおよびcTnIからなる群から選択される、請求項15に記載の方法。
- 卒中またはTIAのインビトロ診断のための、proBNP(1−108)またはRAPRSP配列(SEQ ID NO:1)を含むproBNP(1−108)のフラグメントの使用。
