Abstract
Description
本願は、米国仮特許出願第61/231,370号(名称「Surgical Wound Dressing」、2009年8月5日出願)の利益および優先権を主張し、この出願の全開示は、本明細書に参考として援用される。
本開示は、概して、開放創傷を治療する装置に関し、より具体的には、複数のビードを組み込む創傷被覆システムであって、ビードは、それによって埋め込まれた電極を有する、絶縁され、接続された細長部材を画定する、システムに関する。
創傷閉鎖は、創傷が閉鎖するまで、創傷に隣接した皮上および皮下組織の創傷の中心に向かう移動を伴う。残念ながら、大きい創傷または感染した創傷については、閉鎖が困難である。そのような創傷では、鬱血帯(すなわち、組織の局部的な膨張が、組織への血液の流れを制限する領域)を創傷の表面近傍に形成する。十分な血流が無ければ、創傷を包囲する上皮および皮下組織は、受け取る酸素および栄養が減少するだけでなく、正常に微生物感染と戦うことがより困難になり、したがって、創傷を閉鎖することがより困難になる。そのような創傷は、長年にわたって医療関係者に問題を提示してきた。
例えば、本発明は、以下の項目を提供する。

創傷にわたって位置するように構成される流体透過性カバー層であって、該創傷からの滲出液が該カバー層を通過することを可能にするように構成されるカバー層と、
該創傷内に設置可能であり、該カバー層によって該創傷の中に保持される複数のビードであって、絶縁され、相互接続された細長部材を画定するビードと、
該細長部材の少なくとも一部の中に埋め込まれ、それを通って延在する電極と
を備え、外部エネルギー源によって生成された電流が、該電極を電気的に流れる、システム。

前記創傷に減圧を供給する導管をさらに備える、項目1に記載の創傷被覆システム。
(項目3)
前記複数のビードは、ヒドロゲル材料から作成される、項目1に記載の創傷被覆システム。
(項目4)
前記埋め込まれた電極は、前記細長部材の全長を通って延在する、項目1に記載の創傷被覆システム。
(項目5)
前記複数のビードのうちの各ビードは、第1の長さを有し、
該複数のビードのうちの隣接するビードは、第2の長さだけ相互から分離され、
該第1の長さは、該第2の長さを上回っている、項目1に記載の創傷被覆システム。
(項目6)
前記複数のビードのうちの少なくとも1つのビードの一部は、伝導性塗膜を含む、項目1に記載の創傷被覆システム。
(項目7)
前記伝導性塗膜は、Ag、Ag/Cl、Cu、Au、炭素ゴム、炭素フィルム、およびアルミニウムフィルムのうちの少なくとも1つである、項目6に記載の創傷被覆システム。
(項目8)
周辺電極と、該周辺電極と動作可能に連絡している回転子とをさらに備える、項目1に記載の創傷被覆システム。

前記周辺電極に固着され、該周辺電極を適所に固着するように構成される支持層をさらに備える、項目8に記載の創傷被覆システム。
(項目10)
前記支持層は、少なくとも部分的に、前記周辺電極の上に位置し、前記創傷の周辺に沿って設置される、項目9に記載の創傷被覆システム。
(項目11)
前記流体透過性支持層の中に埋め込まれた電圧源をさらに備える、項目1に記載の創傷被覆システム。
(項目12)
前記電圧源は、前記細長部材の一端に設置される、項目11に記載の創傷被覆システム。
(項目13)
前記電圧源は、前記細長部材を少なくとも2つの部分に分割するように設置される、項目11に記載の創傷被覆システム。
(項目14)
前記複数のビードのうちの各ビードは、隣接するビード間に画定される空間を通る、前記滲出液の通過を促進するほど十分に剛性である、項目1に記載の創傷被覆システム。
(項目15)
創傷被覆構成を製造する方法であって、該方法は、
流体透過性カバー層を提供するステップであって、該カバー層は、創傷にわたって位置するように構成され、該創傷からの滲出液が該カバー層を通過することを可能にするように構成される、ステップと、
該創傷内に設置可能な複数のビードを提供するステップであって、該ビードは、絶縁され、相互接続された細長部材を画定する、ステップと、
該細長部材の少なくとも一部の中に埋め込まれ、それを通って延在する電極を提供するステップと
を含み、外部エネルギー源によって生成された電流が該電極を電気的に流れる、方法。

前記創傷に減圧を供給するステップをさらに含む、項目15に記載の方法。
(項目17)

前記細長部材の一端に前記電圧源を設置するステップをさらに備える、項目25に記載の方法。
(項目27)
前記電圧源は、前記細長部材を少なくとも2つの部分に分割するように設置される、項目25に記載の方法。
(項目28)
前記複数のビードのうちの各ビードは、隣接するビード間に画定される空間を通る、前記滲出液の通過を促進するほど十分に剛性である、項目15に記載の方法。
(項目29)
創傷被覆構成を使用する方法であって、該方法は、
複数のヒドロゲルビードを創傷内に留置することであって、該ビードは、(i)絶縁され、相互接続された細長部材を画定し、(ii)該細長部材の少なくとも一部の中に埋め込まれ、それを通って延在する電極を有する、ステップと、
周辺創傷電極をヒトに留置するステップと、
カバー層が該創傷と外部環境との間に障壁を形成するように、該カバー層を該創傷上に取着するステップと、
エネルギーを該埋め込まれた電極および該周辺電極に伝達するステップと、
該埋め込まれた電極および該周辺電極に伝達される該エネルギーを監視するステップと
を含む、方法。
Claims (29)
- 創傷被覆システムであって、
創傷にわたって位置するように構成される流体透過性カバー層であって、該創傷からの滲出液が該カバー層を通過することを可能にするように構成されるカバー層と、
該創傷内に設置可能であり、該カバー層によって該創傷の中に保持される複数のビードであって、絶縁され、相互接続された細長部材を画定するビードと、
該細長部材の少なくとも一部の中に埋め込まれ、それを通って延在する電極と
を備え、外部エネルギー源によって生成された電流が、該電極を電気的に流れる、システム。 - 前記創傷に減圧を供給する導管をさらに備える、請求項1に記載の創傷被覆システム。
- 前記複数のビードは、ヒドロゲル材料から作成される、請求項1に記載の創傷被覆システム。
- 前記埋め込まれた電極は、前記細長部材の全長を通って延在する、請求項1に記載の創傷被覆システム。
- 前記複数のビードのうちの各ビードは、第1の長さを有し、
該複数のビードのうちの隣接するビードは、第2の長さだけ相互から分離され、
該第1の長さは、該第2の長さを上回っている、請求項1に記載の創傷被覆システム。 - 前記複数のビードのうちの少なくとも1つのビードの一部は、伝導性塗膜を含む、請求項1に記載の創傷被覆システム。
- 前記伝導性塗膜は、Ag、Ag/Cl、Cu、Au、炭素ゴム、炭素フィルム、およびアルミニウムフィルムのうちの少なくとも1つである、請求項6に記載の創傷被覆システム。
- 周辺電極と、該周辺電極と動作可能に連絡している回転子とをさらに備える、請求項1に記載の創傷被覆システム。
- 前記周辺電極に固着され、該周辺電極を適所に固着するように構成される支持層をさらに備える、請求項8に記載の創傷被覆システム。
- 前記支持層は、少なくとも部分的に、前記周辺電極の上に位置し、前記創傷の周辺に沿って設置される、請求項9に記載の創傷被覆システム。
- 前記流体透過性支持層の中に埋め込まれた電圧源をさらに備える、請求項1に記載の創傷被覆システム。
- 前記電圧源は、前記細長部材の一端に設置される、請求項11に記載の創傷被覆システム。
- 前記電圧源は、前記細長部材を少なくとも2つの部分に分割するように設置される、請求項11に記載の創傷被覆システム。
- 前記複数のビードのうちの各ビードは、隣接するビード間に画定される空間を通る、前記滲出液の通過を促進するほど十分に剛性である、請求項1に記載の創傷被覆システム。
- 創傷被覆構成を製造する方法であって、該方法は、
流体透過性カバー層を提供するステップであって、該カバー層は、創傷にわたって位置するように構成され、該創傷からの滲出液が該カバー層を通過することを可能にするように構成される、ステップと、
該創傷内に設置可能な複数のビードを提供するステップであって、該ビードは、絶縁され、相互接続された細長部材を画定する、ステップと、
該細長部材の少なくとも一部の中に埋め込まれ、それを通って延在する電極を提供するステップと
を含み、外部エネルギー源によって生成された電流が該電極を電気的に流れる、方法。 - 前記創傷に減圧を供給するステップをさらに含む、請求項15に記載の方法。

- 前記複数のビードのうちの各ビードは、隣接するビード間に画定される空間を通る、前記滲出液の通過を促進するほど十分に剛性である、請求項15に記載の方法。
- 創傷被覆構成を使用する方法であって、該方法は、
複数のヒドロゲルビードを創傷内に留置することであって、該ビードは、(i)絶縁され、相互接続された細長部材を画定し、(ii)該細長部材の少なくとも一部の中に埋め込まれ、それを通って延在する電極を有する、ステップと、
周辺創傷電極をヒトに留置するステップと、
カバー層が該創傷と外部環境との間に障壁を形成するように、該カバー層を該創傷上に取着するステップと、
エネルギーを該埋め込まれた電極および該周辺電極に伝達するステップと、
該埋め込まれた電極および該周辺電極に伝達される該エネルギーを監視するステップと
