実施形態1. 関節リウマチの1つまたは複数の症状を軽減または治療するために有効量の三酸化二ヒ素を含む組成物を、それを必要とする対象に投与することを含む、関節リウマチおよび他の炎症性関節炎の1つまたは複数の症状を治療または予防する方法。
実施形態2. 滑膜細胞の増殖を低減もしくは阻害するため、またはそのアポトーシスを促進するために、滑膜細胞に有効量の三酸化二ヒ素を接触させることを含む、滑膜細胞の増殖を低減もしくは阻害する、または滑膜細胞のアポトーシスを促進する方法。

A.三酸化二ヒ素
三酸化二ヒ素は、急性骨髄性白血病の治療抵抗性を示す前骨髄球性(M3)サブタイプの治療に非常に有用である。経口の三酸化二ヒ素(As2O3)は、再発性の急性前骨髄球性白血病に非常に有効である(Auら、Blood、112、3587〜90頁(2008))。経口のAs2O3はQT間隔の延長が小さく、したがって白血病を治療するかなり安全な薬物である。最近、経口のAs2O3では心臓におけるQT延長が非常にわずかであることも実証されており、これは長期間の使用に安全であることを意味する(Siuら、Blood、108、103〜6頁(2006))。
以下の送達システムが、As2O3を投与するための組成物の代表である。
注射可能な薬物送達システムには、薬学的に許容される溶液剤、懸濁剤、ゲル剤、微小球状剤(microspheres)およびインプラント剤が含まれる。通常、これらは蒸留水、リン酸緩衝生理食塩水、または静脈もしくは皮下注射用の他のビヒクルの形態となる。
経口送達システムには、溶液剤、懸濁剤、および錠剤(例えば、圧縮錠剤、糖衣錠剤、被覆錠剤および腸溶剤)、カプセル剤(例えば、ハードもしくはソフトなゼラチンカプセル剤または非ゼラチンカプセル剤)、ブリスター剤(blisters)およびカシェ剤などの固形剤形が含まれる。これらは、結合剤(例えば、ヒドロキシプロピルメチルセルロース、ポリビニルピロリドン、他のセルロース性物質およびスターチ)、賦形剤(例えば、ラクトースおよび他の糖、スターチ、リン酸二カルシウムおよびセルロース性物質)、崩壊剤(例えば、スターチポリマーおよびセルロース性物質)、および滑沢剤(例えば、ステアレートおよびタルク)などの賦形剤を含有することができる。固形剤形は、当技術分野で周知の被覆剤(coatings)および技術を用いて被覆することができる。
経粘膜送達システムには、貼付剤、錠剤、坐剤、ペッサリー剤、ゲル剤およびクリーム剤が含まれ、かつ溶解補助剤および促進剤(enhancers)(例えば、プロピレングリコール、胆汁酸塩およびアミノ酸)、および他のビヒクル(例えば、ポリエチレングリコール、脂肪酸エステルおよび誘導体、ならびにヒドロキシプロピルメチルセルロースおよびヒアルロン酸などの親水性ポリマー)を含有することができる。
A.関節リウマチの治療
As2O3を含む組成物は、滑膜細胞の増殖を阻害または低減する有効量、滑膜細胞におけるIL-6受容体の細胞表面発現を阻害または低減する有効量、または関節リウマチの1つまたは複数の症状を発症するリスクを治療または低減する有効量で、RAを有する個体に投与される。As2O3を含む開示組成物は、治療的または予防的に投与され得る。
As2O3を含む組成物は、関節リウマチに伴う症状の開始前、開始中または開始後に投与され得る。対象にAs2O3を含有する開示組成物を投与するために、当業者に公知の許容される任意の方法が使用され得る。
As2O3を含む組成物は、単独で、または1つもしくは複数の追加的な治療用薬剤、または予防用薬剤と組み合わせて投与することができ、あるいは治療を実施するために、外科的、放射的または他のアプローチと連携することができる。例えば、As2O3を含む組成物は、1つまたは複数の抗炎症剤を組み合わせて投与することができる。
材料および方法:
線維芽細胞様滑膜細胞MH7A細胞系
MH7Aは、理研バイオリソースセンター(Riken BioResource Center)(Tsukuba、日本)から購入した。5%CO2中37℃において、10%の熱不活化ウシ胎児血清(FBS;Invitrogen社、Carlsbad、CA、米国)を補給したRPMI1640(Invitrogen社)に細胞を維持した。
試薬には、As2O3およびジメチルスルホキシド(DMSO)(Sigma社、St.Louis、MO、米国)、塩化アンモニウム(NH4Cl)(Amresco社、Solon、OH、米国)、プロテアソーム阻害剤MG115、パン-カスパーゼ阻害剤Z-VAD-FMK、JNK阻害剤SP600125およびその陰性対照(Merck社、Darmtadt、ドイツ)、ならびに組み替えヒトIL-6(Peprotech社、Rocky Hill、NJ、米国)が含まれた。一次抗体には、ウサギ抗IL-6受容体α(IL-6Rα)およびgp130(C-20)抗体(Santa-Cruz Biotechnology社、Santa Cruz、CA、米国)、ウサギ-IL-6抗体(Merck社)、ウサギ抗リン酸化JNK(Thr183/Tyr185)、抗カスパーゼ3、抗β-アクチンおよび抗NFκB抗体(Cell Signaling Technology社、Beverly、MA、米国)、ならびにマウス抗ユビキチン抗体FK1およびFK2(Biomol社、Plymouth Meeting、PA、米国)が含まれた。二次抗体には、西洋わさびペルオキシダーゼ複合化ヤギ抗ウサギIgGおよびウサギ抗マウスIgG(Invitrogen社)が含まれた。
細胞増殖は、MTTアッセイによって評価した(GE Healthcare社、Piscataway、NJ、米国)(Cheungら、Cancer Lett.、246、122〜8頁(2007))。細胞(96ウェルプレートに2×104細胞/ウェル)を一晩インキュベートした後、対照または試薬で2〜6日間処理した。次に、処理細胞をMTT標識用溶液(10μL/ウェル)と共にインキュベートした。4時間インキュベートした後、細胞は溶解し、ホルマザン結晶は37℃において一晩で可溶化した。ホルマザンのシグナルを570nmで検出した(μ-quant(商標)マイクロプレートスペクトロメーター(microplate spectrometer)、Bio-Tek Instruments Inc.社、VT、米国)。取得データを、KCジュニアソフトウェア(junior software)(BLD Science社、Garner、NC、米国)によって解析した。
ウエスタンブロット解析を実施した(Pangら、Gastroenterology、132、1088〜103頁(2007))。細胞溶解物およびタンパク質の採取は、標準的プロトコルに準拠して実施した。タンパク質サンプル(通常、30μg)を、12%の溶解用ゲル中でドデシル硫酸ナトリウム-ポリアクリルアミドゲル電気泳動(SDS-PAGE)によって分離し、ニトロセルロース膜へ電気的に転写した(400mAで2時間)。5%の脱脂乳を含有するtris-バッファー生理食塩水-TWEEN(TBS-T)で、室温において30分間ブロッキングした後、膜を一次抗体および5%のウシ血清アルブミンを含むTBS-Tと共に、4℃で一晩インキュベートした。次に、該膜をTBS-Tで3回洗浄し、1:2000の西洋わさびペルオキシダーゼ複合化二次抗体(Amersham-Pharmacia Biotechnology社、Piscataway、NJ、米国)と共に、室温で1時間インキュベートした。免疫反応バンドは、スーパーシグナルウエストピコケミルミネッセンス基質(SuperSignal West Pico Chemiluminescent Substrate)(Pierce Chemical Co.社、Rockford、IL、米国)を利用した化学発光にて検出し、またX線フィルムに可視化した。バンドシグナルのデンシオメトリー定量は、ImageJ 1.36bソフトウェア(National Institutes of Health、米国)を使用して実施した。実験はすべて3組で実施した。
アポトーシスアッセイ用に、1×106の細胞を10cmのプレート内で一晩インキュベートした後、対照または試薬で24時間処理した。次に、細胞をトリプシン処理し、氷冷したリン酸緩衝生理食塩水(PBS)で2回洗浄し、結合バッファー500μL中に再懸濁させて、FITC複合化アネキシン-Vおよびヨウ化プロピジウム(PI)(Immunotech社、Fullerton、CA、米国)と共に、氷上で10分間インキュベートした。アポトーシス細胞(アネキシン-V-陽性、PI-陰性)を、適切な色調補正後にフローサイトメトリーによって3回計数した(Epics、Beckman Coulter社、Fullerton、CA、米国)。データ解析を、WinMDI 2.8ソフトウェア(The Scripps Research Institute、La Jolla、CA、米国)によって実施した。
As2O3は、約5μMの50%阻害濃度で、用量かつ時間依存的なMH7A細胞増殖の阻害を誘発した(図1A)。フローサイトメトリー解析により、細胞の増殖阻害はアポトーシスの誘発によって媒介されることが裏付けられた。MH7A細胞をAs2O3(5μM)と共に0〜24時間インキュベートし、アポトーシス細胞と死細胞を区別するためにヨウ化プロピジウム(PI)およびアネキシン-Vで染色した。代表的なアッセイでは、13%の対照細胞がアネキシン-Vに陽性、ヨウ化プロピジウムに陰性(アポトーシス)、および2.36%がアネキシン-Vに陽性、ヨウ化プロピジウムに陽性(死亡)となった一方、処理細胞の38.2%がアネキシン-Vに陽性、ヨウ化プロピジウムに陰性(アポトーシス)、および5.76%がアネキシン-Vに陽性、ヨウ化プロピジウムに陽性(死亡)となった。FACS解析の結果は、As2O3処理後にアポトーシス細胞が増加することを示した。
材料および方法:
逆転写ポリメラーゼ連鎖反応(RT-PCR)
TRIzol(Invitrogen社)を用いて、全RNAを調製した。逆転写PCR(RT-PCR)を実施した(Cheungら、Cancer Lett.、246、122〜8頁(2007))。Superscript(商標)IIIファースト-ストランド合成システム(First-Strand Synthesis System(Invitrogen社))を用い、製造元の取扱書に準拠してRNAを逆転写した。反応混合物は、1.2μLのcDNA、18μLのPlatinum(登録商標)PCRスーパーミックス(Supermix)(Invitrogen社)、および各プライマー対200nMを含有した。各プライマー一式について実施したアニーリング温度および増幅サイクル数を以下の表に挙げた。
イムノブロット解析をgp130のユビキチン化に実施した。MH7A細胞を、100μMのAs2O3と共に、0時間、4時間または6時間インキュベートする。全細胞溶解物の免疫沈降(IP)、その後の抗ユビキチン抗体であるFK1およびFK2とのウエスタンブロット解析のために、gp130に対する抗体を使用する。非免疫ウサギ血清(NIS)とのIPが対照となる。
GP130用の定量的RT-PCR(Q-PCR)を、Assays-on-Demand(商標)遺伝子発現システム(Gene Expression System)(AssaylD: Hs00174360_ml、PE Biosystems社、Foster City、CA、米国)を利用し、製造元の取扱書に準拠して実施した。反応混合物は、2μLのcDNA、10μLのTaqMan(登録商標)ユニバーサルPCRマスターミックス(Universal PCR Master Mix)、1μLのAssay-on-demand(商標)遺伝子発現アッセイミックス(Assay Mix)、および20μLの容量とするためのRNase不含水を含有した。Q-PCRをABI Prism 7700配列検出器(Sequence Detector)(PE Biosystems社)を用いて実施した。熱サイクルは、50℃で2分間の初期設定で開始し、次いで95℃で10分間の第1変性ステップ、次に95℃で15秒間(変性)および60℃で1分間(アニーリングおよび伸長反応)を40サイクルした。cDNA導入用の内部標準として、GAPDHを使用した。GAPDHに関しては、増幅は、50℃で2分間の初期ステップ、95℃で10分間加熱する第1の変性ステップ、次に95℃で20秒間(変性)および62℃で1分間(アニーリングおよび伸長反応)を40サイクル実施した。リアル-タイムPCR増幅データを連続収集して、ABI Prism 7700配列検出器で解析した。内部標準に対して正規化を行った、対照キャリブレータに対する相対的遺伝子発現を、ΔΔCT法(ABI user bulletin number 2、PE Biosystem社)によって算出した(Cheungら、Cancer Lett.、246、122〜8頁(2007))。実験はすべて3組で実施した。
MH7A増殖がIL-6に依存的であるかを検討するため、細胞増殖に対するIL-6の効果を検討した。外因性IL-6は、MH7A細胞の増殖を増加しなかった(図2A)。次に、細胞増殖に対するIL-6の中和抗体の効果を検討した。興味深いことに、本結果は、IL-6を中和すると、MH7A増殖が有意に阻害されることを示した(図2B)。モノ-およびポリ-ユビキチン化タンパク質の両方を識別するFK2によって、As2O3誘発性のgp130のユビキチン化は示されるが、ポリ-ユビキチン化タンパク質を識別するFK1によっては示されない。これらの結果は、gp130がモノ-ユビキチン化されたことを実証している。

材料および方法は、上記の実施例1〜3に記載の通りであった。
As2O3の阻害作用が、IL-6自己分泌経路の標的化により媒介されているかを検討するために、初めに、IL-6転写に対するAs2O3の効果を検討した。本結果は、As2O3はIL-6遺伝子転写に影響を及ぼさないことを示した。次に、2つのサブユニットであるIL-6受容体αおよびgp130を含むIL-6受容体に対するAs2O3の効果を試験した。ウエスタンブロット解析は、両サブユニットが発現することを示した。As2O3処理により、IL-6αは変化しないまま、用量かつ時間依存的にgp130が減少する結果になった。半定量的RT-PCRは、IL-6受容体αおよびgp130が活発に転写されること、およびAs2O3処理は両遺伝子の転写に効果がないことを示した。これらの結果をさらに確認するために、GP130遺伝子についてリアル-タイム定量的ポリメラーゼ連鎖反応を実施した。本結果は、As2O3処理はGP130遺伝子転写に効果がないことを裏付けた(図2E)。これらの知見は、転写後レベルでIL-6受容体複合体のgp130成分を下方調節することによって、As2O3はMH7AにおけるIL-6シグナル伝達経路を標的とすることを暗示した。
材料および方法:
材料および方法は、上記の実施例1〜3に記載の通りであった。
As2O3が媒介するgp130の転写後の下方調節はgp130の分解を増強することを示唆した。この問題に対処するために、タンパク質分解の3つの経路であるリソソームタンパク質分解、プロテアソームタンパク質分解およびカスパーゼ依存的タンパク質分解について試験した。MH7A細胞をリソソーム阻害剤である2.5mMのNH4Clと共に予備インキュベートすると、As2O3誘発性のgp130の低下が有意に防止された(図3A)。一方、プロテアソーム阻害剤MG115(10μM)およびカスパーゼ阻害剤Z-VAD-FMK(25μM)と共に予備インキュベートしても、As2O3誘発性のgp130の低下に効果がなかった。これらの知見は、As2O3誘発性のgp130の低下にリソソームの分解が関与していることを示唆した。
材料および方法:
免疫沈降
免疫沈降は、標準的手順によって実施した(Pangら、Gastroenterology、132、1088〜103頁(2007))。細胞を1mMのオルトバナジン酸ナトリウムおよびコンプリートプロテアーゼ阻害剤カクテル(Complete protease inhibitor cocktail)(Complete;Roche Molecular Biochemicals社)を補給した氷冷PBSで洗浄し、バッファー(50mM Tris-HCl、pH7.5、100mM NaCl、1% Triton X-100、4μg/mLアプロチニン、100μMフェニルメチルスルホニルフルオリド、200μMオルトバナジン酸ナトリウム、2μg/mLロイペプチン、1mM ジチオスレイトールおよび1×Complete)に、4℃で15分間溶解させた。溶解物を採集して遠心分離した。タンパク質をアッセイして(Bio-Rad Protein Assay Kit、Philadelphia、PA、米国)、1μg/μLに調整した(通常、800μgから1000μg)。タンパク質サンプルを適切な抗体(通常、4μg)または対照の非免疫血清と共に、4℃で一晩、穏やかに振とうしながらインキュベートすることによって免疫沈降を実施した。抗体-タンパク質複合体を、30μLのrec-Protein G SEPHAROSE beads(登録商標)(Invitrogen社)と共に4℃で2時間、穏やかに振とうしながらインキュベートすることによって沈降させた。次に、Protein G beadsを500μLの氷冷溶解バッファーで3回洗浄した。上澄み液をアスピレートして、50μLの2×Laemmliバッファーを添加した。抗体-タンパク質複合体を、95℃で10分間加熱することによって、ビーズから切り離した。次に、免疫沈降物を3組で、ウエスタンブロット法によって解析した。
タンパク質にモノ-ユビキチン部位が加わると、タンパク質をリソソームに選別することが可能となり、タンパク質はリソソームで最終的に分解される。As2O3がgp130のユビキチン化を増加させるか検討するために、MH7A細胞からの細胞溶解物を、As2O3処理の前後に抗gp130抗体と共に沈降させ、次にポリ-ユビキチン化タンパク質を識別した抗体FK1、ならびにモノ-ユビキチン化タンパク質およびポリ-ユビキチン化タンパク質の両方を識別したFK2とのウエスタンブロット解析を行った。本結果は、ユビキチン化gp130のみがFK2とのイムノブロットにおいて、(高分子量の塗抹として)検出され、FK1とのイムノブロットは検出されないことを示した。したがって、As2O3はgp130のモノ-ユビキチン化を誘発した。さらに、より長いAs2O3処理ほどより多量のユビキチン化gp130が得られ、時間依存的なモノ-ユビキチン化を示唆した(図3B)。これらの知見は、As2O3がgp130のリソソームの分解を誘発することをさらに裏付けた。

重要なIL-6シグナル伝達カスケードの1つは、JNKの活性化を含むMAPK経路である(Heinrichら、Biochem. J.、374、1〜20頁(2003))。興味深いことに、他の細胞系では、ヒ素はまたJNKを活性化することも示されている(Davisonら、Blood、103、3496〜502頁(2004))。したがって、MH7A細胞においてAs2O3誘発性の、IL-6シグナル伝達抑制において考えられるJNK活性化の関与について検討した。As2O3処理は、JNKのリン酸化反応を有意に増加させ、したがってJNKを活性化する(図4A)。JNK阻害剤SP600125(30μM)と共に予備処理すると、As2O3誘発性のJNKのリン酸化反応の増加が阻止された。As2O3誘発性のJNK活性化の阻害により、As2O3が媒介するMH7A細胞増殖の抑制が顕著に阻止されるので、JNK活性化には生物学的に意味があった(図4B)。

細胞周期解析
細胞周期解析を記載の通りに実施した(Pangら、J. Pathol.210、19〜25頁(2006))。細胞をトリプシン処理して、氷冷PBSで2回洗浄し、500μLのPBSに再懸濁させて、氷上、PIで10分間染色した(DNA Prep(商標)、Beckman Coulter社)。細胞周期をフローサイトメトリーによって決定し、データをWinMDI 2.8ソフトウェアによって解析した。
As2O3誘発性のJNK活性化の増殖阻害効果が、カスパーゼ-3によって媒介されるかどうかを検討するために、As2O3処理前にMH7A細胞を、JNK阻害剤SP600125(30μM)で処理した。JNK阻害は、As2O3誘発性のカスパーゼ-3の活性化に影響を及ぼさず、増殖阻害に至るカスパーゼ非依存的な経路が、As2O3誘発性のJNK活性化に関与していることを示唆する。この知見は、図1に示される結果と一致した。JNKがp53をリン酸化することは公知であり、またp53のリン酸化反応は増殖停止の誘発に重要なステップであるので(Wu、Cancer Biol.Ther、3、156〜61頁(2004))、p53のリン酸化反応に対するAs2O3の効果を試験した。図5Cに示されるように、As2O3処理により、アポトーシス促進性遺伝子の転写活性化に重要な部位であるセリン46におけるp53のリン酸化反応が、顕著に増加した(Wu、Cancer Biol. Ther.、3、156〜61頁(2004))。SP600125によるJNKの活性化阻害は、As2O3媒介性のp53のリン酸化反応を顕著に抑制しており、p53のリン酸化反応がAs2O3誘発性のJNK活性の下流エフェクターであることを裏付けた。細胞周期解析は、As2O3処理によりG2Mが有意に停止することを示した(図5D)。PIで細胞標識してフローサイトメトリーによって決定した、5μMのAs2O3でMH7A細胞を24時間にわたり処理した効果を示すためにヒストグラムを用いた。
材料および方法:
材料および方法は、上記の実施例1〜7に記載の通りであった。
もう1つの重要なJNKのクロス-トークパートナーはNFκBである(LiuおよびLin、Oncogene、26、3267〜78頁(2007))。NFκBはまた、関節炎の重要な炎症誘発因子である(Simmondsら、Rheumatology、47、584〜90頁(2008))。図6に示されるように、NFκBはMH7A中に構成的に発現した。As2O3処理により、NFκBのレベルが用量(図6A)および時間(図6B)依存的に顕著に低下した。
Claims (24)
- 関節リウマチの1つまたは複数の症状を軽減または治療するために有効量の三酸化二ヒ素を含む組成物を、それを必要とする対象に投与するステップを含む、関節リウマチおよび他の炎症性関節炎の1つまたは複数の症状を治療または予防する方法。
- 滑膜細胞の増殖を低減もしくは阻害するため、または滑膜細胞のアポトーシスを促進するために、滑膜細胞に有効量の三酸化二ヒ素を接触させるステップを含む、滑膜細胞の増殖を低減もしくは阻害する、または滑膜細胞のアポトーシスを促進する方法。
- 三酸化二ヒ素をリソソームに向けることによりIL-6受容体の分解を促進するために、細胞に有効量の三酸化二ヒ素を接触させるステップを含む、細胞におけるIL-6受容体の細胞表面での発現を低減する方法。
- IL-6受容体が滑膜細胞において発現する、請求項3に記載の方法。
- 滑膜細胞が関節リウマチ患者に存在する、請求項2または4に記載の方法。
- 滑膜細胞が炎症性関節炎患者に存在する、請求項2または4に記載の方法。
- 三酸化二ヒ素が患者に経口的に投与される、請求項5に記載の方法。
- 三酸化二ヒ素が滑膜関節に局所的に投与される、請求項5に記載の方法。
- ステロイド系または非ステロイド系抗炎症剤を投与するステップをさらに含む、請求項1に記載の方法。
- 滑膜細胞の増殖を阻害するため、または滑膜細胞のアポトーシスを促進するために、有効量の三酸化二ヒ素、および場合により薬学的に許容される賦形剤を含む、対象における関節リウマチまたは他の炎症性関節炎の1つまたは複数の症状の治療用医薬組成物。
- 三酸化二ヒ素が経口投与用の薬学的に許容される担体中に存在する、請求項10に記載の医薬組成物。
- 三酸化二ヒ素が滑膜関節への局所投与用の薬学的に許容される担体中に存在する、請求項10に記載の医薬組成物。
- 三酸化二ヒ素が1mgから10mgの量で存在する、請求項10から12のいずれか一項に記載の医薬組成物。
- 三酸化二ヒ素が5mgから10mgの量で存在する、請求項13に記載の医薬組成物。
- 溶液剤、懸濁剤、乳剤、シロップ剤、錠剤およびカプセル剤からなる群から選択される経口投与用単位剤形にある請求項10に記載の医薬組成物。
- 関節リウマチおよび他の炎症性関節炎の1つまたは複数の症状を治療または予防するための医薬の調製における三酸化二ヒ素の使用。
- 滑膜細胞の増殖を低減もしくは阻害するため、または滑膜細胞のアポトーシスを促進するための医薬の調製における三酸化二ヒ素の使用。
- 細胞においてIL-6受容体の細胞表面発現を低減するための医薬の調製における三酸化二ヒ素の使用。
- IL-6受容体が滑膜細胞において発現する、請求項18に記載の使用。
- 滑膜細胞が関節リウマチ患者に存在する、請求項17または19に記載の使用。
- 滑膜細胞が炎症性関節炎患者に存在する、請求項17または19に記載の使用。
- 三酸化二ヒ素が患者に経口的に投与される、請求項20に記載の使用。
- 三酸化二ヒ素が滑膜関節に局所的に投与される、請求項20に記載の使用。
- ステロイド系または非ステロイド系抗炎症剤を投与するステップをさらに含む、請求項16に記載の使用。
