Abstract
Description
〔実施例1〕
急性心筋梗塞(acute myocardical infarct; AMI)は、うっ血性心不全を引き起こすことが多い。現在の薬理学的および機械的な血行再建にもかかわらず、梗塞を起こした心筋を置換するような、効果的な治療法は実験的に確立されていない。AMI後の再モデリング過程の不可欠な構成要素は、AMI後の炎症反応および血管再生の進行である。これらの過程は、梗塞を起こした心筋中のサイトカインおよび炎症細胞によって媒介され、アポトーシス組織および壊死組織を貪食し、間質性樹状細胞(interstitial dendritic cell; IDC)およびマクロファージの自動誘導を開始する。全身的な免疫抑制(ステロイド)によって梗塞のサイズが拡大し、心筋の治癒が遅延したので、AMIによって誘導される炎症反応を減弱させることを目的とする臨床治験を、実施した。以上のデータから、AMI後の炎症応答が、組織の安定化および瘢痕の形成の原因であると結論した。離れた幹細胞が損傷部位を検知し、構造面の修復および機能面の修復を促進することを研究者が観察したときに、再生循環器学における新分野が出現した。この手法を用いて、Orlicらは、c−キット陽性の血管内皮前駆細胞(endothelial progenitor cell; EPC)を、実験的なAMIと増加した血管再生ならびに心筋構造および血管構造の再生との境界領域に注射した。この研究は、「細胞治療」の再生能力を実証する多数の発表の引き金となった。ただし、この治療効果が、移植した細胞自身によってもたらされたのか、常在性の心筋幹細胞の補充によってもたらされたのか、あるいは、未知のパラクリンおよび免疫性のメカニズムの活性化によってもたらされたのか、依然はっきりしないままである。梗塞した心筋における虚血はアポトーシス過程を引き起こし、死にかけている細胞の細胞表面の脂質の変化を惹起する。最もよく特徴がわかっている変化は、リン脂質の非対称の喪失およびホスファチジルセリン(phosphatidylserine; PS)の曝露である。これらのPSは、配位子(例えばトロンボスポンジン、CD14、およびCD36)を介して、マクロファージおよび樹状細胞(抗原提示細胞、APC)によって認識される。これらの受容体は、生理的条件の下では、アポトーシス性および壊死性の組織片を貪食し、静かな「清掃」過程を開始するように作用する。このAPCによる貪食過程は、表現型の抗炎症性反応を引き起こし、これは増大したIL−10およびTGF−βの生成および阻害されたAPCの機能によって決定される。臨床的に関連する報告は、アポトーシス細胞の注射が、造血細胞(hematopoietic cell; HC)の移植モデルにおいて、同種のHCの移植および致死的な急性移植片対宿主病(graft−versus−host disease; GVHD)を遅延させることを実証した報告である。また、臓器移植モデルでは、ドナーのアポトーシス細胞を注射すると、心臓の移植片生着期間が長くなった。炎症とは反対に、また、骨髄(bone marrow; BM)からの前駆細胞の補充に関連して、アポトーシス細胞のオプソニン作用によって、APCによるVEGFおよびCXC8/IL−8の生成の増加が誘導されることが示された。この後者のサイトカインに加えて、MMP9も、骨髄からのEPCの補充および遊離にとって非常に重要であるということがわかった。

生体内実験を実施するために、健康な若いボランティアから血液を採取した。インビボ(ラットのPBMC)実験を実施するために、60Gy(ヒトPBMCの場合)または45GyのCs−137セシウムを照射することによって、アポトーシスを誘導した。0.2%のゲンタマイシン硫酸塩(Sigma Chemical Co.社、アメリカ)、0.5%のβ−メルカプトエタノール(Sigma Chemical Co.社、アメリカ)、1%のL−グルタミン(Sigma Chemical Co.社、アメリカ)を含有するが、血清を含有しないUltra Culture培地(Cambrex Corp.、アメリカ)中に、細胞を再懸濁し、加湿雰囲気中で24時間培養して、試験管内実験(細胞濃度、1×106ml)に備えた。アポトーシスの誘導を、アネキシンV−フルオレセイン/ヨウ化プロピジウム(FITC/PI)の共染色(Becton Dickinson、アメリカ)によって、フローサイトメーターで測定した。PBMCのアネキシン陽性度を、>70%であると判断し、その結果、IA−PBMCと称することにする。放射線の照射を受けていないPBMCをコントロールとして使用し、生存PBMCと称することにする。上清を、どちらの実験的設定環境からも収集し、以下のように(SN−生存PBMC、SN−IA−PBMC)と記載し、実験用実体として使用した。
ヒトPBMCおよび単球(純度>95%)を、磁気ビーズシステム(Negative Selection、Miltenyi Biotec社、アメリカ)を用いて分離した。互いに異なる濃度のアポトーシス自系PBMC(アネキシンの陽性度>70%)およびリポ多糖(1ng/ml LPS;Sigma Chemical Co.社、アメリカ)を用いて、PBMCおよび単球を4時間共インキュベートした。上清を確保し、次の試験まで−80℃で凍結保管した。IL−6およびIL−1βの放出量を、市販のELISAキット(BenderMedSystems社、オーストリア)を用いて決定した。
PBMCを、フィコール・パク(Ficoll−paque:GE Healthcare Bio−Sciences AB社、スウェーデン)を用いた標準的な密度勾配遠心によって、健常なドナーのヘバリン処置した全血から単離した。T細胞および単球を、MACS法(Miltenyi Biotec社)を用いた磁気的な選別によって分離した。CD11b、CD14、CD16、CD19、CD33、およびMHCクラスIIの陽性細胞の、各単クローン抗体を用いた陰性除去によって、精製済みT細胞を得た。ビオチン化済みCD14 mAb VIM13(純度95%)を使用することによって、単球を濃縮した。精製済み血中単球をGM−CSF(50ng/ml)とIL−4(100U/ml)とを組み合わせて用いて7日間培養することによって、DCを生成した。次に、DCに異なる刺激を与えた。大腸菌(血清型0127−B8、Sigma Chemie社)から得た100ng/mlのLPSだけを24時間添加するか、あるいは、LPSを2時間添加し、さらに樹状細胞をアポトーシス細胞とともに1:1の比で22時間培養するかのいずれかによって、成熟を誘導した。また、アポトーシス細胞だけを用いて(1:1)、DCを24時間処理した。混合白血球反応(MLR)を起こすために、同種間の、精製済みT細胞(1×105個/ウェル)を、96個のウェルを有する細胞培養プレート(Corning Costar社)中で、異なる刺激を与えたDCを段階的に増やしながら6日間インキュベートした。3組の分析を実施した。5日後に添加する[メチル−3H]チミジン(ICN Pharmaceuticals社)の取り込み量を測定することによって、T細胞の増殖を観察した。細胞を18時間後に回収し、取り込まれた[メチル−3H]チミジンを、ミクロプレート・シンチレーションカウンターで検出した。
IA−PBMC、生存PBMC(細胞は1×106個、どちらもUltra Cultureの培地中で24時間培養)、およびSN−生存PBMC/SN−IA−PBMCに曝露した線維芽細胞を(Cascade Inc.(アメリカ)から入手した1×105個の線維芽細胞を、10%のウシ胎仔血清(FBS、PAA社、オーストリア)、25mMのL−グルタミン(Gibco社、BRL、アメリカ)、および1%のペニシリン/ストレプトマイシン(Gibco社)を補充したダルベッコ変法イーグル培地(DMEM、Gibco社 BRL、アメリカ)中で培養し、12個のウェルを有するプレートに播種して調べた。線維芽細胞を、SN−生存PBMCおよびSN−IA−PBMCを用いてそれぞれ4時間および24時間共インキュベートした)。製造者の指示にしたがって(RNeasy(QiIAGEN社、オーストリア)を用いて)、PBMCおよび線維芽細胞のRNA抽出を実施した後、取扱説明書の記載にしたがってiScript cDNA合成キット(BioRad社、アメリカ)を用いて、cDNAを転写した。
LightCycler Fast Start DNA Master SYBR Green I(Roche Applied Science社、Penzberg、ドイツ)を製造者のプロトコルにしたがって用いて、mRNAの発現を、リアルタイムPCRによって定量化した。VEGFのプライマーは、順方向が5´−CCCTGATGAGATCGAG−TACATCTT−3´、逆方向が5´−ACCGCCTCGGCTTGTCAC−3´であった。IL−8のプライマーは、順方向が5´−CTCTTGGCAGCCTTCCTGATT−3´、逆方向が5´−TATGCACTGACATCTAAGTTCTTTAGCA−3´であった。MMP9のプライマーは、順方向が5´−GGGAAGATGCTGGTGTTCA−3´、逆方向が5´−CCTGGCAGAAATAGGCTTC−3´であった。β−2−ミクログロブリンβ 2Mのプライマーは、順方向が5´−GATGAGTATGCCTGCCGTGTG−3´、逆方向が5´−CAATCCAAATGCGGCATCT−3´であった。標的遺伝子の相対的な発現量を、Wellmann et al.(Clinical Chemistry. 47 (2001) 654-660, 25)の式を用いてハウスキーピング遺伝子β2Mと比較することによって算出した。プライマー対の効率は記載にしたがって決定した(A. Kadl, et al. Vascular Pharmacology. 38 (2002) 219-227)。
IA−PBMC(5×105個)および生存PBMCを、加湿雰囲気中で24時間インキュベートした。上清を24時間後に収集し、−80℃でただちに凍結し、評価を実施するまで凍結させた。各PBMC細胞の溶解液をコントロールとして使用した。血管新生促進因子(VEGF−A、CXCL−8/IL−8、GMCSF、GCSF)およびMMP9(つまり、c−キット細胞に許容されている遊離因子)の放出を、製造業者の指示にしたがってELISA(R&D社、アメリカ)を使用して分析した。プレートを、Wallac Multilabel counter 1420(PerkinElmer社、アメリカ)を用いて、450nmで読み取った。
生体内実験用に、同系のラットのPBMCを、前もってヘバリン処置しておいたラットから心臓を穿刺(punctuation)することによって得られた全血から密度勾配遠心によって分離した。生体内実験を実施するために、Cs−137セシウムを45Gyで照射することによって、アポトーシスを誘導し、上述のように18時間培養した(アネキシン染色>IA−PBMCの80%、アネキシン染色<生存PBMCの30%、1×106個/ml)。
上述のようにLADを結紮することによって、雄の成体のスプラーグドーリーラットにおいて心筋梗塞を誘導した(Trescher K, et al. Cardiovasc Res. 2006: 69(3): 746−54)。簡潔に説明すると、キシラジン(xylazin)(1mg/身体重量100g)およびケタミン(ketamin)(10mg/身体重量100g)の混合物を用いて、動物に腹腔内麻酔し、機械を用いて人工呼吸を行った。左側方の開胸手術を実施し、6−0 proleneを用いて左心房の下方のLADの周囲を結紮した。虚血の開始直後に、0.3mlの細胞培養培地に懸濁した8×106個のアポトーシスPBMCを、尾静脈を介して注射した。細胞培養培地だけの注射、生存PBMCの注射、および偽処理を、それぞれ、この実験の設定環境においてネガティブコントロールとして使用した。ラットの実験計画を、図1(図1中a、図1中b)に示す。
8×106個の同系のラットのPBMCを、15μMのカルボキシフルオセイン二酢酸サクシニミジルエステル(carboxyfluorescein diacetate succinimidyl ester; CFSE、Fluka Bio−Chemika社、Buchs,スイス)を用いて室温で10分間標識した。標識を、ウシ胎仔血清(FCS)を添加することによって停止させた。アポトーシスを誘導し(アネキシンV>70%)、結紮術後に細胞を注射した。手術の72時間後にラットを屠殺し、肝臓、脾臓、および心臓を標準的な手順にしたがって処理し、凍結切片(n=4)を得た。試料を、上述のように共焦点レーザ走査顕微鏡法(ZEISS LSM 510レーザ走査顕微鏡、ドイツ)によって分析した(Ker-jaschki D, J Am Soc Nephrol. 2004; 15: 603-12)。
すべての動物を、実験的に梗塞を起こしてから72時間後または6週間後に屠殺した。心臓を外植し、次に、梗塞を起こした最大の領域(n=8〜10)を薄片化した。薄片を10%の中性の緩衝化ホルマリンで固定し、(免疫)組織染色をするためにパラフィンに埋め込んだ。組織試料を、ヘマトキシリン−エオシン(H&E)およびelastic van Gieson(evg)を用いて染色した。免疫組織的評価を、CD68(MCA 341R、AbD Serotec社、イギリス)、VEGF(05−443、Upstate/Milipore、アメリカ)、Flk−1(sc−6251、Santa Cruz Biotechnology社、アメリカ)、CD34(sc−52478、Santa Cruz Biotechnology社、アメリカ)、c−キット(sc−168、Santa Cruz Biotechnology社、アメリカ)、S100β(sc−58841、Santa Cruz Biotechnology社、アメリカ)を対象とする以下の抗体を用いて実施した。組織試料を、Olympus Vanox AHBT3顕微鏡(Olympus Vanox AHBT3、Olympus Optical Co. Ltd.社、日本)を用いて拡大率×200で評価し、ProgRes Capture−Pro C12 plus カメラ(Jenoptik Laser Optik Systeme GmbH社、ドイツ)を使用してデジタル方式で撮像した。
梗塞を起こした領域の大きさを決定するために、Image Jプラニメーター分析用ソフトウェア(Rasband, W.S., Image J, U. S. National Institutes of Health、アメリカ)を使用した。梗塞を起こした心筋組織の広がり(左心室における%)を、梗塞を起こした領域の外周の面積を左心室の心内膜および心外膜の外周の面積全体で割り算することによって算出した。壊死した領域が比較しやすいようにevgで染色した組織試料に対して、面積測定評価を実施した。梗塞の大きさを、左心室の面積全体に対する比率%で示す。
心筋梗塞の誘発の6週間後に、100mg/kgケタミン(ketamin)および20mg/kgキシラジン(xylazin)を用いて、ラットを麻酔した。Vivid 5システム(General Electric Medical Systems社、アメリカ)を用いて、超音波検査を実施した。動物を割り当てられる投与グループに対して、経験豊富な観察者が盲検式で分析を行った(EW)。Mモード追跡を、傍胸骨の短軸図から記録し、機能的な収縮および拡張パラメータを得た。心室の直径および体積を、収縮期および拡張期において評価した。分画収縮率(fractional shortening)を下記のように算出した。

SPSSソフトウェア(SPSS Inc.、アメリカ)を用いて、統計解析を実施した。すべてのデータは、平均値±標準偏差として与えられている。Kolmogorov−Smirnov試験を利用して、正規分布を検証した。従属変数を対象とする対応のある両側t検定、および独立変数を対象とする対応のないt検定を使用して、有意性を算出した。ボンフェローニ・ホルム補正を用いて、複数の試験のp値を調節した。p値(<0.05)は、統計的に有意であると考えられた。
〔セシウムの照射によるアポトーシスの誘導(IA−PBMC)〕
アポトーシス細胞の免疫調節能力を評価するために、まず、ヒトの末梢血単核球(PBMC)において、セシウム照射によるアポトーシスの誘導に対する細胞の反応を、フローサイトメーターを用いて、アネキシン−V/PI染色を利用したフローサイトメトリーによって判定した。生存PBMCと比較すると、セシウム照射したPBMCはアネキシンに対して時間に依存する陽性を示し、24時間以内にピーク値を示した。生細胞をコントロールとして使用した(図2a)。アネキシン−V結合は、インビトロですべてさらに24時間後に最高であったので、この培養期間後に調査を実施した(IA−PBMC)。生存PBMCを、RT−PCRを用いた実験および上清を用いた実験において、コントロールとして使用した。
インターロイキン−1βおよびIL−6は、インビボにおける心筋梗塞の主要な炎症促進メディエーターであると考えられている。IA−PBMCが細胞反応に対して効果を有するかどうかという仮説をテストするために、ヒトの単球およびPBMCをIA−PBMCとともに共インキュベートし、標的細胞をLPSで刺激した。どちらの細胞型でも、ELISAによって評価したところ、培養物中におけるIL−1βおよびIL−6の分泌が投与量に応じて減少することがわかった(図2中b、図2中c)。同系モデルにおけるIA−PBMCの抗増殖効果を検証するために、混合リンパ球反応(MLR)を使用した。同系の精製済みT細胞を採用し、これらのエフェクター細胞を、IA−PBMCに添加して、あるいは添加せずに、投与量を段階的に変化させた樹状細胞とともにインキュベートした。図2中dは、IA−PBMCの共インキュベートによって、増殖速度が用量に応じて低下することを示している。
EPCの分離に関連することが知られているタンパク質のmRNAの転写が、照射により増加するかどうかを調査するために、分離後、およびアポトーシスの誘導(24時間)後に、PBMCを分析した。生存PBMCをコントロール(生存PBMCまたはIA−PBMC)として使用した。RNA転写は、RT−PCRによって決定したVEGFの発現に比べてほとんど差を示さなかったが、IL−8/CXCL8およびMMP9の大幅な増加を示した。IA−PBMCにおけるIL−8/CXL8の場合のピークとなる誘導は、それぞれ、生細胞における2倍に対して6倍であり、MMP9の場合は5倍に対して30倍であった(図2中e)。
IA−PBMCおよび生存PBMCに由来するSNを、24時間の培養の後に、ELISAを使用して、VEGF、IL−8/CXCL8、GMCSF、GCSF、およびMMP9について定量した。図2中fからわかるように、VEGF、IL−8/CXCL8、およびMMP9は増加を示した。GM−CSFおよびG−CSFは検出されなかった。注目すべきは、MMP9が細胞溶解物においてピーク値を示すという知見であった。
骨髄中の間質細胞は構造的には線維芽細胞であるから、線維芽細胞とIA−PBMCおよび生存PBMCに由来するSNとの共インキュベーションには、EPC分離の原因となる、VEGF、IL−8/CXCL8、およびMMP9のmRNA転写因子を増加させる能力があるかどうかを調べることが必要であった。RT−PCRを4時間後および24時間後に実施した。IA−PBMC SN中で培養された細胞におけるIL−8/CXCL8について、最高レベルの誘導が検出された。誘導レベルは、コントロールに比べると4時間後にほぼ120倍に到達した。この反応は24時間後にも存在する。同等の反応がVEGFの場合に見られた。その一方で、MMP9の上方制御は、主に24時間が経過してから見られた。このデータは、BMにおける血管新生促進効果の原因となる、線維芽細胞によるmRNAの生成量の増加を促進するパラクリン因子を、SNが含んでいることを示唆している(図2中g)。
培養されたIA−PBMCはインビトロでは抗炎症性でもあり、血管新生促進性でもあることは証明できたので、IA−PBMCおよび生存PBMCを、急性のラットAMIモデルに注射した。まず、これらの培養細胞が梗塞後にどこに自動誘導されているのかを決定することが必要であった。LADの動脈結紮の直後に、CFSE標識IA−PBMCをラットの尾静脈に注射した。代表的な組織像は、図3中a、図3中b、図3中cに示すとおりである。CFSE IA−PBMCは大部分が、脾臓組織および肝組織において72時間以内に捕捉される。心臓で観察された細胞はなかった。
H.E.染色をさらに詳しく調べると、コントロールの梗塞および生存白血球(生存PBMC)で処理したAMIのラットは、創傷領域において、肉芽組織に応じた混合細胞浸潤を示し、AMIから72時間以内に多量の好中球、マクロファージ/単球、リンパ単核細胞、線維芽細胞、および活性化された増殖性内皮細胞が、ジストロフィーの心筋細胞と混合されていた(図3中d、図3中e)。その一方で、IA−PBMCで処理したAMIのラットは、好酸球性の細胞質、高密度な核、および円形状から紡錘形状の形態を有する中程度の大きさの単球様の細胞からなる創傷領域において、高密度で単形性の浸潤を示した(図3中f)。また、リンパ単核細胞、特に血漿細胞、線維芽細胞、および内皮細胞は、ほとんど検出できなかった。免疫組織学的解析によって、IA−PBMC AMIのラットにおける細胞浸潤は、残りの2つのグループでは非常に弱い、大量のCD68+単球/マクロファージ(図3中i)からなることがわかった(それぞれ、MCI、生存PBMC、IA−PBMC、強拡大視野、HPF、60.0±3.6、78.3±3.8、285.0±23.0(SEM))(図3中g、図3中h)。ビメンチンについて陽性の間葉系細胞の含有物は、すべてのグループにおいて同様であった。その一方で、S100+樹状細胞は、コントロール梗塞(処置グループ(図3中j、図3中k、図3中hの代表的な組織像、n=5)に比べると、それぞれ、AMI、生存PBMC、IA−PBMC、HPF 15.6±1.7、12.4±2.3、8.4±1.2(SEM))において、多く見られた。
IA−PBMCは、好酸球性の細胞質および高密度な核を有する中程度の大きさの単球様の細胞からなる創傷領域において、高密度で単形性の浸潤を示したので、血管再生および再生力に関連する複数の表面マーカーを調査した。IA−PBMCで処理したAMIグループにおいてH.E.染色で特定されるこの細胞集団は、血管内皮増殖因子(VEGFa)、Flk−1、およびc−キット(CD117)について、非常に高い陽性の染色を示した(図4中c、図4中f、図4中i)。どちらのマーカーの発現も、コントロールAMIおよび生存PBMCで処理したAMIグループにおいて低下した(図4中a、図4中b、図4中d、図4中e、図4中j)。興味深いことに、IA−PBMCで処理したAMIは、細胞が非常に密集している梗塞を起こした領域の内部において、CD34+細胞の増加を示した。なお、この領域は、コントロール(G、H)に比べて、34+細胞(I)のコロニー形成を推定的に指す、血管の構造に由来している(代表的な組織像、n=5)。
心筋梗塞を誘発させてから6週間後に外植した心臓から得た、EVGで染色した組織試料に対して実施したプラニメーター分析において、生理食塩水を与えられたラットは、左心室の24.95%±3.58(SEM)を超えて延び、拡張の徴候を有するコラーゲンの瘢痕を示す。IA−PBMCで処理したラットでは、これらの徴候はほとんど抑止され、梗塞のサイズは5.81%±2.02(SEM)であった。なお、生存PBMCで処理したラットの梗塞のサイズ14.3%±1.7(SEM)であった(図5中a、図5中b、図5中c)。
同系培養IA−PBMCを静脈内に注入すると、生存PBMCまたは培地で処置した動物に比べて、心エコー検査のパラメータが大幅に向上する。収縮分画率(SF)は、偽処理を行った動物では29.16%±4.65(SEM)、培地で処理したAMI動物では18.76%±1.13(SEM)、生存PBMCで処理したAMIグループでは18.46%±1.67(SEM)、IA−PBMCで処理したラットでは25.14%±2.66(SEM)であった(図5e)。駆出分画率(EF)は、偽処理を行ったラットでは60.58%±6.81(SEM)であり、培地で処理したAMIの動物では42.91%±2.14(SEM)まで低下、生存PBMCを与えられた動物では42.24%±3.28(SEM)まで低下した。その一方で、IA−PBMCで処理したラットは、EFが53.46%±4.25であった。
これらの知見によって、放射線の照射を受けたアポトーシスPBMC(IA−PBMC)が、インビトロで免疫抑制を誘導すること、および血管新生促進タンパク質の分泌に関連することが実証された。したがって、培養済み生存PBMCおよびIA−PBMCを急性のラットAMIモデルに注射し、この治療法がFLK1+/c−キット+陽性のEPCの梗塞心筋への大量の自動誘導を72時間以内に惹起し、6週間以内に有意な機能回復を引き起こすことを実証した。
活性化された末梢血単核球(PBMC)およびその上清(SN)は、創傷の再生において有益であると考えられている(Holzinger C et al. Eur J Vasc Surg. 1994 May; 8(3): 351-6.)。不活性化PBMCおよび不活性化PBMCに由来するSNには、実験的な急性心筋梗塞(AMI)および創傷を形成したモデルにおいて薬効があることが、実施例1において示された。PBMCの不活性化は実験的に検証されなければならないので、PBMCの培養が、T細胞活性化マーカー(CD69、CD25)の強化または炎症性サイトカインの分泌(単球の活性化=TNFα、T細胞活性化=INFγ)の強化につながるかどうかを調べた。対照実験では、培養されたT細胞が、CD3 mAb刺激または植物性血球凝集素(Phytohemagglutinin; PHA)によって誘導される。
健常人からの静脈血をEDTA管に収集した。フィコール・ハイパック密度勾配分離(density grade separation)を実施した後に、PBMCを収集し、生存細胞と放射線の照射を受けたアポトーシス細胞(IA−PBMC)とに分けた。アポトーシス細胞を得るために、PBMCを60Gy(セシウム137)に暴露した。フローサイメトリー法による分析を実施するために、500,000個のPBMCを200μlの無血清培地中で培養した。細胞に対してPHA(7μg/ml)もしくはCD3−mAb(10μg/ml)で刺激を与えるか、または刺激を全く与えなかった。24時間のインキュベーションの後に、細胞を洗浄し、CD3、CD69、およびCD25について染色(R&D System社)し、FC500(Coulter社)で表面活性化マーカーについて評価した。ELISAアッセイを実施するために、PBMCを、PHA刺激またはCD3刺激を与えるものと与えないものとに分けて、2.5×106個/mlの密度で一晩培養した。24時間後に、上清を回収し、−20℃で凍結させた。TNF−α(R&D社)用およびINF−γ(Bender社)用の市販のELISAキットを購入した。簡潔に説明すると、MaxiSorpプレートをTNF−αおよびINF−γに対する抗体でコーティングし、一晩保存した。24時間後に、プレートを洗浄し、試料を各ウェルに2個ずつ添加した。インキュベーションならびに検出用抗体およびStrep−tavidin−HRPの添加の後、TMB基質を各ウェルに添加した。発色後、スルヒック酸(sulphic acid)を添加することによって、酵素反応を停止させた。光学濃度値を、Wallac Victor3プレート読み取り装置で読み取った。
FACS分析
CD3で刺激されたT細胞およびPHAで刺激されたT細胞は、24時間のインキュベーション後に、活性化マーカーCD69およびCD25の上方制御を示した。無刺激の細胞およびアポトーシス細胞は、ほんのわずかな量のCD69およびCD25を発現させるだけであった(図6中a(代表的な試料、図6中b、ヒストグラム、n=4)。統計的有意性は星印で表わす(xxp<0.001、xp<0.05)。
無刺激のPBMCに由来する上清においてはTNF−αもINF−γもいずれも検出されなかったが、PHAまたはCD3で刺激を与えたPBMCから得られた上清は、ELISA分析が示唆するように、これらのサイトカインが高い値であることを示した(星印**p<0.001、*p<0.05、n=8)。この結果は、無刺激のPBMCとは炎症性サイトカインの分泌パターンが異なることを明確に示している。
以上のデータは、「無刺激PBMC」が、刺激PBMC(PHAおよびCD3 mAb)とは異なる別の表現型(活性化マーカー、サイトカインの分泌)であることを示唆している。
本実施例の目的は、特異的(CD3)、非特異的(レクチン、PHA)、および同種のT細胞の誘発(混合リンパ球反応、MLR)を、2日(CD3、PHA)および5日(MLR)間の刺激アッセイにおいて利用する免疫アッセイに比べると、PBMCは増殖活性を有しないことを証明することである。
PBMCを、フィコール密度勾配遠心法によって若い健常人から分離し、0.2%のゲンタマイシン硫酸塩(Sigma Chemical Co.社、アメリカ)および1%のL−グルタミン(Sigma Chemical Co.社、アメリカ)を含有するRPMI(Gibco社、アメリカ)中で、200μLにつき細胞1×105個の割合で再懸濁した。レスポンダー細胞に、MoAbによってCD3(10μg/ml、BD社、NJ、アメリカ)、PHA(7μL/ml、Sigma Chemical Co.社、アメリカ)に対して刺激を与えるか、あるいは、放射線の照射を受けた同種のPBMCを1:1の比(MLRの場合)で用いることによって刺激を与えた。プレートを48時間または5日間インキュベートし、その後、3[H]チミジン(3.7×104Bq/ウェル;Amersham Pharmacia Biotech社、スウェーデン)を用いて18時間パルスを与えた。細胞を回収し、3[H]チミジンの取り込み量を液体シンチレーション計数器において測定した。
刺激PBMCは、3[H]チミジン取り込み法によって測定した増殖速度が、RPMI中で無刺激で培養される生存PBMCに比べて高いことを示した(図8)。この効果は、T細胞に特異的な刺激(PHA、CD3)を付加することによって、増殖が抗原提供細胞(MLR)によって誘導されたアッセイにおいても、観察された。
この一連の実験は、培養液中で最長5日間まで保持される生存PBMCが増殖しない一方で、異なる方法で刺激を与えられたPBMCは顕著な増殖反応を示したことを示唆している。以上のデータから、無刺激でPBMCを培養しても増殖反応を引き起こさないと結論できる。
血管再生と炎症とはインビボにおいて強く関連しているので、これらのPBMCのセクレトームがT細胞に対して抗増殖効果をも示し、その結果、炎症の免疫反応に干渉するかどうかを調べた。
フィコール密度勾配遠心法によって若い健常人から分離したPBMC(2.5×106個/ml)を、0.2%のゲンタマイシン硫酸塩(Sigma Chemical Co.社、アメリカ)および1%のL−グルタミン(Sigma Chemical Co.社、アメリカ)を含有するRPMI(Gibco社、CA、アメリカ)中で24時間インキュベートすることによって、セクレトームを得た。上清を細胞分画から分離し、−80℃で保存した。増殖アッセイを実施するために、分離後に、同種のPBMCを200μLのRPMIにつき細胞1×105個の割合で再懸濁した。レスポンダー細胞として、MoAbによってCD3(10μg/ml、BD社、アメリカ)またはPHA(7μL/ml、Sigma Chemical Co.社、アメリカ)に対して刺激を与えた。上清の異なる希釈物を添加した。プレートを48時間インキュベートし、その後、3[H]−チミジン(3.7×104Bq/ウェル;Amersham Pharmacia Biotech社、スウェーデン)を用いて18時間パルスを与えた。細胞を回収し、3[H]−チミジンの取り込み量を液体シンチレーション計数器において測定した。
3[H]−チミジン取り込み法によって測定する増殖速度は、同種のPBMCのセクレトームにおいて、正のコントロールに比べて有意に低減することを示した(図9)。この効果は用量に依存し、PHA刺激時とともに、抗CD3刺激時にも観察された。
この一連の実験は、生存PBMCから得られるセクレトームを24時間培養液中に保持すると、有意な抗増殖効果をインビトロで示すことを示唆している。以上のデータは、PBMCに由来する上清または凍結乾燥された状態にある上清は、低酸素症によって誘発される炎症またはその他の高炎症疾患(例えば自己免疫疾患、炎症性皮膚疾患など)に関連するヒトの疾患を治療するための、治療用処方物として作用する潜在的な可能性があることを示唆している。
実施例1では、末梢血液に由来し、放射線の照射を受けた培養後のアポトーシス細胞を注射することによって、ラットにおける実験的な心筋梗塞の後の機能的な心臓の回復が大幅に改善されることが示された。この改善は、アポトーシス細胞が持つ免疫を抑制する特性、血管新生促進効果、およびc−キット+血管内皮前駆細胞(EPC)の増大した自動誘導の誘導に基づいている。
〔インビトロアッセイにおけるヒトPBMCの細胞培養〕
ヒト末梢血単核球(PBMC)を、上述のフィコール密度勾配遠心法(Ficoll density grade centrifugation)によって採取した。ヒトPBMCにおいてアポトーシスを誘導するために、細胞に60Gyの放射線を照射した(irradiation automat for human blood products、Department of Hematology、General Hospital Vienna)。生存PBMCおよび放射線の照射を受けたアポトーシス(IA−)PBMCを、どちらも各種の細胞密度(1×106、5×106、10×106、および25×106個の細胞/ミリリットル、n=5)で37℃で24時間インキュベートした。その後、上清を得て、分泌されるタンパク質のレベルを、製造業者が提供するプロトコルにしたがって、酵素結合免疫吸着測定法(ELISA、R&D System社、Minneapolis、アメリカ)によって測定した。
生体内実験に用いる同系ラットのPBMCを、前もってヘバリン処置しておいたラットから、右心房を穿刺することによって得られた全血から、密度勾配遠心法によって分離した。生体内実験を行うために、45GyのCs−137セシウムを照射することによって、アポトーシスを誘導し、37℃で25×106個の細胞/ミリリットルの細胞密度で培養した。照射によるアポトーシスの誘導を、フローサイトメトリー(IA−PBMCの場合はアネキシンV染色>80%、生存PBMCの場合はアネキシンV染色<20%)によって測定した。細胞を加湿雰囲気(5% CO2、37℃、相対湿度95%)中で24時間インキュベートした。上清を除去し、50mMの酢酸アンモニウムに対して3.5kDaのカットオフ(Spectrum laboratories、Breda、オランダ)で4℃で一晩透析した。その後、上清を無菌濾過し、凍結乾燥した。凍結乾燥したセクレトームを−80℃で保存し、実験を行うたびに新たに再懸濁した。セクレトームを無作為抽出して、そのpH値を計測した。凍結乾燥した粉末を、次の実験を実施するまで−80℃で保存した。
ウイーン医科大学動物研究委員会(committee for animal research, Medical University of Vienn)から、動物実験の許可を得た。実験はすべて米国国立衛生研究所(National Institutes of Health (NIH))が発行するGuide for the Care and Use of Laboratory Animalsにしたがって実施した。左前側の下行動脈(LAD)を結紮することによって、雄の成体スプラーグドーリーラットにおいて、心筋梗塞を誘導した。簡潔に説明すると、キシラジン(xylazin)(1mg/身体重量100g)およびケタミン(ketamin)(10mg/身体重量100g)の混合物を用いて、動物に腹腔内麻酔し、機械を用いて人工呼吸を行った。左側方の開胸術を実施し、6−0 proleneを用いて左心房の下方のLADの周囲を結紮した。虚血の開始直後に、0.3ml細胞培養培地中に再懸濁された8×106個のアポトーシスPBMCから得られた、凍結乾燥させた上清を、大腿静脈に注射した。細胞培養培地だけの注射、生存PBMCの上清の注射、および偽処理を、それぞれ、この実験の設定環境において、ネガティブコントロールとして使用した。実験設計を図1に示す。

ソフトウェア(Graph Pad Prism、アメリカ)を用いて統計解析を実施した。すべてのデータを、平均値±標準誤差として与える。従属変数を対象とする対応のある両側t検定、および独立変数を対象とする対応のないt検定を使用して有意性を算出した。
〔ELISAによるIA−PBMCおよび生存PBMCが分泌するパラクリン因子の決定〕
結果を図10〜15に示す。
実施例1では、急性心筋梗塞(acute myocardical infarct; AMI)の動物モデルにおける、PBMCセクレトームの抗炎症効果が示された。本実施例では、AMIを誘発後のPBMCセクレトームの適用が、免疫応答を大きく下方制御することによって心筋の炎症性損傷を阻害することを示す。
〔PBMCセクレトームの生成〕
健常人から得られるPBMCを、フィコール密度勾配遠心法によって分離した。細胞を、Ultra Cultureの培地(Lonza社、Basel、スイス)中で1×106個/ml(sup liv)の濃度で再懸濁した。アポトーシスPBMCからセクレトームを生成するために、60Gy(sup APA)を照射することによって、アポトーシスを誘導した。細胞を、加湿雰囲気(5% CO2、37℃、相対湿度95%)下で24時間インキュベートした。上清を除去し、50mMの酢酸アンモニウムに対して3.5kDaのカットオフ(Spectrum laboratories、Breda、オランダ)で4℃で一晩透析した。その後、上清を無菌濾過し、凍結乾燥した。凍結乾燥したセクレトームを−80℃で保存し、実験を行うたびに新たに再懸濁した。セクレトームを無作為抽出して、そのpH値を計測した。
MACSビーズシステム(Miltenyi社、Bergisch Gladbach、ドイツ)を使用して、CD4+T細胞以外の細胞を枯渇させることによって、CD4+細胞を分離した。細胞を新たに準備し、ただちに各実験に使用した。
アポトーシスを、市販のアネキシンV/PIキット(BD社、New Jersey、アメリカ)を用いてフローサイトメトリーによって検出した。アポトーシスをアネキシン陽性染色法によって定義し、後期アポトーシスをPIの陽性度によって定義した。
PBMCまたは精製済みCD4+細胞を、96個の丸底ウェルを有するプレートにおいて、0.2%のゲンタマイシン硫酸塩(Sigma Chemical Co.社、St. Louis、MO、アメリカ)、0.5%のβ−メルカプトエタノール(Sigma Chemical Co.社、St Louis、MO、アメリカ)、および1%のGlutaMAX−I(インビトロゲン社、Carlsbad、CA、アメリカ)を補充したUltra Culture中で、1×105個/ウェルの濃度にまで希釈した。PHA(7μg/ml、Sigma Chemical Co.社、アメリカ)、CD3(10μg/ml、BD社、New Jersey、アメリカ)、IL−2(10U/ml、BD社、アメリカ)を用いて、または放射線の照射を受けた(60Gy)同種のPBMCを1:1の比で用いて、MLRを得るために、細胞に刺激を与えた。細胞を、複数の濃度のPBMCセクレトーム、IL−10、またはTGF−βを用いて48時間または5日間(MLR)インキュベートした。その後、3[H]チミジン(3.7×104Bq/ウェル;Amersham Pharmacia Biotech社、Uppsala、スウェーデン)を用いて、細胞に18時間パルスを与えた。細胞を回収し、3[H]チミジンの取り込み量を液体シンチレーション計数器において測定した。
精製済みCD4+細胞を、抗CD3(10μg/ml)を用いて刺激し、複数の濃度のPBMCセクレトームで共インキュベートした。細胞を、標準的なフローサイメトリー法の染色プロトコルにしたがって、CD69およびCD25について染色し、フローサイトメーターFC500(Beckman Coulter社、Fullerton、CA、アメリカ)で分析した。
予備実験において、生細胞(sup liv)から得られるPBMCの上清の抗増殖特性について試験を行った。抗CD3刺激実験およびPHA刺激実験では、増殖速度が、セクレトーム(n=10)を添加することによって大幅に低下した。
これらの実験によって、PBMCのセレクトームがインビトロで免疫抑制特性を有することが初めて示された。上清は、a)抗CD3刺激実験、PHA刺激実験、およびMLR刺激実験において増殖速度を低下させ、b)T細胞の誘導時にCD4+細胞のアポトーシスを誘導し活性化を阻害する効力を有することが示された。
Claims (16)
- 体内の炎症症状(好ましくは虚血に関連する体内の症状)を治療するための薬剤であって、
a)末梢血単核球(peripheral blood mononuclear cell; PBMC)またはそのサブセットを含有する生理的溶液、または
b)上記溶液a)の上清を包含し、
上記溶液a)が、PBMCまたはそのサブセットを、PBMC増殖物質およびPBMC活性化物質を含有しない生理的溶液中で、少なくとも1時間培養することによって得られる、薬剤。 - 上記炎症症状が、心筋虚血、肢虚血、組織の虚血、虚血再灌流障害、狭心症、冠動脈疾患、末梢血管疾患、末梢動脈疾患、脳卒中、虚血発作、心筋梗塞、うっ血性心不全、外傷、腸疾患、腸間膜梗塞、肺梗塞、骨折、歯を移植した後の組織の再生、自己免疫疾患、リウマチ性疾患、同種移植、および同種移植の拒絶からなる群より選択されることを特徴とする、請求項1に記載の薬剤。
- 上記末梢血単核球(PBMC)のサブセットが、T細胞、B細胞、またはNK細胞であることを特徴とする、請求項1または2に記載の薬剤。
- 上記生理的溶液が、生理食塩水(好ましくは生理的NaCl溶液)、全血、血液分画物(好ましくは血清)、または細胞培養培地であることを特徴とする、請求項1〜3のいずれか一項に記載の薬剤。
- 上記細胞培養培地が、RPMI、DMEM、X−vivo、およびウルトラカルチャー(Ultraculture)からなる群より選択されることを特徴とする、請求項4に記載の薬剤。
- 上記PBMCが、ストレスを誘発する条件の下で培養されたものであることを特徴とする、請求項1〜5のいずれか一項に記載の薬剤。
- 上記ストレスを誘発する条件には、低酸素症、オゾン、熱、放射線、化学物質、浸透圧、pHの変化、またはこれらの組み合わせが含まれることを特徴とする、請求項6に記載の薬剤。
- 上記PBMCまたはそのサブセットが、少なくとも10Gy(好ましくは少なくとも20Gy、より好ましくは少なくとも40Gy)、オゾン、高温、またはUV線によって培養中にストレスを与えられたものであることを特徴とする、請求項1〜7のいずれか一項に記載の薬剤。
- 請求項1〜8のいずれか一項に記載の薬剤であって、皮下投与、筋肉内投与、器官内投与、および静脈内投与を実施できるように構成されたことを特徴とする、薬剤。
- 上記溶液a)または上記上清b)が凍結乾燥されたものであることを特徴とする、請求項1〜9のいずれか一項に記載の薬剤。
- 上記PBMCまたはそのサブセットが、上記溶液中で少なくとも4時間(好ましくは少なくとも6時間、より好ましくは少なくとも12時間)培養されたものであることを特徴とする、請求項1〜10のいずれか一項に記載の薬剤。
- 請求項1〜11のいずれか一項に記載の薬剤を、体内の炎症症状(好ましくは虚血に関連する体内の症状)を治療するための薬物の製造のために用いる使用方法。
- 請求項1〜11のいずれか一項に記載の薬剤を調製する方法であって、
a)末梢血単核球(PBMC)またはそのサブセットを準備する工程と、
b)上記工程a)のPBMCを、PBMC増殖物質およびPBMC活性化物質を含有しない生理的溶液中で少なくとも1時間培養する工程と、
c)上記工程b)の細胞および/またはこの細胞の上清を単離する工程と、
d)上記工程c)の細胞および/または上清を用いて上記薬剤を調製する工程とを含む、方法。 - 上記工程b)の前または上記工程b)の途中で、上記PBMCを、ストレスを引き起こす条件に暴露し、
上記ストレスを引き起こす条件には、低酸素症、オゾン、熱、放射線、化学物質、浸透圧、pHの変化、またはこれらの組み合わせが含まれることを特徴とする、請求項13に記載の方法。 - 上記工程b)の前または上記工程b)の途中で、上記PBMCが、少なくとも10Gy(好ましくは少なくとも20Gy、より好ましくは少なくとも40Gy)、オゾン、高温、またはUV線に暴露されることを特徴とする、請求項13または14に記載の方法。
- 請求項13または15に記載の方法によって得られる薬剤。
