Abstract
Description

生理学
自律神経系(automatic nervous system)(ANS)は、「不随意(involuntary)」器官を調節し、一方、随意(骨格)筋の収縮は、体性運動神経によって制御される。不随意器官の例は、呼吸および消化器官を含み、また同様に、血管および心臓を含む。しばしば、ANSは、不随意的で再帰的な方法で機能して、たとえば、腺を調節し、皮膚、眼、胃、腸、および膀胱の筋肉を調節し、心筋および血管の周りの筋肉を調節する。
治療
本主題は、迷走神経刺激などの神経刺激を提供するシステム、デバイス、および方法に関する。種々の実施形態は、神経刺激治療を提供するための、外部かまたは内部の、独立型デバイスを提供する。たとえば、本主題は、MI後治療または心不全治療の一部として神経刺激によって抗リモデリング治療を送出してもよい。神経刺激はまた、制限ではなく、例として、高血圧治療および条件付け療法において使用されてもよい。本主題はまた、てんかん、うつ病、疼痛、肥満、高血圧、睡眠障害、および精神神経傷害を処置するための治療などの、非心臓用途で実施されてもよい。種々の実施形態は、神経刺激を、徐脈ペーシング、抗頻脈治療、リモデリング治療などのような1つまたは複数の他の治療と統合するシステムまたはデバイスを提供する。
神経刺激治療
神経刺激治療の例は、睡眠呼吸障害などの呼吸問題、高血圧を処置するなどのための血圧制御、心臓調律管理、心筋梗塞および虚血、心不全、てんかん、うつ病、疼痛、片頭痛、摂食障害、および肥満のための神経刺激治療を含む。多くの提案される神経刺激治療は、迷走神経の刺激を含む。他の神経刺激治療のこの一覧表作成は、網羅的な一覧表作成であることを意図しない。神経刺激は、電気、音響、超音波、光、および磁気治療を使用して提供されうる。電気神経刺激は、神経カフ、静脈内給送式リード線、または経皮的電極の任意のものを使用して送出されうる。
心筋刺激治療
種々の神経刺激治療が、種々の心筋刺激治療と統合されうる。治療の統合は、相乗効果を有する可能性がある。治療は、互いに同期されることができ、検知されるデータは、治療間で共有されうる。心筋刺激治療は、心筋の電気刺激を使用して心臓治療を提供する。心筋刺激治療の一部の例が、以下に提供される。
システム
本主題の種々の実施形態は、神経刺激(neural stimulator)(NS)デバイスまたはコンポーネントに関する。神経刺激器の例は、高血圧を処置するのに使用される抗高血圧(ant-hypertension)(AHT)デバイスまたはAHTコンポーネント、および、MI後治療について神経刺激を提供するのに使用されるデバイスまたはコンポーネントを含むが、それに限定されない。本主題の種々の実施形態は、独立型の埋め込み型圧受容器刺激器システムを含み、NSコンポーネントと心臓調律管理(CRM)コンポーネントを統合した埋め込み型デバイスを含み、無線で、または、埋め込み型デバイスに接続するワイヤリード線によって、互いに通信することが可能な少なくとも1つの埋め込み型NSデバイスおよび埋め込み型CRMデバイスを有するシステムを含む。同じデバイス内かまたは別個のデバイス内で実施されるNS機能とCRM機能を統合することは、これらの治療がインテリジェントに一緒に働くことを可能にすることによってNS治療および心臓治療の態様を改善する。
検出された生理的状況に反応するNSシステム
種々の実施形態は、生理的心臓状況などの生理的状況の検出によって神経刺激を自動的に調整する(たとえば、圧受容器刺激を高める)。たとえば、圧反射治療強度が増大されて、血管拡張性反応が高められ、またおそらく、心筋虚血損傷を防止するかまたは低減しうる。種々の実施形態は、圧受容器を電気的に刺激するための刺激リード線を同様に有する心臓調律管理デバイス(たとえば、ペースメーカ、ICD、またはCRTデバイス)内にフィードバックメカニズムを含む。デバイスは、既存の方法によって心臓電気活動を監視する。心室細動(ventricular fibrillation)(VF)および心房細動(atrial fibrillation)(AF)、予め規定されたレートを超える心室頻脈(ventricular tachycardia)(VT)および心房頻脈(atrial tachycardia)(AF)、ならびに、分時換気量センサによって検出される呼吸困難、狭心症、代償不全、および虚血などの有害な心臓事象の場合、デバイスは、最大限許容可能なレベルまで圧受容器刺激を高めることによって応答する。結果として、血圧は、一時的に下がり、おそらく、虚血による心筋損傷が防止されるかまたは低減される。たとえば、高血圧を処置するデバイスの機能は、圧受容器刺激の程度を一時的に調節することによって有害な心臓事象に応答しうる場合、拡張されうる。心臓状況を識別する事象検出アルゴリズムは、神経刺激を自動的に調節し、埋め込み型デバイスが、副交感神経反応を高めることによって、検出される心臓状況に応答することを可能にし、おそらく、心筋虚血損傷が防止されるかまたは低減される。
Claims (22)
- 埋め込み型医療デバイスであって、
短期神経刺激治療について示される生理的状況を検出するよう構成された検出器と、
長期神経刺激治療および前記短期神経刺激治療を送出することが可能な神経刺激器と、
前記神経刺激器を制御して前記長期神経刺激治療を提供し、生理的状況が検出されたというインジケータを前記検出器から受信し、前記インジケータに応答して、前記神経刺激器を制御して前記短期神経刺激治療を前記長期神経刺激治療と統合するよう構成されたコントローラとを備えるデバイス。 - 前記検出器は、
虚血、
急性心筋梗塞、
不整脈、
心原性ショック、
代償性心不全の発症を検出するよう構成される、請求項1に記載のデバイス。 - 前記長期神経刺激治療は、心不全リモデリング治療を含む、請求項1に記載のデバイス。
- 前記長期神経刺激治療は、心筋梗塞後治療を含む、請求項1に記載のデバイス。
- 前記長期神経刺激治療は、抗高血圧治療を含む、請求項1に記載のデバイス。
- 埋め込み型医療デバイスであって、
第1神経刺激治療を実施して、第1の生理的状況を処置する手段と、
第2神経刺激治療について示される心臓状況である第2の生理的状況を検出する手段と、
前記第2の生理的状況を検出することに応答して、前記第1神経刺激治療および前記第2神経刺激治療を統合して前記第1および第2の生理的状況についての統合された神経刺激治療にする手段とを備えるデバイス。 - 前記第2の生理的状況は、不整脈、急性虚血事象、または急性心筋梗塞を含む、請求項6に記載のデバイス。
- 前記第1の生理的状況は、高血圧または心室リモデリングを含む、請求項7に記載のデバイス。
- 心臓調律管理治療を送出する手段をさらに備える請求項6に記載のデバイス。
- 前記第1神経刺激治療を実施する前記手段は、前記第1神経刺激治療を長期的に実施する手段を含む、請求項6に記載のデバイス。
- 前記第1の生理的状況は高血圧であり、
前記第1神経刺激治療は、抗高血圧神経刺激治療を含む、請求項10に記載のデバイス。 - 前記第1の生理的状況は心室リモデリングであり、
前記第1神経刺激治療は、心室リモデリングの進行を緩和するための抗リモデリング治療を含む、請求項10に記載のデバイス。 - 前記第1神経刺激治療は、神経刺激の無い時間によって分離される神経刺激時間を有する断続的神経刺激治療であり、
前記第1神経刺激治療および前記第2神経刺激治療を統合して前記第1および第2の生理的状況についての統合された神経刺激治療にする前記手段は、前記第2神経刺激治療の一部として送出される神経刺激のタイミングをとって、前記断続的神経刺激治療の神経刺激時間の間に起こるようする手段を含む、請求項6に記載のデバイス。 - 前記第1神経刺激治療および前記第2神経刺激治療を統合して前記第1および第2の生理的状況についての統合された神経刺激治療にする前記手段は、前記第2の生理的状況の治療が終了するまで、前記第1神経刺激治療を中止する手段を含む、請求項6に記載のデバイス。
- 前記第1神経刺激治療および前記第2神経刺激治療を統合して前記第1および第2の生理的状況についての統合された神経刺激治療にする前記手段は、前記第2の生理的状況の治療が終了するまで、前記第1神経刺激治療の強度を増加させるか、または減少させる手段を含む、請求項6に記載のデバイス。
- 第2の生理的状況を検出する前記手段は、第2の生理的状況の重篤度を検出する手段を含み、統合する前記手段は、前記第2の生理的状況の重篤度に基づいて前記第1神経刺激治療および前記第2神経刺激治療を統合する手段を含む、請求項6に記載のデバイス。
- デバイスであって、
迷走神経刺激治療を実施して慢性生理的状況を処置する手段と、
虚血を検出する手段と、
虚血を検出することに応答して、前記慢性生理的状況について前記迷走神経刺激治療を調整する手段とを備えるデバイス。 - 前記慢性生理的状況を処置するための前記迷走神経刺激治療は、高血圧を処置する治療または心筋梗塞後治療を含む、請求項17に記載のデバイス。
- 虚血は生理的心臓状況であり、デバイスは少なくとも第2の生理的状況を検出する手段をさらに備え、前記迷走神経刺激治療を調整する前記手段は、前記第2の生理的心臓状況について前記迷走神経刺激治療を調整する手段を含む、請求項17に記載のデバイス。
- 少なくとも第2の生理的状況を検出する前記手段は、前記第2の生理的状況および第3の生理的状況を検出する手段を含み、前記迷走神経刺激治療を調整する前記手段は、虚血、前記第2の生理的状況、および前記第3の生理的状況の組合せについて前記迷走神経刺激治療を調整する手段を含む、請求項19に記載のデバイス。
- 不整脈を検出する手段をさらに備え、前記迷走神経刺激治療を調整する前記手段は、不整脈を検出することに応答して前記迷走神経刺激治療を中止する手段を含む、請求項17に記載のデバイス。
- 急性心筋梗塞を検出する手段をさらに備え、前記迷走神経刺激治療を調整する前記手段は、迷走神経の刺激によって前記検出された心筋梗塞に応答する手段を含む、請求項17に記載のデバイス。
