排尿の問題は、恐らく、人間の歴史と同じだけの長きにわたり付きまとってきた。歴史には、古代中国人が急性尿閉の人々の苦痛を緩和するのに玉ねぎの茎を使用していたことが記されている。文献は、この様な問題が、遥か2000年以上も前の紀元前206年に遡ることに触れている。古代ローマ人はカテーテルを使用していたことが知られており、カテーテルは、紀元前3世紀のギリシャ人医師Erasistratusによって最初に発明されたと考えられている。ローマ時代のカテーテルは、青銅で作られた細い管であった。ローマ人の婦人科医Soranusは、どの様にカテーテルを使えば、石を進路から押し出して膀胱腔へ戻し、尿の流れを回復させることができるかを説明している。ポンペイの発掘では、数点の青銅カテーテルが出土した。これらの器具は、巧く作られているが、比較的単純であり、カテーテルの設計が西暦79年の時期から西暦1700年頃まで殆ど変わっていないことが分かる。
継続的なカテーテル法では、留置カテーテルは、水が充填されたバルーンによって膀胱内に維持される。留置カテーテルは、尿を膀胱から脚又はベッドに取り付けられている袋に継続的に排出する。袋には、間を置いて尿を空けることができるように栓が設けられている。カテーテルは、通常、医師又は看護師によって挿入され、4週間から6週間毎に交換される。しかし、この設計には設置の難しさが常に付きまとっていた。これは、比較的剛性の高い厚肉のカテーテルに傷つき易い粘膜で覆われた尿道を横断させる必要のある従来型の「押し出し前進式」技術のせいである。
間欠式カテーテル
間欠式カテーテル法では、プラスチック、ゴム、又は金属製の単純なカテーテルが、患者又は介助人によって、ちょうど膀胱を完全に空にできるだけの期間、即ち、通常は約1分間挿入される。これらの一時的なカテーテルは、大抵、同じサイズの留置用カテーテルよりも直径が小さくてより堅い。男性では尿道が長く前立腺内に急な曲がりを有するため、この堅さはカテーテル法を困難にする。更に、外括約筋に到達した時、括約筋の筋肉が収縮して通過を困難にする。殆どの患者は、自分達でカテーテル法を習得し、高い自立性を獲得する。このプロセスは、日中は約3時間乃至4時間毎に繰り返され、夜間は必要に応じて繰り返される場合もある。
一部の患者で長期の膀胱排出を維持するのに使用される代わりの器械と方法として、恥骨上管の使用がある。
拡張は、段階的に大きいサイズの尿道拡張管を連続的に押し込んで尿道管腔のサイズを広げることによって達成されており、患者にとっては苦痛で外傷を被る手法である。狭窄の外科的治療には、外科的な危険性のみならず、感染、出血、及び再狭窄を含む合併症が伴い、再狭窄の場合には更なる治療が必要になることが多い。
オクルーダーは、一部の症例で、失禁を制御するのに使用される。先行技術のオクルーダーは、上で説明したカテーテル及び拡張器と同じ押し出し前進式の概念を当てはめて作られており、従って、同じ不都合に悩んでいる。基本的なオクルーダーは、尿道内に挿入して尿道を通る尿の正常なの流れを停止又は阻止するか、又は遥か膀胱の中まで操って、例えば、膀胱からの尿の流出を阻止するための栓として尿道頚部に着座させる、シャフト上の球頭部又は栓である。
ステントは、尿の流れが許容されるように狭窄部を開存支持するために植え込まれる管状の金属メッシュ装置である。ステント本体は、解剖学的構造に応じて3.5cmから6.5cmの長さであり、所定の場所に固定されるように設計によって拡張可能になっている。ステントは、メッシュであるため、孔があり、それら孔を通って組織が増殖することができるため、抜去が難しくなり、かさぶたができて尿の流れを減少させる。
尿道内弁付きカテーテルは、遠隔的に作動させる一体型の弁によって尿の流れを制御するために植え込まれる装置である。全カテーテル長さが尿道内に在るため、外部感染の可能性は低減する。現在の設計の固定機構は、バルーン又はカテーテルからの「花弁状」突起体で達成されている。上記設計は共に、装着が複雑で、抜去が難しく、弁が失陥すれば、患者は痛みを伴う危険な状況に置かれることになる。
押し出し前進式の方法論の不満を表明する先行技術の特許活動があった。カテーテルは、押し込み式装置の明らかに残された課題である外傷と苦痛を和らげることを意図したものと記載されている多種多様な螺旋状及びねじの切られた造形で飾り立てられてきた。Alvordの米国特許第207,932号、Peyretの仏国特許第564,832号、Hayesの米国特許第1,644,919号、及びJacobyの米国特許第1,888,349号は、これらの代表である。どの場合も、これらの開示は、基本的な押し出し前進式技法には根本的に欠陥があり、放棄すべきであることを認識し損い、押し出し前進方式に代わる回転前進に必要な構造の重要な特徴を解明し損なっている。
GI(消化器)管の視診及び治療に使用される現在の装置は、可撓性内視鏡である。この装置は、装置が1960年代初頭に発明されてから変わらない基本的な押し出し前進式設計のせいで、使用には高度な技量を要し、操作が難しく、患者にとっては非常に苦痛である。内視鏡の遠位先端部は、通常は、以下の部分、即ち、

b.頭方向(頭部に向かって)に盲腸から肝湾曲部まで走っている上行結腸と、
c.肝湾曲部の上四分の一区分から脾湾曲部の左上四分の一区分まで走っている横行結腸と、
d.尾方向(足部に向かって)脾湾曲部から左下四分の一区分まで走っている下行結腸と、
e.左下四分の一区分からS状結腸移行部まで走っているS状結腸と、
f.肛門管まで下方向に伸びている直腸と、
に分けられる。
大腸の全長を調べるには、高度な技術を有する施術者が必要であり、そのため処置費用が高くつく。しかしなお、同処置は患者にとっては非常に苦痛であり、鎮静が必要になる。これは、「押し出し前進式」設計に付きものの欠点のせいである。
a.十二指腸と、
b.空腸と、
c.回腸と、に分けられる。
留置及び間欠式カテーテル
この回転前進式の挿置形式によって可能になる可撓性を有する薄壁の留置及び間欠式カテーテルと関連装置及び送達用スタイレットは、外傷性が低く、医療施術者又は患者にとって使用がより簡単である。本発明のカテーテルは、螺旋又は回転技術を使用して、制御された挿入と尿道を巧く通り抜けるための可撓性を提供することによって、従来型の装置の問題を解消する。螺旋設計は、括約筋を弛緩させて、痙攣を軽減又は抑える安定した速度で通路の事前拡張を達成する。装置は、設置後、螺旋の半径方向変位と密なピッチによって固定され、身体運動又は流体の流れによって長手方向に移動しないようにされる。
膀胱出口部閉塞の治療に適用される本発明の禁制カテーテルは、TURPを受けることができないか又は受けないことを選択するBPH患者に使用することを意図している。本発明のこの実施形態は、前立腺の区域内の尿道を開存維持することができる。このカテーテルの近位(外側)端には、膀胱を空にするために押圧するか又は別のやり方で開ける流量弁を設けてもよい。カテーテルは、一度使用されたら必要に応じて取り換えられる無菌の一回限定使用の使い捨て式の品目として製造してもよい。
患者は、カテーテルを尿道口に挿入し、シャフトを回転させてカテーテルを膀胱の中へ進めるだけでよい。これは、朝、自宅で手軽に行える。ユーザーは、排尿が必要になると、可撓性シャフトの弁端を衣類から露出させ、弁を開けて、膀胱を空にする。装置は排尿の都度取り出して再挿入することはないので、感染の危険性が減る。一日の終わりに、カテーテルは簡単に抜去され廃棄される。
本発明の男性又は女性用尿道内弁付きカテーテルは、膀胱制御の治療に適用される。本発明のこの実施形態は、カテーテル内に在る弁機構によって尿の流れを制御できるようにしている。この弁は、直接的にスタイレットの様な道具の挿入によって作動させてもよいし、遠隔的に磁界装置を使用することによって作動させてもよい。
膀胱出口部閉塞の治療に適用される本発明のステントは、尿道の狭窄区域を開在維持する。ステント本体は、解剖学的構造に応じて3.5cmから6.5cmの長さであり、ステントを前進させて保持するために、本体の外径上に螺旋要素を有している。ステントの側壁は、前立腺の圧力によって押し潰されないように補強手段を有していてもよい。ステントは、蛍光透視下に、ステント本体の近位端にキー係合する取り外し可能な可撓性スタイレットを使用して尿道に挿入することができ、外来患者の処置では局所麻酔を使って挿入してもよい。
本発明の螺旋形に改造された拡張器とオクルーダーは、同様に、回転式に前進後退させるものであって、螺旋要素が或る程度までの引き延ばし機能を果たす。それぞれに直径を一回り大きくした拡張器を使用して、段階的に効果が強くなっていくようにしてもよい。
本発明を、膀胱又は他の泌尿生殖器系器官の排出のための古典的な経腹壁穿刺に使用される恥骨上カテーテルに適応させて改造したものでは、カテーテルの遠位端の螺旋部を、器官の壁に、螺旋状の羽根が器官壁の両側から伸びるように十分奥に挿置できるようになるため、カテーテルの器官に対する長手方向の滑動前進後退運動は螺旋状の羽根によって阻止される。これにより、器官壁進入点における刺激及びそれに伴う合併症の発生源が低減する。
本発明の実施形態は何れも、蛍光透視法を使用して、装置の挿置又は抜去を、或いは装置の回転方向及び回転運動さえも、監視できるように、X線不透過性であってもよいし、X線不透過性の造形、マーカー、又は、他の構成要素を設けてもよい。
ねじ付きカメラ導入器システムは、簡単に言えば、視覚化センサー及び他の用具を結腸に導入しその全長を挿通させるための新規な手段を提示している。導入器の基本構造は、本発明の、回転前進式の構造と方法論に一致しており、近位端の回転力を結腸壁に対する牽引作用に変換する柔らかく柔軟性のある螺旋状のねじを備えた、大きく柔らかい可撓性を有する虫様の管状装置である。
1.装置の長さ方向に伸びている螺旋ねじ又は渦巻き部は、
a)流体を結腸/通路に運ぶため、

2.渦巻き部は、更に、進入時に、完全なねじの形を得てカテーテルをカメラ要素の回転式に把持又は固定できるように流体で膨らませ、その後、結腸を通して装置を引くことによって装置を非回転式に抜去できるように萎ませてもよく、
3.回転式カメラ導入器を前進させる際にそれを通して見える映像画面又は画面上の画像は、処置を施術管理している人にとって都合のよいように非回転静止方式で画像が保持されるように電子的に処理してもよく、
4.装置の遠位部分は、結腸/通路の経路に沿った追跡性を高めるために、相対的により高い可撓性を持たせてもよく、
5.装置は、直腸の全長さにおいて内部支持無しに装置の遠位部分を回転させることができるだけの、近位端から遠位端へのトルク伝達能力を有してもよく、
6.遠位先端部又は帯域は、結腸壁を把持し、装置を身体の中へ進めて曲がり角を通り抜けさせる一次的な「牽引力」を提供するだけのねじ高さを有し、一方では、引きずりの無い回転式前進を支援し、結腸壁が束ねられたり手繰り寄せられないようにするには、装置の残り部分に沿っては、多少低目のねじ高さが適切であり、
7.この160cm長の器具を収納し、確実に、施術野内に留まるようにするに制御するには、少なくとも3つの方法、即ち、
a)図34に示されている小出し装置、
b)真っ直ぐな管状の構成要素、又は

という特徴と利点を含んでおり、
2.より一層「従来的な様式の内視鏡」−従来型の内視鏡を本発明の構造及び方法に合わせて改造することによってもたらされるもの−として使用することができ、その場合、本発明の利点は、以下の従来の機能、即ち、

本発明のシステムは、手動で(例えば、外科医がカテーテルを手で回転させることによって)回転させることもでき、又は代わりのやり方として、システムは動力駆動することもできるものと理解頂きたい。本発明の好適な形態では、より容易に且つより正確に、カテーテルを体内通路の中へ前進させ又はカテーテルを体内通路から後退させられるように、カテーテルを回転させるのに動力駆動装置を使用してもよい。
本発明の1つの好適な形態では、カテーテルの前方部からカスを一掃するための、洗浄システムを設けてもよい。多くの状況では、カテーテルを受け入れる体内通路はカスで覆い隠されているため、同システムは、内視鏡を体内通路に通して進める際に解剖学的構造が鮮明に見えるようにするのに役立つことであろう。洗浄システムは、内視鏡挿入中に、流体を使って体腔通路からカスを洗い流すために設けてもよい。一例として、洗浄システムは、糞便を砕いて結腸から除去するのに使用してもよく、そうすれば、結腸を通してカテーテルを進める際に、解剖学的構造が更に鮮明に見えるようになる。
これより図1、図2、及び図3を参照してゆくが、男性用のねじ付きカテーテル101は、流れ制御装置104に取り付け可能な、外ねじ103の付いた管102で構成されている。管102は、ポリウレタン材料から押出成形されており、0.06インチの内径及び0.125インチの外径103dを有し、大凡13インチの長さがある。デュロメータ硬度は、管の滑らかな外壁で測定してショアA硬度85である。遠位端105は閉鎖しており、その先端は約0.06インチの均一半径に丸められている。管102の近位端106は、直角に切り落とされ、流れ制御装置104に取り付けられている。管102は、その長さの大部分が尿道内に入っている時に、尿道の外の管の下端部に指の力で加えられるトルクに耐えて、そのトルクをねじに伝達できるだけの強度を有している。
0.25インチのねじピッチ103c対0.6インチのねじ直径103dの円周の比であるRは、1対1より遥かに小さく、その結果、1/1より大きい比の場合に比べ、回転を長手方向の牽引力に変換するスクリューねじの梃子比が上がる。
図5に示す様に、ルーメン118につながっている2つの側方排出ポート117は、形状が楕円であり、楕円の長軸は管112の軸に平行で短軸の約1.5倍であり、これは、ルーメンの直径とほぼ等しい。2つの側方ポート117は、半径方向に180度離間するように構成されており、ねじの巻きと巻きの間に嵌るように長手方向に間隔を開けて配置されている。
先ず、ユーザーは、滅菌が施されたねじ付きカテーテル101又は111と、尿、石鹸、及び水用の容器と、水溶性潤滑剤(カテーテルに事前に潤滑剤が塗布されていない場合)と、鏡(女性の場合)と、ティッシュとを含む器材を組み立てる。ユーザーは、次いで、石鹸と水を使って手と尿道口を洗い、少量の潤滑剤を清潔なティッシュに絞り出し、カテーテルの遠位先端部を潤滑剤に浸け、手を使ってカテーテルの先端部を尿道口の中に入れる(鏡は、女性の場合に尿道口の場所確認の支援に役立つであろう)。
次に図13、図14、及び図15に示すように、拡張器201とオクルーダー211及び221は、可撓性シャフト202の上端部205を、その遠位端付近に先細の球頭部204が設けられているように構成し、その表面に1つ又は2つのねじ区分203を配置した同様の構造である。これらのねじは、図2及び図3のカテーテル101のねじ103と同様であり、ねじの高さは、シャフト202の直径の少なくとも20パーセント(20%)であり、ねじピッチ対ねじ直径の円周の比は球頭部又はシャフト上の何れの所与の位置でも1対1(1/1)より小さい。ここでも、ねじ203の両端は、図2及び図3のねじ付きカテーテルと同様に、前進後退を容易にするため先細になっている。
次に、図16から図18に示すように、ポリウレタン材料で作られたねじ付き尿道ステント301は、均一ピッチの外ねじ303を備えた管302を有している。ねじ303は、図2及び図3のカテーテル101のねじ103と同様であり、ねじの高さは、シャフト302の直径の少なくとも20パーセント(20%)であり、ねじピッチ対ねじ直径の円周の比は、1対1(1/1)より小さい。ねじ303の両端は、通路を通して容易に前進後退が行えるよう先細になっている。ステントの遠位端305には、内部肩部304(図17)があり、内部肩部304より近位側には、比較的硬い材料のブッシング307(図17)が配置されている。ブッシング307は、ブッシングの一端の全径から正六角形アパーチャ309まで伸びる先細の内部壁308を有している。コイル状の側壁補強部材310が、ステント301内のブッシング307と内部肩部304の間に取り付けられている。代わりの実施形態では、ねじの区分は、前立腺又は括約筋の様な筋肉組織の帯域によって把持される「くびれ」を提供できるように、より低い高さ又はゼロ高さまでテーパが付いていてもよい。補強部材310は、管302の側壁と一体的に構成又はモールド成形することもできる。
次に図25から図29を参照していくが、図25と図26のねじ付き恥骨上カテーテル401は、可撓性の管402で作られ、ルーメン408が近位端と遠位端の軸方向のポートを接続し、遠位端には均一ピッチの外ねじ403が設けられている。先に図2と図3のカテーテル101について説明した様に、ねじピッチ403c対ねじ直径403dの円周の比は、1対1(1/1)より遥かに小さい。管402は、近位端で加えられる指の回転力を受容し、それを遠位端に伝達できるだけの捩り強さを有している。ねじ403の両端は、カテーテルを腹部に通して膀胱壁の中へ容易に前進及び後退させることができるように先細になっている。
次に図30、図31A、及び図31Bを参照すると、平均的サイズの成人の結腸に適したねじ付きカメラ導入器カテーテル500は、長さ約5フィートで管直径502dが1インチの柔らかい可撓性の管502へとつながっている球頭状の先端部501を備えている。ルーメン508は、先端部501の遠位端の窓511の内部面から先端部501と管502を通り管502の近位端まで伸びている。
図30から図34には、カメラアッセンブリ520を体内通路内に配置するのに使用することができるねじ付きカメラ導入器カテーテル500が示されている。螺旋型カメラ導入器の利点は、他にもあるが中でも特に大きな利点として、体内通路内の視覚化器械(例えば、内視鏡)を安定させて視覚化診断の成果を高める能力である。例えば、螺旋型カメラ導入器は、内視鏡が粘膜に覆われている結腸の曲がり付近を引かれている間、内視鏡が安定するように助けるので、重大な病変が見逃される恐れが減少する。
本発明の1つの形態では、カメラ導入器カテーテル600は、以下で更に詳しく論じてゆくが1つ又は複数の回転結合部605を設けたこととその使用法以外は、上で説明したカメラ導入器カテーテル500と実質的に同じであるのが望ましい。より具体的には、カメラ導入器カテーテル600は、一般的に管610を備え、その上に螺旋ねじ615が形成されている。管610は、管の近位端に加えられた回転が管の遠位端に伝達されるだけの剛性を有し、同時に、管610は、管が結腸の湾曲部付近で曲がるだけの可撓性も有している。更に、螺旋ねじ615は、カメラ導入器カテーテル600を結腸内に配置する時、カテーテルの近位端を回転させると、螺旋ねじ615がカメラ導入器カテーテル600を本発明の回転前進様式で結腸に沿って引いてゆける幾何学形状を有している。
本発明のもう1つの形態として、次に図39Eを見ると、回転前進式の内視鏡780が示されており、同内視鏡は、内視鏡の外部側壁795の一部又は全部に沿って螺旋形のスクリューねじ790を有する従来型の内視鏡785を備え、内視鏡を回すと、螺旋ねじが体内通路内で内視鏡を長手方向に動かすようになっている。
癌性増殖の近接照射療法による治療は多くの文献に記されている。1つの手法に、放射線源を目標組織にできるだけ近づけて配置することを目的に、放射性物質を癌性増殖部に外科的に植え込む方法がある。その様な移植は、難しく、効果が発揮されるのに時間を要する。更に、後に放射線の線量を修正したり被爆を短期間だけに限定する必要が生じた場合、植え込みプロセスを逆行させることは難しい。
限定ではなく一例として、新規な近接照射療法器械の1つの好適な用途として、装置は、冒されている前立腺の領域に放射性物質を送達せねばならない前立腺癌の治療に使用してもよい。この症例では、一般に、本発明のカニューレ形式を使用するのが望ましい。
次に図40を見ると、動脈又は静脈などの体内導管への迅速且つ効果的なアクセスを提供するのに使用できる導管継手800が示されている。
大腸の目視検査(結腸鏡)は、結腸鏡を腸の全長に逆行式に通すやり方で、直腸から開始して盲腸まで前進させることにより行われる。
本発明の1つの好適な形態として、次に図44を見ると、ねじ付きカテーテル1005と動力駆動装置1010を備えたカテーテル法のシステム1000が示されている。ねじ付きカテーテル1005は、器具、例えば、内視鏡1013を、中に受け入れるための中央ルーメン1012を備えている。動力駆動装置1010は、ねじ付きカテーテル1005を回し、それによりねじ付きカテーテル1005を体内通路に沿って前進させるのに使用することができる。
動力駆動装置1010は、(以上及び/又は以下に説明している様に)空気/水/吸引及び道具用の通路として、ねじ付きカテーテルを通って伸びる導管を追加的に備えたシステムで使用してもよいものと理解頂きたい。
下部消化器官の病態を適切に診査及び治療するのに、通常、患者は糞便を除去するために瀉下処置を受ける。この処置が首尾よく行われないと、体内通路を鮮明に映し出すのは一般に非常に難しくなる。このことは、解剖学的異常が内視鏡から隠れてしまう可能性があることから、非常に望ましくない。
次に図46を見ると、本発明により形成された1つの好適な泌尿器ステントの構造が示されている。

次に図56から図62を見ると、小腸にアクセスして、小腸内に内視鏡770Aを配置するのに使用される螺旋ねじ付きカメラ導入器システム710Aが示されている。上で論じた様に、螺旋形のカメラ導入器710Aの大きな利点は、視覚化及び診断の成果を改善するのみならず療法のための安定したプラットホームを提供することを目的として、視覚化器械(例えば、内視鏡770A)を体内通路(即ち、小腸)内で制御できるようになっていることにある。一例として、螺旋形のカメラ導入器システム710Aは、小腸の蛇行して傷つき易い解剖学的構造の中へ挿入する間、及びそこから引き出す間、内視鏡が安定するように助力することができる。
本発明の以上の好適な実施形態は、回転前進式カテーテル法のシステムの有効性を向上させる数多くの追加設計を含んでいる。これらの追加設計は、螺旋ねじ構造に関係がある。
本発明の別の実施形態によれば、螺旋は、装置に対する組織(又は材料)の動き方が異なるように、装置の長さに沿って少なくとも2つの異なるねじピッチを持たせた構造にしてもよい(例えば、図51のシャフト1405上に配置されている螺旋1400を参照)。一例として、可変ピッチ螺旋構造は、冗長な結腸を内視鏡の上に手繰り寄せたり、又は結腸内の廃棄物を除去し易くする上で有利である。また、可変ピッチ螺旋構造は、解剖学的構造内での装置の固定を最適化するのに使用することもできる。
本発明のもう1つの好適な実施形態では、螺旋のねじ面は、装置の前進又は固定が向上するように、表面に突起又は窪みを持たせた構造でもよい(例えば、螺旋1400上の突起1410を示している図52と図53を参照)。
上で指摘した様に、ねじ要素は、中空であってもよいし、及び/又は流体が充填されていてもよい。ねじは、剛性を有する材料、弾性を有する材料、又は双方の材料の組合せで作られていてもよい。一例として、ねじ要素は、PVC、ポリウレタン、TPE、シリコン、TFE、医用等級ステンレス鋼、タンタル、チタン、ニッケル−チタン合金などから形成してもよい。反対に、材料は、螺旋のねじ要素の抜去の必要性が無くなるように、特に生体分解吸収性のものを選定してもよい。代わりに、ねじ要素は、所望の複合的な属性、例えば、硬度、摩擦力、整合性、及び/又は放射線不透過性などが得られるように、少なくとも2つの異なる属性を有する材料から作ってもよい。
本発明のもう1つの好適な実施形態では、螺旋装置は、温度、圧力、放射能、位置、及び/又は処置中の診断的又は療法的処置についてのその他の状況の様な状態を表示させるために1つ又はそれ以上のセンサーを備えていてもよい。
本発明のもう1つの実施形態では、結合部は、内視鏡又は装置に、様々な方法で固定してもよい。取付のための力は、例えば、機械力、油圧、空気圧、磁力、及び/又は接着力であってもよい。或いは、半径方向の力を利用した設計を用いるなら、変形可能な要素を利用して摩擦式クランプを作り出し、これを逆にすると結合が外れるようにしてもよい。結合は、単一方向への回転(即ち、時計回り限定か反時計回り限定)を許容する一方向クラッチを組み入れたものを提供してもよい。1つの実施形態では、クラッチ方向は、一方向に前進させ反対方向に回して引き出すのがやり易くなるように、施術者によって変更される。もう1つの実施形態として、一方向オーバーライドクラッチは、左巻きに巻き付けられたばねを使用してもよい。これは、装置を前進させることを許容し、引き出す際は、ばねを僅かに巻き戻せばクラッチが外れてIDが広がり把持が阻止されるようにしている。他の一般に知られているクラッチ設計を結合部内に一体化することもできる。
カテーテルシステムの長さには、螺旋装置を容易に回転できるようにするため、人間工学に基づく単数又は複数の把持部を組み込んでもよい。これらの把持部は、永久的であってもよいし、剥ぎ取り式の様な一時的なものにして、処置中に取り外し又は位置直しが行えるようにしてもよい。把持部は、弾性であっても剛性であってもよく、また、手にしっくり馴染むサイズに作られていてもよい。それらは、把持部内の動力駆動装置と一体化されていてもよい。

Claims (20)
- 体内通路の内部を遠隔場所で視覚化するための方法において、
前記体内通路の中に配備するための視覚化システムを提供する段階であって、前記視覚化システムは、
管と、
前記管のルーメン内に配置されている視覚化器械と、
前記管の外部表面に配置されている螺旋ねじであって、前記管を前記体内通路の中に配置した時に、前記螺旋ねじが前記体内通路の内部側壁に係合し、前記管を回転させると、前記管と前記体内通路の前記側壁との間に相対運動が引き起こされるだけの、構造的完全性と表面輪郭とを有している、螺旋ねじと、を備えている、視覚化システムを提供する段階と、
前記視覚化システムを、前記体内通路の中の視覚化する予定の部位から遠隔の場所に挿入する段階と、
前記管を回転させて、前記体内通路を前記管の上へ襞を付ける要領で引き寄せ、これにより、視覚化する予定の部位を視覚化器械に向ける段階と、
前記視覚化器械を使用して、前記体内通路の内部を視覚化する段階と、を備えている方法。 - 前記体内通路は小腸を備えている、請求項1に記載の方法。
- 前記視覚化システムは、前記小腸の中へ幽門括約筋に隣接して挿入される、請求項2に記載の方法。
- 前記管は、可撓性を有する管を備えている、請求項1に記載の方法。
- 前記管は、動力駆動装置によって回転する、請求項1に記載の方法。
- 前記視覚化器械は、内視鏡を備えている、請求項1に記載の方法。
- 前記視覚化器械は、前記管が前記通路の中に設置される前に、前記管の中に配置される、請求項1に記載の方法。
- 前記視覚化器械は、前記管が前記通路の中に設置された後に、前記管の中に配置される、請求項1に記載の方法。
- 前記管は、前記視覚化器械から独立して回転する、請求項1に記載の方法。
- 前記視覚化器械は、前記管を長手方向に動かすと、その結果、前記視覚化器械が長手方向に動くように、前記管に解放可能に係止されていてもよい、請求項1に記載の方法。
- 前記視覚化器械は、実質的に前記管と面一に終端している、請求項1に記載の方法。
- 前記視覚化器械は、前記管より先まで伸びている、請求項1に記載の方法。
- 前記体内通路の一部が前記管上に引き寄せられた後、前記視覚化器械は前記体内通路の中を前記管より更に奥へ進められる、請求項1に記載の方法。
- 前記体内通路の一部が前記管上に引き寄せられた後、前記管を反対方向に回すと、前記体内通路が前記管から解ける、請求項1に記載の方法。
- 組織を視覚化するための器械において、
管のルーメン内に配置される視覚化器械を受け入れるサイズに作られている管と、
前記管の外部表面に配置されている螺旋ねじであって、前記管を前記体内通路の中に配置した時に、前記螺旋ねじが前記体内通路の内部側壁に係合し、前記管を回転させると、前記管と前記体内通路の前記側壁との間に相対運動が引き起こされるだけの、構造的完全性と表面輪郭とを有している、螺旋ねじと、を備えており、
前記螺旋ねじは、部分的に変形可能で、それにより更に沿わせ易く外傷性のより低い前記組織との係合が得られるように構成されている、器械。 - 前記器械は、前記管の前記ルーメン内に配置されている視覚化装置を備えている、請求項15に記載の器械。
- 前記螺旋ねじは、中空構造を備えている、請求項15に記載の器械。
- 前記螺旋ねじは、半卵形断面ねじ輪郭を有している、請求項15に記載の器械。
- 前記螺旋ねじは、非対称断面を有している、請求項15に記載の器械。
- 前記螺旋ねじは、螺旋の長さに沿って変化する輪郭を有している、請求項15に記載の器械。
