(抗体およびサイトカイン) ヤギ抗hMIF IgGおよびrhMIF(R&D Systems,Minneapolis,NIN)を、それぞれ、PBSおよび0.1%
BSA含有PBS中に再構成して、アリコートして、使用するまで−20℃で保管した。ウサギ抗−ヤギIgG−HRP(BioRad Corp.,Hercules,CA)は、使用するまで4℃で保管した。
LPS(Sigma−Aldrich Corp.,St.Louis,Mo.)を腹腔内(i.p.)に注射して、CO2窒息および引き続く頚部脱臼によって、注射後、テキストに示された時点で屠殺した。注射していないマウスをコントロールとして用いた。2つの抗MIF抗体(III.D.9およびXIV.15.5,Rockland Immunochemicals,Inc.,Gilbertsville,PA)およびそれらのアイソタイプコントロール(HB−49,Rockland Immunochemicals,Inc.,Gilbertsville,PA)を、心エコー研究におけるLPSチャレンジの90分前に腹腔内に(i.p.)注射した(100μgを200μl PBS中に含有)。心臓全体を取り出して、液体窒素中で急速凍結させ、−80℃で保管するか、または免疫組織化学のために、10%天然の緩衝化ホルマリン中で24時間固定して、70%エタノールに入れた。
(タンパク質抽出およびウエスタンブロッティング) 心臓を解凍して、1% NP40、0.5%デオキシコール酸、0.1% SDS、2mM EDTA、および1mM PMSFを含有するTris−緩衝化生理食塩水(TBS,50mM Tris,150mM NaCl、pH7.5)中で氷上でホモジナイズした。溶解物の濃度は、Bio−Rad Protein Assay(Hercules,CA)を用いて定量した。タンパク質(20μg)を、Laemmliサンプル緩衝液(Bio−Rad,Hercules,CA)を用いて1:1希釈して、還元条件下で18% SDSポリアクリルアミドゲル上で再溶解した。このゲルを、半乾燥転写装置(semi−dry transfer apparatus)(Bio−Rad,Hercules,CA)を15Vで15分間用いて、PVDFメンブレン(NEN,Boston,MA)に転写した。メンブレンを、0.5%脱脂粉乳を含むTBS/0.1% Tween−20(TBS−T)を用いて30分間ブロックして、ヤギ抗−hMIF IgG(1:750)を含有するTBS/0.1% Tween−20/5%脱脂乳とともに4℃で一晩インキュベートした。このメンブレンをTBS−T中で10分間3回洗浄して、抗ヤギIgG−HRP(1:1000)とともに室温で1時間インキュベートして、TBS−Tを用いて10分間4回洗浄した。このメンブレンを、アルカリ条件(SuperSignal West Pico,Pierce,Rockford,IL)のもとでルミノールに過酸化水素を加えた5mlの混合物に5分間曝して、得られた化学発光反応を、Kodak X−OMAT AR Film(Eastman Kodak Co.,Rochester,New.York)によって検出した。
(RNA抽出、プローブ調製およびノーザンブロッティング) 全RNAを、製造業者のプロトコールに従って氷上で解凍した心臓からトリゾール(Invitrogen,Carlsbad,CA)で抽出して、分光測定法によって定量した。MIF特異的ノーザンプローブを、MIFプラスミド(Research Genetics,Huntsville,AL)からDNA(DNeasy Tissue Kit,Qiagen,Valencia,CA)を単離すること、および引き続きECOR1およびNOT I制限酵素(Fisher Scientific,Pittsburgh,PA)を用いてそれを切断することによって調製した。得られたDNAを1.2%アガロースゲル上で分離して、精製し(GenElute Agarose Spin Columns,Supelco,Bellefonte,PA)、Ready−To−Go Labeling Beads(Amersham Pharmacia,Piscatany,NJ)を用いて、5μl 32P−dCTP(3000 Ci/mmol)(PerkinElmer,Boston,MA)で標識して、製造業者のプロトコールに従って、ProbeQuant Microcolumns(Amersham Pharmacia,Piscatany,NJ)中で精製した。

(Mモード測定) データは、その各々が少なくとも3つの選択された拍動の平均にあたる少なくとも2つの別のスキャンの平均に相当する。心臓拡張末期は、最大のLV拡張期径として規定され、そして心臓収縮末期は、後壁運動のピークとして規定された。収縮機能は、以下のようにLV径から円周短縮率(FS)として算出された:図5Aに示されるとおり、FS(%):LVED−LVES/LVED×100。
(統計学的分析) ノーザンおよびウエスタンのデータは、平均±標準誤差として表され、そして一元分散分析を用いて統計学的に分析される。実験群とコントロール群との間の有意差の決定は、チューキー法を用いて行なった(p≦0.05)。Langendorff調製によって決定した心臓機能は、平均±標準誤差として表し、そして別の分析を各々のLVP、+dP/dtmax、および−dP/dtmaxについて、処置群の関数として、そして冠血流速度について、反復測定分散分析を用いて行なった。Bonferroni法を使用する多重比較手順を用いて、群間の有意差を決定した(p<0.05)。心エコー図によって決定した心機能は、円周短縮率%(LVED−LVES/LVEP×100.)±標準偏差によって表され、そして一元の反復測定分散分析を用いて分析される。チューキー検定を用いてさらなる比較を行ない、特定の群の間の有意差を決定した(p<0.05)。全ての統計学的分析は、SigmaStat 2.03(SPSS Inc.,Chicago,IL)およびMicrosoft Excel(Microsoft Corp.,Seattle,WA)を用いて行なった。
MIFタンパク質は、インビボにおいて心筋細胞によって構成的に発現されて、LPSチャレンジに応答して放出される。心臓組織で行なった免疫組織およびウエスタン分析の両方によって、心室および心房の筋細胞を含む心臓細胞におけるMIFの存在が、ベースラインのコントロール条件下で実証された(図1および2)。内毒素チャレンジ後、免疫組織化学およびイムノブロット分析の両方によって、内毒素後の心臓組織のMIFの有意な減少が証明される。この減少は、12時間で最も顕著(75%低下)であったが、24時間までにほぼベースラインのコントロールレベルに戻った。心臓におけるこの発現パターンは、肝臓および脾臓において証明されたパターンと同様であり(図2)、そしてMIFがLPSチャレンジ後に組織内の予め形成された貯蔵から放出されるという仮説と一致している。組織からのMIFの放出は、4時間でイムノブロットで証明され(図1)、内毒素曝露後の血清レベルの増大と相関している(表1)。
(内毒素チャレンジ後の心筋でのMIF MRNA発現) コントロールのマウスまたはLPSチャレンジマウスのいずれかの心臓から所定の時点で得られたRNAのノーザン分析によって、MIF mRNAは、コントロールマウスで構成的に発現されること、およびLPSチャレンジ後に、心臓全体の調製物においてMIF mRNAの濃度に有意な変化は検出できないことが示される。
MIFは、収縮期および拡張期の心機能障害を誘発する。MIFが心機能に直接影響するか否かを決定するために、自発的に拍動している正常なマウス心臓(Langendorff調製)を、組み換えMIF(rMIF)を用いて、敗血性ショックを有するヒトの血清で実証された濃度(20)に近い20ng/mlの濃度で灌流させた。MIFに対する応答は、C57BL/6JマウスおよびC3H/HeJマウスの両方由来の心臓において確認された。C3H/HeJマウスは、内毒素(41−43)に対して耐性であり、従って、観察される任意の抑制が灌流液中の微量の内毒素に起因し得る可能性は限定される。表2は、コントロールの灌流液、または20ng/mlの組み換えMIFを含有する灌流液での1.5ml/分の逆行性の心房灌流に対する、両方のマウス系統の応答を図示する。MIFでの灌流によって、両方のマウス系統でLVP、+dP/dtmax、および−dP/dtmaxの有意な低下がもたらされた。図4は、ある範囲の冠血流速度を超えるMIFの効果を例示する。実験群の割り当てに関わらず、全ての心臓において収縮能力の段階的な増大が存在する。MIFに曝された心臓とコントロールの心臓との比較によって、機能曲線の下向きのシフトが明らかになり、これによって、20ng/ml rMIFに応答する有意な収縮期および拡張期の抑制が示された(p<0.05)。MIFの効果は、内毒素感受性系統(C57BL/6J)および内毒素耐性系統(C3H/HeJ)の両方で統計学的に同一であった。同様に、rMIFで灌流させたC57BL/6JとC3H/HeJの研究心臓の間でLVP、+dP/dtmaxおよび−dP/dtmaxに相違はなかった。
抗MIF抗体は、インビボにおいてLPS誘発性の心臓抑制を改善する。インビボにおいて心機能障害の病原性におけるMIFの影響を決定するために、連続的な心エコー(Mモード)を、抗MIFモノクローナル抗体、アイソタイプコントロール抗体、または処置なしのいずれかを用いて事前に処置(90分前)されている、LPSチャレンジしたマウスで行なった(図5)。LPSチャレンジ4時間後、全てのLPSチャレンジしたマウスの円周短縮率%(FS%)は、群の割り当てにかかわらず、同様に抑制された(FS%において50%減少)。しかし、LPSチャレンジの8時間後、いずれかの抗MIFモノクローナル抗体を注射されたマウスは、処置なしまたはアイソタイプ抗体コントロールのいずれかを受けている、LPSチャレンジされた群に比較してFS%の統計学的に有意な回復を実証した(図5)。機能のこの増強された回復は、12時間、24時間および48時間継続した。チャレンジ後48時間で、抗MIF処置群は、FS%のほぼ完全な回復を示したが、LPSチャレンジした群は、顕著に抑制されたままであった。48時間にわたって、ニセマウスのFS%は、有意に変化されず、このことは、心機能が麻酔または試験レジメン自体によって影響されなかったことを示している。さらに、全ての時点で、アイソタイプの抗体コントロールを注射されたマウスは、LPSチャレンジされた動物に同一であって、このことは抗MIF抗体効果の特異性を示す。
(材料および方法)
(抗体およびサイトカイン)。ポリクローナルウサギ抗ラットMIF IgG(Torrey Pines BioLabs,Inc.,Houston,TX)を、ウエスタンイムノブロットおよび免疫組織化学に用いた。この抗体は以前にマウスMIFと交差反応することが示されており、前に記載されたとおり調製された(23)。ポリクローナルヤギ抗ウサギIgG−HRP(BioRad Corp.,Hercules,CA)をウエスタンイムノブロットのための二次抗体として用いて、4℃で保管した。2つのモノクローナルマウス抗マウス(およびヒト)MIF IgG1抗体(XIV.15.5およびIII.D.9、Cytokine PharmaSciences,Inc.から贈呈)およびモノクローナルマウスIgG1アイソタイプコントロール抗体(HB−49、Cytokine PharmaSciences,Inc.から贈呈)を、心エコー法研究において用いた。XIV.15.5およびIII.D.9クローンの両方によるMIF活性のインビボ中和は、以前に実証されている。

MIFタンパク質は、インビボにおいて心筋細胞によって構成的に発現されて、熱傷に応答して放出される。このサイトカインであるマクロファージ遊走阻止因子(MIF)は、ウエスタンおよび免疫組織化学によって実証されるとおり、心室および心房の両方の筋細胞にベースラインで存在する(図7および8)。熱傷後、有意な減少(2・1倍)がMIFの組織濃度で、8時間で同定され、これは12時間でベースラインのレベルに戻った(図7)。この発現パターンは、熱傷後の肝臓、脾臓および肺において平行であって(図8)、これは、熱傷の応答メディエーターにおいてMIFが放出されるという仮説と一致している。
TNF−αシグナル伝達がMIF分泌において有する役割、およびその関連する心機能障害を致死量未満の内毒素中毒症のモデルにおいて決定するため、以下の実験を実施例14において実行した。
(抗体およびサイトカイン) マウスMIFと交差反応するポリクローナルウサギ抗ラットMIF IgG(Torrey Pines BioLabs,Inc.,Houston,TX)をウエスタンイムノブロットおよび免疫組織化学のために用いた。ポリクローナルヤギ抗ウサギIgG−HRP(BioRad Corp.,Hercules,CA)をウエスタンイムノブロットのための二次抗体として用いた。2つのモノクローナルマウス抗マウス(およびヒト)MIF IgG1抗体(XIV.15.5およびIII.D.9、Cytokine PharmaSciences,Inc.,King of Prussia,PAから贈呈)およびモノクローナルマウスIgG1アイソタイプコントロール抗体(HB−49,Cytokine PharmaSciences,Inc.から贈呈)を、心エコー研究において用いた。XIV.15.5およびIII.D.9クローンの両方によるMIF活性のインビボ中和は以前に実証されている。TNF−αを中和するために用いた組み換えヒトTNFR:Fc(Enbrel(登録商標))は、Immunex Corp./Amgen,Inc.、(Thousand Oaks,CA)から贈呈された。組み換えヒトMIFは、Bernhagenらの方法に従って合成して、Cytokine Pharmasciences,Inc.によって提供された。
ImxTnfrsflbtm1 Imxマウス(TNFR−/−)の繁殖するペアをJackson Laboratory(Bar Harbor,ME)から購入して、雄性の子孫を遺伝子型決定後に20〜24週(30〜40グラム)で利用した。市販の固形飼料および水道水は自由に利用させた。全ての動物プロトコールは再審査されて、University of Texas Southwestern Medical Center Institutional Animal Care Advisory Committeeによって承認されて、NIHによって公開された、動物使用を管理する規則に従った。

WTマウス、Enbrel(登録商標)で事前処置したWTマウスおよびTNFR−/−マウスの血清MIFレベル。MIFの血清レベルは、LPSチャレンジ後野生型マウスでは8時間で最大(ベースラインの約1.5倍)に達する(図13A)。TNFR−/−マウスにLPSをチャレンジするとき、最大の血清MIFレベルは、12時間で生じる(ベースラインの1.7倍)(図13B)。最大血清MIFレベル(ベースラインの約2.3倍)は、Enbrel(登録商標)で事前に処置し(60分)てLPSをチャレンジした野生型マウスにおいて24時間で確認された(図13C)。

(動物):Charles RiverのC57BL/6マウス(12〜15週齢)には、市販の固形飼料および水道水を自由に利用させて飼育した。全ての動物プロトコールは再審査されて、NIHによって公開された動物使用を管理する規則に従うUniversity of Texas Southwestern IACACによって承認された。

MIFは、LPSチャレンジ後の心筋細胞から分泌され、そして遅発性(>6時間)の心機能障害を直接媒介する。哺乳動物細胞では、CD74は近年、ERK1/2細胞内シグナル伝達経路を介して効果を発揮する、MIFレセプターであることが確認された。CD74が敗血症においてMIF誘発性心機能障害を媒介するか否かを決定するために、本発明者らは、1)LPSを用いて野生型マウス(C57BL/6)をチャレンジし;2)抗CD74モノクローナル中和抗体で事前処置された野生型マウスをLPSを用いてチャレンジし、そして3)LPS(4mg/kg)を用いてCD74ノックアウトマウスをチャレンジした。連続的な心エコー検査を行なって、円周短縮率(FS%)を決定した。24時間で、コントロール(FS%=58±1%)に比較して、LPSを与えたWTマウスでは有意な障害が観察された(FS%=31.6%±3.3%)。LPSを用いてチャレンジされた、抗CD74抗体処置されたマウス、そしてCD74ノックアウトマウスの両方で、心機能はLPSのみを与えられた野生型マウスに比較して有意に改善された(それぞれ、FS%=49±3.6%および53.3±2.4%、p<0.05)。CD74発現は、心臓ではいまだかつて実証されていないので、本発明者らは、イムノブロットおよび組織化学を行なって、これによって、CD74が心臓細胞膜上におよびサイトゾル中に構成的に存在すること;そしてLPSチャレンジ後に実質的に調節された(12時間でほぼなし、4倍以上低下)ことを確認した。図26〜30および表5〜7。これらのデータは、CD74が心筋細胞上で発現されて、心機能障害の重要なメディエーターであることを初めて実証する。

(冠動脈結紮) マウスを1〜1.5%のイソフルランで麻酔して、その後に冠動脈結紮を行なった。アトロピン(筋肉内に0.075mg/kg与えた)、リドカイン(筋肉内に1mg/kg)、および生理食塩水(腹腔内に1ml)を手技の前に与えた。マウスの鼻面にフィットする特注のマスクおよび小型動物用呼吸器((Harvard Apparatus,Inc.,Holliston,MA)を用いて換気を行なった。左胸郭の第四肋間腔に切開(約5mm)を行い、心膜切開術を行なって左室を露出させた。左の冠動脈を、左心耳の約2mm下に8−0のプロレン(prolene)を用いて閉塞した。引き続き、胸部を、層を成して閉じて、シリンジ吸引によって胸部の陰圧を戻した。(R)−3−(4−ヒドロキシフェニル)−4,5−ジヒドロ−5−酢酸イソキサゾリンの酸性メチルエステル(「ISO−1」、「CPSI−26」またはp−ヒドロキシフェノール−イソキサゾリンメチルエステル)を、25μlのDMSO中に含む200mg/kgの投薬量で2週間毎日複空内に与えた。ブプレノルフィン(0.10mg/kg)は手術後1回疼痛のために与えた。ニセの手順は、冠動脈の結紮なしに同じく行なった。結果を図31に示す。
