（関連技術の背景）
外科的に組織を接合するために使用される様々なタイプの外科用器具が当技術分野において公知であり、例えば、横切断（ｔｒａｎｓｅｃｔｉｏｎ）、切除、吻合における組織または器官の閉鎖のために、胸郭処置および腹部処置における器官の閉塞のために、および組織を電気外科的に融合または封鎖するために普通に使用される。
外科的に組織を接合するために使用される様々なタイプの外科用器具が当技術分野において公知であり、例えば、横切断（ｔｒａｎｓｅｃｔｉｏｎ）、切除、吻合における組織または器官の閉鎖のために、胸郭処置および腹部処置における器官の閉塞のために、および組織を電気外科的に融合または封鎖するために普通に使用される。
かかる外科用器具の一例は、外科用ステープリング器具であり、外科用ステープリング器具は、アンビルアセンブリと、外科用ステープルの配列を支持するカートリッジアセンブリと、カートリッジアセンブリとアンビルアセンブリとを接近させる接近メカニズムと、カートリッジアセンブリから外科用ステープルを排出するための発射メカニズムとを含み得る。
外科用ステープリング器具を使用して、外科医がアンビル部材とカートリッジ部材とを接近させることは一般的である。次に、外科医は、ステープルを組織の中に配置するために、器具を発射し得る。さらに、外科医は、同じ器具または別個の器具を使用して、ステープルの列に隣接しているか、またはステープルの列の間の組織を切断し得る。

本開示による外科用ステープリング器具において、ロッドまたは他の部材が、手の動作によって遠位方向に動かされ、ＤＬＵを作動させ、ステープルを配備する。例えば、図１Ａに戻って、固定ハンドル１３０に対する可動ハンドル１３２の少なくとも部分的な作動が、駆動ビーム（図示された実施形態には明確に示されず）を長手方向に並進させ、少なくとも一方の顎部材を他方に対して接近させる。さらに、可動ハンドル１３２の少なくとも部分的な作動（例えば継続された作動）が、発射ロッド（図示された実施形態には明確に示されず）を長手方向に並進させ、外科用ファスナ（例えばステープル）をカートリッジアセンブリ１７０から排出し、かつ／またはナイフを前進させ、組織を切断する。第１のハンドルが顎部材を互いに対して接近させ、第２のハンドルおよび／または第３のハンドルの作動が、ファスナの排出およびナイフの前進を引き起こすことも想定されている。例えばモータ駆動、液体圧、ラチェッティング、その他のような他のタイプのハンドルが使用され得る。
図１Ａを引き続き参照して、レバー１４０が回転ダイアル１２０に隣接して示され、顎部材２２０の関節運動を容易にするために使用され得る。レバー１４０の作動は、顎部材２２０が長手方向軸Ａ−Ａと実質的に整列する第１の位置と、顎部材２２０が長手方向軸Ａ−Ａに対してある角度で配置される第２の位置との間で、顎部材２２０を動かす。レバー１４０を動かすことは、関節運動リンクを長手方向に動かし、これによって少なくとも１つの顎部材の近位部分が近位方向または遠位方向に動くことになることが想定されている。つまり、レバー１４０を第１の方向に動かすことは、関節運動リンクを近位方向に動かし（これが、顎部材２２０を第１の方向に関節運動させる）、レバー１４０を第２の、反対の方向に動かすことは、関節運動リンクを遠位方向に動かす（これが、顎部材２２０を第２の方向に関節運動させる）。

電気外科用鉗子１００ｂと関連した想定される使用において、ユーザは最初に、組織の周りに顎部材２２０を配置する（図２に示されるように、締め付け具２３０および顎部材２２０は開いた位置にある）。次に、ユーザは、締め付け具２３０が圧縮するようにし（図３）、次に顎部材２２０を閉じる（図４）。顎部材２２０が所望の位置で閉じられた後、ユーザは、トリガアセンブリ７０を押し下げ、組織にエネルギーを送達し、組織を封鎖するか、または組織を凝固させる。ユーザは、封鎖／凝固の間に、またはこの後に組織を切断し得る。
理解され得るように、外科用器具１００の様々な作動が達成され得る幾つかの方法がある。例えば、外科用器具１００は、固定ハンドル、締め付け具ハンドル、第１の可動ハンドル、第２の可動ハンドル、および解放ボタンを含み得る。解放ボタンは、締め付け具２３０を解放する（例えばロックされた位置から）ために作動させられ得ることがさらに想定されている。締め付け具２３０の閉鎖、顎部材２２０の閉鎖、およびステープルの発射または組織へのエネルギーの送達をもたらすために、外科用器具１００に１つ以上の可動ハンドルが提供される。

ここで、図５および図６を参照して、締め付け具２３０を接近させるための閉鎖メカニズム２４０が示される。閉鎖メカニズム２４０はピン２４２と、角度の付いたスロット２４４とを含む。ピン２４２は、ＤＬＵ２００ａの締め付け具リンクまたは外科用器具１００（締め付け具２３０の一部分ではない）に配置されている。スロット２４４は、締め付け具２３０の第１の部分２３２に配置されて示されている。図５および図６を参照して理解され得るように、レバー１４０を利用した締め付け具２３０の作動は、外科器具１００の関節運動リンクを動かす。関節運動リンクは、ＤＬＵ２００ａが器具１００に接続されるとき、締め付け具リンクに接続される。締め付け具リンクの動きは、ピン２４２とスロット２４４との間の相対運動を引き起こす。つまり、スロット２４４に対するピン２４２の近位方向の動きは、締め付け具２３０の第１の部分２３２を締め付け具２３０の第２の部分２３４に向かって動かす（例えば、カム、ピボット、その他）。任意の適切な構造が、外科用器具１００および／またはＤＬＵ２００ａに対して組み込まれ得、ピン２４２とスロット２４４との間の相対運動を引き起こし得る。例えば、外科用器具１００は締め付け具カラーを含み得、該締め付け具カラーは、可動ハンドル／ノブ／その他が作動すると、長手方向に可動である。かかる実施形態において、締め付け具カラーの遠位方向の並進は、第２の部分２３４に対して第１の部分２３２をカムで動かし（ｃａｍ）得、このようにして、締め付け具２３０を接近させ得る。さらに、締め付け具カラーの近位方向の並進は、締め付け具２３０を開き得る。さらに、図２〜図４に示されるように、組織を望ましくない態様で挟むことを防止することを助けるために、閉鎖メカニズム２４０は、筐体２４６内に含まれ得る（または少なくとも部分的に含まれ得る）。
さらに、図２に戻ると、本開示の実施形態は、締め付け具２３０に配置されたタブ２５０を含む。（図２において、例えば、タブ２５０は締め付け具の第２の部分２３４に示される）締め付け具２３０が閉じた位置にあるとき、タブ２５０は、締め付け具２３０の相対する部分（例えば、第１の部分２３２）に配置されたアパーチャ２５１（図２において幻影で示される）と嵌合するように構成されることが想定されている。このタブ２５０／アパーチャの組み合わせは、締め付け具２３０を閉じた位置に維持することを助け得、使用中、締め付け具２３０の部分２３２、２３４の整列を維持することも助ける。

Claims (16)
- 外科的に組織を接合するための外科用ステープリング器具であって、
ハンドル部分と、
該ハンドル部分から遠位方向に延び、第１の長手方向軸を画定する内視鏡的部分と、
該内視鏡的部分の遠位端に隣接して配置され、そこから概ね遠位方向に延びる一対の顎部材であって、該顎部材のうちの少なくとも一方は、それらの間で身体組織を係合するため、開いた位置と接近した位置との間で他方に対して可動であり、該一対の顎部材は、アンビルアセンブリとカートリッジアセンブリとを備え、該カートリッジアセンブリは、複数の外科用ステープルを収納し、該複数の外科用ステープルは、該第１の長手方向軸に対して横方向に、該アンビルアセンブリに向かって排出されるように配列されている、一対の顎部材と、
該顎部材に隣接して配置され、該内視鏡的部分から概ね遠位方向に延びる締め付け具であって、該締め付け具は、身体組織を係合するため、開いた位置と接近した位置との間で可動であり、該顎部材に対して独立して可動である、締め付け具と
を備えている、外科用ステープリング器具。 - 前記締め付け具は、第１の部分と第２の部分とを含み、該締め付け具の少なくとも一方の部分は、該締め付け具の近位部分に配置された旋回点の回りで他方の部分に対して旋回可能である、請求項１に記載の外科用ステープリング器具。
- 前記一対の顎部材のおのおのは、前記締め付け具に対して実質的に平行である、請求項１に記載の外科用ステープリング器具。
- 前記一対の顎部材は、前記第１の長手方向軸に沿って直線状に延びる、請求項１に記載の外科用ステープリング器具。
- 前記一対の顎部材は、電気外科エネルギーを組織に送達するように構成されている、請求項１に記載の外科用ステープリング器具。
- 前記一対の顎部材は、前記第１の長手方向軸に対して湾曲している、請求項１に記載の外科用ステープリング器具。
- 前記締め付け具は、前記第１の長手方向軸に対して湾曲している、請求項６に記載の外科用ステープリング器具。

- 前記顎部材は、ローディングユニットの一部分であり、該ローディングユニットは、内視鏡的部分に対して取り付け可能である、請求項１に記載の外科用ステープリング器具。
- 前記一対の顎部材は、前記ハンドル部分に対して回転可能である、請求項１に記載の外科用ステープリング器具。
- 前記一対の顎部材は、第２の長手方向軸を画定し、該一対の顎部材は、該第２の長手方向軸が前記第１の長手方向軸と実質的に整列された第１の位置から、該第２の長手方向軸が該第１の長手方向軸に対してある角度で配置された少なくとも第２の位置へ可動である、請求項１に記載の外科用ステープリング器具。
- 外科用器具を解放可能に係合するように構成されたローディングユニットであって、該ローディングユニットは、
第１の長手方向軸を画定する本体部分であって、該本体部分の近位部分は、該外科用器具の内視鏡的部分を解放可能に係合するように構成されている、本体部分と、
該本体部分から遠位方向に配置された一対の顎部材であって、該顎部材のうちの少なくとも一方は、それらの間で身体組織を係合するため、開いた位置と接近した位置との間で他方に対して可動であり、該一対の顎部材は、アンビルアセンブリとカートリッジアセンブリとを備え、該カートリッジアセンブリは、複数の外科用ステープルを収納し、該複数の外科用ステープルは、該長手方向軸に対して横方向に、該アンビルアセンブリに向かって排出されるように配列されている、一対の顎部材と、

- 前記一対の顎部材は、前記第１の長手方向軸に沿って直線状に延びる、請求項１２に記載のローディングユニット。
