Abstract

Description
本発明は、一般には医療装置および方法に関し、より具体的には、脳の組織を電気的および/または化学的にモジュレート(調節)およびモニタ(監視)するために使用されるリードに関する。
プローブまたはリードなどの医療デバイスを頭蓋骨の中に埋め込むことは、パーキンソン病、本態性振顫、およびジストニアなどの病気の治療に関する手法として、ますます重要になっている。うつ病、癲癇、ジストニア、強迫神経症、肥満、および慢性的疼痛などの数多くの疾患を治療するために、インプラントが使用できる。これらのデバイスのほとんどは、電極によって電流を加えることによって脳と相互に連絡する。さらに、慢性的に埋め込まれたリードを通して薬物を注入することが、とりわけアルツハイマー病やパーキンソン病を患う患者で、主な治療または電気刺激への補助的な治療として医療文献で提案されてきた。

図15は、円230の周囲の点に配置された3つの電気単極子(モノポール)235a、235b、および235cの場合を示す。図15の最大の正味双極子の正方形243が、ここに参照のために重ね合わされる。3つの電気単極子は、最大の正味双極子の六角形によって図示されるように、より効果的に向きを付けられた双極子を生成する。2つの双極子は、1つの極性の1つの単極子および2つの反対または0電荷の単極子によって生成される。3つすべての単極子の電荷の合計は0である。六角形246の点の径方向部位は、2つの双極子の合計によって生成できる最大の正味双極子を表し、どの単極子の最大電荷が大きさ1.2を超えることができない制約に従う。各部分が円周の1/4を占める場合よりも各部分が円周の1/3を占める場合に固定された軸方向長さの各刺激部位の表面積がより大きくなるので、ここでより大きな最大電荷の制約が使用される。電極235a、235b、235cに最も近い六角形の辺は、制約する電極が正の極性を有し、これらと反対側にある六角形の辺は、制約する電極が負の極性を有する場合の状況で生成される。六角形246の径方向の部位が、原点からのほとんどの方向で、正方形243よりも原点から離れていることが理解できる。切断されたリングの固定された軸の範囲に関して、3つの刺激部位が、4つの刺激部位と比べてより大きな効果的な刺激を送ることができる。あるいは、固定された効果的な刺激に関して、3つの刺激部位の切断されたリングの軸方向の長さは、4つの刺激部位の切断されたリングに関するよりも短いものであることができる。本発明の好ましい実施形態は、最大の電流密度が制約される状態に関する電流のよりよい誘導可能性を補助する別のプローブに勝る利点を有する。本発明のこの説明は、異なる切断されたリングの刺激部位が同時または連携して刺激される刺激プロトコルを使用することを除外しない。
6つ(または任意のその他の3の倍数)の刺激部位の切断されたリングを有する刺激プローブが、本発明の利点を得る様式で使用できることが当分野の技術者には明らかである。これは、それぞれが1対の隣接する刺激部位からなる3つの刺激部位として、6つの刺激部位のリングを制御することによって達成できる。
したがって、任意の軸方向部位で、刺激部位の数は素数である。素数は、異なる刺激の向きを得るために、電極のすべての表面を同時に使用するためのより多くの組合せの可能性を生み出す。すべての電極表面を使用することにより、電流密度が可能な限り低く保たれる。好ましい実施形態では、刺激部位の数は3である。別の実施形態では、刺激部位の数は5である。2つ、5つ、または7つの刺激部位による構成も、本発明が達成しようとする電流密度の利点を得ることができるが、より少ない程度しか得られない。
図1を参照すると、組織のモジュレートおよびモニタプローブが示されている。図1は、プローブの好ましい実施形態を示す。それは、可撓性プローブ本体10およびオプションの多接点接続端子20aを備える円筒形のプローブである。多接点接続端子のさらなる詳細は、米国仮特許出願第60/820,914号に記載され、その全体の内容が本明細書に参照によって組み込まれる。その他のコネクタが使用でき、当分野でよく知られている。プローブ30aの遠位端に、1つまたは複数の刺激部位の切断された環状のリングがある。刺激部位はすべてのリングの適合する角度部位と部位合わせすることができ、または異なるリングで異なる角度部位にずらすことができる。局所的な電場電位を記録するのに適した1つまたは複数の周縁の電極バンド(electrode bands )、および最遠位点またはその付近にある記録電極もある。この好ましい実施形態では、多接点端子20aの最大直径は可撓性のプローブ本体10の直径に等しい。
この実施形態では、3つの刺激部位33a、33b、33c、34a、34b、34c、35a、35b、35c、36a、36b、36cが、4つの軸方向部位に、総計で12の刺激部位に関して切断されたリングとして配置される。これらは、図5〜12の断面図でよりよく理解できる。また、この実施形態には、切断されたリングの間の間隙に配置される3つの記録バンド37、38、39がある。記録部位の大きさは、局所的な電場電位を記録するのに適しており、露出した領域が約0.0005mm2から約0.5mm2の範囲にあるが、その領域は約0.8mm2までになることもできる。いくつかの実施形態は、活動電位の細胞外の記録を改善するより小さな記録部位を有する。露出された領域のそのような記録部位は、約1.9×10-5mm2から約0.002mm2の範囲にあることができるが、それらは約0.1mm2程度の大きさであることもできる。記録部位の型式は、絶縁されたワイヤのむき出しの端部、薄膜、金属パッド、またはデバイスの壁の中で導電体を露出するために除去される絶縁の部分を有する絶縁された領域であることができる。別の実施形態は、記録リングをまったく有することができず、またはより多くの記録リングを有することができる。さらなる記録リングまたは点電極がプローブ本体10に沿って、またはプローブ先端32に配置できる。本実施形態は、刺激部位に対する記録電極(バンドおよび/または点)の部位合わせを制限しない。
短絡化を低減し、記録された信号の絶縁を向上させるために、刺激面と記録面の間および記録面の間に少なくとも100μmの非伝導性の隔たりがなければならない。プローブを通って移動する電気信号は、互いに干渉しないことが望ましい。高いレベルの電気刺激信号が低レベルの記録信号と干渉しないことが特に望ましい。したがって、記録信号を運ぶ導電体が内側の螺旋に配設されることが望ましく、刺激信号を運ぶ導電体が外側の螺旋に配設されることが好ましい。刺激導電体および記録導電体の対が、感知できる距離に対して、隣接する経路をまったく横切らないように、2つの螺旋のピッチは、同じまたは異なることができる。これは、刺激導電体と任意の記録導電体の間の容量性結合を最小限に抑える。その他の実施形態では、導電性の被覆を記録導電体の螺旋の外側に施すことができる。これは、2つのタイプの導電体の間の電磁干渉を低下させるために接地できる。別の実施形態では、接地できる金属箔が内側の螺旋と外側の螺旋の間に巻かれる。
別の実施形態では、記録された信号を運ぶ導電体は、電気刺激信号を運ぶ導電体の間に配設される。この実施形態は、導電体が単一の薄層で配設されよりコンパクトかつより可撓性があることができる利点を有するが、場合によっては、この実施形態は刺激電流が刺激導電体をモジュレートする場合、刺激信号が隣接する記録導電体につながる可能性があるという欠点を有する可能性がある。必ずしもすべての刺激導電体が、任意の瞬間に電流を運ぶことを求められないことに留意されたい。したがって、プローブの多くの用途において、記録導電体のうちのいくつかは、任意の瞬間に作動状態の刺激導電体から十分に分離される。別の実施形態では、刺激ワイヤおよび記録ワイヤは、螺旋状に導電体の隣接する組として通る。
ワイヤは機械的な強度を有し、導電性である必要がある。適切な材料には、合金MP35N(コバルト・クロム合金)、ステンレス鋼、およびタングステンまたは刺激部位との導通、および頭蓋外のコネクタへの導通を向上させるために金メッキされたタングステン合金ワイヤが含まれる。材料がMRI適合性を最大にするために磁性が最小限であることが重要である。
刺激部位は、白金、イリジウム、または白金とイリジウムの合金などの安全な電荷移動を最大にする比較的不活性の材料から作製できる。プローブの本体は、プローブが頭蓋骨を出る短い曲率半径による曲げを補助する、シリコーンゴムまたはポリウレタンなどの生体適合性ポリマーによって被覆される。
図2は、プローブ30bの別の実施形態を示す。プローブ30bは、化学物質がプローブ管腔に出入りできることを可能にするポート40を加える点を除いて、図1のプローブ30aと同様である。ポート40を半透明の薄膜によって覆うことができる。あるいは、化学的に反応性のあるヒドロゲルなどの化学的に制御されたゲーティング機構がポートの付近に配置できる。そのようなヒドロゲルは、隣接する媒体の化学組成に基づいて膨張または後退できる。ゲーティング機構は、ポートのかさの膨張および閉塞に基づいて動作することができ、またはヒドロゲルは機械的な付属構造によって形成できる。そのような構造の1つの例に、Applied Physics Letters、81(16):3091~3093、2002 に掲載された「Micromechanical Cantilever as an Ultrasensitve pH Microsensor 」にR.Bashir、J.Z.Hilt、O.Elibol、A.Gupta、およびN.A.Peppasによって述べられるようなバイモルフ・ビーム(bimorph beam)が含まれる。別の例には、Sensor and Actuators B、114(l):9~18、2006 に掲載された「A Microstructured Silicon Membrane with Entrapped Hydrogels for Environmentally Sensitive Fluid Gating 」という名称の1つの論文でA.Baldi、M.Lei、Y.Gu、R.A.Siegel、およびB.Ziaie によって開示された、微細ポートによって開口部を付けられた表面カバーが含まれ、別の例には、Journal of Microelectromechanical Systems、12(5):613~621;2003に掲載された「A Hydrogel-Actuated Environmentally Sensitive Microvalve for Active Flow Control」でA.Baldi、Y.Gu、P.E.Loftness、R.A.Siegel、およびB.Ziaie によって述べられるような閉塞性シールを形成するのに適した要素を移動するパッドが含まれる。これらの参考文献の内容全体が本明細書に参照によって組み込まれる。

図3は、ブローブ30cの別の実施形態を示し、プローブ先端32aが導電性であり、追加の刺激部位として機能する。これは、従来の刺激部位として機能することができ、単極および双極の刺激を補助する。刺激部位36a〜cの遠位リングと共に、それは3次元で先端の付近の電流の誘導を補助する四面体の頂点に中心を合わせられた刺激部位の組を形成する。図3の実施形態は、刺激電極36a〜36cと遠位の刺激電極32aとの間に追加の記録電極42も有する。また、多接点接続端子20cは、米国仮特許出願第60/820,914号にさらに詳しく開示されるような、2つの反円筒形またはD字形のコネクタに沿って軸方向に間隔を置いた複数の電気接点を有し、その内容全体は、参照によって本明細書に組み込まれる。

図6は、刺激導電体50がプローブの周縁の周囲に均一に間隔を置いて配置されるのではなく組になって配置される別の実施形態を示す。4本からなる3つの組が示されるが、その代わりに導電体は3本からなる4つの組に配置することができる。そのような実施形態により、中央の管腔70とプローブの外側との間を連通するポートを設け、または導電体の組の間の壁厚を低下させると共に、プローブの可撓性を改善することが可能になる。
図7は、図1の断面線101での別の実施形態の軸方向断面図を示す。この実施形態では、導電体が分離された前の実施形態とは異なり、刺激導電体および記録導電体は、プローブの同じ環状の空間にある。この実施形態は、両方の導電体を同じ管状の空間に配置するので、中央の管腔70はより大きいものであることができる。好ましい実施形態では、刺激導電体50および記録導電体60は、螺旋の周囲に交互になるが、別の実施形態では、刺激導電体および記録導電体は別々の組として通ることができる。別の実施形態では、刺激導電体50と記録導電体60の間に、電気的中性の点に連結できる別の導電体があることができる。別の実施形態では、電気的中性の点に連結できる管72を導電体材料によって被覆できる。
図8は、記録導電体60および刺激導電体50が組に分けられた別の実施形態を示す。この実施形態は、図7に示される実施形態と比較して刺激導電体と記録導電体の間に望ましくない容量性結合が起こる可能性を低下させる利点があるが、個々の記録導電体の間に望ましくない容量性結合が起こる可能性を増加させる。
図9は、流体(気体または液体)の物質または薬物の送出またはサンプリング、あるいは液体または揮発性物質のサンプリングを可能にするための、中央管腔70および管状管腔71の二重管腔を有する実施形態を示す。管腔は、図2および13A〜13Cに40として示されるポートと連通することができ、そのような連通は、電気的または化学的に開閉制御することができる。管腔の遠位端は、管腔の内容または管腔の内容の一部を排出するために、閉じられ、透過可能であり、選択的に透過可能であり、または開くことができる。2つの管腔の遠位端は、一方がレボドパなどの薬物を含む液体、あるいは一酸化炭素または一酸化二窒素などの生体活性効果を有する気体の媒体を送出し、もう一方の管腔が1つまたは複数の物質をプローブ内のポート40またはその他の開口の付近の媒体と交換する機会を得た後に媒体を回収するように、互いに連通することができる。米国特許第6,094,598号および米国特許第6,227,203号に開示されるもののような送出できるその他の治療薬剤が当分野でよく知られており、その両方は、その内容が参照によって本明細書に組み込まれ、脳脊髄液(CSF)などの細胞外流体がしばしば標本として抜き取られる。この実施形態では、プローブ内の追加の壁78によって形成された追加の環79内に電気刺激および記録用の導電体が共に通る。

図16は、モニタおよびモジュレート用リードの斜視図を示す。図16では、リード上の4つの刺激領域が、それぞれ3つの独立の刺激電極を含む。すべての3つの刺激電極36a、36b、36cは、最遠位領域でのみ可視である。2つの刺激電極は、35a、35b、34a、34b、33a、33bを含むリードのその他の領域で可視である。さらに、リードは、3つの記録電極37、38、および39、ならびに遠位のリード先端32の付近に追加の記録電極52を有する。内側のシャフト53が、リード本体10内に収容され、本明細書に前述したガイドワイヤ、スタイレット、管腔等を収めるように適合できる。

下記の表1は、環状のリングの周りの異なる刺激部位を刺激することによって異なる機能的な刺激効果が達成できることを実証する集計されたデータを要約したものである。図16に示されるものと同様のリードが、麻酔をかけた猫の大脳基底核に挿入された。最遠位の環状のリング(36a、36b、および36c)の刺激部位が、脳を電気的に刺激するために共に、および独立に通電された。接地が側頭筋に配置された。十分な大きさの電気的刺激が同側または対側、あるいは両方の顔面筋での反応を引き起こした。刺激の大きさが電圧の段階で送られ、運動反応が順序尺度に基づいて段階評価された(NR〜反応なし、THR、反応閾値、より大きな数がより高い大きさの閾上反応に対応する)。部位36aのみが刺激された場合、同側の動きに関する反応閾値が対側の動きよりも低かった。部位36bのみが刺激された場合、同側の動きおよび対側の動きに関する反応閾値は同じであった。部位36cのみが刺激された場合、対側の動きに関する閾値が同側の動きに関するものよりも低かった。すべての3つの部位が同時に刺激された場合、同側運動に関する閾値は対側運動に関するものよりも低かったが、同側運動および対側運動の両方に関する閾値は、任意の単一の部位の刺激の場合よりも低かった。この試験からのデータが下記の表1に概要を示され、この刺激の識別閾値(differential stimulation thresholds )のパターンにより、環状のリング内の異なる部位を刺激することが電流を脳内で誘導することが実証される。
図18A〜18Cは、リードが電場電位を記録でき、異なる記録部位が異なる電位を記録することを実証する。記録は、上記に論じたような図16に示される同じリードから、同じ配置で得られた。反応は、目の前で閃光を揺らすことによる視覚経路の感覚刺激によって引き起こされた。図18Aでは、トレースT1が記録部位38から記録され、図18Bでは、トレースT2が記録部位39から記録される。これらのトレースのスペクトル分析は、180Hzおよび300Hzでの振動を示し、それは電力網との意図しない結合から生じるものと考えられる。クリスチャーノ・フィッツジェラルド・フィルタ(Christiano-Fitzgerald filter)がこれらの周波数の付近の信号エネルギーを除去するために加えられ、図18A〜18Cに示されるようにT1aおよびT2aで表される。図18Cのトレース△は、差T1a−T2aである。トレースは同様に見えるが、それらが主に電気的なクロストークから生じる場合のように釣り合っていない。位置Aでは、T1/T1aは、T2/T2aと比較してより持続的な陽性を有する。位置Bでは、トレースの陽性T1/T1aおよびT2/T2aがほぼ同一である。位置Bと位置Cの間の三相波の振幅は、トレースT1/T1aおよびT2/T2aにおいて大幅に異なる。この記録された電位の振幅は、リードの位置を反映して、最適に記録された電場電位の振幅に比べて幾分少ない。
図19A〜19Cは、灰白質の中心での配置の自発的活動の電場電位の特徴をリードが記録できることを実証する。記録は図18A〜18Cでの記録が得られる部位に対して3mm背部の部位から得られた。この記録の振幅は電力網からの干渉の振幅よりもはるかに大きかったので、Christiano−Fitzgeraldフィルタリングが必要でなかった。図19AのトレースT1は記録部位38から記録され、図19BのトレースT2は記録部位39から記録される。図19Cのトレース△は、差T1−T2である。トレースは、電場電位の記録の時間経過および振幅の特徴によって同様に見える。差のトレース△は、活動電位波形の0.5から3.5msecの継続時間を伴ういくつかの非定常波、および数十ミリボルトの振幅の特徴を有する。図18A〜18Cに示される記録と共に、これらのデータは、図16に示されるようなリードが白質および灰白質からの電場電位を記録でき、適切な信号処理によって活動電位スパイクも記録できることを実証する。

- 組織を刺激およびモニタする装置であって、
近位端および遠位端を有する細長い部材と、
前記細長い部材に沿って軸方向に並べられ、前記遠位端の付近に配置された複数の環状の刺激電極にして、組織内に電流を流すようになされ、少なくとも1つの環状の刺激電極が少なくとも3つの独立の刺激点を有する環状の刺激電極と、
前記刺激電極に隣接して配置された複数の記録電極にして、局所的な組織の電位を測定するようになされた複数の記録電極と、

組織にプローブを埋込みをするステップにして、前記プローブが複数の記録電極、および複数の環状の刺激電極を有し、前記環状の刺激電極の少なくとも1つが少なくとも3つの独立の刺激点を有するステップと、
前記組織を前記環状の刺激電極からの治療電流によって刺激するステップと、
局所的な組織の電位を前記刺激に応答して前記記録電極によって測定するステップと、
前記刺激の効果に関するフィードバックを行うために前記測定された局所的な組織の電位を分析するステップと、
前記フィードバックに応答して前記刺激を調整するステップと含む方法。 - 前記組織が脳組織である、請求項27に記載の方法。
- 前記組織を治療薬剤によって刺激するステップをさらに含む、請求項27に記載の方法。
- 前記プローブをアンカーによって前記組織に解放可能に連結するステップをさらに含む、請求項27に記載の方法。
- 前記プローブが磁気共鳴映像と適合性を有する、請求項27に記載の方法。
- 前記刺激の効果に基づいてフィードバックを行うために、前記組織から化学物質を集めるステップをさらに含む、請求項27に記載の方法。

前記プローブに電気的刺激を与えるようになされた埋込み可能および制御可能なパルス発生器と
を備えるシステム。 - 治療される前記組織が脳組織である、請求項35に記載のシステム。
- 前記プローブを患者の頭部に取外し可能に固定するようになされた定着デバイスをさらに備える、請求項35に記載のシステム。
- 前記パルス発生器を制御するようになされた患者プログラマをさらに備える、請求項35に記載のシステム。
- 前記記録電極および刺激電極と電気的に接続された多接点コネクタをさらに備える、請求項35に記載のシステム。
- 前記組織プローブが磁気共鳴映像と適合性を有する、請求項35に記載のシステム。
