Abstract
Description
本出願は、2006年5月17日出願の米国仮特許出願第60/800,853号明細書(第‘853号出願)に対する優先権を主張する。本出願はまた、現在係属中の2006年12月29日出願の米国特許出願第11/647,312号明細書(第‘312号出願)に対する優先権も主張する。第‘853号出願及び第‘312号出願は、双方とも本明細書に完全に記載されたかのように本明細書によって全体として参照により援用される。
本発明は、体内組織を穿刺又は刺通するための針アセンブリ及び方法に関し、例えば、右心房から心房中隔を横通することにより左心房に到達するための経中隔的なアクセスシステム及び方法を含む。特に、本発明の実施形態は、経中隔穿刺が要求される心臓学的な手技においてカテーテルアセンブリと共に使用される医療器具を対象とする。穿刺アセンブリは、拡張器内で格納位置に付勢されている可動穿刺器具を有し、この器具の位置は正確に配置可能である。穿刺アセンブリは好ましくはアセンブリの長さの大部分に沿って可撓性を有し、従って任意の所定形状の任意のカテーテルアセンブリと共に使用でき、且つ経中隔手技での使用に必要な場合には、シャフト内の所定位置から拡張器の端部を越えて伸展する位置まで調節可能である。
Claims (21)
- 経中隔医療器具であって、
近位端と遠位端とを有する拡張器、
前記拡張器内に配置される穿刺器具、及び
前記穿刺器具と前記穿刺器具の近位端で操作可能に連結される変位機構、
を備え、前記変位機構に力がかかると、前記変位機構が前記穿刺器具の遠位部を前記拡張器内の初期位置から前記拡張器外部の位置まで前進させることができるとともに、前記変位機構から前記力が取り除かれると、前記穿刺器具の前記遠位部が前記拡張器内の初期位置に後退する、経中隔医療器具。 - 前記穿刺器具の長さが前記拡張器の長さと実質的に等しい、請求項1に記載の経中隔医療器具。
- 前記穿刺器具の長さが前記拡張器の長さより実質的に短く、それでもなお前記変位機構により前記拡張器外部の位置に伸展することが可能である、請求項1に記載の経中隔医療器具。
- 前記穿刺器具が、前記拡張器の前記遠位端に近接して位置する遠位端と可撓性の部分とをさらに備える、請求項1に記載の経中隔医療器具。
- 前記穿刺器具が針を備える、請求項4に記載の経中隔医療器具。
- 前記穿刺器具が湾曲した針を備える、請求項4に記載の経中隔医療器具。
- 前記穿刺器具が、前記可撓性部分と前記変位機構との間にさらに剛性部分を備える、請求項4に記載の経中隔医療器具。
- 前記拡張器が拡張器遠位端及び拡張器近位端をさらに備え、前記拡張器遠位端の断面積が前記拡張器近位端の断面積より小さい、請求項1に記載の経中隔医療器具。
- 前記穿刺器具の少なくとも一部がさらに可撓性ポリマーから構成される、請求項1に記載の経中隔医療器具。
- 前記穿刺器具の少なくとも一部がさらに可撓性金属から構成される、請求項1に記載の経中隔医療器具。
- 前記穿刺器具の少なくとも一部がさらに、ポリマー、金属、又はこれらの組み合わせの可撓性コイルから構成される、請求項1に記載の経中隔医療器具。
- 前記変位機構が前記拡張器と操作可能に連結され、その結果前記変位機構が操作されると前記拡張器が前記変位機構の近位端に向かって動く、請求項1に記載の経中隔医療器具。
- 前記変位機構が前記穿刺器具と操作可能に連結されるばね機構をさらに備える、請求項1に記載の経中隔医療器具。
- 前記ばね機構が非付勢位置にあるとき、前記穿刺アセンブリを前記拡張器内に保持する、請求項13に記載の経中隔医療器具。
- 近位端と遠位端とを有する細長チューブ状部材、
前記細長チューブ状部材内に配置される穿刺器具、及び
前記穿刺器具と前記穿刺器具の近位端で操作可能に連結される変位機構、
を備える経中隔医療器具であって、前記変位機構に力がかかると、前記変位機構が前記穿刺器具の遠位部を前記細長チューブ状部材内の初期位置から前記細長チューブ状部材外部の位置まで前進させることが可能な、経中隔医療器具。 - 前記変位機構に引込み力をかけ、それにより前記穿刺器具を前記細長チューブ状部材内部の位置に後退させるための機構をさらに備える、請求項15に記載の経中隔医療器具。
- 心臓の中隔を穿刺するための方法であって、
拡張器内に収容されている穿刺器具を中隔の標的領域に近接する心臓の領域内に導入するステップ、
中隔の前記標的領域に近接する前記拡張器外部の位置に前記穿刺器具を伸展させるステップ、
中隔の前記標的領域を穿刺するステップ、及び
前記穿刺器具を前記拡張器内の位置に後退させるステップ、
を含む、方法。 - 前記穿刺器具を後退させる前に前記拡張器を中隔の前記標的領域を通じて前進させるステップをさらに含む、請求項17に記載の方法。
- 前記穿刺器具を後退させた後に前記拡張器を中隔の前記標的領域を通じて前進させるステップをさらに含む、請求項17に記載の方法。
- 前記穿刺器具を後退させるステップが、中隔の前記標的領域を穿刺している間に使用者によりかけられている力が解放されると自動的に起こる、請求項17に記載の方法。
- 内部ルーメンを有する拡張器を提供するステップ、
前記拡張器の前記内部ルーメン内に可撓部を有する穿刺器具を提供するステップ、
変位機構を前記穿刺器具と操作可能に連結して、前記変位機構に力がかかると前記穿刺器具が前記拡張器内の第1の位置から前記拡張器の遠位端外部の第2の位置に伸展され、前記変位機構から前記力が取り除かれると、前記穿刺アセンブリが前記第1の位置に自動的に後退するようにするステップ、
を含む、伸展可能な経中隔医療器具を作製するための方法。
