Abstract
Description
母乳で育てられた子供は、調製乳で育てられた子供よりも健康でアレルギーおよび感染症の発病率が低い傾向にあり、またやせ型の傾向がある[1]。これらの保護的効果の理由は完全には分かっていないが、乳タンパク質は明らかに役割を果たしている。さらに、従来の研究から、人乳タンパク質が乳児において胃腸、神経、および免疫学上の発達に影響を及ぼすことが実証されている[2−6]。

RHMB長期コホートのみにおいては、一人の訓練された研究看護師が母体の身体計測を測定した(B.S.D.)。母親の体重は、E−Zキャリー携帯型デジタル体重計(E-Z Carry Portable Digital Scale)(米国メリーランド州バルチモア所在のHopkins Medical Products)を使用して毎月の訪問時に測定された。女性達は靴を脱ぎ外出着で計測を受けた。身長は、最初の訪問時に、対象者がソックスを着用し、かかとを合わせ、つま先を45度の角度に離し、頭をフランクフルト水平面にして、起立した状態で計測した。身長を壁に記録し、床との垂直距離を計測した。体容量指数(BMI)を、体重(kg)を伸長(メートル)の二乗で割ったものとして算出した(kg/m2)。
脂質はラジオイムノアッセイ(RIA)に干渉するので、スキムミルクを使用し、また同じ人物(W.B.)が3つのコホートの全サンプルをアッセイした。乳汁サンプルを融解し渦流混合した。遠心分離(1,500μg、20分、4℃)した後に脂肪層を取り除くことによって、スキムミルク(水相)を調製した。免疫反応性のアディポネクチンを、1:3に希釈したスキムミルク100μL(33.3μL相当)を使用して、市販のRIAキット(米国ミズーリ州セントチャールズ所在のLinco Research)を用いて二連で測定した。測定間および測定内のアッセイの変動係数はそれぞれ8.5%および3.9%であった。RHMB臨時コホートのサンプルでは、レプチンについても、Houseknechtらの試
験計画書[47]に従って市販のRIAキット(米国ミズーリ州セントチャールズ所在のLinco Research)を使用して、スキムミルク中にて二連でアッセイした。測定間および測定内のアッセイのCVはそれぞれ4.5%および5.0%であり、検出限界は0.3ng/mLであった。
統計学的解析は、SASのバージョン9.1.3.を使用して実施した。記述統計は、データが正規性でないためメジアンおよび範囲として報告される。統計学的モデルに使用される乳汁アディポネクチンの正規性を改善するために、データを自然対数(ln)に変形した。8つの乳汁レプチン濃度は検出限界(0.3ng/mL)未満であり、これらの値は0.2ng/mLに設定した。0に設定した検出限界未満の値でも分析を実施したが、結果は実質的に変化がなかったので報告しない。
ムとして扱う、反復測定を伴う混合モデルを構築した。データに基づいて導かれたベイズ情報量規準(BIC)を使用して、最良の相関構造(この場合、様々な時点にわたって得られたln(乳汁アディポネクチン)の間で等価な相関)が選択される。従属変数はln(乳汁アディポネクチン)であり、独立変数は授乳日であった。
スキムミルクの系列希釈物(10〜40μL)のアディポネクチン濃度は、標準曲線にパラレルであった。スキムミルクサンプルに、5、20、および100ng/mLのヒトアディポネクチン標準品をスパイクすると、添加量の回収率は平均109%であった。
免疫反応性のアディポネクチンはすべてのサンプル中においてスキムミルク中で検出された(表2)。乳汁アディポネクチン濃度は妊娠期間には関係していなかった(p≧0.18)。
RHMB臨時サンプル(n=30)では、授乳月はln(乳汁アディポネクチン)と負に相関し、β=−0.059±0.024、p=0.02であった。これらの結果はRHMB長期サンプルにおいて確認され、同サンプルでも授乳月がln(乳汁アディポネクチン)と負に相関した(β=−0.059±0.007、p<0.0001)(図2)。これらの結果に基づき、乳汁アディポネクチンは、授乳1週と比較して、授乳7ヶ月までに6.9ng/mL低くなると予測される。
の誤差を最小限にしているので、上記の違いは生理的な根拠を有する可能性が最も高い。さらに、乳汁中のアディポネクチン濃度の人種差は、血清中のアディポネクチン濃度に類似している。いくつかの研究から、白人は、アジア人[54、55]、アメリカインディアン[29]、およびアフリカ系アメリカ人[56]より高い血清中アディポネクチン濃度を有していることが実証されている。非ヒスパニックの白人とヒスパニックとの間の血清中アディポネクチン濃度は直接比較されていないが、メキシコの集団は元来アメリカインディアン集団と白人集団との組み合わせであると考えられる[57]。したがって、メキシコ人の乳汁中のアディポネクチン濃度がより低いことが見出されると予想される。
RHMB長期コホートにおいて横断的には、妊娠後の母親のBMIは、ほとんどの時点においてln(乳汁アディポネクチン)濃度と有意に正の相関を有していた(表3)。統計学的に有意ではなかった時でも、その関係の程度は、授乳1〜7ヶ月において比較的一貫していた(β(範囲)=0.08〜0.13、p(範囲)=0.01〜0.16)。

上述のようなCBC由来の22セットの縦断的サンプルを使用して、人乳中のアディポネクチンと乳児の体重との縦断的関係を調査した。データと反復測定の相関構造を明らかにした後では、乳汁中のアディポネクチン濃度は、乳児の年齢および年齢に合わせて調節して、28週までの乳児において体重増加と負に相関する(β±SE:−0.20±0.12kg、p=0.082)。この関係は統計学的に有意ではないが、我々の小規模なサンプルサイズではこの結果を検出するための検出力がなかった。この関係を図5に示すが、図5では、年齢に合わせて調整された乳児の体重の残差に対して乳汁アディポネクチンの自然対数がプロットされている。人乳アディポネクチンと乳児の発育との関係をさらに検討するために、産後1ヶ月で集められた36サンプルを、306組のメキシコの母体・乳児ペアのコホートから無作為に選択した。メキシコの母親と米国の母親との間の全体的な乳汁アディポネクチンレベルの差にもかかわらず、1ヶ月の乳汁アディポネクチンの自然対数と、産後1ヶ月および4ヶ月の乳児の脂肪過多との関係は、メキシコのコホートとシンシナティのコホートにおいて程度が類似している(表4)。
マウス乳汁中の自然なアディポネクチンレベルを泌乳期間にわたって測定した。アディポネクチンレベルは泌乳期間の早期に最も高く、泌乳期間の後期に最も低いことが分かった(図7)。乳汁中のアディポネクチンが血清中へ吸収されるのであれば、乳児の血清中レベルは消費された乳汁中のアディポネクチンレベルの差によって影響を受けるはずである。乳児の血清をアディポネクチンレベルについて分析し、血清アディポネクチンが泌乳期間の最初の数日に最も高く泌乳期間の後期に最も低いことが分かった(図8)。これらのデータは、乳汁アディポネクチンが哺乳動物の乳児における血清アディポネクチンの供給源であることと一致している。
アディポネクチンが腸粘膜を横切り、若い哺乳動物の血清中に吸収される能力を測定するために、機能的な組換えヒトアディポネクチン(哺乳動物細胞において発現)を仔マウスの胃に経口投与した。対照として生理食塩水を投与した。ヒトアディポネクチンに特異的なELISAアッセイを使用して、管腔液、腸管および血清中のヒトアディポネクチンを計測した。図9に示されるように、ヒトアディポネクチンを経口投与されたマウスの腸管の管腔液は、投与されたヒトアディポネクチンの量に比例したヒトアディポネクチンレベルを含んでいた。図9に示されるように、最も高量のヒトアディポネクチンを投与されたマウスは腸管内腔中に最も高レベルのヒトアディポネクチンを有し、生理食塩水のみを投与されたものは腸管内腔にヒトアディポネクチンを全く有していなかった。したがって、経口投与の結果無傷のアディポネクチンがうまく腸の中に達した。ヒトアディポネクチンは腸の組織内に輸送され、この場合も最も高量のアディポネクチンを投与されたマウスにおいて腸組織で最も高レベルとなった。さらに、腸内に投与されたヒトアディポネクチンは、血清中に吸収された。図9に示されるように、最も高量のヒトアディポネクチンを投与されたマウスは、最も高い血清中アディポネクチンレベルを示した。したがって、ヒトアディポネクチンは若い哺乳動物において腸から血清中へ吸収可能であり、吸収されたアディポネクチンは、アディポネクチンの乳汁中レベルと乳児における生理学的結果との関係に関与している可能性が考えられる。
アディポネクチンは、薬学的に許容可能な担体、例えばリン酸緩衝生理食塩水、水中エタノール混合物、水、および油/水乳濁液もしくは水/油乳濁液のような乳濁液、ならびに様々な湿潤剤もしくは賦形剤を含んでいる組成物に含めて投与することができる。アディポネクチンは、患者に投与された時に副作用、アレルギー反応またはその他望ましくない反応を生じない材料と組み合わせることができる。使用される担体または媒体は、溶剤、分散剤、コーティング剤、吸収促進剤、制御放出剤、および1以上の不活性の賦形剤(デンプン、多価アルコール、造粒剤、微結晶セルロース、希釈剤、滑沢剤、結合剤、崩壊剤などがある)、などを含むことができる。望ましい場合には、開示の組成物の錠剤投与量に標準的な水性または非水性の技術でコーティングを施してもよい。
オティクス、鎮痛剤および抗炎症剤のような治療薬も含むことができる。
適切な投与量は当業者によって決定され、例えば、患者の免疫状態、体重および年齢のような要因によって変化する。ある場合には、投与量は、ヒトの母乳中に存在するアディポネクチンについて見出される濃度と同じような濃度になるだろう。

Claims (31)
- 精製アディポネクチンまたはその生物学的活性を有するフラグメントを含有する組成物を経口投与することを含む、患者において肥満症を治療またはリスク低減する方法。
- 精製アディポネクチンまたはその生物学的活性を有するフラグメントを含有する組成物を経口投与することを含む、患者においてメタボリック症候群を治療またはリスク低減する方法。
- 精製アディポネクチンまたはその生物学的活性を有するフラグメントを含有する組成物を経口投与することを含む、患者において炎症性腸疾患、高血糖症、インスリン抵抗性、インスリン抵抗性を伴うメタボリック症候群、2型糖尿病、メタボリック症候群であって高血圧、アテローム性動脈硬化、冠性心疾患または虚血性心疾患を伴うもの、から選択される病気を治療またはリスク低減する方法。
- 精製アディポネクチンまたはその生物学的活性を有するフラグメントを含有する組成物を経口投与することを含む、乳幼児において壊死性小腸大腸炎を治療またはリスク低減する方法。
- 患者は18歳未満である、請求項1〜3のいずれか1項に記載の方法。
- 患者は15歳未満である、請求項1〜3のいずれか1項に記載の方法。
- 患者は10歳未満である、請求項1〜3のいずれか1項に記載の方法。
- 患者は1歳未満である、請求項1〜3のいずれか1項に記載の方法。
- 乳幼児は早産で生まれたことを特徴とする、請求項4に記載の方法。
- アディポネクチンまたはその生物学的活性を有するフラグメントはグリコシル化されている、請求項1〜4のいずれか1項に記載の方法。
