Abstract

Description
(発明の分野)
本発明は、身体導管(body conduit)の壁の平滑筋を処置するための方法および装置に関し、そしてより詳細には、本発明は、身体導管を取り囲む平滑筋の塊(bulk)を、平滑筋の放射エネルギー処置を用いて減少させることにより、医学状態を処置するための方法に関する。
喘息は、肺の気道を取り囲む平滑筋の収縮を引き起こす多くの刺激に対する気管気管支樹(tracheobronchial tree)の強まった反応を含む疾患である。気道の反応亢進は、気道の異常な組織反応から生じ、この組織反応は、免疫学的に誘導されるか、または他の正常な機能応答の生化学的不均衡および神経液性(neurohumoral)不均衡に由来する。健常な患者においては、この患者が咳き込んでいるときなどに、気道を通る限局的な空気流を増加させ、そして肺に入る吸入した粒子を吐き出すように、気道を取り囲む平滑筋は収縮する。喘息患者においては、気道が反応亢進である。空気中のごく少量の花粉、アレルゲン、または他の物質は、これらの反応亢進気道で多量の平滑筋収縮または痙攣を刺激する。この反復される平滑筋の収縮は、筋肉を訓練させ、このことは、筋肉を肥大させ、かつ大きくする。平滑筋を取り囲む組織は、比較的剛性であり、肥大した平滑筋は、気道の方向に拡大して、空気の通り道のための気道の内径を縮小させる。例えば、健常個体は、気道を取り囲む平滑筋の厚さが約0.01mmであるが、喘息患者における平滑筋厚さは、約2または3mmまでに増大し得る。これに対応して、気道の内径は、健常個体については、約2〜3mmであるのが、喘息個体では約0.5mm以下まで減少され得る。気道が狭いことにより、喘息と関連した笛状音またはぜん鳴が引き起こされる。

(項目2) 上記光源が約240nm〜約280nmの波長を有する光を送達する、項目1に記載の装置。
(項目3) 上記光源が赤色可視域にある光を送達する、項目1に記載の装置。
(項目4) 上記光源が、上記気道の上記壁に適用されたときに、該気道を取り囲む平滑筋細胞中のDNAを架橋する波長および強度を有する光を送達し、そして該平滑筋細胞の複製を妨げる光を送達する、項目1に記載の装置。
(項目5) 上記光源が、上記気道の上記壁に適用されたときに、該気道を取り囲む粘液腺細胞中のDNAを架橋する波長および強度を有する光を送達し、そして該粘液腺の複製を妨げる光を送達する、項目1に記載の装置。
(項目6) 上記光方向付け部材が、上記ファイバーの長軸から離れた方向で上記光伝達ファイバーからの光を再方向付けする、実質的に円錐状の反射面を備える、項目1に記載の装置。
(項目7) 上記反射面が、凹状断面である、項目6に記載の装置。
(項目8) 上記反射面が、実質的に平面状断面である、項目6に記載の装置。
(項目9) 上記反射面が、実質的に放物型断面である、項目6に記載の装置。
(項目10) 上記光方向付け部材が、上記ファイバーの長軸から離れた方向で上記光伝達ファイバーからの光を方向付けする、拡散レンズを備える、項目1に記載の装置。
(項目11) 上記光伝達ファイバーが、上記気道への送達のためのシースにより取り囲まれている、項目1に記載の装置。
(項目12) 上記シースが透明遠位末端部分を備える、項目11に記載の装置。
(項目13) 上記シースが、上記光方向付け部材により再方向付けられた光が、該シースを出ることを可能にするための複数の透明ウィンドウを有する遠位部分を備える、項目11に記載の装置。
(項目14) 食道の壁を処置するための装置であって、該装置は、以下:
食道へ挿入されるように形成された細長本体であって、該デバイスは近位末端と遠位末端とを有する、細長本体;
該食道へ光を伝達するために、該細長本体の近位末端から遠位末端へ延びる光伝達ファイバー;
該食道の壁に適用されたときに、平滑筋組織の複製を妨げる、該平滑筋組織の変化を引き起こす強度で該細長本体に光を送達するための光源;
該光源に該細長本体を接続するための、該細長本体の該遠位末端上のコネクタ;および

(項目15) 尿管または尿道の壁を処置するための装置であって、該装置は、以下:

該尿管または尿道へ光を伝達するために、該細長本体の近位末端から遠位末端へ延びる光伝達ファイバー;

平滑筋細胞の減少または除去により気管支痙攣を制御する工程;
を包含する、方法。
(項目17) 上記気道の上記壁に適用された光が約240nm〜280nmの波長を有する、項目16に記載の装置。
(項目18) 上記気道の上記壁に適用された光が赤色可視域にある、項目16に記載の方法。
(項目19) 上記光が、上記気道に沿って光送達デバイスを動かすことにより上記壁に適用される、項目16に記載の方法。
(項目20) 呼吸状態を処置して粘液栓形成を制御する方法であって、該方法は、以下:
粘液腺細胞に変化を引き起こし、そして該粘液腺細胞の複製を妨げる波長および強度で、肺における気道の壁に照射する工程;ならびに
粘液腺の減少および除去により粘液栓形成を妨げる工程;
を包含する、方法。
(項目21) 上記気道の上記壁に適用された光が、約240nm〜280nmの波長を有する、項目20に記載の装置。
(項目22) 上記気道の上記壁に適用された光が赤色可視域にある、項目20に記載の方法。
(項目23) 上記光が、上記気道に沿って光送達デバイスを動かすことにより上記壁に適用される、項目20に記載の方法。
(項目24) 食道を処置してアカラシアまたは食道痙攣を低減させる方法であって、該方法は、以下:
平滑筋組織細胞に変化を引き起こし、そして該平滑筋細胞の複製を妨げる波長および強度で、該食道の上記壁に照射する工程;ならびに
該平滑筋組織の除去または減少により該平滑筋組織の痙攣を妨げる工程;
を包含する、方法。
(項目25) 上記食道の壁に適用された光が、約240nm〜280nmの波長を有する、項目24に記載の方法。
(項目26) 上記食道の上記壁に適用された光が赤色可視域にある、項目24に記載の方法。
(項目27) 尿管または尿道を処置して、痙攣を制御するための方法であって、該方法は、以下:
平滑筋組織細胞に変化を引き起こし、そして該平滑筋細胞の複製を妨げる波長および強度で、該尿管または尿道の上記壁に照射する工程;および
該平滑筋組織の除去または減少により該平滑筋組織の痙攣を妨げる工程;
を包含する、方法。
(項目28) 上記尿管または尿道の上記壁に適用された光が、約240nm〜280nmの波長を有する、項目27に記載の方法。
(項目29) 上記尿管または尿道の上記壁に適用された光が赤色可視域にある、項目27に記載の方法。
(発明の要旨)
本発明は、平滑筋組織の複製を妨げるように脈管壁の平滑筋組織に放射エネルギーを印加することによって身体で脈管を処置するためのデバイスおよび方法に関する。平滑筋組織の処置により、経時的に平滑筋組織の量を低減させ、そしてこれは、身体導管の内径を増加させ、そして平滑筋痙攣を妨げる。
図1は、身体導管の壁12に光を送達するための光送達デバイス10を例示する。光送達デバイス10は、光伝達ファイバー18を取り囲む外側カテーテルまたはシース16を備える。光方向付け部材20は、導管壁に光を方向付けるために光送達デバイス10の遠位末端に配置される。本発明は、肺の気道の処置に関して詳細に記載されるが、本発明はまた、他の身体導管の処置のために使用され得ることが理解されるべきである。
