Abstract
Description
本出願は、米国特許法第119条(e)項に基づき、ともに「種々の型の癌に関する診断における腫瘍関連抗原の組合せ」と題された、2004年6月17日に出願された米国特許仮出願第60/580,969号に対して優先権を主張する、2005年6月17日に出願された米国特許出願第11/155,288号の一部継続であり、これらの記載内容は各々、参照により本明細書中で援用される。

T細胞応答を誘導する。したがって、腫瘍組織によるMHC発現を確定することは有益である。PCR及び抗体ベースの方法を含むMHCの検出のための試薬は、当該技術分野で広く知られている。クラス、遺伝子座及び型に特異的な試薬が、一般に利用されている。クラスI発現は、β2−マイクログロブリンの検出によっても判断される。クラス及び遺伝子座に特異的な試薬は、広範に適用可能な均一手法の利点を提供する。型特異的試薬は、発現及びMHC型を同時に確定することを可能にする。抗体ベースの手法は、細胞表面でのタンパク質発現を直接的に検出するという利点を提供するが、これは、表面発現のみが推測されるRT−PCR等とは対照的に臨床的関連性を有する。TCR四量体ベースのアッセイは、MHC及び標的抗原(実際には標的エピトープも)の発現の同時確定を可能にし、これらのアッセイは本質的に型特異的である。

例えばワクチンを含む免疫性組成物は、完全な抗原又はエピトープペプチドを用いて調製することができる。ペプチド免疫原は、例えば、当業者に既知である標準的ペプチド合成手法を用いて容易に調製することができる。免疫原は、化学合成を行う多数の企業の1
つにより商業的に調製され得る。このような企業の例として、卸売業者がCLINALFA AG(Laufelfingen、スイス)であるAmerican Peptides, Inc.がある。抗原又は免疫原はGMPスタンダードに基づいて調製することができ、HPLC解析により純度を評価することができる。産物はアミノ酸解析により特性化され、滅菌性及び発熱原が存在しないことを試験することができる。

効果的な外因性サイトカインの例は、GM−CSFである。GM−CSFは、抗原提示細胞(APC)上のB7−1又はB7−2のようないわゆる「共刺激」分子の発現を増強することが報告されている。これらの共刺激分子は、APCによるCTLの刺激中に起こる様々な相互作用において、重要な役割を担う。また、GM−CSFはAPCの活性化を誘導すると共に、APCの成長及び分化を促進することが知られており、これらのAPCを、多量に、有効な形で得られる重要なCTL刺激細胞としている。

胞メモリは繰り返し誘導され、再増幅及び再活性化される。抗原及びアジュバントは、生分解性ミクロスフェア又はリポソームとして調製されることが示唆されているが、これまでは、これらの製剤のいずれもが長期間にわたって癌細胞又は病原体を攻撃するのに有用なCTL応答を提供しなかった。好ましくは、抗原の送達は、目的の期間、目的の応答を得るための抗原レベルを維持するのに十分なレベルに維持される。一つの実施形態において、液体抗原組成物を有する貯蔵器は、抗原が動物のリンパ系に到達するように送達するのに用いることができる。以下の考察のほとんどは、抗原を送達するための輸液の使用に焦点を当てているが、リンパ系へ直接的にボーラス注射を用いることも可能であり、その数及び頻度は、使用する抗原の特定の型、及び配合により与えられる抗原の持続性による。

TuAAとDNAワクチンとの組合せを利用するいくつかのアプローチがある。第1のアプローチは、所定の組合せの全ての抗原又は全ての抗原からのエピトープを単一DNA発現ベクター中に含めることである。このアプローチは、製造及び患者への投与に関する簡便性という利点を有する。しかし、場合によっては、エピトープ競合がこのアプローチの有用性を限定する。すなわち、全てのTuAAを提示しているいくつかのエピトープを組み合わせて有するワクチンが患者に与えられた場合、最も免疫原性が高いエピトープのみが、免疫応答を誘発する可能性がある。また、全てのエピトープが高い有効性で発現するDNAワクチンを設計、構築することはより難しい。それにもかかわらず、各種類の癌において患者を処置する手順が簡便且つ均一であるため、以下に記載される他のアプローチよりもコストが低くなる見込みがある。

記載されている数値は、実行可能な程度に正確に報告されている。本発明のいくつかの実施形態で与えられる数値は、これらそれぞれの試験測定において見出される標準偏差において必ず生じるある程度の誤差を含む。
TuAA解析及び組合せの選択
TuAAの存在を、リアルタイムPCR(RT−PCR)により測定した。すなわち、全RNAを標準的な方法を用いて腫瘍検体から単離し、cDNAを標準的な逆転写手順を用いて作製した。相補的DNA(cDNA)を、cDNAのみとアニーリングするがゲノムDNAとはアニーリングしないように特異的に設計された遺伝子特異的プライマーを用いて増幅した。12個の卵巣腫瘍検体及び7個の結腸直腸腫瘍検体のTuAA発現パターンを、RT−PCRにより解析した。以下の表4に結果をまとめた。
卵巣癌
卵巣癌の場合、解析した全てのサンプルはPRAMEに対して陽性であった。したがって、PRAMEを含む組合せは、卵巣癌の症例における有効範囲を改善する。
結腸直腸癌
結腸直腸癌の場合、PRAME及びPSMAはそれぞれ、解析された7サンプルのうち5サンプルで検出された。7サンプルのうち6サンプルで、PRAME又はPSMAのいずれかが検出された。SSX−2は7サンプルのうち2サンプルでしか検出されなかったが、SSX−2−PRAME及びSSX−2−PSMAの組合せの両方が、有効範囲を7サンプルのうち6サンプルに増加した。同様に、NYE−SO−1は7サンプルのうち1サンプルのみで検出されたが、NY−ESO−1−PRAMEの組合せ、及びNY−ESO−1−PSMAの組合せは、有効範囲を、7サンプルのうち6サンプルに増加した。PRAME及びPSMAの組合せへのSSX−2又はNY−ESO−1の添加は、有効範囲を、7サンプルのうち7サンプルへと改善した。したがって、アセンブリパネルに関してPRAME、PSMA及びNY−ESO−1の組合せ、又はPRAME、PSMA及びSSX−2の組合せは、患者の大部分に良好な有効範囲及び抗原の余剰を提供した。PRAME、PSMA、NY−ESO−1及びSSX−2の組合せはさらなる余剰を提供した。
膵臓癌
リアルタイムPCR(RT−PCR)を使用して、PRAME、SSX−2、NY−ESO−1及びPSMAの存在を決定した。すなわち、5つの膵臓腫瘍検体から、全RNAを標準的な方法により単離し、cDNAを標準的な逆転写手順を用いて作製した。相補的DNA(cDNA)を、cDNAのみとアニーリングするがゲノムDNAとはアニーリングしないように、特別に設計された遺伝子特異的プライマーを用いて増幅した。
実施例5
腎細胞癌腫に関しては、SSX−2、PSMA及びPRAMEがそれぞれ、5、100及び40%の頻度で検出された(Sahin, U et al., Clin Cancer Res. 2000, 6: 3916; Chang SS et al., Urology 2001, 57: 801; Neumann E et al., Cancer Res. 1998, 58: 4090、それぞれ参照によりその全体が本明細書に援用される)。したがって、PSMA及
びPRAMEの組合せは、腎細胞癌腫に対して優れた有効範囲及び余剰を提供する。PSMA及びPRAMEの組合せにSSX−2を添加すると、余剰が改善される。
非小細胞肺癌
非小細胞肺癌に関しては、NY−ESO−1、SSX−2、MAGE−3、BAGE、Her2/neuの過剰発現及びPSMAの報告された存在は、それぞれ、21、15、60、6、16及び100%であった(Scanlan MJ et al., Cancer lett 2000, 150: 155; Chang SS et al., Cancer Res 1999, 59: 3192; Selvaggi G et al., Cancer 2002, 94: 2669、それぞれ参照によりその全体が本明細書に援用される)。したがって、NY−ESO−1、SSX−2、MAGE−3及びPSMAの組合せは、非小細胞肺癌の免疫療法に対する有効範囲及び抗原余剰を提供する。
メラノーマ
メラノーマに関しては、Melan−A、チロシナーゼ、NY−ESO−1及びSSX−2がそれぞれ、腫瘍検体の92、92、41及び35%で存在すると報告された(Fetsch PA, et al., Cancer 1999, 87: 37; Fetsch PA, et al., Cancer 2000, 90: 252; Schultz-Thater E et al., Br J Cancer 2000, 83:204; Sahin, U et al., Clin Cancer Res. 2000, 6: 3916)。したがって、Melan−A、チロシナーゼ、NY−ESO−1及びSSX−2の組合せは、メラノーマの免疫療法に対して優れた有効範囲及び抗原余剰を提供する。かなりの余剰が、チロシナーゼ及びMelan−Aを共に使用すること、又はNY−ESO−1及びSSX−2をチロシナーゼ又はMelan−Aのいずれかと組み合わせることにより達成される。
上記腫瘍型に関するさらなる調査は、観察された発現パターン及びTuAAの好ましいパネルを確立した。様々な供給業者から得た、合計で34個の卵巣、44個の大腸、18個の腎臓及び13個の膵臓の組織サンプルを、腫瘍関連抗原発現に関して、qRTPCRを使用して解析した。これらの検査の結果から、PRAME及びPSMAは、調査された4種類全ての腫瘍において頻繁(68%〜100%の範囲)に発現したことがわかった。NY−ESO−1及びSSX−2は、卵巣及び膵臓腫瘍の20%〜40%において発現した。
2つの抗原からのエピトープ発現プラスミドを用いた免疫化のスケジュール
2つの群のHHDマウス(n=4)を、図1に示されるように、Melan−A26-35A27L(ELA)発現pSEM及びSSX−241-49発現pCBPのいずれかを混合物として、又はpSEMを左の鼠経リンパ節及びpCBPを右の鼠経リンパ節に2回(0日目及び4日目)、リンパ節内注射より免疫化した。プラスミド量は25μg/プラスミド/用量であった。2週間後、動物を屠殺し、パルス化した又はペプチドを有さないT2細胞に対する細胞傷害性を計測した。
異なる抗原を担持する異なるベクターの同時投与
実施例9に記載されたように免疫化した動物を屠殺し、各群からの脾細胞をプールし、2つのペプチド(ELA又はSSX−241-19)で並行して刺激した。細胞傷害性を、Cr51−タグ付けしたペプチド添加T2標的細胞でインキュベートすることにより計測した。図2のデータは、様々な標的細胞に対する特異的細胞傷害性の平均(n=4/群)を示す。
ペプチド追加免疫工程の付加による多面的応答のレスキュー
4つの群のHHDマウス(n=6)を、図3に示されるように、pSEM及びpCBPを混合物として、又はpSEMを左の鼠経リンパ節及びpCBPを右の鼠経リンパ節に2回(0日目及び4日目)、リンパ節内注射より免疫化した。対照として、pSEM又はpCBPプラスミドのいずれかでマウスを免疫化した。プラスミド量は25μg/プラスミド/用量であった。2週間後、動物をMelan−A及び/又はSSX−2ペプチドで追加免疫し、プラスミド免疫化用量及び組合せを反映させた。2週間後、動物をCFSEで染色し、Melan−A又はSSX−2ペプチドを添加するか又は有さない脾臓細胞で攻撃誘発し、in vivoでの細胞傷害性を評価した。
ペプチド増幅は、非優性エピトープの免疫性をレスキューする
実施例11に記載されたようにマウスを免疫化し、ELA又はSSX−2ペプチドで被覆したHHD同腹子脾臓細胞で攻撃誘発した。同時に2つの抗原標的の特異的溶解を評価することが可能な3ピークのCFSE in vivo細胞傷害性アッセイを採用した。同数の対照CFSElo、SSX−241-49−CFSEmed、及びELA−CFSEhi細胞を、免疫化マウスに静脈内注入し、18時間後にマウスを屠殺し、フローサイトメトリを用いて、CFSE蛍光により脾臓における標的細胞消失を計測した(図4)。図4は、個々のマウスからのSSX−2及びMelan−A抗原標的の特異的溶解の割合、並びに各群のSEMの平均を示す。
同調−増幅免疫化に関する臨床的実践
図2及び図4のデータは、図5に示される、臨床における強い多面的応答を達成する2つのシナリオを示唆する。第1のシナリオ(A)では、追加免疫のためのペプチドの使用は、プラスミド及びペプチドが混合物として使用されたとしても、多面的免疫応答を回復させる。第2のシナリオ(B)では、プラスミド成分及びペプチド成分の分離はそれぞれ、多面的免疫応答の誘導を可能にする。
MKC1207:メラノーマに対する同調−増幅療法薬
MKC1207は、プラスミドpSEM(開示の全てが参照により本明細書に完全に援用される、2002年11月7日付けで出願された米国特許出願第10/292,413号(公開番号2003−0228634A1)、2004年2月10日付けで出願された同第10/777,053号(公開番号2004−0132088A1)、2004年4月30日付けで出願された同第10/837,217号(公開番号 )において開示され、その中でpMA2Mとして記載される)、並びにMelan−A26−35及びチロシナーゼ369−377に相当するペプチドを含む。上記プラスミドは、Melan−AエピトープのA27Lアナログ及び天然チロシナーゼエピトープ配列をコードする。上記プラスミドは、これらエピトープの両方がpAPCにより発現、提示されるように、これらエピトープ両方をコードする。療法薬の代替的な実施形態においては、上記ペプチドは、参照により本明細書に完全に援用される、「エピトープアナログ」と題された2005年6月17日付けで出願された米国特許出願第11/156,369号(公開番号

MKC1106:癌種に対する四価同調−増幅療法薬

- 予め選択された、増殖因子又はシグナル伝達タンパク質を含むパネル中の2つ以上の発現腫瘍関連抗原(TuAA)に関して患者の腫瘍組織をアッセイし、当該腫瘍に関する抗原プロファイルを構築する工程、及び
前記プロファイルに基づいて、プロファイル中の1つ又は複数の発現腫瘍関連抗原を標的とする、前記患者に対する免疫療法物質を選択する工程
を包含する、患者における癌の状態と免疫療法物質とを適合させる方法。 - TuAAのうちの少なくとも1つが癌精巣抗原、組織特異的抗原、癌胎児性抗原、分化抗原、増殖因子、増殖因子受容体、接着因子、シグナル伝達タンパク質、転写因子、癌遺伝子産物、腫瘍抑制遺伝子産物及び微生物抗原から成る群から選択される、請求項1記載の方法。

- 前記腫瘍が膵臓癌であり、TuAAのパネルがSSXタンパク質、PRAME、NY−ESO、LAGE、PSCA、PSMA及びMAGEタンパク質から成る群から選択される少なくとも2つのTuAAを含む、請求項1記載の方法。
- 抗原発現がRT−PCR、転写物決定、タンパク質決定、エピトープ決定又はその任意の組合せのうちの少なくとも1つを含む手法により検出される、請求項1記載の方法。
- 抗原発現が腫瘍細胞又は腫瘍関連間質細胞あるいはその両方で検出される、請求項1記載の方法。
- 前記腫瘍関連間質細胞が新血管系である、請求項23記載の方法。
- 前記新血管系関連抗原がPSMAであり、腫瘍細胞の抗原がNY−ESO−1、SSX−2、LAGE及びPRAMEから成る群から選択される、請求項24記載の方法。
- 前記腫瘍組織が原発性腫瘍組織を含む、請求項1記載の方法。
- 前記腫瘍組織が転移性腫瘍組織を含む、請求項1記載の方法。
- 前記増殖因子がVEGF−Aである、請求項1記載の方法。
- 前記シグナル伝達タンパク質がPLK1である、請求項1記載の方法。
- 予め選択された、増殖因子又はシグナル伝達タンパク質を含むパネル中の2つ以上の発現腫瘍関連抗原(TuAA)に関して患者の腫瘍組織をアッセイし、当該腫瘍に関する抗原プロファイルを構築する工程、及び
前記プロファイルに基づいて、プロファイル中の2つ以上の発現腫瘍関連抗原を標的とする1つ又は複数の免疫療法薬の投与を含む免疫療法レジメンを選択する工程
を包含する、患者における癌の状態と免疫療法レジメンとを適合させる方法。 - 前記レジメンが能動免疫療法薬及び受動免疫療法薬をともに投与することを含む、請求項30記載の方法。
- 前記増殖因子がVEGF−Aである、請求項30記載の方法。
