Abstract
Description
図1は、一実施形態による、中枢性呼吸器疾患を治療するために呼吸を検出し制御するシステム100を示す。このシステムは、中枢性呼吸器疾患を治療するために呼吸の神経制御を行う埋込可能医療装置120を含む。装置120は、中枢性呼吸器疾患の発生を表す検出信号を受け取る検出回路102を含む。装置は、また、神経刺激信号を生成するように適応された神経刺激器104と、中枢性呼吸器疾患の発生の検出に応じて所望の標的を刺激するために検出回路102と通信し神経刺激器104を制御するコントローラ106を含む。
図1に示したシステム100は、少なくとも1つの電極110を有する少なくとも1つの神経刺激リード108と、生理学的パラメータを監視して中枢性呼吸器疾患の発生を検出するように適応された少なくとも1つのセンサ112を備える。システム100は、また、少なくとも1つのリード108と少なくとも1つのセンサ112に結合された埋込可能医療装置120を有する。一実施形態によれば、装置120は、リード109を介して少なくとも1つのセンサ112に接続される。一実施形態によれば、装置120は、無線接続によって少なくとも1つのセンサ112に接続される。
図3Aは、人間の呼吸器の神経生理機能を示す。図は、延髄304、舌咽神経306、迷走神経308、頚動脈小体310、大動脈体312、肋間筋314、肺外気道316、肺318を示す。呼吸の制御と関係する神経の検出は、いくつかのセンサからの知覚入力を含む。一実施形態において、中枢性CRが検出される。これらの中枢性CRは、第9及び第10脳神経の出口の部位の近くの延髄の中央面を含む。一実施形態において周辺CRが検出される。これらは、頚動脈圧受容器と関連付けられておらず、pO2、pCO2、pHに応答する大動脈体と頚動脈小体を含む。
図4は、一実施形態による、中枢性呼吸器疾患を治療するために呼吸の神経制御を行う埋め込み可能なシステムの概略図である。このシステムは、埋込可能医療装置(IMD)401、IMD401に結合された電気リード420、少なくとも1つの電極425を有する。IMDは、コントローラ回路405、記憶回路410、遠隔測定回路415、神経刺激回路435を含む。コントローラ回路405は、電気的な神経刺激治療を行うために、記憶回路に記憶された命令で動作可能である。治療は、神経刺激回路435によってリード420と電極425を介して加えられる。遠隔測定回路415は、外部プログラム装置430との通信を可能にする。プログラム装置430を使用して、IMD401によって提供されるプログラムされた治療を調整することができ、IMDは、装置データ(バッテリや負荷抵抗など)と治療データ(検出データや刺激データなど)を、例えば無線遠隔測定を使ってプログラム装置に報告することができる。種々の実施形態によれば、IMD401は、後述する図6に示し方法に開示したように、中枢性呼吸器疾患の発生を検出するために1つ又は複数の生理学的パラメータを検出し、検出したパラメータを検出したパラメータの所望の値範囲と比較し、検出したパラメータの1つ又は複数が所望の範囲内にない場合は神経刺激治療を行う。図示したシステムは、また、少なくとも1つのセンサ445に結合されたセンサ回路440を有する。コントローラ回路405は、センサ回路からのセンサデータを処理し、センサデータに応じた治療を行う。
図6は、一実施形態による、中枢性呼吸器疾患を治療するために呼吸を検出し制御する方法の流れ図を示す。この方法600は、605で、中枢性呼吸器疾患の発生を検出するために1つ又は複数の生理学的パラメータを検出する段階を含む。この方法は、また、610で、検出したパラメータを検出したパラメータの所望の値範囲と比較する段階を含む。この方法は、更に、615で、検出したパラメータの1つ又は複数が所望の範囲内にない場合に正常なCR活動とMR活動を模倣するように神経刺激治療を行う段階を含む。
Claims (33)
- 中枢性呼吸器疾患の発生を表す検出信号を受け取る検出回路と、
神経刺激信号を生成するように適応された神経刺激器と、
検出回路と通信し、検出した中枢性呼吸器疾患の発生に応じて所望の神経標的を刺激するように神経刺激器を制御するコントローラと
を有する埋込可能医療装置。 - 神経刺激器は、中枢性呼吸器疾患を治療するために化学的受容器を刺激するように適応された請求項1に記載の埋込可能医療装置。
- 神経刺激器は、中枢性呼吸器疾患を治療するために機械的受容器を刺激するように適応された請求項1に記載の埋込可能医療装置。
- 神経刺激器は、中枢性呼吸器疾患を治療するために迷走神経を刺激するように適応された請求項1に記載の埋込可能医療装置。
- 神経刺激器は、中枢性呼吸器疾患を治療するために舌咽神経を刺激するように適応された請求項1に記載の埋込可能医療装置。
- 神経刺激器は、中枢性呼吸器疾患を治療するために横隔神経を刺激するように適応された請求項1に記載の埋込可能医療装置。
- 神経刺激器は、中枢性呼吸器疾患を治療するために脳内の呼吸中枢性を刺激するように適応された請求項1に記載の埋込可能医療装置。
- 神経刺激器は、中枢性呼吸器疾患を治療するために横隔膜筋肉を刺激するように適応された請求項1に記載の埋込可能医療装置。
- 検出信号は、化学的受容器と機械的受容器の神経活動を表す信号を含む請求項1に記載の埋込可能医療装置。
- 少なくとも1つの電極を備えた少なくとも1本の神経刺激リードと、
中枢性呼吸器疾患の発生を検出するために生理学的パラメータを監視するように適応された少なくとも1つのセンサと、
少なくとも1つのリードと、少なくとも1つのセンサとに結合された埋込可能医療装置であって、
中枢性呼吸器疾患の発生を表す検出信号を受け取る検出回路と、
神経刺激信号を生成するように適応された神経刺激器と、
検出回路と通信し、検出された中枢性呼吸器疾患の発生に応じて所望の神経標的を刺激するために神経刺激器を制御するコントローラとを有する埋込可能医療装置と
を含むシステム。 - 埋込可能医療装置は、心臓リズム管理治療を行うように適応された請求項10に記載のシステム。
- 少なくとも1つのセンサは、化学的受容器の神経活動を監視するように適応された請求項10に記載のシステム。
- 少なくとも1つのセンサは、機械的受容器の神経活動を監視するように適応された請求項10に記載のシステム。
- 少なくとも1つのセンサは、身体の動きを監視するように適応された請求項10に記載のシステム。

を有するシステム。 - 神経刺激治療を行う手段は神経カフ電極を備える請求項25に記載のシステム。
- 神経刺激治療を行う手段は経血管的刺激リードを備える請求項25に記載のシステム。
- 中枢性呼吸器疾患の発生を検出するために1つ又は複数の生理学的パラメータを検出する段階と、
検出したパラメータを検出したパラメータの所望の値範囲と比較する段階と、
検出したパラメータの1つ又は複数が所望の範囲内にない場合に、正常な化学的受容器及び機械的受容器活動を模倣するように神経刺激治療を行う段階とを含む方法。 - フィードバック信号が入手できない場合に代替標的に神経刺激治療を行う段階を更に含む請求項28に記載の方法。
- フィードバック信号が入手できない場合に予めプログラムされた刺激信号を印加する段階を更に含む請求項28に記載の方法。
