Abstract
Description
(i)約915MHz〜約2.45GHzの周波数範囲でマイクロ波エネルギーを提供する、マイクロ波アンテナに連結されるマイクロ波電源;
(ii)高周波(RF)電極に連結されるRF電源;
(iii)光ファイバ又はライトパイプに連結されるコヒーレント光源;
(iv)光ファイバに連結される非コヒーレント光源;
(v)加熱流体を受容するように構成される、閉じているか又は少なくとも部分的に解放されている管腔を有するカテーテルに連結される加熱流体;
(vi)冷却流体を受容するように構成される、閉じているか又は少なくとも部分的に解放されている管腔を有するカテーテルに連結される冷却流体;
(vii)低温流体;
(viii)ac導電性ワイヤに連結される抵抗加熱源;
(ix)超音波放射器に連結される超音波電源、ここで、この超音波電源は、約300kHz〜約3GHzの範囲内の超音波エネルギーを発生する;
(x)及びこれらの組み合わせ。
Claims (24)
- 人体の内臓又は骨の中の腫瘍を処置するための外科用装置であって、
腫瘍組織(T)に貫入するための貫入針アセンブリ(N/N’/N’’/N’’’/N’’’’)と、
前記針アセンブリに接続され、使用中に、前記針アセンブリの周囲の領域(R)内の標的腫瘍組織を切除するように構成されるエネルギー伝達手段(2)と、
使用中に、前記領域内にある前記標的腫瘍組織内に電気内部浸透及び任意選択的な電気分解を引き起こすように構成される少なくとも1つの陰極(1)とを備えることを特徴とする外科用装置。 - 前記エネルギー伝達手段は、前記陰極(1)に接続され、前記領域(R)においてRF切除を引き起こすように構成されるAC源(2)を含み、
前記AC源は、少なくとも5Wの電力、より好ましくは少なくとも10Wの電力、最も好ましくは少なくとも20Wの電力を生成するように構成されていることを特徴とする請求項1に記載の外科用装置。 - 前記患者の体内組織に挿入されるように構成される陽極(C’)をさらに備えることを特徴とする請求項1又は請求項2に記載の外科用装置。
- 前記陽極は針(C’)の形状であることを特徴とする請求項3に記載の外科用装置。
- 前記陰極(1)に負の電位にバイアスをかけるように構成されるDC源(4)をさらに備えることを特徴とする請求項1から請求項4までのいずれか1項に記載の外科用装置。
- 前記負の電位は、前記DC源(4)の正極に対して9V〜12Vの範囲内にあることを特徴とする請求項5に記載の外科用装置。
- 前記陰極(1)が負の電位にバイアスをかけられて電気内部浸透を引き起こす初期の処置段階と、前記エネルギー伝達手段が起動されて前記初期の処置段階において水和した標的腫瘍組織(T)を切除する後続の処置段階とを適用するように構成される切替手段(SW)を備えることを特徴とする請求項5又は請求項6に記載の外科用装置。
- 前記針アセンブリは、カニューレ(C/C1、C2、C3/C’)と、
前記カニューレ内に配置され、前記カニューレの遠位端から拡張可能であり、標的組織(T)に貫入するように形作られる少なくとも1つの前記陰極(1)とを備えることを特徴とする請求項1から請求項8までのいずれか1項に記載の外科用装置。 - 複数の電極(1)が、前記カニューレ(C/C’)内に配置され、前記カニューレの前記遠位端から拡張可能であり、標的腫瘍組織(T)内に貫入するように形作られ、使用中に、前記領域(R)内にある標的腫瘍組織内に電気内部浸透を引き起こすように構成されることを特徴とする請求項8に記載の外科用装置。
- 前記電極(1)は全て、使用中に、同じ極性でバイアスをかけられるように構成され、 前記装置は、前記患者の体で逆の極性の電気的接続を確立するための手段(P/C’)
をさらに備えることを特徴とする請求項9に記載の外科用装置。 - 前記電極(1)のうちの1つ又は複数は、1つの極性でバイアスをかけられるように構成され、
前記電極のうちの他の1つ又は複数は、逆の極性でバイアスをかけられるように構成されることを特徴とする請求項9に記載の外科用装置。 - 前記電極のうちの1つ又は複数の極性を選択的に入れ替えるための手段をさらに備えることを特徴とする請求項9から請求項11までのいずれか1項に記載の外科用装置。
- 前記針アセンブリ(N’)は、導電性液体、又は細胞破壊物質、又はその前駆物質の供給源と連通する管腔を有し、前記導電性液体又は物質を前記標的腫瘍組織(T)内に導入するように構成され、
前記陰極は、前記導電性液体又は物質と電気的に接触し、前記領域内にある標的組織内に電気内部浸透を引き起こすように構成されることを特徴とする請求項1から請求項8までのいずれか1項に記載の外科用装置。 - 前記針装置のさらなる管腔と連通し、前記標的組織(T)から液体、固体又は気体の堆積物を抽出するように構成される抽出手段をさらに備えることを特徴とする請求項13に記載の外科用装置。
- 前記針アセンブリ(N)は、前記領域内で電気分解を引き起こすように構成され、
前記装置は、前記領域(R)内で電気分解によって生成される気体を検出するように構成される超音波撮像手段(US)をさらに備えることを特徴とする請求項1から請求項14までのいずれか1項に記載の外科用装置。 - 前記超音波撮像手段はドップラ超音波撮像装置(US)であることを特徴とする請求項15に記載の外科用装置。
- 前記針アセンブリ(N/N’’’)は、或る体積の腫瘍組織を取り囲むように展開可能であり、使用中に、前記取り囲まれた体積の周辺を選択的に切除するように構成される、拡張可能な電極アレイ(1)を備えることを特徴とする請求項1から請求項16までのいずれか1項に記載の外科用装置。
- 人体の内臓内の標的組織を壊死させるための外科用装置であって、
使用中に標的組織に貫入し、前記標的組織内で電気分解を引き起こすように構成される少なくとも1つの貫入陽極(1)を備えることを特徴とする外科用装置。 - 前記標的組織内に電解質を導入するための手段をさらに備え、
前記電解質は、原位置において薬剤を生成するように電気分解可能であることを特徴とする請求項18に記載の外科用装置。 - 請求項1から請求項19までのいずれか1項に記載の無菌外科用装置。
- 人間又は動物の体内の臓器又は骨の中の腫瘍を処置する方法であって、
標的腫瘍組織(T)内で電気内部浸透及び任意選択的な電気分解を引き起こすことと、
前記標的腫瘍組織を別個に、又は同時に切除することとを含むことを特徴とする方法。 - 前記電気内部浸透は、前記標的腫瘍組織(T)を切除する前に引き起こされることを特徴とする請求項21に記載の方法。
- 前記電気内部浸透は、前記標的腫瘍組織(T)を切除する前に、60秒〜600秒の時間にわたって、好ましくは120秒〜400秒の時間にわたって、最も好ましくは240秒〜360秒の時間にわたって引き起こされることを特徴とする請求項22に記載の方法。
- 前記標的腫瘍組織(T)は、RF切除装置(2、4、I)によって切除されることを特徴とする請求項21から請求項23までのいずれか1項に記載の方法。
