Description
本願は、2004年9月17日に出願された米国仮特許出願第60/610,781号明細書の権益主張出願であり、この米国仮特許出願を参照により引用し、その記載内容を本明細書の一部とする。
本発明は、一般に、患者の体内に植え込み可能であり、欠陥のあるまたは疾患のある血管のための医療器具プラグ(栓)として使用するのに特に適した閉塞性を有する医療器具に関する。これら形式の医療器具は、展開配備時に選択された場所における閉塞または他の治療性を高めるのに適した多孔率特性を有する。
本発明の恩恵を受けることのできる医療器具としては、中空内部を有することを特徴とし、管腔内的に導入され、展開配備時に拡張して患者の体内の問題の場所を塞ぐ医療器具が挙げられる。カテーテルおよび導入器による展開配備を容易にするために押しつぶし状態または形態と拡張状態または形態との間で動く器具が存在する。本明細書による開示は、身体の血管内の疾患場所のための閉塞器具、特に、血管系内への植え込み可能な寸法、および形状の器具ならびに神経血管用途の器具に焦点を当てている。
本発明によれば、担持フレームを備えていて、薄膜メッシュ構造体が担持フレームの少なくとも一部を覆って延びると共にこれに固定された閉塞器具が提供される。薄膜メッシュ構造体は、担持フレームを覆い、または担持フレームの内部を内張りするのがよく、あるいは、担持フレームを薄膜の2つの層相互間に嵌め込むのがよい。担持フレームおよび薄膜メッシュ構造体は各々、管腔内展開配備を容易にする縮めまたは押しつぶし展開配備前形態ならびに体内での拡張または展開配備形態を有する。体内に展開配備されると、閉塞器具は、薄膜メッシュ構造体が血管の疾患部分への血液の流れを実質的に減少させまたは完全に遮断するプラグとして働くよう位置決めされる。例えば、閉塞器具は、薄膜メッシュ構造体が動脈瘤の頸部を覆いまたは施栓するよう展開配備される。
必要に応じて、本明細書において本発明の詳細な実施形態が開示されるが、開示される実施形態は、本発明の単なる例示に過ぎず、本発明は、種々の形態で実施できることは理解されるべきである。したがって、本明細書において開示する特定の細部は、本発明を限定するものと解釈されてはならず、単に特許請求の範囲の理解のための基礎として、また当業者に本発明を実際にかつ適当な仕方で様々に用いるのを教示する代表的な基礎として解されるべきである。
本発明の具体的な実施態様は、次の通りである。
(1)閉塞特性を備えた拡張可能な医療器具において、
規定された長さを有する細長い担持フレームであって、押しつぶし状態から拡張状態に拡張可能である、担持フレームと、
前記細長い担持フレームに固定された薄膜メッシュと、
を有し、
前記薄膜メッシュは、前記薄膜メッシュを貫通して設けられていて、前記担持フレームが前記押しつぶし状態と前記拡張状態との間で動くにつれて開き度が変化する複数個の開口部を有する、
拡張可能な医療器具。
(2)実施態様(1)記載の拡張可能な医療器具において、
前記薄膜メッシュは、形状記憶材料で作られている、拡張可能な医療器具。
(3)実施態様(2)記載の拡張可能な医療器具において、
前記形状記憶材料は、ニチノールを含む、拡張可能な医療器具。
(4)実施態様(1)記載の拡張可能な医療器具において、
前記薄膜メッシュは、第1の層、および、第2の層、を有し、
前記担持フレームは、前記薄膜メッシュの前記第1の層と前記第2の層との間に嵌められている、拡張可能な医療器具。
(5)実施態様(1)記載の拡張可能な医療器具において、
前記担持フレームは、自己拡張型担持フレームを含む、拡張可能な医療器具。
前記担持フレームは、内面および外面を備えた全体として管状のステントを含む、拡張可能な医療器具。
(7)実施態様(6)記載の拡張可能な医療器具において、
前記薄膜メッシュは、前記全体として管状のステントの前記内面の少なくとも一部を内張りしている、拡張可能な医療器具。
(8)実施態様(6)記載の拡張可能な医療器具において、
前記薄膜メッシュは、前記全体として管状のステントの前記外面に沿って少なくとも部分的に延びている、拡張可能な医療器具。
(9)実施態様(6)記載の拡張可能な医療器具において、
前記薄膜メッシュは、前記全体として管状のステントの前記外面のほぼ全体にわたってぐるりと延びている、拡張可能な医療器具。
(10)実施態様(1)記載の拡張可能な医療器具において、
前記複数個の開口部は、前記担持フレームが前記押しつぶし状態から前記拡張状態に動くと、スロットになるよう開口するスリットを含む、拡張可能な医療器具。
前記複数個の開口部は、前記担持フレームが前記押しつぶし状態から前記拡張状態に動くと、スリットになるよう閉じるスロットを含む、拡張可能な医療器具。
(12)実施態様(1)記載の拡張可能な医療器具において、
前記薄膜メッシュの厚さは、約0.1μmを超えるが、約5μm未満である、拡張可能な医療器具。
(13)閉塞特性を備えた拡張可能な医療器具において、
規定された長さおよび表面領域を有する細長い担持フレームであって、押しつぶし状態から拡張状態に変形可能である、担持フレームと、
前記細長い担持フレームに固定された薄膜メッシュと、
を有し、
前記薄膜メッシュは、前記担持フレームの前記表面領域の少なくとも一部に沿って変化する閉塞特性を与える、
拡張状態な医療器具。
(14)実施態様(13)記載の拡張可能な医療器具において、
前記薄膜メッシュは、前記担持フレームに沿って変化する閉塞特性を与えるために前記担持フレームの表面領域の全広がり未満の広がりで存在する、拡張可能な医療器具。
(15)実施態様(13)記載の拡張可能な医療器具において、
前記薄膜メッシュは、第1の領域を有し、前記第1の領域は、前記薄膜メッシュの第2の領域により与えられる閉塞特性よりも高い閉塞特性を有する、拡張可能な医療器具。
高い閉塞特性を備えた前記薄膜メッシュの前記第1の領域は、低い閉塞特性を備えた前記薄膜メッシュの前記第2の領域よりも高いメッシュ密度を有する、拡張可能な医療器具。
(17)実施態様(13)記載の拡張可能な医療器具において、
前記薄膜メッシュは、前記医療器具が押しつぶし状態にあるときにある長さを有し、前記医療器具が拡張状態にあるとき、別のより短い長さを有する、拡張可能な医療器具。
(18)実施態様(13)記載の拡張可能な医療器具において、
前記薄膜メッシュを前記担持フレームに連結する引き伸ばし可能なアーム、
を更に有し、
前記引き伸ばし可能なアームは、前記医療器具を前記押しつぶし状態から前記拡張状態に動かすにつれて長さが伸びる、
拡張可能な医療器具。
(19)実施態様(13)記載の拡張可能な医療器具において、
前記担持フレームは、自己拡張型担持フレームである、拡張可能な医療器具。
(20)実施態様(13)記載の拡張可能な医療器具において、
前記薄膜メッシュは、形状記憶合金である、拡張可能な医療器具。
前記担持フレームは、全体として管状のステントを含む、拡張可能な医療器具。
(22)実施態様(13)記載の拡張可能な医療器具において、
前記薄膜メッシュは、前記担持フレームの一部にのみ沿って延びる、拡張可能な医療器具。

- 閉塞特性を備えた拡張可能な医療器具において、
規定された長さを有する細長い担持フレームであって、押しつぶし状態から拡張状態に拡張可能である、担持フレームと、
前記細長い担持フレームに固定された薄膜メッシュと、
を有し、
前記薄膜メッシュは、前記薄膜メッシュを貫通して設けられていて、前記担持フレームが前記押しつぶし状態と前記拡張状態との間で動くにつれて開き度が変化する複数個の開口部を有する、
拡張可能な医療器具。 - 請求項1記載の拡張可能な医療器具において、
前記薄膜メッシュは、形状記憶材料で作られている、拡張可能な医療器具。 - 請求項2記載の拡張可能な医療器具において、
前記形状記憶材料は、ニチノールを含む、拡張可能な医療器具。 - 請求項1記載の拡張可能な医療器具において、
前記薄膜メッシュは、第1の層、および、第2の層、を有し、
