Abstract
Description
発明の分野
本発明は、一般的には手術用移植片に関し、より詳細には靱帯および腱を治療するための手術用移植片および手法に関する。
靱帯および腱は筋骨格系を支持し、かつ筋骨格系に安定性を与える。一般的に繊維性の結合組織の帯(band)またはシートから成るが、靱帯および腱は損傷した際に痛みを伴い、しばしば衰弱することもある。これらの結合組織の治療は縫合のような手段による修復を含むか、または他の生体物質若しくは合成材料による完全な置換または部分的な置換を包含していてもよい。これらの組織の複雑さおよび機能性、かつ一般的な修復の考察の結果、治療はこれらの組織をそれらの損傷以前の状態に保ち、かつ戻すことが一般的に好ましい。
本発明は、靱帯および腱を修復するあるいは治療するために、靱帯および腱に移植片を導入する。

Claims (30)
- 靱帯若しくは腱上に、または靱帯若しくは腱中に複数の微粒子を含む移植片を供給することを含む靱帯または腱の治療方法。
- 前記靱帯または腱における損傷を確認することをさらに含む請求項1に記載の方法。
- 前記移植片は微粒子の少なくとも一部を含む生物学的な足場(biological scaffold)を形成し、前記生物学的な足場は、前記靱帯または腱における少なくとも一部の結合組織として機能する請求項1に記載の方法。
- 前記移植片は、宿主組織により部分的に置換されるように構成される請求項1に記載の方法。
- 前記移植片がコラーゲンをさらに含む請求項1に記載の方法。
- 前記移植片が、水、生理食塩水、界面活性剤、放射線不透過性色素および嫌色素(chromophobe)のうちの少なくとも1種を含む請求項1の方法。
- 前記微粒子が組織適合性固体を含む請求項1の方法。
- 前記微粒子が実質的に球状であり約15〜約200ミクロンの範囲にある直径を有する請求項1の方法。
- 前記微粒子が、ポリメタクリレート、ポリメチルメタクリレート(PMMA)、硬化ポリマー、完全に重合したポリマー、およびガラスの1種以上を含む請求項1の方法。
- 前記靱帯または腱が、前縦靱帯、後縦靱帯、棘上靱帯、棘下靱帯(intraspinous ligament)、関節包靱帯、前十字靱帯(ACL)、後十字靱帯(PCL)および回旋筋腱板の少なくとも1種を含む請求項1の方法。
- 損傷の位置が、筋腱連結部、腱の領域、および腱画(tendon insertion)の起始部(insertion)のうちの少なくとも1つである請求項1の方法。
- 前記移植片を接合部位に供給することを含む請求項1の方法。
- 組織反応を促進するために、靱帯または腱または隣接する組織上または内部で組織処置を行うことを含む請求項1の方法。
- 損傷を確認することが、先天性、特発性、または後天性脊柱側彎症または脊柱後彎を有する患者における損傷を確認することを含み、かつ
前記移植片を供給することが、異常彎曲を示す患者の部位における靱帯または腱に移植片を供給することを含む
請求項2の方法。 - 前記彎曲の進行を阻止すること、または前記彎曲を部分的に修正することを含む請求項14に記載の方法。
- 前記移植片は嫌色素(chromophobe)を含み、前記方法は、前記靱帯または腱におけるまたはその周囲の反応を促進するために、レーザを用いて前記嫌色素を活性化することをさらに含む請求項1の方法。
- 前記移植片が放射線不透過物質を含み、前記方法は、前記放射性不透過物質を用いて前記移植片を可視化することをさらに含む請求項1の方法。
- 前記放射線不透過物質がバリウムを含む請求項17の方法。
- スコープを通して前記靱帯または腱を観察することをさらに含む請求項1の方法。
- 前記スコープがビデオフルオロスコープを含み、前記移植片の供給を蛍光透視しながら導く請求項19の方法。
- 微粒子を含む移植片と、
少なくとも1種の腱または靱帯を修復するように構成されている1種以上の手術道具と
を含む医療用品一式。 - 前記移植片を供給するように構成された供給デバイスをさらに含む請求項21に記載の医療用品一式。
- 損傷した靱帯または腱を修復するのに用いる、複数の微粒子を含む移植片。
- 前記移植片が、生体適合性媒体中の実質的に球状の微粒子を含む請求項23の移植片。
- 前記生体適合性媒体がコラーゲンを含む請求項24の移植片。
- 腱または靱帯を治療する方法であって、複数の粒子を前記靱帯または腱の少なくとも一部と接触するように配置することを含む方法。
- 前記粒子を前記靱帯または腱の少なくとも一部の中に配置する請求項26の方法。
- 前記粒子を前記靱帯または腱の損傷部分上に配置する請求項26の方法。
- 前記複数の粒子を、コラーゲンを含む溶液中に懸濁する請求項26の方法。
- 前記粒子は実質的に球状であり、前記粒子が約15ミクロン〜約200ミクロンの直径を有する請求項29の方法。
