Abstract

本発明は一般に、抗癌療法の分野に関する。より詳細には、これは、有機ヒ素化合物、ならびに白血病および固形腫瘍などの癌の治療にそれらを用いるための方法を提供する。
白血病の治療法の進歩にもかかわらず、白血病を有するほとんどの成人患者は依然として疾患の進行のために死亡している。無機化合物である三酸化ヒ素は、再発性または不応性の急性前骨髄球性白血病(APL)を有する患者の治療に対して承認されており、他の型の白血病に対する治療法としても評価が進められている。しかし、中国からの予備的データおよび米国での最近の経験は、三酸化ヒ素が他の血液系癌にも役立つ可能性を示唆している。その結果、抗白血病薬としての三酸化ヒ素の活性が多くの型の白血病で現在調査されている。それらの結果は調査されている白血病の型のうちいくつかの奏効率に関しては好ましいように思われるが、三酸化ヒ素の全身毒性は問題となっている(Soignet et al., 1999;Wiemiket al., 1999;Geissler et al., 1999;Rousselot et al., 1999)。

「Cx-yアルキル」という用語は、x個〜y個の炭素を鎖内に含む、直鎖アルキル基および分枝鎖アルキル基を含む置換型または非置換型の飽和炭化水素基のことを指し、これにはトリフルオロメチルおよび2,2,2-トリフルオロエチルなどのハロアルキル基が含まれる。C0アルキルは、基が末端位置にあれば水素を表し、内部にあれば結合を表す。「C2-yアルケニル」および「C2-yアルキニル」という用語は、長さおよび可能な置換に関しては上記のアルキルと類似しているが、それぞれ少なくとも1つの二重結合または三重結合を含む、置換型または非置換型の不飽和脂肪族基のことを指す。
三酸化ヒ素の使用はその毒性のために制約される。これに対してOAははるかに毒性が低く、インビボでの無機ヒ素へのOAへのメチル化は解毒反応であるとみなされているほどである。OAであるモノメチルアルシン酸およびジメチルアルシン酸は、無機ヒ素の主要代謝産物である(Hughes et al., 1998)。三酸化ヒ素を含む無機ヒ素剤は、心血管系、胃腸管、腎臓、皮膚、神経系および血液を含む、多くの臓器系に対してさまざまな影響を及ぼす。無機ヒ素剤は特に肝臓に対する毒性が強く、浸潤、中心壊死および硬変を引き起こす(IARC, 1980:ACGIH, 1991;Beliles et al., 1994;Goyer et al., 1996)。現在では、無機ヒ素化合物がヒトにおける皮膚癌および肺癌の発癌物質であるという十分な証拠がある(Goyer et al., 1996)。
本発明の有機ヒ素剤は、すべての固形腫瘍、および白血病、リンパ腫、多発性骨髄腫、脊髄形成異常症または骨髄増殖性疾患を含むすべての血液系癌を含む、さまざまな癌を治療するために用いることができる。OAをまた、他の形式の治療に対して不応性となった血液系癌を治療するために用いることもできる。
少なくとも1つの有機ヒ素剤または付加的な有効成分を含む薬学的組成物の製剤は、参照として本明細書に組み入れられる「Remington's Pharmaceutical Sciences」、18th Ed. Mack Printing Company, 1990に例示されているように、本開示に鑑みて当業者には公知であると考えられる。さらに、動物(例えば、ヒト)への投与の場合、製剤が、FDAの生物基準局(Office of Biological Standards)で要求されているような、無菌性、発熱性、全般的な安全性および純度に関する基準を満たす必要があることも理解されるであろう。
有機ヒ素剤を別の作用因子または治療方法、好ましくは別の癌治療法と併用しうることは、本発明の1つの局面である。数分から数週間の範囲の間隔をおいて、有機ヒ素剤を他の作用因子による治療の前または後に用いることができる。他の作用因子および発現構築物を対象に対して別個に適用する態様においては、一般に、作用因子および発現構築物が細胞に対して有利な併用効果を依然として発揮しうるように、各々の送達時点の間に有効な期間が過ぎないように徹底されると考えられる。例えば、このような場合には、細胞、組織または生物体に、2つ、3つ、4つまたはそれ以上の治療手段を有機ヒ素剤と実質的に同時に(すなわち、1分間未満のうちに)接触させることが同定される。また別の局面において、1つまたは複数の作用因子を、有機ヒ素剤を投与する前および/または後に、約1分、約5分、約10分、約20分、約30分、約45分、約60分、約2時間、約3時間、約4時間、約5時間、約6時間、約7時間、約8時間、約9時間、約10時間、約11時間、約12時間、約13時間、約14時間、約15時間、約16時間、約17時間、約18時間、約19時間、約20時間、約21時間、約22時間、約23時間、約24時間、約25時間、約26時間、約27時間、約28時間、約29時間、約30時間、約31時間、約32時間、約33時間、約34時間、約35時間、約36時間、約37時間、約38時間、約39時間、約40時間、約41時間、約42時間、約43時間、約44時間、約45時間、約46時間、約47時間〜約48時間またはそれ以上の時間以内に投与してもよい。ある種の別の態様において、作用因子は、有機ヒ素剤を投与する前および/または後に、約1日、約2日、約3日、約4日、約5日、約6日、約7日、約8日、約9日、約10日、約11日、約12日、約13日、約14日、約15日、約16日、約17日、約18日、約19日、約20日〜約21日以内に投与してもよい。しかし、状況によっては、各投与の間に数週間(例えば、1、2、3、4、5、6、7もしくは8週またはそれ以上)の間隔をおき、治療のための期間を大きく延長することが望ましいこともある。

癌治療法はまた、化学物質または放射線に基づく治療法の双方とのさまざまな併用療法も含みうる。併用される化学療法薬には、例えば、シスプラチン(CDDP)、カルボプラチン、プロカルバジン、メクロレタミン、シクロホスファミド、カンプトテシン、イホスファミド、メルファラン、クロランブシル、ブスルファン、ニトロソウレア、ダクチノマイシン、ダウノルピシン、ドキソルビシン、ブレオマイシン、プリコマイシン、マイトマイシン、エトポシド(VP16)、タモキシフェン、ラロキシフェン、エストロゲン受容体結合薬、タキソール、ゲムシタビエン、ナベルビン、ファルネシルプロテイントランスフェラーゼ阻害薬、トランスプラチナ、5-フルオロウラシル、ビンクリスチン、ビンブラスチンおよびメトトレキサート、または前記のものの任意の類似体または誘導体変異体が含まれる。
DNA損傷を引き起こし、幅広く用いられている他の因子には、γ線、X線、および/または腫瘍細胞に対する放射性同位体の直接送達として一般に知られているものが含まれる。マイクロウェーブおよびUV照射といったその他の形態のDNA損傷性因子も想定している。これらの因子はすべて、DNA、DNAの前駆体、DNAの複製および修復、ならびに染色体の構築および維持に対して広範囲の損傷を引き起こす可能性が高い。X線に関する線量の範囲は、1日量50〜200レントゲンを長期間(3〜4週間)から単回線量2000〜6000レントゲンまでの範囲にわたる。放射性同位体に関する線量の範囲は非常に幅広く、同位体の半減期、放出される放射線の強度および種類、ならびに新生物細胞による取り込みに依存する。「接触される」および「曝露される」という用語は、細胞に対して適用される場合、治療用構築物および化学療法薬もしくは放射線療法薬が標的細胞に対して送達される、または標的細胞に直接並列して配置される工程を記載するために本明細書では用いられる。細胞の死滅または静止を実現するためには、いずれの薬剤も細胞を死滅させる、またはその分裂を阻止するために有効な複合量として細胞に送達される。
免疫治療薬は一般に、免疫エフェクター細胞、および癌細胞を標的として破壊する分子の使用に依拠している。免疫エフェクターは、例えば、腫瘍細胞の表面にあるある種のマーカーに対して特異的な抗体であってよい。抗体単独でも治療法のエフェクターとしての役割を果たす場合があり、またはそれが他の細胞を動員して実際にはそれに細胞死滅を行わせる場合もある。また、抗体を薬物または毒素(化学療法、放射性核種、リシンA鎖、コレラ毒素、百日咳毒素など)と結合させ、単にターゲティング薬剤として利用することもできる。または、エフェクターは、腫瘍細胞標的と直接的または間接的に相互作用する表面分子を有するリンパ球であってもよい。種々のエフェクター細胞には細胞傷害性T細胞およびNK細胞が含まれる。
さらにもう1つの態様において、二次的な治療は、治療用ポリヌクレオチドを第1の治療薬の前、後またはそれと同時に投与する二次的な遺伝子治療である。治療薬を、遺伝子産物をコードするベクターとともに送達することには、標的組織に対して複合的な過剰増殖抑制効果があると考えられる。
癌を有する人の約60%は何らかの種類の外科手術を受けると考えられ、これには予防的、診断的または病期判定用、根治的および姑息的な外科手術が含まれる。根治的外科手術は、本発明の治療法、化学療法、放射線療法、ホルモン療法、遺伝子治療、免疫療法および/または代替療法といった他の治療法と併せて用いうる癌治療法である。治癒的外科手術には、癌組織の全体または一部を物理的に除去、摘出および/または破壊する切除術が含まれる。腫瘍切除術とは、腫瘍の少なくとも一部の物理的除去のことを指す。腫瘍切除術のほかに、外科手術による治療法には、レーザー外科手術、冷凍外科手術、電気外科手術および顕微鏡制御外科手術(モース外科手術)が含まれる。さらに、本発明を表在癌、前癌または偶発的な量の正常組織の除去と併用することも想定している。
以下の実施例は、本発明の好ましい態様を示すために含められる。以下の実施例において開示される技術が、本発明の実施において十分に機能することが発明者らによって見いだされた技術を代表し、そのため、その実施のための好ましい様式を構成するとみなしうることが当業者には理解されるはずである。しかし、当業者は、本発明の開示に鑑みて、開示された特定の態様にさまざまな変化を加えることができ、それでもなお本発明の精神および範囲から逸脱することなく同様または類似の結果が得られることも理解するはずである。
S-ジメチルアルシノ-チオコハク酸(MER1)、S-ジメチルアルシノサリチル酸(SAL1)およびS-(ジメチルアルシノ)グルタチオン(SGLU1)の合成
MER-1:メルカプトコハク酸4.5gを、250mLの丸底フラスコ内にある100mLのグリム(1,2-ジメトキシエタン)に加えた。4mLのジメチルクロロアルシン(0.03mol)を滴下させながら添加し、続いて4mLのジエチルアミン(0.04mol)を同じく滴下した。反応混合物を室温で20時間攪拌した。塩酸ジエチルアミンの白色沈殿物が形成され、これを濾過によって分離した。グリム中にあるMER1溶液の容積を減圧下での蒸発によって大きく減少させた。MER1の白色結晶を濾過によって分離し、冷却した蒸留水で洗浄した。続いて無色の結晶性生成物をエタノール-水から再結晶化させ、150℃という一定の融点となるようにした。
インビトロ評価のためのアッセイ
本発明のヒ素剤化合物、組成物および/または製剤に対する癌細胞の応答を判定するために、さまざまなインビトロアッセイを用いた。アッセイした応答のいくつかには、細胞生存度、細胞周期、アポトーシスおよび成熟度が含まれる。本発明者らはまた、本発明のヒ素組成物に対する感受性にとっての癌細胞におけるPML/RARα遺伝子の必要性を評価するためのアッセイもデザインした。以下に提示するものは、これらのアッセイの説明である:
種々のヒト癌細胞を、マイクロタイタープレート上で、指定濃度のMER1、SAL1またはSGLU1の存在下または非存在下で48時間インキュベートし、その後にスルホロダミンB色素を培養物に添加した。スルホロダミンB色素はタンパク質結合色素であり、生細胞を標識する。結果は、非処理対照細胞と比較した場合の処理細胞の増殖率として報告している(負のデータは細胞死滅を表す)。
これらのアッセイのためには、白血病患者および正常ドナーの末梢血試料からの単核細胞をフィコール-ハイパック分画によって分離し、DMEM完全培地中に再懸濁させた。または、場合によっては細胞株の細胞を用いた。種々のヒト細胞株からの悪性細胞(通常は5×104個/mL)または白血病患者および健常ドナーの末梢血からの単核細胞(1×106個/mL)を、αMEMまたはRPMI1640中において、さまざまな濃度のMER1、SAL1またはSGLU1の存在下または非存在下でインキュベートした。それぞれの実験条件を3つずつ行った。指定日数(通常3日間)にわたってMER1、SAL1またはSGLU1に曝露させた後に、ウェルに対する色素(MTTまたはトリパンブルーのいずれか)の添加によって細胞生存度を評価した。MTT色素は、ウェル内の生細胞の存在に依存して色調を変化させる。MTT処理下での細胞の生存度は、対照細胞の増殖に対する百分率として評価した。トリパンブルー色素は死細胞内に浸透し、生細胞を顕微鏡下で算定して生存率を評価することができる。
クローン原性またはコロニー形成は、(正常ドナーまたは白血病患者から)末梢血単核細胞を入手し、それを組換えサイトカインを含む半流動培地中に再懸濁させ、4×104個/0.1mLの密度で、96ウェルマイクロタイタープレートに0.1mL/ウェルを4つずつプレーティングすることによって分析した。5%CO2加湿雰囲気下、37℃でのほぼ10日間のインキュベーション後に、50個を上回る細胞から構成される細胞凝集物を1つのコロニーとして算定した。増殖阻害は、対照(薬剤なしの)試料におけるコロニー増殖と比較したコロニー増殖の百分率として評価した。
アポトーシスの分析には、アポトーシス経路における種々のイベントをアッセイすることにより、3種類の方法を用いた。本発明のヒ素誘導体によって誘導されたアポトーシス細胞の百分率をフローサイトメーターを用いて評価した。アポトーシスに関して細胞を染色する種々の方法を、アポトーシスカスケードの種々の局面を評価するために利用した。
アネキシンVは細胞膜の外層表面にホスファチジルセリンを発現する細胞と結合し、一方、ヨウ化プロピジウムは細胞膜に損傷を来した細胞の細胞DNAを染色する。これにより、生細胞(どちらの蛍光色素でも染色されない)をアポトーシス細胞(アネキシンVのみによって染色される)および壊死細胞(アネキシンおよびPIの両方によって染色される)と識別することが可能になる。
ミトコンドリア膜の電位の変化を評価するため、ヒ素誘導体による指定時間にわたる処理の後に、1μMを下回る濃度のMitoTrackerプローブ中で細胞をインキュベートした。MitoTrackerプローブは原形質膜を介して受動的に拡散し、活性ミトコンドリア中に蓄積する。細胞は、MitoTracker Red CMXRos(Molecular Probes)およびMitoTracker Green FM(Molecular Probes)という2種類の着色剤で染色した。細胞をPBSで洗浄し、MitoTracker色素で染色して、暗下にて37℃で1時間インキュベートした。CMXRosはミトコンドリア膜電位によって駆動されてミトコンドリア内に組み入れられ、チオール残基と反応してチオールエステル共有結合を形成する。MitoTracker Green FM色素はミトコンドリア膜電位とは関係なくミトコンドリア内に選択的に蓄積するため、ミトコンドリア量を判定するための有用なツールとなる。
蛍光性基質PhiPhiLux G1D1(Oncoimmunin)を、フローサイトメトリーによってカスパーゼ活性をモニターするために用いた。PhiPhiLux G1D1は、生細胞におけるカスパーゼ3活性およびカスパーゼ3様活性の検出および測定のための基質である。本発明のヒ素誘導体で指定期間にわたって処理した後に、細胞をPBSで洗浄し、5μLの基質溶液中に再懸濁させ、暗下にて37℃で1時間インキュベートした。インキュベーション後に細胞を洗浄し、分析時に壊死細胞を除外するために、フローサイトメトリー分析の数分前にPIを添加した。
細胞周期は以下の通りに分析した:本発明の種々のヒ素化合物との72時間のインキュベーション後に、細胞(1×106個)をPBSで2回洗浄した。細胞ペレットを、低張溶液(RNアーゼ溶液、Triton X-100、クエン酸ナトリウム、PEG)およびPI(25μg/mL)を含む染色溶液中に再懸濁させた。細胞を暗下にて室温で15分間インキュベートし、その後にそれらをフローサイトメーターにより、CellQuestプログラム(Becton-Dickinson)を用いて分析した。
ヒト急性前リンパ球性白血病細胞株NB4を、白血病細胞の成熟に対する本発明のヒ素剤の効果について試験するために用いた。フィコエリトリン結合抗CD11bモノクローナル抗体(Becton-Dickinson)を、成熟骨髄球のマーカーとして用いた。薬剤との72時間のインキュベーション後に、細胞をPBSで洗浄した。続いて、密度1×106個/mLの細胞を、希釈度1:10のモノクローナル抗体とともに暗下にて室温で15分間インキュベートした。インキュベーション後に細胞をPBSで洗浄し、ペレットを500μLのPBS中に再懸濁させた。非特異的な結合を除外するために、適切なアイソタイプ対照を同じ様式で調製した。細胞をフローサイトメーターを用いて選別し、CellQuest Document Analysisを用いて分析した。
三酸化ヒ素は急性前リンパ球性白血病の治療を目的として承認されており、これはAPL細胞を死滅させるが、その大部分はPML/Rarα遺伝子およびタンパク質の発現に起因する。白血病細胞におけるPML/RARα融合タンパク質の存在が、SGLUおよびMER1に対して観察されている白血病細胞の感受性に寄与するか否かを確かめるために以下の系を用いた。三酸化ヒ素に対する耐性があることが知られているU937細胞に対してPML/RARα遺伝子をトランスフェクトした。トランスフェクト細胞(U937/PR9)はDr. Michael Andreeff(M.D. Anderson Cancer Center)より寄贈いただいた。PML/RARα遺伝子は亜鉛の存在下で機能的になる。U937/PR9細胞株におけるPML/RARα遺伝子のZn2+による誘導性発現が、Grignani et al.(Cell, (1993)74: 423-431)に記載されている。PML/RARα発現を確かめるために、細胞を0.1mM ZnSO4で3時間処理し、その後にヒ素化合物を72時間にわたり添加した。PML/RARα発現は、典型的には、細胞に対する亜鉛の添加から約3時間後に確かめられ、48時間にわたって安定的である。
MER1、SAL1およびSGLU1の抗癌活性のインビトロ評価
MER1の抗白血病活性を、6種類のヒト白血病細胞株:AML2、AML3およびHL60(AML由来の細胞株)、NB4(APL由来の細胞株)、K562(CML-BP由来の細胞株)およびKBM7(AML由来の細胞株)に対する3日間のMTTアッセイ/トリパンブルー排除法によって評価した。MER1はNB4細胞に対して最も効果的であり、IC50(非処理対照細胞と比較して、細胞の50%の生存を引き起こす濃度)は1μMであった(図1)。MTTアッセイによるAML2細胞およびKBM7細胞の分析ならびにトリパンブルーアッセイによるAML2細胞(図2参照)、AML3細胞、K562細胞およびHL60細胞の分析を含め、他の細胞株のMER1処理で認められたIC50値は1.5〜4μMの範囲であった。この活性は、これらの細胞株に対する三酸化ヒ素の活性と同程度であった(三酸化ヒ素活性の例は図1および図2に示されている)。MER1は、National Institute Of Health(NIH)により、スルホロダミンBアッセイを用いて、インビトロで60種類という一群の腫瘍細胞株に対する抗癌活性についても試験されている(図3)。この化合物はさまざまな腫瘍細胞株に対して低濃度で活性があるという証拠を示し、試験した白血病細胞に対して特にそうであった。1μMというMER1の濃度で、試験した6種類の白血病細胞株のすべてで増殖が有意に遅延した(増殖が20%未満に;図3、最初の図面)。
悪性および正常血液細胞に対するMER1およびSGLU1の毒性の判定
MER1を、白血病患者5例(3例はAML、1例はCML-BPおよび1例はALL)からの血液単核細胞(芽球が80%超)に対して試験した(図9〜13)。短期的な細胞培養物では、MER1は三酸化ヒ素と同程度に有効であった(図9、10および12)。さらに、健常ドナー4例からの試料における、正常末梢血単核細胞に対するMER1の毒性も評価した。MTTアッセイによる短期的な細胞懸濁培養物では、白血病患者からの悪性細胞よりも正常細胞に対するMER1の毒性の方が低かった(図14)。最も重要なことに、長期的なクローン原性アッセイでは、MER1は三酸化ヒ素よりも正常細胞に対する毒性が低かった(図15)。
MER1の製剤化および安定性
MER1はリン酸緩衝水中に溶解した場合に少なくとも2カ月にわたって安定であることを示すデータが、この期間中に行ったインビトロ実験で溶液が同じレベルの細胞傷害活性を保ったことから得られた(図19)。さらに、MER1およびSGLU1の詳細な薬学的評価も行った。
MER-1は、臨床的状況での投与に対して許容される十分な溶解性および安定性を有することが見いだされた(以下のデータを参照)。これはまた、動物試験に、あるいは前期第I相試験に用いるために溶液を即時に調合しうる程度には十分に安定である。しかし、より大規模な臨床試験に用いるための液体製剤のより大規模なバッチを製造するため、および長期的貯蔵が必要とされる市場での流通のためには、溶液の安定性は十分ではない。これらの用途のためには、使用時に再構成される凍結乾燥製剤が想定される。このような凍結乾燥組成物の調製物は当技術分野で周知である。
MER-1の水への溶解度は約15mg/mLである。0.1N水酸化ナトリウムを用いてpHを6に調製することにより、最大で約150mg/mLというより高いMER-1濃度を実現することができる。エタノールに対するMER-1の溶解度は100mg/mLを上回る。
MER-1の水溶液の自然なままのpH値は以下の通りである:

0.9%塩化ナトリウム注射液において10mg/mLの濃度で、さまざまなpH値の影響を評価した。pHが2.3(中性pH)の試料ならびに水酸化ナトリウムでpH 5、7.1および8.5に調整した試料を、冷蔵下で3カ月間にわたって評価した。pH 5の試料は相対的に良好な安定性を示し、3カ月後に初期濃度の約89%を保っていた。pH 7.1および8.5の溶液は、14日間後にはそれぞれ約92%および96%を保ったが、それ以降は90%未満に低下した。pH 2.3の試料は7日間は安定していたが、それ以降は沈殿物を生じた。表2を参照のこと。
SGLU-1は、臨床的状況での投与に対して許容される十分な溶解性および安定性を有することが見いだされた。これはまた、動物試験に、あるいは前期第I相試験に用いるために溶液を即時に調合しうる程度には十分に安定である。しかし、より大規模な臨床試験に用いるための液体製剤のより大規模なバッチを製造するため、および長期的貯蔵が必要とされる市場での流通のためには、溶液の安定性が十分ではない。これらの用途のためには、使用時に再構成される凍結乾燥製剤が想定される。
SGLU-1の水への溶解度は約60mg/mLであった。0.1N水酸化ナトリウムを用いて溶液のpHを高めることにより、より高いSGLU-1濃度が得られた。しかし、この薬剤はアルカリ環境では不安定であるように思われた。SGLU-1はエタノールには溶解しない。
SGLU-1の水溶液の自然なままのpH値は以下の通りである:

0.9%塩化ナトリウム注射液において2.5mg/mLの濃度でさまざまなpH値の影響を評価した。pHが3(中性pH)の試料ならびに水酸化ナトリウムでpH 5および7に調整した試料を、冷蔵下で30日間にわたって評価した。pH 5の試料は幾分良好な安定性を示し、30日後に初期濃度の約90%を保っていた。pH 3および7の溶液はそれぞれ約84%および82%を保っていた。表4を参照のこと。
MER1、SAL1およびSGLU1に関する機序
アポトーシスの誘導、細胞周期に対する影響、成熟の誘導および異常なPML/Rarα融合タンパク質の分解はいずれも、三酸化ヒ素の作用機序であることが示されている。本発明者らは、MER1がHL60ヒト白血病細胞におけるアポトーシスを誘導する能力について検討した(アッセイ期間1〜3日間)。アポトーシスの誘導は、生存細胞の百分率に関して類似した低下を示した(図20、21および22)。MER1およびSGLUの両方を用いた別の試験により、これらの化合物によるアポトーシスの誘導(アネキシンV染色)は、ミトコンドリア膜電位の変化(CMXRos染色)およびカスパーゼの活性化(PhiPhiLux染色)を伴うことが確かめられた。図23A、23B、23C、23D、23Eおよび23Fを参照のこと。
MER1、SAL1およびSGLU1の治療能力のインビボ評価
ヒト白血病の動物モデルは、ヒト白血病を有する重症複合免疫不全(SCID)マウスによって代表させた。このモデルは、動物におけるヒト白血病の増殖が患者で認められるものに類似した様式である点でユニークである。これにより、新たな薬剤のインビボでの有効性をさまざまな投薬レベルおよびスケジュールで迅速に試験する可能性が得られた。さらに、動物の生存度をモニターしうるだけでなく、疾患の播種パターンに対する治療の効果もモニターすることが可能である。SCIDマウスの治療は、典型的にはヒト白血病細胞の接種から2日後に開始した。1種類のヒト白血病細胞株を注入したSCIDマウスにおける初期インビボ実験により、他のマウスモデルならびに初期ヒト試験のためのMER1、SAL1またはSGLU1の投与量およびスケジュールが決定された。
最初のインビボ実験にはSCIDマウスの4つの群を用いた。1群当たり5匹のマウスに対してさまざまな種類のヒト白血病細胞を腹腔内に接種した:HL60(AML)、KBM5(CML-BP)、KBM7-急性骨髄性白血病およびZ119(ALL)。HL60細胞およびKBM5細胞は優れた生着を示した:HL60群ではすべてのマウスが接種後第31日から第36日までの範囲で死亡し、一方、KBM5群のマウスは第34日から第36日までの範囲で死亡した。生着はヒトHLA-DQαのDNA配列に対するPCRを行うことによって実証した(検査結果はすべてのマウスからのすべての組織で陽性であった)。第100日の時点で、KBM7群ではマウス5匹中4匹、Z119群ではマウス5匹中5匹が依然として生存していた。その日にすべてのマウスを屠殺し、組織をHLA-DQαに対するPCRによって分析した。検査結果は陰性であり、白血病細胞の生着が存在しないことが示された。治療試験のためには同じ種類の代替的な細胞株が必要である。
毒性試験のためには、免疫能のあるSwiss Websterマウスを用いた。三酸化ヒ素に関するLD50濃度は10mg/kgであることが確認された。
SGLU1の毒性を調べるために、Swiss-Websterマウスに対して2種類の試験を行った。第1の試験では、SGLU1を178mg/kg;285mg/kg;および357mg/kgの用量でIP経路を介して投与した。毒性はマウスの死亡率によって測定した。マウスは用量178mg/kgおよび285mg/kgのSGLU1には十分な忍容性があることが見いだされた。この試験のデータは表6にまとめられている。
MER-1の毒性を調べるために、Swiss-Websterマウスに対して2種類の試験を行った。第1の試験では、MER-1を用量71mg/kg;107mg/kg;および143mg/kgでIP経路を介して投与した。毒性はマウスの死亡率によって測定した。マウスは用量71mg/kgおよび107mg/kgのMER-1に対して十分な忍容性があり、死亡は皆無であることが見いだされた。この試験のデータは表8にまとめられている。
以上の実験と同様に、SAL1の簡易毒性試験により、SAL1に関するLD50濃度が50mg/kgであることが確かめられた。
MER1、SAL1およびSGLU1の薬物動態
MER1、SAL1およびSGLU1の薬物動態的な配置は、尾静脈を介した静脈内投与後のマウスで評価する。以前に決定されたMTD付近の用量でまず試験を行う。血液試料を、薬剤投与後のさまざまなサンプリング時点(0(投与前)、5、10、15、30、45、60分および2、3、4、6、8、12、16、24、48、72時間)で採取する(1つの時点につきマウス8匹)。血液採取のためには、マウスをCO2吸入によって麻酔した後に断頭し、血液を全採血によって採取する。血液試料をヘパリンを含む試験管に集め、遠心処理して血漿を分離し、分析時まで-80℃で保存する。試験を繰り返し、Amicon Centrifreeマイクロパーティションユニットによる血漿の2000g×20分間の遠心処理によって血漿限外濾液を採取する。限外濾液は分析時まで-80℃で保存する。選択した群では、さまざまな組織を死後に採取し、組織への配置の分析のために凍結する。血漿試料および限外濾液試料中のヒ素含有量を黒鉛炉(無炎)原子吸光分析法によって測定する。薬物動態パラメーターを得るために、測定された薬剤濃度をコンパートメント的に分析する。
毒物学的試験
多回投与毒物学的試験
MER-1およびSGLU-1のマウス群における反復投与に伴う用量規定毒性を明らかにするためにさらにほかの試験を行った。
HPLC分析法の開発およびバリデーション
有機ヒ素剤の使用に関する方法の開発およびバリデーションにはHPLCが用いられる。HPLC法は以下を含む:標準曲線および直線性、再現性(最低で10回の注射)、感度(最小定量可能濃度;最小検出可能濃度)、精度(独立に調製した0.025mg/mL、0.1mg/mL、1mg/mLの3種の溶液を用いるなど)、熱、塩基性溶液、酸性溶液およびH2O2による意図的な分解、ならびに無傷薬剤、バルク不純物および出発材料および分解産物に関するピークの定義。バルク原薬剤を、比較標準ロットにおいて、HPLCによる純度分析、乾燥による損失、旋光性、融点および外観について分析する。
剤形の開発
有機ヒ素剤の剤形は、Pharmacology Laboratoryによって開発された製剤溶媒システムによって開発される。これは、水性媒質中および濾過に対する安定性を判定すること、さらなる開発のための標的濃度を選択すること、浸透圧およびpHを検査すること、ならびに必要に応じてパッケージおよび閉鎖形状を選択すること、熱安定性を判定すること(オートクレーブ処理)、外観および微粒子量を検査すること、ならびに標的pH値および許容される標的濃度の範囲を決定することを含む。
臨床試験
本実施例は、ヒ素化合物MER1、SGLUおよびSAL-1、ならびに本発明の組成物またはその医薬製剤を用いたヒト治療プロトコールの開発にかかわる。これらの組成物は、白血病ならびに他の形態の固形癌および腫瘍を含む、さまざまな癌の臨床的治療において有用である。
S-ジメチルアルシノグルタチオンの代替的な合成
以下の手順は、S-ジメチルアルシノグルタチオンの調製の様式を記載している。用いる量を増やしても減らしてもよく、それぞれの比が維持されるならば同じく成功が得られる。
ジメチルアルシン酸、(CH3)2As(O)OHは、Luxembourg Chemical Co., Tel Aviv, Israelから供給を受けた。この製品にはその純度が表記されており、純度99.7%として供給された。ジメチルアルシン酸を水-塩酸中に添加してpH 3とした。二酸化硫黄ガスの流れをこの溶液中に約1時間通過させた。ジメチルクロロアルシンは比重の高い無色の油として分離した。2つの液層、水/(CH3)2AsClを分離用漏斗を用いて分離した。クロロジメチルアルシンをジエチルエーテル中に抽出し、このエーテル溶液を無水硫酸ナトリウム上で乾燥させた。乾燥させた溶液を蒸留フラスコに移し、これを徐々に加熱してエーテルを蒸発させた。残った液体であるジメチルクロロアルシンを蒸留によって精製した。106〜109℃で沸騰する画分を採取した。この生成物である無色の油は、1.65ppmで単純な1H NMR共鳴を呈した。
500mLフラスコ内に、Aldrich Chemical Co.から純度98%として受け取ったグルタチオン7gを用い、250mLの1,2-ジメトキシエタン中に溶解させた。この溶液に3.3gのジメチルクロロアルシンを添加した。これに続いて3.5gのピリジンを添加した(NaOHペレット上乾燥させた後に再蒸留した)。この溶液を1時間環流させ、その後に室温で3時間攪拌した。
S-ジメチルアルシノグルタチオン(GLU)のピリジン塩酸を用いない合成
ジメチルアルシノグルタチオンは、Chen(Chen, G. C. et al., Carbohydrate Res.(1976)50: 53-62)を改良したものを用いて作製され、その内容は全体にわたった参照として本明細書に組み入れられる。手短に述べると、ジチオールビス(ジメチルアルシノグルタミン)を窒素中でジクロロメタン中に溶解させる。テトラメチルジアルシンを溶液に滴下させながら添加し、反応物を窒素中にて室温で一晩攪拌し、続いて空気に1時間曝露させる。続いて混合物を蒸発させて乾燥させ、残留物を水で洗浄し、乾燥させて粗固体を得た上で、それをメタノールから再結晶化させてS-ジメチルアルシノグルタチオンを得る。
塩酸ピリジンを用いないS-ジメチルアルシノグルタチオン(GLU)の第3の合成
S-ジメチルアルシノグルタチオンは、Cullen et al.(J. Inorg. Biochem.(1984)21: 179-194)の手順を用いて作製され、その内容は全体にわたって参照として本明細書に組み入れられる。手短に述べると、ジメチルアルシン酸およびグルタチオンを窒素雰囲気下で水中に溶解させて攪拌する。その結果得られた溶液を12時間攪拌し、続いて加熱せずに減圧下で蒸発させて乾燥させて固体を得て、それを冷メタノールで抽出する。続いてこのメタノール溶液を減圧下で蒸発させて乾燥させ、その結果生じた固体をメタノール/水から再結晶化させて収集し、乾燥させてS-ジメチルアルシノグルタチオンを得る。
GMZ27の抗癌活性のインビトロ評価
を有する有機アルシンであるGMZ27を、種々のヒト急性骨髄性白血病(AML)細胞株に対して72時間MTSアッセイで試験したところ、IC50は0.56〜0.86μMであることが見いだされた。この活性は、これらの細胞株に対する三酸化ヒ素の活性よりも高かった(図27A)。続いて、GMZ27の抗白血病活性を、細胞を半流動培地中で増殖させる長期(7日間)コロニー形成アッセイで評価した。GMZ27は、ヒト白血病細胞株および急性または慢性白血病の患者から入手した白血病細胞のいずれに対しても、三酸化ヒ素よりも有意に高い活性を有していた(図27B)。
N-(2-S-ジメチルアルシノチオプロピオニル)グリシンの調製
N-(2-メルカプトプロピオニル)グリシン(0.02mol、3.264g)を1,2-メトキシエタン(50mL)中に加え、ジメチルクロロアルシン(0.025mol、3.52g)を滴加した。反応混合物を室温で4時間攪拌した。続いてトリエチルアミン塩酸塩の白色沈殿物を濾過によって分離し、溶液の容積を減圧下での蒸発によって減少させた。その結果得られた残留物をカラムクロマトグラフィーによって精製し、所望の生成物(3.5g)を得た。
2-(S-ジメチルアルシノ)チオニコチン酸の調製
2-メルカプトニコチン酸(0.02mol、3g)をジクロロメタン(50mL)に加え、ジメチルクロロアルシン(0.025mol、3.52g)を滴加した。反応物を環流させながら4時間攪拌した。続いてジクロロメタンを蒸発によって除去し、残留物をジエチルエーテル(50mL)中に溶解させ、水で洗浄した(3回)。この溶液をNa2SO4上で乾燥させて濾過して、減圧下での濃縮後に所望の生成物を淡黄色の固体として得た。
L-(+)-2-アミノ-3-(ジメチルアルシノ)チオ-3-メチルブタン酸
L-(+)-2-アミノ-3-メルカプト-3-メチルブタン酸(0.01mol、1.55g)をジクロロメタン(50mL)に加え、ジメチルクロロアルシン(0.015mol、2.1g)入りのジクロロメタン(5mL)を滴加し、その後にトリエチルアミン(1.6g)を滴加した。混合物を4時間攪拌すると、所望の生成物が反応混合物の濾過後に浮遊性の白色結晶性固体として得られた。この結晶性固体をジクロロメタン、酢酸エチルおよびアセトンで逐次洗浄して、所望の生成物(1.6g;mp 107〜109℃)を得た。
当業者は、定型的な範囲の実験により、本明細書に記載された化合物およびその使用方法に対する多くの等価物を認識する、または確認しうると考えられる。このような等価物は、本発明の範囲内に含まれ、以下の特許請求の範囲によってカバーされるとみなされる。当業者はまた、本明細書に記載された態様のすべての組み合わせが本発明の範囲に含まれることを認識するであろう。
