Description
本願は2004年3月11日出願の「モジュール式血管内移植片(Modular Endovascular Graft)」という名称の米国特許仮出願連続番号第60/552,132号の優先権を主張するものであるが、該仮出願の完全な開示内容は、ここに援用することにより本明細書の一部をなすものとする。
Claims (20)
- モジュール式血管内移植片であって、
第1の壁部材によって境界を区切られた第1の液流管腔と、第1の壁部材の上に配置されている第1の取付け要素と、第1の液流管腔を取り巻いているとともに、膨張状態になると、第1の壁部材から放射方向に張り出す第1の膨張可能な折返し帯とを有する第1の移植片本体部と、
第2の壁部材によって境界を区切られた第2の液流管腔と、第2の壁部材上に配置された第2の取付け要素とを有する第2の移植片本体部とを備え、前記第2の取付け要素が前記第1の取付け要素に固着されると、前記第1の液流管腔は前記第2の液流管腔に封鎖固定されるように構成される、
ことを特徴とする、モジュール式血管内移植片。 - 前記第1の移植片本体部は、その上を覆って分布させられて膨張可能な折返し帯と流体連絡状態になる、膨張可能なチャネルのネットワークを更に備えており、膨張可能なチャネルのネットワークが膨張状態になると、構造剛性を供与するとともに、第1の移植片本体部を支持するように構成したことを特徴とする、請求項1に記載のモジュール式血管内移植片。
- 前記第1の移植片本体部の端部に固着されたステントを更に備え、前記第1の取付け要素はステントの反対側で第1の移植片本体部の端部に配置されることを特徴とする、請求項1に記載のモジュール式血管内移植片。
- 前記第1の移植片本体部の前記壁部材と前記第2の移植片本体部の前記壁部材の上に配置されたコネクタ部材を更に備えており、前記ステントはコネクタ部材に固着されることを特徴とする、請求項3に記載のモジュール式血管内移植片。
- 前記第1の取付け要素は少なくとも一部が前記第2の取付け要素に固着されて、軸線方向の長さを有する軸線方向の重畳部を形成し、軸線方向の重畳部の軸線方向の長さは、前記両取付け要素が互いに固着された際の前記第1の移植片本体部と前記第2の移植片本体部の相対的な軸線方向の位置に基づいて変動するように構成したことを特徴とする、請求項1に記載のモジュール式血管内移植片。
- 前記第1の取付け要素には、複数の可撓性フックが互いに隣接して前記第1の壁部材の実面積を覆って設けられており、前記第2の取付け要素には、複数の可撓性ループが互いに隣接して前記第2の壁部材の実面積を覆って設けられており、可撓性フックは、第1の取付け要素が第2の取付け要素に押圧されると、可撓性ループと機械的に係合するように構成されることを特徴とする、請求項1に記載のモジュール式血管内移植片。
- 前記第1の取付け要素には、拡大ヘッド部を有する複数のボタンが互いに離隔されて前記第1の壁部材の表面に設けられており、前記第2の取付け要素は、複数の開口部を有する拡張可能なメッシュ構造から構成されており、メッシュ構造が周方向に抑制された状態になっている間、開口部にボタンの拡大ヘッド部が入ってこられるようにするとともに、メッシュ構造が周方向に拡張した状態になっている間、ボタンの拡大ヘッド部を捕獲することができるように構成されることを特徴とする、請求項1に記載のモジュール式血管内移植片。
- 前記第1の取付け要素には、前記第1の壁部材の表面から放射方向に張り出した複数のピンが設けられており、前記第2の取付け要素は、複数の開口部を有する拡張可能なメッシュ構造から構成されており、前記第1の取付け要素が第2の取付け要素に押圧されると、開口部にピンが入るのを可能にするように構成されることを特徴とする、請求項1に記載のモジュール式血管内移植片。
- 前記膨張可能な折返し帯は硬化可能材を保有しており、前記第2の取付け要素は、棘部材を設けた拡張可能な部材から構成されており、拡張可能な部材は外方向に張り出して膨張可能な折返し帯と硬化可能材に入り込むように構成されることを特徴とする、請求項1に記載のモジュール式血管内移植片。
- 前記第1の取付け要素には、前記第1の移植片本体部に固着された拡張可能な円筒状部材の外面から放射方向に張り出した複数の突起部が設けられており、前記第2の取付け要素は、複数の開口部を設けた拡張可能なメッシュ構造から構成されており、拡張可能な円筒状部材が拡張すると、開口部に突起部が入ってこられる構成になっており、少なくとも1個の突起部が第2の取付け要素の拡張可能なメッシュ構造の開口部に押し入るように構成されることを特徴とする、請求項1に記載のモジュール式血管内移植片。
- モジュール式血管内移植片であって、
第1の壁部材によって境界を区切られた第1の液流管腔と、第1の壁部材の上に配置されている第1の膨張可能部材が設けられた第1の取付け要素とを有している第1の移植片本体部と、
第2の壁部材によって境界を区切られた第2の液流管腔と、第2の壁部材上に配置された第2の取付け要素とを有している第2の移植片本体部とを備え、第2の取付け要素は、第1の膨張可能部材が膨張状態になると、第1の膨張可能部材と嵌合して、第1の移植片本体部と第2の移植片本体部が軸線方向に分離するのを阻止するように構成される、
ことを特徴とするモジュール式血管内移植片。 - 前記第1の膨張可能部材には、膨張状態になると、前記第1の移植片本体部の前記第1の壁部材の内面に接する、周囲寸法を減じた第1のショルダー部が設けられており、
前記第2の取付け要素には、第1のショルダー部と機械的に嵌合して前記第1の移植片本体部と前記第2の移植片本体部が軸線方向に分離するのを阻止するように構成され、かつ周囲寸法を減じた第2のショルダー部が設けられる、
ことを特徴とする、請求項11に記載のモジュール式血管内移植片。 - 前記第1の移植片本体部は、その上を覆って分布させられた膨張可能なチャネルのネットワークを更に備えており、膨張可能なチャネルのネットワークが膨張状態になると、構造剛性を供与するとともに、第1の移植片本体部を支持するように構成したことを特徴とする、請求項11に記載のモジュール式血管内移植片。
- 前記第1の移植片本体部には、複数の第1の膨張可能部材が第1の移植片本体部の長手方向軸線に沿って互いに軸線方向に離隔されて設けられており、前記第2の移植片本体部の前記第2の取付け要素は、第1の膨張可能部材のいずれかが膨張状態になると、その第1の膨張可能部材と嵌合して、配備時に、互いに接合された両移植片本体部の軸線方向の長さを調節することができるように構成されることを特徴とする、請求項11に記載のモジュール式血管内移植片。
- 前記第2の移植片本体部には、複数の第2の取付け要素が第2の移植片本体部の長手方向軸線に沿って互いに軸線方向に離隔されて設けられており、前記第1の移植片本体部の前記第1の膨張可能部材は、第1の膨張可能部材のいずれかが膨張状態になると、その第1の膨張可能部材と嵌合して、配備時に、互いに接合された両移植片本体部の軸線方向の長さを調節することができるように構成されることを特徴とする、請求項11に記載のモジュール式血管内移植片。
- 前記膨張可能部材は硬化可能材を保有しており、前記第2の取付け要素は、棘部材を設けた拡張可能な部材から構成されており、拡張可能な部材は外方向に張り出して前記膨張可能部材と硬化可能材に入り込むように構成されることを特徴とする、請求項11に記載のモジュール式血管内移植片。
- 前記第1の移植片本体部は、前記第1の壁部材に設けられて前記第1の膨張可能部材に軸線方向に隣接する弾性部材を更に備えており、前記第2の取付け要素との嵌合を向上させるように構成されることを特徴とする、請求項11に記載のモジュール式血管内移植片。
- 前記第2の取付け要素には、前記第1の膨張可能部材を受容して嵌合するような構成の窪みポケットが設けられることを特徴とする、請求項11に記載のモジュール式血管内移植片。
- 前記第2の移植片本体部は勾配を設けた部分を更に備えており、前記第2の液流管腔は周囲寸法が増した部分まで勾配が設けられて、前記第1の液流管腔の内面と嵌合するように構成されることを特徴とする、請求項11に記載のモジュール式血管内移植片。
- 患者の体液管を治療する方法であって、
第1の液流管腔および第1の膨張可能部材とを有している第1の移植片本体部と第2の液流管腔を有している第2の移植片本体部とをモジュール式血管内移植片に設ける段階を含んでおり、第1の膨張可能部材には、第1の膨張可能部材が膨張状態になると、第1の移植片本体部の内面に接する、周囲寸法を減じた第1のショルダー部が設けられており、第2の取付け要素の周囲寸法が減じられた第2のショルダー部によって第2の移植片本体部が第1の移植片本体部に固着されており、第2のショルダー部は第1のショルダー部と機械的に嵌合して、第1の移植片本体部と第2の移植片本体部が軸線方向に分離するのを阻止しており、前記方法は、さらに、
患者の体液管の所望部位の内側に第1の移植片本体部を配備する段階と、
第1の移植片本体部に隣接して第2の移植片本体部を配備する段階と、
第1の移植片本体部に相関的に第2の移植片本体部を設置して、第2の取付け要素が第1の膨張可能部材に隣接するようにする段階と、
第1の膨張可能部材を膨張させて、第2の取付け要素と嵌合させるとともに、第2の移植片本体部に第1の移植片本体部を固着させる段階とを含む、
ことを特徴とする方法。
