Abstract
Description

本発明は、白内障、老眼、ブドウ膜炎、黄斑変性、若しくは限定されるものでないが糖尿病性網膜症を挙げることができる他の網膜症、脈絡膜新生血管膜(CNVM)、嚢胞様黄斑浮腫(CME)、網膜前膜(ERM)(黄斑萎縮)および黄斑円孔、ならびに、限定されるものでないが炎症、外傷、照射損傷、角膜血管新生を挙げることができる角膜(ならびに周囲の眼瞼および結膜)の障害、ならびにフックスジストロフィー、円錐角膜、格子状ジストロフィーおよび地図状斑点状指紋萎縮症のような多様なジストロフィー、ならびに乾性眼症候群、眼瞼炎および眼の酒さを表す、または進展させつつある若しくは進展する危険にさらされている患者の眼に局所投与し得る化合物および組成物を提供する。こうした化合物は、化学的断片として、白内障の進展の遅延において有効であることが以前
に知られているヒドロキシルアミン種を包含することが理解されうる一方、局所適用し得る化合物の成果は、治療の技術分野における非常に重要な進歩である。実際、Ziglerの特許の譲受人である国立保健研究所は、局所適用による白内障若しくは黄斑変性のための治療において有効であり得る化合物を同定することを試みたがしかし失敗した。この情況において、Ziglerの特許は注入を介するtempol−Hのようなある種の組成物の投与を列挙しており、そして点眼薬を介する局所投与の望ましさを認識しているが、しかしながら、この提案された投与経路は実務において利用可能であることが見出されなかったことが指摘される。従って、本発明は、「先駆け」でありかつ当該技術分野における長い間切望されてきたがしかし対応されていない必要性をかなえたとして見られるべきである。

[実施例1]
方法:エステル化合物の0.1〜0.5%溶液を、DTPA若しくはEDTAを含有する緩衝液(pH4.5〜5.0)中で調製した。該溶液を褐色ガラスバイアルに充填し、これを封止しかつ40℃に維持される制御温度容器中に入れた。サンプルバイアルを定期的に取り出し、そしてHPLC、GC若しくはGC/MS分析方法により分析されるまで0〜5℃で保存し、そしてこれらの条件下で3か月後に安定であることが見出された。

抗白内障剤として以前に試験された薬物と対照的に、tempol−Hおよびtempolは周囲の液体から水晶体組織に浸透する顕著な能力を有する。本節に記述される実験は、臓器培養条件下でのラット水晶体とのインキュベーション後の水晶体中の時間経過、有効成分濃度および化合物分布を測定した。

動物の角膜を横断する代表的化合物の吸収
実施例16に記述したin vivoウサギ試験からの眼房水サンプルを、化合物1およびその代謝物すなわち眼エステラーゼによる化合物1の加水分解により形成されるtempol−Hおよびカルボン酸(R1COOH)の存在について、GC/MSにより同定した。双方の代謝物は観察されたがしかし化合物1はされなかった。これは、化合物1がその代謝物に完全に転化されたことを確認した。
[実施例19]
3.5%の化合物1を含有する点眼薬を、2羽の覚醒ウサギの各眼に1時間間隔で6回投与した。薬物は良好に耐えられ、そしてこの予備試験において有害な所見は示されなかった。

熱損傷を生じさせることが可能なレベルより十分に下の網膜傷害を生じさせる光の能力は、光化学的網膜傷害と命名されている。光化学的網膜傷害の作用機序は、フリーラジカルの光誘発性の産生であると考えられている。光化学的網膜傷害を生じさせる、臨床眼科で一般に使用される光源の能力は十分に認識されている。眼科手術で使用される手術用顕
微鏡は、網膜の光化学的傷害を生じさせる能力を有することが示されている。この観察結果を、光化学的網膜傷害を阻害する多様な剤の能力を試験するためのウサギ眼のモデルを製作するのに利用した。検眼鏡で見ることができる網膜病変は、手術用顕微鏡への曝露のわずか約2.5分後に検出可能であることが決定された。下に示されるプロトコルを利用して、硝子体内注入を介して与えた化合物1若しくはtempol−Hが手術用顕微鏡により生じられる検眼鏡で見ることができる光化学的網膜病変の進展を阻害する能力を有するかどうかを決定する。
[実施例67]
硝子体内注入後のウサギの網膜組織中への化合物1若しくはtempol−Hの取り込みの評価
スキームに従って、両側で硝子体内注入により処置する。
1.標識した化合物1若しくはtempol−Hでの両側硝子体内注入。(N=12)
2.プラセボ(ベヒクル)での両側硝子体内注入。(N=12)
第1、7、14および28日にそれぞれ6羽のウサギ(それぞれ各処置アームから3羽)を、致死用量のペントバルビタールナトリウムで殺す。殺した直後に両側の摘出を実施し、そして網膜組織をシンチレーション分析のため取り出す(網膜組織は、液体の喪失を予防するために摘出後に凍結しうる)。網膜組織をスラリーに粉砕し、そしてアルカリ若しくは四級アンモニウム化合物中で溶解する。シンチレーションの読み取りを実施して、網膜組織中に存在する化合物1若しくはtempol−Hの量を定量する。
[実施例68]
背景:萎縮性加齢黄斑変性(AMD)の病因は完全に理解されていないとは言え、AMDは、網膜色素上皮細胞の死、光受容細胞の変性、およびその後に生じる結果として生じる失明で開始することが一般に受け入れられている。RPEに蓄積するリポフスチンをこれらの細胞の死と結びつける増大する多数の証拠が存在する。例えば、リポフスチン濃度は黄斑の下にあるRPE細胞中で最高であるのみならず、眼底の自己蛍光の検出によりRPEのリポフスチンをモニターすることもまた、RPE萎縮の領域が以前に増大された蛍光の部位で進展することを示した。RPEのリポフスチンとRPE細胞死との間の関連を検査することに関する研究は、PREのリポフスチンの主構成要素すなわちビスレチノイドフルオロフォアA2Eが細胞膜を破壊し得かつRPEに対する青色光損傷を媒介し得ることを示した。A2Eの光励起は、一重項酸素の生成、およびエポキシドが形成されるようなA2Eのレチノイド由来の側枝に沿った炭素−炭素二重結合への後者の付加につながる。こうして、A2Eは、変動する数のエポキシドをもつ化合物の混合物(A2Eエポキシド)に転化される。これらの高度に反応性のエポキシドは細胞を損傷することが示されている。最終的には、RPE細胞中でA2Eの照射により惹起される光化学的事象が、細胞基質を切断するためのシステイン依存性プロテアーゼ(カスパーゼ)の参画を必要としかつミトコンドリアタンパク質bcl−2により調節される経路によって、細胞死を開始する。

2.全部の死細胞の核が膜非透過性色素による染色(Dead Red核酸染色、Mo
lecular Probes)により赤色に見え、全細胞の核がDAPIで青色に染色する、二色蛍光アッセイ。細胞生存率を青色光照射8時間後に評価した。各ウェル中の照射領域内の5顕微鏡視野内の細胞を数えることにより複製物をアッセイし、また、実験あたり3ウェルをアッセイした。データは3回の実験のに基づく。

Claims (67)
- 患者における黄斑変性、網膜症、緑内障、老眼、結膜疾患、眼瞼障害、角膜疾患若しくはブドウ膜炎の処置方法であって、式:
R3およびR4は独立にC1ないしC3アルキルであり;また、
式中R1およびR2は一緒になって、若しくはR3およびR4は一緒になって、または双方がシクロアルキルであってもよく;
R5はH、OH若しくはC1ないしC6アルキルであり;
R6はまたはC1ないしC6アルキル、アルケニル、アルキニルまたは置換アルキル若しくはアルケニルであり;
R7はC1ないしC6アルキル、アルケニル、アルキニルまたは置換アルキル若しくはアルケニルであるか、
あるいは式中R6およびR7、若しくはR5、R6およびR7は一緒になって環中に3から7個までの原子を有する炭素環若しくは複素環を形成する]
を有する最低1種の化合物を含有する眼科で許容できる担体若しくは希釈剤を含んでなる組成物をこうした患者の眼に投与することを含んでなる、上記方法。 - R1、R2、R3およびR4がC1−C3アルキルである、請求項1に記載の方法。

- 前記化合物が、約50μMないし約100μMの組織および液体中の濃度を達成するように投与される、請求項1に記載の方法。
- 白内障、老眼、緑内障、加齢性黄斑障害若しくは眼の他の加齢に関係した状態の進展の予防若しくは遅延方法であって、式:
R3およびR4は独立にC1ないしC3アルキルであり;また、
式中R1およびR2は一緒になって、若しくはR3およびR4は一緒になって、または双方がシクロアルキルであってもよく;
R5はH、OH若しくはC1ないしC6アルキルであり;
R6はまたはC1ないしC6アルキル、アルケニル、アルキニルまたは置換アルキル若しくはアルケニルであり;
R7はC1ないしC6アルキル、アルケニル、アルキニルまたは置換アルキル若しくはアルケニルであるか、
あるいは式中R6およびR7、若しくはR5、R6およびR7は一緒になって環中に3から7個までの原子を有する炭素環若しくは複素環を形成する]
