Description
本発明は、概ね、患者における灌流不全を評価する方法及び装置に関する。
非常に低い血液の流れ或は低全身灌流は、典型的には低動脈圧によるものであり、そして、大出血、敗血、心停止を含む、多数の要因によって生じ得る。身体はそうしたストレスに対して消化管への血流を低減して、他のより重大な臓器のために取っておくことによって応答する。よって、心臓からの血液の低減された流れがあると、血流に関しての一時的な大きな低減によって、その生存が脅かされず且つより重要でない臓器への血流を制限する一方で、身体は血液の連続的な供給無しには長期に生き延びないであろう脳等の重要な内臓に、血液のより高い部分を指向させる。例えば、胃及び腸そして更に食道や口腔/鼻腔に供給する臓器の脈管構造への血液の流れは、心臓からの低減された血流がある場合、劇的に低減される。この理由のため、臓器血管への減少された血流はこうして患者の灌流不全を表す。外科医は、共通して、灌流不全を評価すべく胃及び腸におけるCO2採取及びpH測定によってこの現象を利用する。
様々な方法及び装置が、患者内のショックを示す灌流不全等の循環器機能の欠陥を、患者の胃腸(GI)管及び/或は上気道/上部消化管と連結する口或は鼻を介して、アクセス可能な粘膜表面に隣接する血流の測定によって評価するために提供されている。組織の灌流は組織を通じて流れる血球の速度と血球の数との双方の機能であって、組織を通じて流れる血液は、単独で使用されるpH或はCO2よりも組織灌流のより直接的な測定である。以前における当業界での意見は、減少された血流は灌流不全中の局所的な現象であることである。今日、判明されたことは、減少血流、減少pH、並びに、組織CO2における増大は、灌流不全中に身体の隅々にわたって生ずることであり、そして特に胃、空腸、結腸、並びに、直腸ばかりではなく、食道、咽喉、口、鼻、並びに、関連された領域でも生ずる。こうして、患者の胃腸管及び/或は上気道/上部消化管の組織内の血流を測定することによって、患者の灌流不全及び灌流レベルを評価するために新規で有用な方法及び装置が今日提供されている。

Claims (14)
- 患者における全身灌流の度合いを評価する装置であって、
患者の表面灌流圧を測定する表面灌流圧モニタと、
患者の血圧を測定する血圧モニタと、を備え、
指標が、患者の全身灌流の前記度合いを評価すべく、前記灌流圧測定及び血圧測定から引き出されることから成る装置。 - 前記表面灌流圧モニタがドップラー・センサを含む、請求項1に記載の装置。
- 前記引き出される指標が表面灌流圧指標である、請求項1に記載の装置。
- 表面灌流圧モニタが光電脈波を含む、請求項1に記載の装置。
- 前記引き出される前記指標が光学的プレチスモグラフィー指標であるである、請求項4に記載の装置。
- 舌下腺PCO2及びSaO2から成るグループから選択された測定であって、直接的或は間接的に為す測定のため、患者身体内の粘膜表面に隣接して位置決めされるべく適合する血流センサを更に含む、請求項1に記載の装置。
- pH測定するセンサを更に含む、請求項6に記載の装置。
- 患者における全身灌流の度合いを評価する方法であって、(i)表面灌流圧モニタを利用することによって患者における前記表面灌流圧を測定する段階と、(ii)患者における血圧を測定する段階と、(iii)前記表面灌流圧測定及び前記血圧測定から引き出される指標を計算する段階と、(iv)患者の全身灌流の度合いを評価する段階と、の諸段階を含む方法。
- 患者における前記表面灌流圧を測定する前記段階が、ドップラー・センサで測定を為すことを含む、請求項8に記載の方法。
- 前記表面灌流圧測定及び前記血圧測定から引き出される指標を計算する前記段階が、表面灌流圧指標を計算することを含む、請求項8に記載の方法。
- 患者における前記表面灌流圧を測定する前記段階が、光電脈波で測定を為す、請求項8に記載の方法。
- 前記表面灌流圧測定及び前記血圧測定から引き出される指標を計算する前記段階が、光学的プレチスモグラフィー指標を計算することを含む、請求項11に記載の方法。
- 患者の身体内における口によってアクセス可能な粘膜表面に隣接して、舌下腺PCO2及びSaO2から成るグループから選択された血流測定を為す段階(v)を更に含む、請求項8に記載の方法。
- pHを測定するpHセンサを提供する段階(vi)を更に含む、請求項13に記載の方法。
